1.A Case of Confluent and Reticulated Papillomatosis: Treatment with Etretinate.

Confluent and reticulated papillomatosis(CRP) is a rare dermatosis. The lesions are hyperkeratotic, verrucous papules and plaques, resulting in a confluence of lesions centrally and a reticulated pattern at the periphery. It is more common in women and during puberty. The histopathological findings show papillomatosis, hyperkeratosis and a sparse superficial perivascular lymphocytic infiltrate. We report a case of CRP that responded to oral etretinate in an 18-year-old man.

2.Shear bond strength of veneering porcelain to zirconia and metal cores.

STATEMENT OF PROBLEM: Zirconia-based restorations have the common technical complication of delamination, or porcelain chipping, from the zirconia core. Thus the shear bond strength between the zirconia core and the veneering porcelain requires investigation in order to facilitate the material's clinical use. PURPOSE: The purpose of this study was to evaluate the bonding strength of the porcelain veneer to the zirconia core and to other various metal alloys (high noble metal alloy and base metal alloy). MATERIAL AND METHODS: 15 rectangular (4x4x9mm) specimens each of zirconia (Cercon), base metal alloy (Tillite), high noble metal alloy (Degudent H) were fabricated for the shear bond strength test. The veneering porcelain recommended by the manufacturer for each type of material was fired to the core in thickness of 3mm. After firing, the specimens were embedded in the PTFE mold, placed on a mounting jig, and subjected to shear force in a universal testing machine. Load was applied at a crosshead speed of 0.5mm/min until fracture. The average shear strength (MPa) was analyzed with the one-way ANOVA and the Tukey's test (alpha= .05). The fractured specimens were examined using SEM and EDX to determine the failure pattern. RESULTS: The mean shear strength (+/- SD) in MPa was 25.43 (+/- 3.12) in the zirconia group, 35.87 (+/- 4.23) in the base metal group, 38.00 (+/- 5.23) in the high noble metal group. The ANOVA showed a significant difference among groups, and the Tukey's test presented a significant difference between the zirconia group and the metal group. Microscopic examination showed that the failure primarily occurred near the interface with the residual veneering porcelain remaining on the core. CONCLUSION: There was a significant difference between the metal ceramic and zirconia ceramic group in shear bond strength. There was no significant difference between the base metal alloy and the high noble metal alloy.

3.Present status of Enterobius vermicularis infestation among children in kindergartens.

BACKGROUND: An Epidemiological study was undertaken to evaluate the prevalence of E. vermicularis infestation among children in kindergartens. METHODS: A total of 247 cellotape anal swab specimens from kindergarten children in Inchon were examined for E. vermicularis eggs. Each specimen was collected in the morning. RESULTS: Out of 247 specimens from two kindergartens in Inchon, E. vermicularis eggs were detected in 168 specimens, the egg detection rate being 68.0%. The egg detection rates in boys and girls were 63.9% out of 133 children and 72.8% out of 114 children, respectively. The egg detection rate in girls was higher than that of boys. The egg detection rates by age group was 65.5%(age 3), S7.4%(age 4), 72.0%(age 5) and 75.4%(age 6), respectively. CONCLUSIONS: Enterobiasis is a cosmopolitan infection. Surveys for children of kindergartens indicate that E.vermicularis is probably the most widely distributed human helminth, its incidence varing from 59.2% to 73.8% in groups studied since pinworm infection is frequently a familial problem that infestation of the household frequently results in reinfection. Careful washing of hands and cleansing of fingernails before meals and after use of the toilet are the most practical measures, all member of family should be treated simultaneously when practicable.

4.The Diagnostic Value of Laboratory Tests in Acute Appendicitis.

PURPOSE: Acute appendicitis has atypical clinical findings that make accurate preoperative diagnosis difficult. The aim of this study was to investigate the usefulness of laboratory analyses, especially total leukocyte count (WBC) and neutrophil count, C-reactive protein (CRP) in the diagnosis of acute appendicitis. METHODS: Sixty-nine patients who visited to the Department of Surgery, Sangju St. Mary's hospital with the diagnosis suspected acute appendicitis were studied. WBC, neutrophil count, CRP, and erythrocyte sedimentation rate (ESR) were analysed. Surgery for acute appendicitis was performed in fifty-four patients. According to histopathological findings of the appendix, the patients were divided into four groups: complicated (20 patients with perforated & gangrenous type), suppurative (9 patients), phlegmonous (25 patients), and normal (15 patients). The sensitivity and specificity of the laboratory tests in the diagnosis of acute appendicitis were calculated. RESULTS: The sensitivity of WBC>10, 000/mm3, neutrophil count>70% or >7, 000/mm3, and CRP>12 mg/L in acute appendicitis was 68.5%, 85.1%, and 74.0%, and the specificity was 86.0%, 73.3%, and 80.0% respectively. The combination of neutrophil count or CRP was 94.4% sensitivity. The severity of appendicitis had positive relationship with WBC, neutrophil count, and CRP (P=0.0001, P<0.0001, and P= 0.0014, respectively). The appendicitis and normal groups were significantly different in WBC, neutrophil count, and CRP (P<0.05). CONCLUSION: WBC, neutrophil count, and CRP have diagnostic value among the myriad causes of right lower quadrant abdominal pain and show a positive relationship with the severity of appendicitis.

