1.Comparison of Two-Dimensional Doppler Echocardiography and Fick Method with Cardiac Catheterization of Qp, Qs and Qp/Qs Ratio in Ventricular Septal Defect in Children.

This study was performed to compare the amount of blood flow from left and right side heart using Doppler echocardiography and Fick method. The pulmonary blood flow (Qp), aortic blood flow (Qs) and Qp/Qs ratio were noninvasively evaluated by two dimensional Doppler echocardiography in 10 patients with ventricular septal defect in children, aged 2 years to 15 years (mean+/-SD=7.1+/-4.9 years). Cardiac catheterization was performed within 2 days after examination of Doppler echocardiography in all patients. Systemic and pulmonary blood flow was calculated by using Fick principle, based on measured oxygen consumption and measured blood oxygen saturation. The Doppler frequency shift was analyzed by computer system combined with Meridian Echocardiography System. Comparison of Qp, Qs and Qp/Qs ratio obtained by invasive methods and by two-dimensional pulsed wave Doppler echocardiography showed a correlation of 0.679 in Qp, 0.710 in Qs and 0.639 in Qp/Qs ratio (p<0.05).

5.Post-transcriptional control of c-erb B-2 overexpression in stomach cancer cells.

The growth factor receptor oncogene, c-erb B-2, is frequently overexpressed in the adenocarcinomas of breast, ovary, lung and stomach. Although the mechanism of erb B-2 overexpression is thought as the result of transcriptional upregulation in many primary human carcinomas, expression rate of c-erb B-2 at mRNA level is usually lower than the level of translated protein. We also found that the expression of erb B-2 in SNU-1 stomach cancer cells was greater at post-transcription level (Bae et al., 1993). To explore the underlying mechanism of erb B-2 protein overexpression, we have chosen two cells lines, SNU-1 and SNU-16 where transcription rate of erb B-2 was closely resemble to each other while expressed protein levels were quite different. The synthesis rate of erb B-2 protein in SNU-1 cells was faster than SNU-16 cells while levels of erb B-2 mRNA were found to be similar in both cell lines. The half-life of the expressed erb B-2 protein was not significantly different in both cell lines. Analysis of the 5' untranslated region (UTR) of erb B-2 mRNA (-1approximately-323) showed no sequence abnormality in both cell lines. However, ribonuclease protection assay using cloned 5 UTR sequence revealed that the size of 5' UTR of erb B-2 mRNA which associate with transcription initiation site(s) in SNU-1 cells was longer than that in SNU-16. These results suggest that the increased erb B-2 protein synthesis rate possibly due to the redundant selection of transcription initiation might be a mechanism of erb B-2 overexpression in SNU-1 cells.

6.Long Term Corticosteroids Effects on Cardiac Function in Childhood Nephrotic Syndrome.

Corticosteroids has been used in treatment of wide varieties of disease, but there was no report which related to the effects on cardic function with the long term corticosteroids therapy in children. We had been evaluated 18 patients with nephrotic syndrome who were admitted to department of pediatrics, Kyung Hee University Hospital until March 1987. Also we examined interventricular septal thickness, left ventricular posterior wall thickness and all parts of internal dimension of heart, and cardiac output and STI(Systolic Time Interval) by M-mode and Doppler echocardiography, and compared with body surface matched control group. Result are as follows;age and sex distribution were 4 4/12 to 14 6/12(9.11+/-3.55 years) and 15 males and 3 females. The duration of follow up was from 6 to 132 months(39.28+/-36.12 months). The interventricular septal thickness and left ventricular posterior wall thickness were more thickened in corticosteroids group than those of control. Systolic time interval, the index of left ventricular function, was more increased in corticosteroids group than in control group, but there was no differences in cardiac output between two groups. lthough longterm follow up studies are needed to confirm the cardiac effects of long term corticosteroids therapy, we conclude that long term corticosteroids therapy cause significant hypertrophy of cardiac muscle and periodic evaluation of cardiac function such as noninvasive Doppler echocardiographic studies are mandatory in patients with long term corticosteroids therapy.

7.Histochemical and ultrastructural changes of cyclosporin A and prednisolone on proximal tubules and glomeruli in rabbit kidney.

