1.A Study on the Development of the Thoracic Aorta in Human Fetus.

Prenatal development of the thoracic aorta of the human during the period ranging from gestation weeks 7 (C-R length 20mm) to 30 (C-R length 260mm) was examined by transmission electron microscopy and the following results were obtained. The early form of cuboidal or columnar endothelial cells at 7-9 weeks of gestation changed gradually to typical flat endothelial cells at 12-14 weeks of gestation. At 9 weeks of gestation, the mesenchymal cells begin to differentiate to myoblasts, which have small clusters of myofilaments with dense bodies and rough endoplasmic reticulum. And from 14 weeks the differentiating cells begin to form a parallel concentric lamellar structure. At 12th week of gestation, elastic fibers were first seen in subendothelial connective tissue and the intercellular spaces between smooth muscle cells. Elastic fibers appeared as small globular shape which composed of a central core of elastic and peripheral microfibrils. From this period the amount of elastic fibers and their aggregation increases gradually in both the subendothelial space and the intercellular spaces between smooth muscle cells. At 30th week of gestation, subendothelial elastic fibers almost completed the internal elastic lamina and also well formed elastic laminae were seen between the smooth muscle cells adjacent to endothelial cells. However, in the space between the smooth muscle cells near the adventitia the elastic lamina formation is delayed. In the adventitia elastic fiber were scanty but collagen fibers are abundant.

2.The Incidence of Urinary Incontinence and influences on quality of life among elderly Women.

The purpose of this study was to identify the incidence of urinary incontinence of elderly women in Korea, to identify the life style for urinary incontinence. The sample consisted of 237 women of Seoul and 5 Provinces. Data for this study was collected from January 16 to June 23 by structured questionnaire. Urinary Symptom Questionnaire developed by Jackson and demographic questionnaire were used to collect the data. The data was analyzed by frequency, percentage, Chi-Square test, Chronbach alpha coefficient with SPSS/PC+ program. 1. It was reported that 64.5% of the subjects have experienced urinary incontinence. And stress, urge, and mixed type incontinence was 34.0%, 8.5%, 57.5% respectively. 2. Characteristics related to urinary incontinence are : Daily frequency 28.7%, nocturia 71.5%, urgency 74.0%, bladder pain 36.7%, unexplained incontinence 35.1%, noctural incontinence 23.5%, frequency of incontinence 23.3%. Wearing protection 42.3%, changing outer clothing 31.0%, hesitancy 17.1%, straining 24.7%, intermittent stream 27.2%, abnormal strength of stream 28.4%, retention 13.6%, burning 5.0%, incomplete emptying 13.6%, Inability to stop mid stream 32.5%. 3. The life style and incidence with urinary incontinence were as following. Fluid intake restriction 18.6%, interference in daily task 30.3%, avoidance of places and situation 28.0%, interference in physical activity 22.3%, interference in relationship with other people 16.6%, interference in relationship with husband/companion 4.5%, time after attack of urinary symptoms 81.5%. In terms of the feeling about the rest of life with urinary symptom, perfectly happy 5.0%, pleased 10.6%, mostly satisfied 18.7%, mixed feelings 28.6%, mostly dissatisfied 20.1%, very unhappy 16.8%, desperate 0.3%. 4. Significant relation between incidence of urinary incontinence and life style. Significant differences between Fluid intake restriction(X2=8.876, P=0.002), affected daily task(X2=32.113, P0.000), avoidance of places and situation(X2=30.155, P=0.000), interference in physical activity(X2=30.209, P=0.000), interference in relationship with other people(X2=26.091, P=0.000). In terms of the feeling about the rest of life with urinary symptom(X2=43.425, P=0.000) of life style and incidence of urinary incontinence. In conclusion, this study were preliminary study to provide nursing practices guidelines for elderly urinary incontinence. Nurses working with elderly should develop and provide adequate care for the incontinent elderly subjects.

3.A Case of Eruptive Vellus Hair Cyst Associated with Steatocystoma multiplex.

Eruptive vellus hair cysts and steatocystoma multiplex have clinical similarities in terms of age of onset, location and appeararice of the lesions, but histopathologic feitures are distinctively differen-tiated. We present herein a 37-year-old woman with eruptive vellus hair cyst and steatocystoma multiplex as well. The histopathologic sections from 2 different lesions showed features of both eruptive vellus hair cyst with sebaceous gland in and near the cyst wall, and steatocy toma multiplex with rudimentary hair follicle near the cyst wall. This case may support the premie that eruptive vellus hair cyst and steatocystoma multiplex have a common developmental origin.

4.Comprehensive orthodontic treatment of a young girl with an odontogenic keratocyst and impacted teeth in the mandible

Odontogenic keratocysts (OKCs) are one of the most aggressive cysts in the oral and maxillofacial area because of their high recurrence rate and infiltrative behavior. In growing patients with OKCs, a radical treatment approach might cause numerous complications, including the disturbance of jaw growth and loss of the involved tooth. This case report describes successful comprehensive orthodontic treatment combined with marsupialization of the cyst in a young girl who exhibited an OKC with impacted teeth. The 10-year-old girl presented with an OKC extending from the mandibular symphysis through the left mandibular body, with ectopic impaction of the mandibular left canine and first premolar, as well as congenitally missing bilateral mandibular second premolars. Interestingly, spontaneous improvement of the positions of the ectopic impacted teeth, along with a reduction in the size of the cyst, occurred during marsupialization. The sequential use of removable and fixed appliances enabled orthodontic traction of the impacted teeth. The treatment outcome was stable at 2.5 years after the end of the treatment. We speculate that comprehensive orthodontic treatment combined with marsupialization can be an effective treatment strategy for patients with OKCs, especially when they are encountered in young, growing patients with impacted teeth.
5.A case of perifollicular fibroma.

