2.A Statistical Study of Dermatoses (2000-2004).

BACKGROUND: The incidence of skin diseases in the university hospital may not truely indicate their province in the population, nevertheless it offers the only available source of information.Objective and Method: To study the prevalence of common dermatoses and compare this with previous reports, we reviewed 20, 000 new outpatients who visited the Department of Dermatology at Wonju Christian Hospital from 2000 to 2004, and have statistically analyzed the findings. RESULTS: The study results are summarized as follows: 1. Of the 20, 000 outpatients, the total number of male patients was 9, 871 (49.4%), and the numbers of female patients was 10, 129 (50.6%). 2. As for age distribution, the most frequent age group was the first decade (15.3%), fifth decade (15.1%), fourth decade (13.9%) and sixth decade (13.7%), comprising 58% of the total outpatients. 3. The 19 most common dermatoses were found to be dermatophytosis, atopic dermatitis, allergic contact dermatitis, other eczema, urticaria, alopecia, herpes zoster, seborrheic dermatitis, psoriasis, bacterial infection, pruritus, drug eruption, vitiligo, acne, verruca, irritant contact dermatitis, benign epidermal tumors, nevocellular nevus and hyperpigmentary disorders. 4. Those dermatoses which showed a decreasing tendency to be treated by medicine included allergic contact dermatitis, other eczema, bacterial infections, acne, and irritant contact dermatitis. An increasing tendency was noted in dermatophytosis, urticaria, alopecia, herpes zoster, seborrheic dermatitis, psoriasis, pruritus, drug eruption, vitiligo, verruca, benign epidermal tumors and nevocellular nevus. There was no significant annual difference in incidences of hyperpigmentory disorders compared to atopic dermatitis. 5. Diseases with high seasonal distribution in summer were dermatophytosis and verruca. However, atopic dermatitis, alopecia, herpes zoster and pruritus showed high incidence in the winter period. Acne showed a peak incidence in both summer and winter. 6. As for gender distribution, dermatophytosis, alopecia, seborrheic dermatitis, bacterial infection and vitiligo showed a high frequency in males, whereas allergic contact dermatitis, urticaria, herpes zoster, psoriasis, pruritus, drug eruption, acne, epidermal benign tumors, nevocellular nevus and hyperpigmentary disorders showed a high freguency in female. 7. In age distribution, atopic dermatitis, bacterial infection, vitiligo, verruca and nevocellular nevus were found to be the most frequent in the first decade, and acne and alopecia in the second decade. Urticaria and psoriasis were most frequent in the third decade; dermatophytoses, allergic contact dermatitis, irritant contact dermatitis and hyperpigmentary disorders in the fifth decade; other eczema, seborrheic dermatitis and pruritus in the sixth decade; herpes zoster and drug eruption in the seventh decade; and epidermal benign tumors in the eighth decade. 8. In the distribution of dermatoses as disease groups, eczema (25.5%), diseases of the skin appendages (10.8%), dermatophytosis (10.0%), erythema, urticaria and drug eruption(10.2%), viral infections (8.4%), papulosquamous disease (4.1%), and pruritus and neurocutaneous disease (3.9%) constituted 72.5% of the total. CONCLUSION: The distribution of skin disease in our hospital was similar to studies wnducted in other provinces. Comparing the alone results with earlier studies carried out in the same area by Lee (3), Won (2) and Song (1), there was a relatively low incidence of dermatoses caused by animal parasites. However, a predominently high incidence of age related dermatoses was found.

3.The Traumatic False Aneurysm of Rt Femoral Artery: A Case Report

A 42 years old male with the traumatic flase aneurysm that occurred in right femoral artery after injury of knife was reported at the department of Orthopedic surgery of Hanyang University Hospital. In this case, a false aneurysmal sac was located at the anterolateral aspect of right femoral artery just below the sartorius muscle, and repaired by resection of sac and interrupted suture of arterial wall with 6~0 silk. The postoperative result was good, and there has been no evidence of recurrence.

4.The Effect of clonidine on the Induction Time of Midazolam.

