1.Two Cases of Allergic Reactions to Mesna which Imitate Malar Rash.

Hemorrhagic cystitis is potentially life-threatening sequellae of chemotherapy using oxazaphosphorine alkylating agents (cyclophosphamide and ifosfamide). Mesna contains a sulfhydryl group that is believed to bind acrolein within the urinary collecting system and reduce the hemorrhagic cystitis without affecting the chemotherapeutic potential. To date, about thirty cases of hypersensitivity or allergic reactions of the delayed and urticarial type associated with mesna have been reported. We reported two patients with systemic lupus erythematosus who developed facial rash and flushing associated with mesna which imitate malar rash.

2.Efficacy of the Preliminary Ultrasonographic Examination before Suprapubic Cystostomy by Trocar.

Suprapubic cystostomy by trocar is a simple, safe and useful method for urinary diversion in acute urinary retention. But rarely, severe complications occur during or after suprapubic cystostomy such as bowel perforation, through and through perforation of the bladder, rectal injury and severe hematuria. We studied the complications of suprapubic cystostomy retrospectively and compared 19 patients who performed ultrasonographic examination with 30 patients without ultrasonographic examination. The complications in 30 patients without preliminary ultrasonographic examination were initial failure in 4 cases (13.3%), extravesical Foley indwelling in 2 cases (6.7%), puncture site closed to the pubic bone in 2 cases (6.7%), spontaneous bladder rupture in 1 case (3.3%) and bowel injury in 1 case (3.3%). But, the complication in 19 patients with preliminary ultrasonographic examination was only 1 retried case (5.3%) because of extravesical Foley indwelling. As above results, the brief preliminary ultrasonographic examination of the bladder is a very useful method with minimal risk during suprapubic cystostomy by trocar.

3.A Case of Penile Cutaneous Horn.

The cutaneous horn rarely occurs on the penis and was reported only 43 cases since l854. The lesion is characterized by an overgrowth and a cornification of the epithelium, forming a solid protuberance. On microscopic examination there is extreme hyperkeratosis, dyskerstosis, and acanthosis This process usually develops over a pre-existing skin condition (i.e. wart, nevus, area of trauma or malignant change) and may enlarge quite rapidly Wide excision with removal of a margin of normal tissue around the base is the treatment of choice Recently we have experienced a case of penile cutaneous horn treated with partial penectomy.

4.Expression Pattern of Tumor Progression and Metastasis-related Gene Proteins - CD44H, CD44v6, erbB-2, and p53 -in Gastric Carcinoma.

Immunohistochemical studies of the molecules associated with gastric tumor progression and metastasis were done to evaluate their relationship with known prognostic factors and their usefulness in assessment of the progression of gastric carcinoma in 127 gastric carcinoma tissues. The 4 antibodies used in this study were CD44H, CD44v6, erbB-2, and p53. The CD44H expression was detected in 76 (59.8%), CD44v6 in 63 (49.6%), erbB-2 in 18 (14.2%), and mutant p53 in 98 (77.2%) out of 127 cases of gastric carcinomas. There was no significant correlation between the expression rates of each four proteins. The expression rates of all 4 proteins were not significantly correlated with age and sex of the patients and lymph node metastasis, but the correlation between CD44v6 expression and the depth of tumor invasion and tumor stage was significant (p<0.05). These results suggest that CD44v6 is closely associated with tumor invasion, and high levels of CD44H, erbB-2 and p53 are associated with tumorigenesis of the stomach as they are highly expressed in early as well as in advanced gastric carcinomas. The findings also support the conclusion that the loss of control of alternative CD44 mRNA splicing resulted in production of CD44v6 splicing variant in tumor cell facilitates tissue invasion by increased adherence of the tumor cell to an extracellular matrix or by tumor cell migration. It can be expected that CD44v6 overexpression in tumor cells appears to be an important prognostic indicator for gastric tumor progression.

