DNA isolated from human tumor cells can induce malignant transfarmation of tissue culture cells. The DNA is then called an oncogene. Its protein produets have been detected in animal and human tumors and are considered to play a significant role in carcinogenesis. In order to evaluate whether the oncogenes are involved in development of tumors of epidermis and whether they could be used as tumor markers, immunoperoxidase staining was performed for the ras product in sections of squamous cell carcinoma, Bowen's disease, actinic keratosis, keratoacanthoma and seborrheic keratosis. Three cases of sgamous cell carcinoma showed 10~20 positive cells per high power field(HPF). Three cases of Bowen's disease revealed 1-9 positive cells per HPF, whereas the actinic keratosis 1~9 or no positive cells per 10 HPF in all three cases. The keratoacanthoma and seborrheic keratosis showed 1~9 or no positive cells in all observed cases. The positive staining was observed in the cytoplasm. The increasing positivity in parallel with the increase of malignant potential strongly suggests that the ras oncogene is closely related to development of epidermal malignancy and also point out the possibility of ras as a cancer marker.

Paragonimus westermani mainly infests the lung parenchyma, and it is also causes ectopic parasitism of various sites, such as brain, liver and subcutaneous tissue but there have been no reports in literature about paragonimiasis infesting lumbar epidural space. A 58-year-old housewife complained of lumbago and radiating pain to both lower extremities for 20 years. Calcified extradural masses were found by preoperative plain X-ray, myelogram, and CT scan. Intraoperatively, cystic mass in the epidural space at L2-3 was found and confirmed to be paragonimiasis by histopathological study. A case of ectopic paragonimiasis in the lumbar epidural space at L2-3 level is presented.

PURPOSE: We applied lateral lithotomy position to the severe ureteral stricture cases supposed to fail with only retrograde approach. MATERIAL AND METHODS: From October 1997 to April 1999, 13 patients with severe ureteral stricture (lenghth>2cm or complete obstruction) supposed to fail with only retrograde approach and one patient with study. The causes of ureteral strictures were pelvic malignancy in 5, tuberculosis in 4, trauma in 2 and others in 2. The patient's ipsilateral shoulder was rotated and fixed like lateral position. And ipsilateral pelvis was elevated with sandbag or pad and rotated about 45 degrees, too. The retrograde approach was tried at first, if fail, antegrade approach was combined. RESULTS: We could insert ureteral stent via retrograde approach only in 3 patients and we needed aid of antegrade approach for passage through ureteral stricture in the other 11 patients (79%). Percutaneous antegrade approaches were combined in those 11 patients and we could pass the guide wire and indwell the stent in 10 of 11 patients (91%) using this position. CONCLUSIONS: The lateral lithotomy position was very helpful to the simultaneous retrograde and antegrade approach in severe fibrotic or malignant ureteral strictures.

BACKGROUND AND PURPOSE: The present study was carried out to investigate transient arteriolar vasospasm following reperfusion in the gerbil as an ischemic model and relationship between vasospasm and the changes of morphologic, physiologic and regional blood flow. METHODS: The transient arteriolar vasospasm was observed under the microscope. All gerbils were perfusion fixed and ischemic neuronal damage was checked in the CA1 subfield of the dorsal hippocampus. Metabolic parameters were serially measured during the occlusion and after recirculation, and cerebral blood flow was measured with a laser Doppler flowmetry. RESULTS: 1. Among the 18 gerbils with BCAO(bilateral carotid artery occlusion) for 2 minutes, 8 gerbils showed vasospasm after reopening, namely post-ischemic vasospasm, and 10 gerbils did not show vasospasm. 2. In all animals, metabolic parameters showed decreased PaCO2 and high pH just after the occlusion but increased PaCO2 and lowered pH following the recirculation. BCAO caused marked increase in systemic blood pressure but it returned to preischemic levels following the recirculation. 3. In all animals, BCAO caused marked decrease of cerebral blood flow compared to preischemic levels immediately following the occlusion but it returned to preischemic levels following the recirculation. And it took some longer the onset and duration of the transient increase in blood volume after reopening in the animals with vasospasm than those without vasospasm. 4. There was no morphological neuronal damage in the hippocampal CA1 subfield, at 1, 3, 7 days following 2-minute ischemic insult. CONCLUSION: The present study confirmed that BCAO in gerbils resulted in forebrain ischemia caused transient cortical vasospasm in the gerbil. But there was no close relationship between vasospasm and the changes of morphologic, physiologic and regional blood flow.

The shoulder fusion is one of the good method to relieve pain, improve the function and stabilize the flail shoulder joint. But recent advance of arthroplasty, the procedure is less popular. Authors studied and analized 23 patients who received shoulder arthrodesis from Aug. 1978 to Aug. 1986 and reported functional outcome after shoulder arthrodesis. Among twenty three patients, brachial plexus palsy were eighteen, upper extremity residual poliomyelitis in three and shoulder joint turberculosis in two respectively. Postoperative follow up was 12 months to 9 years and 6 months, average being 2 years and 6 months. It took 14.4 weeks in average for shoulder arthrodesis. The postoperative abduction fusion angle was 33.8 degrees in average(20 degrees to 50 degrees). The active abduction after shoulder fusion was mean 51.8 degrees(20 degrees to 85 degrees). The abduction fusion angle between 20 degrees and 40 degrees was presented satisfactory results in the point of pain relief, functional assessment and patients satisfaction. High abduction fusion angle(more than 40 degrees) revealed pain around the fused shoulder area. The combined extra and intraarticular arthrodesis revealed better results. The Saber-cut approach was more valuable for secondary elbow reconstruction than Henry approach.

BACKGROUNDS: The Rh antigens are important m clinical practice. The classification of Rh phenotype is usually based on the antigen detection done by conventional serologic method, but it has many limitation such as delicate grading of antigen expression. Recently, Flowcytometry has been introduced in immunohematology to detect and quantitate cell bound immunoglobulins to assess blood cell antigens and related antibodies. So, we tried these method to detect Rh(D) antigen and measure its density and evaluated the possibility of clinical usage. MATERIALS AND METHODS: We performed a flowcytometric analysis for the expression of D antigen in D-positive, negative and weak D group in indirect immune fluorescence assay by using polyclonal antibodies. We measured the intensity of immunofluorescence as a degree of antigen density and analysed the difference of mean channel fluorescence value(MCF) among these groups. RESULTS: Weak D groups had the lower fluorescence than D-positive group, while D-negative sample showed the same fluorecence as negative ntrol. The difference of fluorescence intensity of D antigen between that of C antigen were not observed and were statistically insignificant. CONCLUSION: Flowcytometry appear to be a good tool for antigen density measurement in immunohematologic areas and shows the possibility of application to other aspect.

Thrombolysis using urokinase solution is one of the effective methods in treatment of intracerebral hematoma. The present study was undertaken in order to 1) determine the most effective concentration of urokinase solution, 2) determine the most suitable time interval of irrigation of urokinase solution through the measurement of hemoglobin and FDP(Fibrin / Fibrinogen Degradation Product) of drained solution, 3) estimate the size of unresolved hematoma without taking brain CT. The results are summarized as follows : 1) The most effective and economic concentration of urokinase solution was 1000 u/ml. 2) The most preferable time interval of irrigation of urokinase solution was about one hour. 3) It was possible to estimate the size of unresolving hematoma by means of measurement of hemoglobin and hematocrit of patient, hemoglobin and volume of the thrombolysed solution, so that it was unnecessary to take brain CT for measurement of remaining hematoma.
