2.Treatment of Infected uncemented Total Hip Arthroplasty: Two-stage revision procedure by preserving well-fixed femoral components in four cases.

In infected uncemented hip arthroplasty, 4 cases which showed well-fixed femoral stem were managed by staged procedure. Thorough debridement, removal of acetabular socket and insertion of antibiotics impregnated cement spacer was done without removal of femoral stem. No recurrence of infection or loosening was developed after staged procedure and revisoin was performed after infection evidence disappeared. The interval between the two procedures was 4 months (3-6) on average. In Weber's criteria, all 4 cases were graded as having definitive cure of infection on both clinical and radiological criteria (Grade I) .

4.The Risk Factor of Delirium after Transurethral Resection of the Prostate.

Purpose: Transurethral resection of the prostate (TURP) is the surgical treatment for symptomatic benign prostate hyperplasia with a good result, but some cases of postoperative delirium have been associated with this treatment. The purpose of this study is to develop and validate clinical prediction of postoperative delirium using the preoperative and postoperative data. Materials and Methods: We analyzed 417 patients who were admitted for TURP between January 1999 and December 2004. All patients underwent evaluations, including a medical history and laboratory tests, and we recorded the operative time, resection volume, complications, medications and length of stay from the medical records. Postoperative delirium was diagnosed using DMS-IV. Logistic regression test was used to assess the data of the patients and to determine the risk factor of postoperative delirium. Results: Postoperative delirium occurred in 33 patients (7.9%) of the 417 patients. The hospital stay of the patients with delirium was longer than that of the patients without delirium. Delirium was associated with an age>or=70 years, a postoperative Hb<10g/dl and decreased Hb>or=2.0g/dl (p<0.05). A postoperative Hb<10g/dl and a decreased Hb>or=2.0g/dl were associated with an increased risk of delirium on the multivariate analysis (odds ratio=4.6 and 5.7). Conclusions: The incidence of postoperaitve delirium in elderly patients with symptomatic benign prostate hyperplasia was 7.9%, and a postoperative Hb<10g/dl and a decreased Hb>or=2.0g/dl were identified as risk factors. Because delirium was associated with a variety of adverse outcomes, difficult treatment and high mortality, those patients with risk factor for delirium must be kept under close observation after TURP.

5.Trido Mitral Valve Replacement with Dacron Collar Prosthetic Valve due to Paravalvular Leak.

The paravalvular leak after redo mitral prosthetic valve replacement is rare but serious complication when it does happen. This condition should be corrected surgically to increase life span and improve symptoms. But simple closure or patch closure of paravalvular leak are not effective in cases of weak annulus or broad defect. We report 3 cases of trido mitral valve replacement using mechanical valve with its sewing ring expanded by a collar of Dacron sheet. The prosthetic sewing ring is anchored on the weak mitral annulus with multiple interrupted sutures, while the Dacron collar is contineuously sutured to the left atrial wall for blood tight sealing. All of the three cases showed event free postoperative course.

10.Relationship of Air Pollution and Pediatric Respiratory Diseases in Incheon City.

PURPOSE: It is generally accepted that the air pollution can cause acute respiratory diseases. This study was undertaken to investigate the relationship between the increase in the level of air pollutants and hospital visits for pediatric respiratory diseases in Incheon city. METHODS: We retrospectively reviewed the medical records of pediatric patients who visited the outpatient department(OPD) of Inha University Hospital for respiratory diseases(such as bronchial asthma, pneumonia, bronchitis, bronchiolitis and URI) from January 1 to December 31 1997. We converted the data into an average per day that included meteorologic data of air pollution(such as O3, CO, SO2, NO2, and PM10) and weather data(such as atmospheric temperature, relative humidity) in three different places in Incheon city. We used a Poisson distribution, selecting a lowess statistics model. We also used the S-PLUS statistics program. RESULTS: Ozone(O3), CO, and SO2 were all found to have significant associations with the OPD visits for pediatric respiratory diseases. After meteorologic variables(such as CO, SO2, NO2, PM10, atmospheric temperature, and relative humidity) controlled, we determined the relative risk between the increase in the OPD visits for respiratory diseases and every 0.01 ppm increase in O3. The relative risk was 1.16 and the 95% confidence intervals were 1.12-1.20. CONCLUSION: There was a significant relationship between the incidence of pediatric respiratory diseases and the increase in the level of air pollutants, especially O3, CO, and SO2 in Incheon city. Further studies on the effects of continuous daily exposure to low or ambient levels of air pollutants are needed.