7.Collective Review of Cases of Spinal Metastases

The authors have reviewed and analysed 238 cases of the apinal metastases. Those cases were treated from January, 1980 to December, 1986 at the department of orthopaedic surgery of Kosin medical center. Among these cases, 37 were received radiotherapy alone ; 10, surgery alone ; 9, combined treatment and the rest of 137 cases were treated symptomatically. In doing the surgical decompression, it was performed in an emergent basis, designed to minimize the progression of the neurologic dificit and to prevent the serious consequences. The effectiveness of the treatment was discussed. And our review concludes the followings : 1. The most frequent primary tumor is breast cancer in woman and lung cancer in man. A sex ratio is more frequent in woman, and peak age is 6th decade. An average survival time is longest in nasopharyngeal cancer(16.6 mos.). 2. The most frequent metastatic deposit is the thoracolumbar spine(28.57%). 3. The prognosis is best in the breast cancer(83.3%), and cervical involvement(66.7%). 4. The prognosis is good for cases with incomplete paraplegia, intact sphincter control, a long duration of pain and neurologic deficit, and the cases with cord compression of gradual onset. 5. The radiotheraphy is the most effective method of treatment.

8.Korea National Health Insurance Service Financial Status and Prospect

Background:
Korea National Health Insurance Service (NHIS) is operated as a social insurance system in which people pay a portion of their monthly income as insurance premiums and receive benefits when they experience illness or injury. Since 2005, the national health insurance remained surplus accumulating cumulative reserves each year. However, as of 2018, NHIS revenue recorded 62.11 trillion won and spending of 62.29 trillion won. The deterioration of NHIS finances is expected to accelerate with the aging population, income growth, new medical technology development, and enhanced security policies.
Methods:
To examine the financial health and sustainability of NHIS, we estimated the future revenue and spending until 2030 using the data from Korea Health Insurance Review and Assessment Service statistical yearbook. 2004–2018 average percentage change in NHIS revenue and spending was calculated. We estimated the future NHIS financial status using two methods. In the first method, we calculated the revenue and spending of the future NHIS by applying the 2004–2018 average percentage change to the subsequent years consecutively. In the second method, we estimated the future NHIS financial status after adjusting for the predicted demographic changes such as the aging population and declining birth rate in South Korea.
Results:
The estimates from this study suggest that the NHIS’s cumulative reserves will run out by 2024.
Conclusion
In terms of spending on current health insurance, there should be a search for ways of more efficient spending and funding options.

10.The Differences of the Smoking Habit Between Emphysema and Chronic Bronchitis.

BACKGROUND: Smoking is the most important and consistent determinant of the development and progression of COPD(Ed Note : Define COPD). The fact that cigarette smokers develop a different type of COPD, chronic bronchitis and emphysema, with different clinical and pathological aspects, suggests that the development of COPD has a relationship with other smoking-associated factors beyoud just a simple smoking history. The aim of this was to analyze the smoking habits and history of patients with COPD and to evaluate the development of different types of COPD accordint to patient's smoking habits. METHOD: To evaluate the differences in the smoking patterens of patients with chronic bronchitis and emphysema, a pulmonary function test was conducted, and the smoking history and patterns was obtained through a smoking history questionnaire by a direct personal interview from 333 male cigarette smokers diagnosed with COPD, in the Yeungnam university medical center(190 patients diagnosed with chronic bronchitis, 143 patients diagnosed with emphysema). RESULT: The patients with emphysema smoked earlier and had a higher smoking history(ie, more pachyears, more total amounts of smoked cigarette, and more deep inhalation and longer duration of plain cigarette exposure) than those with chronic bronchitis. The depth of ingalation was also significantly higher in the emphysema patients after taking into account age, cumulative cagarette consumption and the type of cigarette smoked. CONCLUSION: Emphysema was more associated with the increasing degree of inhalation as assessed by the depth of inhalation. A high alveolar smoke exposure may be a significant risk factor for the development of emphysema.