This experiment was designed to investigate the histochemical and ultrastructural changes of the proximal tubules and glomeruli of rabbits treated with cyclosporin A (CsA) or cyclosporin A and concomitant administration of prednisolone (PD). Rabbits were given with CsA for 1 week (CsAlW) and for 2 weeks (CsA2W) and CsA plus PD for 1 week (CsA +PDIW) and for 2 weeks (CsA+PD2W). The dose of CsA was 5mg/kg/day i.v. and PD was 1 mg/kg/day i.m. At 1 week and 2 weeks after the treatments, the following items were studied of alkaline phosphatase (ALPase). peroxidase, 5'-nucleotidase (SNU) with the ultrastructural enzymatic changes and electron microscopic changes. CsAlW, CsA2W and CsA +PD2W groups showed a decrease of ALPase activity and CsA+PD2W and CsA2W groups showed weakly increased peroxidase activity CsA2W and CsA+PD2W showed a decrease of 5NU activity. Ultrastructural changes of CsAlW CsA2W and CsA+PD2W groups revealed swollen and disrupted mitochondria, loss of brush border in the proximal tubules and thickening of basement membranes, edematous podocytes and fusion of root processes of podocytes in the glomeruli showing the early stage of nephrotoxicity. The concomitant treatment of PD with CsA eased the nephrotoxicity of CsA within 1 week after the treatment.

9.The Effects of Single Epidural Triamcinolone Injection on the Blood ACTH and Cortisol Level.

BACKGROUND: Epidural steroids injections are often used for the treatment of low back pain but their effects on the endocrine system have not been determined. Few studies have quantified the degree or duration of the suppression of the hypothalamic-pituitary-adrenal (HPA) axis in humans given epidural triamcinolone injection (ETI) for low back pain. The evaluation of the blood adrenocorticotropic hormone (ACTH) and cortisol was undertaken to determine the extent of suppression of the HPA axis in patients given ETI. METHODS: Lumbar epidural triamcinolone injections were performed on the painful lumbar intervertebral space with patients in the lateral decubitus position. The injection consisted of 40 mg of triamcinolone acetonide diluted in 10 mL of 1% lidocaine. Patients remained in the lateral position for 10 min after the procedure. Basal blood sampling was performed at 30 min before ETI and tested blood sampling was obtained at 7 days, 10 days, and 14 days after ETI. RESULTS: The blood cortisol level was significantly decreased at 7 days and 10 days but at 14 days was not significantly decreased and the blood ACTH level was not significantly decreased at 7 days, 10 days, and 14 days. CONCLUSIONS: Above results demonstrate that blood ACTH and cortisol level normalize 7 days and 14 days, respectively, after epidural triamcinolone 40 mg injection.

10.The Study of in Vivo Visual Pathway Tracing using Magnetic Magnanese Tracer.

Purpose: To evaluate the tracing of optic nerve tract using manganese enhanced magnetic resonance imaging. Materials and Methods: After injecting 30 microliter of MnCl2 (1 mol) into the retina of female New Zealand white rabbit, the contrast enhancements at major anatomical structures of optic nerve tract were evaluated by high resolution T1-weighted images 12 hours, 24 hours, and 48 hours after MnCl2 injection using 3D FSPGR (Fast Spoiled Gradient Recalled echo) pulse sequence at 1.5 T clinical MR scanner with high performance gradient system. Also, for quantitative evaluation, the signal-to-noise ratios of circular ROI on anatomical locations were measured. Results: The major structures on the optic nerve tract were enhanced after injecting MnCl2. The structures, which showed enhancement, were right optic nerve, optic chiasm, left optic tract, left lateral geniculate nucleus, left superior colliculus. The structures on the contralateral optic pathway to the right retina were enhanced whereas the structures on the ipsilateral pathway did not show enhancement. Conclusion: The Mn transport through axonal pathway of optic nerve system was non-invasively observed after injecting MnCl2 at the retina, which is the end terminal of optic nerve system. This Mn transport seems to occur by voltage gated calcium (Ca2+) channel and in case of direct injection into the retina, the fast transport pathway of voltage gated calcium channel seems to be responsible for Mn transport.
