6.Periodontal Status Following the Alignment of Buccally Impacted Maxillary Canine Teeth with Surgical Uncovering.

The present study examines the effects of orthodontic treatment of surgically exposed impacted upper canines or ectopically erupted upper canines to periodontal condition and whether various opening procedures have significant difference in postoperative periodontal status. The subjects included 23 orthodontic patients(7 men, 16 women) with unilateral upper canine impaction treated either with closed eruption technique(group I), with apically positioned flap procedure(group II), and those with canines ectopically erupted through keratinized gingiva(group III). In each subject, the ectopic canine was orthodontically aligned, and changes in periodontal tissue were assessed by measuring keratinized gingival width, attached gingival width, probing depth and bone probing depth. In all three groups, the width of keratinized gingiva was preserved while showed no signs of detrimental periodontal condition such as gingival recession. In all three groups, no significant difference in periodontal pocket depth from control was observed. The width of attached gingiva was significantly greater in patients treated with apically positioned flap procedure(group II) than in patients on other groups.

7.Evaluation of Accuracy and Interobserver Agreement of MR Cholangiography for Diagnosis of Cholelithiasis.

PURPOSE: To assess the accuracy of MR cholangiography(MRC) in the diagnosis of cholelithiasis, and to determine interobserver agreement. MATERIALS AND METHODS: Between March and September 1999, 43 consecutive patients with biliary obstruction [24 men and 19 women aged 25 -85 (mean, 58) years] underwent MRC using the single-shot fast spin-echo technique. Heavily T2-weighted source images(axial and coronal) 3 -5 mm thick and 12 projection images with 15- degree rotation and 5-cm thickness were obtained. All images were reviewed blindly and indepen-dently by two radiologists specialized in the interpretation of abdominal imaging information. Choledocholithiasis was evaluated in eight segments of the intrahepatic duct(IHD), extrahepatic duct(EHD) and gall bladder lumen. Final diagnosis was established on the basis of operative (n=31) and other radiological (n=12) findings. The sensitivity, specificity and accuracy of the MRC findings were assessed, and using kappa measurement (cross-table analysis,SPSS Windows for 8.0), interobserver agreement was determined. RESULTS: Thirty of the 43 patients, had choledocholithiasis (IHD stones in 7 cases, EHD stones in 15, and GB stones in 18). For radiologist 1, sensitivity, specificity and accuracy were 86%, 100% and 98%, respectively, in the diagnosis of IHD stones; 100%, 89% and 93%, respectively, in the diagnosis of EHD stones; and 81%, 96% and 91%, respectively, in the diagnosis of GB stones. For radiologist 2, the corresponding figures were 86%, 94% and 93% (1HD stones); 87%, 89% and 88% (EHD stones); and 81%, 86% and 84% (GB stones). Interobserver agreement for the diagnosis of choledocholithiasis was excellent in all cases. The kappa mesurement was 0.91 for 1HD stones, 0.77 for EHD stones, and 0.70 for GB stones. CONCLUSION: MRC is an excellent imaging modality for the diagnosis of choledocholithiasis, and interobserver agreement was also excellent.

8.A Case of Trisomy 10p with Vertebral Anomaly and Hypospadias.

Tisomy 10p syndrome is a rarely reported chromosomal abnormality with distinct craniofacial anomalies, severe growth and psychomotor retardation, osteoarticular anomalies, and organ malformations. Most of reported cases were due to translocation of 10p to the other chromosome. Rare causes are tandem duplication, maternal pericentric inversion, isochromosome formation. We experienced newborn infant with craniofacial anomaly, hypospadias and extra vertebra and rib. The characteristic craniofacial anomalies were frontal bossing, wide opened fontanel and suture line, sparse eyebrow, turtle beak mouth, Cytogenic analysis showed 46, XY, rec(10) dup(10p) inv (10)(p11.2q26.1). Karyotype of the father was normal(46, XY). However, karyotype of the mother showed 46, XX, inv(10)(p11.2q26.1). Therefore, chromosome 10 recombination resulting from a maternal pericentric inversion formed trisomy 10p. We report rare chromosome abnormality syndrome, trisomy 10p, with brief review.

9.Retinoblastoma Manifested by Hyphema and Orbital Cellulitis.

The authors experienced a case of retinoblastoma in a two-year-old female who had recurrent hyphema and marked erythematous lid swelling. We treated her for orbital cellulitis and hyphema, but those lesions were not improved. Finally, we performed enucleation for confirming the etiology of a disease which was pathologically proven to be retinoblastoma.

10.Clinical Outcomes of Early Phacoemulsification after Laser Iridotomy in Acute Angle-closure Glaucoma.

PURPOSE: To evaluate long-term change in intraocular pressure (IOP) in eyes undergoing laser iridotomy (LI) and early phacoemulsification after LI in patients with acute angle-closure glaucoma (AACG). METHODS: The retrospective, comparative chart review included patients with AACG, Group A who underwent only LI and Group B who underwent early phacoemulsification within 1 month after LI. Patients were followed up on day 1; week 1; and months 1, 3, 6, and 12 after LI. IOP changes were studied. RESULTS: This study included a total 99 eyes from 99 patients, 37 in group A and 62 in group B. The mean IOP were not significantly different between the two groups at the initial visit or 1 month later. However, group B showed a consistently lower mean IOP that that of group A at 3, 6, and 12 months (p= 0.003, <0.001, <0.001, respectively). The prevalence of IOP increase to greater than 21 mmHg was 3 (8.11%), 5 (13.51%), and 5 patients (13.51%) in group A and 0, 2 (5.41%), and 1 patients (1.61%) in group B at 3, 6, and 12 months, respectively. Group B showed a significantly lower prevalence of IOP increase (p = 0.050, 0.038, 0.026). CONCLUSIONS: We found that patients treated with early phacoemulsification after LI had better outcomes of well-maintained IOP compared to those undergoing LI alone. For AACG patients with coexisting cataract, early phacoemulsification after LI can be considered as a reasonable treatment to maintain IOP.