7.The Temperament and Character of Korean Male Conscripts with Military Maladjustment-A Preliminary Study.

OBJECTIVE: Recently, an increasing number of Korean male conscripts have been retiring because of military maladjustment despite the presence of qualifying medical and psychological screening tests in the Korean army. These problems suggest the presence of a common personality problem. To further examine this possibility, the present study used Cloninger's psychobiological model to investigate the temperament and character of soldiers suffering from military maladjustment. METHODS: Seventy-nine maladjusted male conscripts and eighty-seven controls enrolled at the 1596th unit from April 2011 to June 2012 participated in the present study. To measure participant personality, we used the Korean version of the Temperament and Character Inventory, Revised-Short. We used logistic regression analysis to examine the association between TCI-RS scores and risk of military maladjustment. RESULTS: The maladjustment group had a lower rank, socioeconomic status, education level, and a shorter duration of military service than the control group. The harm avoidance and self-transcendence scores were significantly higher in the maladjustment group, with lower scores for reward dependence, persistence, self-directedness, and cooperativeness scores. However, of these measures, only low cooperativeness was associated with an increased risk of military maladjustment. CONCLUSION: These results suggest that a low level of cooperativeness can predict military inadequacy. Maladjusted male conscripts may have different personality characteristics from normals. To validate our results, further follow-up or cohort studies with a larger sample will be required.

8.A case of an unruptured duplicated middle cerebral artery aneurysm–An unusual presentation of the distal internal carotid artery aneurysm

The duplicated middle cerebral artery (DMCA) is an anatomic variation that arises from the distal internal carotid artery (ICA) and supplies blood to the middle cerebral artery (MCA) territory. Aneurysms of the DMCA have been reported in 36 cases in 2020. We also report a case of a 3.7 mm saccular aneurysm originating from the DMCA. A 52-year-old woman visited our hospital with worsening headache. She had no neurological abnormalities. Magnetic resonance imaging (MRI) and magnetic resonance angiography (MRA) revealed a right distal ICA aneurysm at the anterior choroidal artery. Cerebral angiography was performed to confirm the shape and the size of the aneurysm. Cerebral angiography revealed that the vessel that was originally identified as the anterior choroidal artery by the MRA was actually the duplicated MCA that was originating from the aneurysm neck and was supplying the MCA territory. The patient’s aneurysm was clipped using a transsylvian approach and she recovered without any neurological symptoms. DMCAs are rare and often associated with aneurysms and require preoperative evaluation to confirm the vascular status, aneurysm characteristics, and the shape of the parent artery.

10.Treatment Results of Sinonasal Sarcomas : 20 Cases in Asan Medical Center.

BACKGROUND AND OBJECTIVES: Sinonasal sarcomas are relatively rare tumors, accounting for only about 1 percent of all sinonasal malignancies. The purpose of this study is to analyze the clinical features, treatment modalities and outcomes, survival and prognostic factors of sinonasal sarcomas. SUBJECTS AND METHOD: We retrospectively reviewed the medical records of 20 patients who were diagnosed and treated for sinonasal sarcomas between July 1993 and June 2006. Potential prognostic factors including age, tumor size, histology and adjuvant treatment were evaluated. RESULTS: According to histologic subtype, twenty cases consisted of 7 rhabdomyosarcomas, each 2 cases of spindle cell sarcoma, Ewing's sarcoma, angiosarcoma, and each 1 case of fibrosarcoma, leiomyosarcoma, hemangiopericytoma, peripheral neuroectodermal tumor, myxoid sarcoma, osteosarcoma, chondrosarcoma. The mean follow-up period was 61 months. Fourteen patients (65%) had undergone surgical approach with adjuvant chemotherapy or radiotherapy. Four patients (20%) had undergone surgical treatment only. Two patients (15%) had undergone chemotherapy or radiotherapy. At 5 years, the overall survival rates of sinonasal sarcomas were 65%. The survival rates according to the treatment modality were 100% (surgery only), 66% (surgery with adjuvant chemotherapy or radiotherapy), and 33% (chemotherapy or radiotherapy), respectively. The overall recurrence rates of the sinonasal sarcomas were 25%. The univariate analysis showed that the treatment modality and surgical margin are important prognostic factors for survival. CONCLUSION: The 5-year survival rate of sinonasal sarcomas was 65%. Early diagnosis and wide excision with safe resection margin are important for the treatment of sinonasal sarcomas.
