2.A STUDY THE SHADE CHANGE OF SEVERAL KINDS OF ALL CERAMIC CROWNS USED FOR METAL DOWEL CORE.

The purpose of this study is to compared and analyze the shade changes(deltaE)about In-Ceram(R),IPS-Empress(R),OPC(R) by using of the spectrophotometer arising from initial status and before and after cementation of the resin cement mounted on the metal core. We used a couple of statistics such as 'One-Way ANOVA'and Multiple Range Test. We could be able to verify significantly what is being discussed here up to 95%. The results drawn from our research are as follows: 1. At the time of our experiments regarding the initial shapes of all-ceramic and mounting status of all-ceramic on the metal crown(deltaE1), and a comparison of mounting of all ceramic on the metal crown with all-ceramic cemented on the metal core(deltaE2), at the time of shade change of initial shapes and after we cemented on the metal core. (1) no significant difference among all-ceramics was found. (2) no particular difference was found regarding the In-Ceram(R) (3) a significant difference between the deltaE1 and deltaE2 regarding the IPSS-Empress(R) was found (P<0.05). (4) a significant difference between the deltaE1 and deltaE2 regarding the OPC? was found (P<0.05). 2. When we compared the shade changes (deltaE) resulted from before and after the cementation on each of the parts involved of some all-ceramic . we could be able to find shape change increase from incisal third, middle third, and cervical third in that order in In-Cream(R)(spinell), IPS Empress(R) and OPC(R) all. In addition , we could be able to find a siginificant difference between cervical third and incisal third, middle third. (P<0.05) From what we have just seen, we might conclude that there is a significant shade change difference before and after the cementation with respect to IPS-Empress(R)and OPC(R). In addition, we could also be able to find more shade change difference at the cervical third rather than incisal third and middle third depending on the parts involved.

3.Giant Cavernous Hemangioma of the Esophagus: One Case Report.

Esophageal hemangioma is an extremely rare benign tumor that causes dysphagia and massive upper gastrointestinal bleeding. Although certain abnormalities seen on a barium swallow esophagography or at endoscopy may suggest an esophageal hemangioma, a contrast CT and radionuclide angiography using a blood-pool radiopharmaceutical can characterize the intense vascularity of the tumor. We experienced the ase of a 7x7x3.5 cm in size giant cavernous hemangioma of the lower 1/3 of esophagus in a 40 year old man. A mural cavernous hemangioma was diagnosed with a barium swallowed esophagogaphy, endoscopy, and a contrast CT. It was treated successfully by transthoracic esophagectomy including the tumor and esophagogastrostomy.

4.Association of the CYP1B1 Gene Polymorphism with the Risk of Advanced Endometriosis in Korean Women.

OBJECTIVE: To investigate whether polymorphisms of gene encoding CYP1B1 is associated with the risk of endometriosis in Korean women. METHODS: We investigated 199 patients with histopathologically confirmed endometriosis rAFS stage III/IV and 183 control group women who were surgically proven to have no endometriosis. The genetic distribution of four different CYP1B1 polymorphisms at G119-T, G432-C, T449-C, and A453-G were analyzed by polymerase chain reaction (PCR) and restriction fragment length polymorphism of PCR products. RESULTS: We found no overall association between each individual CYP1B1 genotype and the risk of endometriosis. The odds ratio of genotype GG/GC+GG/TC+TT/AA compared to GG/CC/CC/AA (reference) was calculated as 2.06 with a 95% confidence interval of 1.003~4.216. CONCLUSIONS: This results suggest that CYP1B1 genetic polymorphism may be associated with development of endometriosis in Korean women.

6.Clinical Studies of Aseptic Meningitis in Pusan Area in 1996.

PURPOSE: We report clinical features and viral studies on 153 children with aseptic meningitis in the Pusan area in 1996 and compare the results of this study with those of other reports in 1990 and 1993. METHODS: One hundred fifty-three children with aseptic meningitis who have been admitted to the Department of Pediatrics, St. Benedict Hospital between July and October 1996 were included. We described age and sex distribution, monthly distribution, clinical manifestations, laboratory data and complication. RESULTS: The male to female ratio was 2.32:1 and the most age was 4.21 years. It occurred mostly in July, August and October. It was later than the peak incidence between May and July in 1990 and 1993. The main symptoms were fever, headache and vomiting in this order. Rash was mostly maculopapular form and frequent in those younger than 4 years-old. No specific results were not found in peripheral blood and CSF studies. We failed to isolate the causative agent in the viral culture, but Echovirus 9 was suspected as a causative agent by its clinical features and other similar reports. The duration of clinical symptoms in the early diagnostic group was significantly shorter that in late diagnostic group. CONCLUSIONS: Aseptic meningitis was prevalent in children in the Pusan area, during the summer of 1996. It was frequent in male and the mean age was 4.21 years. It occurred mostly in July, August and October. Peak incidence was later than in 1990 and 1993. The main symptoms: fever, headache, vomiting and rash were observed frequently.

Kimura's disease is an uncommon, chronic inflammatory disease of unknown etiology. It is an important category of reactive lymphadenopathy in the oriental population. The most common sites are the subcutis of the head and neck, and parotid gland. It's clinical course is benign nature. The treatment modalities for this disease are steroid therapy, radiation therapy and surgical excision. We experienced a case of soft tissue mass in the left arm. It was slightly tender and relatively movable. We excised the mass, which was turned out to be Kimura's disease on microscopic examination.

10.Comparative Study of Heparinase Treated Activated Clotting Time with Hephrinase Treated Thromboelastography for Detecting Residual Heparin Effects Following Cardiopulmonary Bypass.

Residual heparin effects after protamine reversal is a potential bleeding disorder associated with cardiopulmonary bypass(CPB). To differentiate this from the other multiple factors causing coagulopathy should be initialized in the setting of management. The purpose of this study was to compare simple activated clotting time(ACT) and thromboelastography(TEG) with heparinase treated ACT and TEG for detecting residual heparin effects to distinguish rapidly the presence of heparin from the effects of other factors because the enzyme heparinase specifically neutralized heparin. After institution approval, 20 patients who required open heart surgery were studied. Baseline kaoline ACT, heparinase ACT, TEG and heparinase TEG(Haemoscope) were obtained before CPB on the same blood sample. The repeated tests were performed on the same blood samples 20 minutes after protamine reversal following CPB. Differences between heparinase treated tests and untreated tests were also evaluated at the same time. Wilcoxon signed ranked test was used to compare the results between before and after bypass. None of patients had significant postoperative bleeding complication. All tests before bypass were normal. Twenty minutes after protamine reversal, 3 patients showed kaoline ACT were extended above 10% of the value of heparinase ACT but all of them remained within normal range. However, nearly all patients showed heparin effects on TEG. The heparin effects on TEG were defined as significant differences in all of parameters, especially in alpha angle and R+K time between simple TEG and heparinase TEG. In Conclusion, heparinase treated ACT and native ACT are not sensitive to residual heparin effects after CPB. Their normal results did not preclude residual heparin effects on heparinase modified TEG. However, it might be further investigated to need additional protamine in the case of residual heparin effects on TEG.