Perifollicular fibromas are very undsual adnexal tumor of the skin. Clinically, they are small firm akin colored or pinkish papules and usually located on the face and neck. We report a case of perifollicular fibroma which occurred in a 30-year-old woman, who had asymptomatic multiple small skin colored papules on the right side of her face for about 20 years. On physical examination, the papules were ferm and non-tender. The biopsy specimen demonstrated concentric proliferation of fibrous tissue surrounding hair follicles ad the collagen fibers within concentric fibrous sheaths surrounding hair follicles stained green with Masson's trichrome stain, which were quite typical for perifollicular fibroma. Because we could not find any epithelial proliferation in the serial sections, fibrofolliculoma could be ruled out. The skin lesions were removed by electrodesiccation and followed up for 5 months without recurrence.

6.Aberrant Expression of p53 in Basal Cell Carcinomas and Squamous Cell Carcvinomas.

BACKGROUND: The p53 gene mutations have been found in a variety of human skin cancers. To date most studies concerning p53 gene mutations in skin cancers focused on squamous cell carcinoma(SCC) and its precancerous lesions such as actinic keratosis and Bowen's disease, especially related to ultraviolet light, but few on basal cell carcinoma(BCC). OBJECTIVE: This study was aimed to investigate the aberrant expression rates on p53 mutant gene in BCCs as well as in SCCs and their changes depending upon their clinicopathologic characteristics. METHODS: An immunohistochemical study was employed using the mouse monoclonal antibody raised against recombinant human p53 in 30 BCCs and 16 SCCs. Also, clinical and histopathologic analyses for age, sex, site, duration and histopathologic classification were performed. RESULTS: p53 immunoreactivity was observed in 57% of BCCs and 75% of SCCs. In BCCs P53 immnoreacticvity was increased by up to 61% in cases(28/30) developed on the sun-exposed area. In SCCs p53 immunoreactivity(89%) on non-sun-exposed area was unexpectantly higher than that(57%) on sun-exposed area. No significant correlation between p53 imunoreactivity and clinicopathologic characteristics in BCCs and SCCs was noted. CONCLUSION: Mutations of p53 may play an important role of the pathogenetic sequence in a large part of the pathogenetic sequence in a large part of BCCs as well as SCCs.

7.Cryotherapy in a patient with acquired tufted angioma.

A 26-year old woman had tender erythematous grouped papules and plaque(10 cm in diameter) with underlying deep seated rodules on the right shoulder for 2 years. A biopsy specimen showed vascular tufts composed of endothelial cells in the mid and lower dermis, which was diagnostic of acquired tufted angioma. After 4-months of cryotherapy with liquid nitrogen, most of the lesions had disappeared as well as her subjective symptom.

8.A Case of Hypergammaglobulinemic Purpura of Waldenstrom.

Hypergammaglobulinemic purpura of Waldenstrom is a distinct syndrome consisting of recurrent episodes of purpura, elevated serum r-globulins, elevated erythrocyteed rnentation rate, and mild anemia. This disease has been divided into the prirnary type without an uncerlying disease, and the secondary type with a known underlying disease. We diagnosed a hypergammaglobulinemic purpura of Waldenstram in a 53-year-old woman who presented sudden onset of showers of purpuric macules and petechiae of 24 hours duration involving her lower extremities with a 10-rnonth history. She showed characteristic laboratory findings consistent with those of hypergammiglobulnemic purpura of Waldenstrorri and also showed positive results of antinuclear antibody and rheumatoid factor. We tried to find out if there we any associated underlying disases such as systemic lupus erythematosus or Sjogren synirorne through various tests but faile it find any. We concluded she showed a primary type of hypergarnmhglobulinemic purpura of Waldcnstrorn.

9.Proliferating Cell Nuclear Antigen Immunohistochemical Study in Melanocytic Skin Lesions.

BACKGROUND: It is well-known that the giant congenital nevi prcgress to malignant, melanama more frequently than other benign melanocytic nevi but to date the laor tory methods for early detection of such progression were not avsilable. The proliferating cell uncleai antigen(PCNA) staining has been regsrded as an useful marker in determining prognosis of some maignant diseases. OBJECTIVE: The PCNA taining was performed as a predictive value of malignant transformation from benign meanocytic skin lesions. We investigated the differcnces between malignant melanoma and the benign lesions. MATERIAL AND METHODS: Immunohistochemical study was employee using anti-PCNA, anti-S-100, and anti-CD45RO antibody in 6 giant congenital nevi, 8 small and medium sized congenital nevi, 10 acquired nevi, and 10 malignant meanomas. Only cells positive for both PCNA and S-100, and negative for CD45RO on the serial sections were identified as melanocyts. RESULTS: The number of PCNA-posit,ive cells per 1000 melanocy,es averaged 6.0+7.5 in giant, congenital nevi, 2.9+1,9 in small and medium sized congenital novi, 3.1+2.7 in acquired nevi, and 61.5+ 39.4 in malignant, melanomas. Malignant melanomas showed onger intensity of PCNA staining than the other melanocytir nevi. CONCLUSION: There was not significant difference of the numter of PCNA-positive cells among the groups of congenital neviind PCNA staining can't be used in prditive measure of malignant. progression and studying mechansm of malignancy in giant congenital nevi. But, PCNA staining is considered as an useful method in differentiat,ing malignant melanoma from mllanocytic nevi.