These studies were conducted to determine the influence of intravenous clonidine on the induction time of midazolam when used for induction of anesthesia and assoeiated hemodynamic effect. Eighty ASA physical status I or II patients were randomly allocated to one of four treatment groups ; normal saline (control, group 1, n=20), clonidine 1.25 ug/kg (group 2, n=20), clonidine 2.5 ug/kg (group 3, n=20), clonidine 5 ug/kg (group 4, n=20). The test drug was admi- nistered 15 min before induction of anesthesia with intravenous midazolam (0.23 mg/kg). After injection of midazolam, induction time (the time from the start of injection to spontaneous closing of eyes and to loss of eye-lash reflex) and apnea ratio were recorded as well as mean arterial pressure, heart rate and oxygen saturation at 3 min intervals up to induction of anesthesia and then at 1 min intervals for 10 min. Significant shortening in induction time were observed in clonidine groups (33+/-8 and 448 sec in group 2, 32+/-7 and 428 sec in group 3, 26+/-8 and 38+/-8 sec in group 4) compared with the control group (68+/-11, 83+/-11 sec in group 1), but there were no significant difference between clonidine groups. Mean arterial pressure and heart rate were significantly decreased by large dose of clonidine (group 3 and 4), but not changed by saline and small dose of clonidine (group 1 and 2). These results suggest that small dose (1.25 ug/kg) of clonidine shortened the induction time of midazolam and did not significantly affect the hemodynamics.

7.A Study on the Availability of the On-Board Imager (OBI) and Cone-Beam CT (CBCT) in the Verification of Patient Set-up.

PURPOSE: On-line image guided radiation therapy (on-line IGRT) and (kV X-ray images or cone beam CT images) were obtained by an on-board imager (OBI) and cone beam CT (CBCT), respectively. The images were then compared with simulated images to evaluate the patient's setup and correct for deviations. The setup deviations between the simulated images (kV or CBCT images), were computed from 2D/2D match or 3D/3D match programs, respectively. We then investigated the correctness of the calculated deviations. MATERIALS AND METHODS: After the simulation and treatment planning for the RANDO phantom, the phantom was positioned on the treatment table. The phantom setup process was performed with side wall lasers which standardized treatment setup of the phantom with the simulated images, after the establishment of tolerance limits for laser line thickness. After a known translation or rotation angle was applied to the phantom, the kV X-ray images and CBCT images were obtained. Next, 2D/2D match and 3D/3D match with simulation CT images were taken. Lastly, the results were analyzed for accuracy of positional correction. RESULTS: In the case of the 2D/2D match using kV X-ray and simulation images, a setup correction within 0.06degrees for rotation only, 1.8 mm for translation only, and 2.1 mm and 0.3degrees for both rotation and translation, respectively, was possible. As for the 3D/3D match using CBCT images, a correction within 0.03degrees for rotation only, 0.16 mm for translation only, and 1.5 mm for translation and 0.0degrees for rotation, respectively, was possible. CONCLUSION: The use of OBI or CBCT for the on-line IGRT provides the ability to exactly reproduce the simulated images in the setup of a patient in the treatment room. The fast detection and correction of a patient's positional error is possible in two dimensions via kV X-ray images from OBI and in three dimensions via CBCT with a higher accuracy. Consequently, the on-line IGRT represents a promising and reliable treatment procedure.

9.Epitope specificity of Porphyromonas gingivalis heat shock protein for T-cell and/or B-cell in human atherosclerosis.

Since periodontal infections are suggested as risk factors for the development of cardiovascular diseases, the present study was performed to evaluate the T cell immune responses specific to Porphylomonas gingivalis(P. gingivalis) heat shock protein(hsp) 60 and T-cell and B-cell epitope specificities for P. gingivalis hsp60 in atherosclerosis. Anti-P. gingivalis IgG antibody titers were elevated in all patients. We could establish P. gingivalis hsp-specific T cell lines from the atheroma lesions, a mixture of CD4+ and CD8+ cells producing the cytokines characteristic of both Th1 and Th2 subsets. of 108 overlapping synthetic peptides spanning whole P. gingivalis hsp60 molecule, ten peptides with common epitopes specificities for both T-cell and B-cell were identified. it was concluded that P. gingivalis hsp60 might be involved in the immunoregulatory process of atherosclerotic diseases with epitope specificities.

10.Presacral Mass in Currarino Triad: Case Report.

The Currarino triad is a unique complex of congenital caudal anormalies including anorectal malformation, sacral bony abnormality, and presacral mass. Histologically the presacral mass has been identified as a meningocele, teratoma, enteric duplication or a combination of this. A Case of female infant with this triad was presented and was successfully treated by surgery. The presacral mass was a lipomyelomeningocele. The clinical features, unique radiologic findings, and importance of a correct diagnosis of currarino triad were reviewed.
