1.Epidemiological Consiteration on Venereal Diseases Control in Korea.

It is recognized not only by the health experts but also by the public that venereal diseases are remarkably increasing in these days. Therefore, every available measure for declining of the diseases are planned and implemented through case-finding, diagnosis, treatment, education and research. The author intended to compare and analyse infection rates of venereal diseases between reporting from the Ministry of Health and author's private V.D. clinic during 1961-1971. The following results are obtained through the study: 1. According to the report of the Ministry of Health, 45.0% of total examinees were sick with gonorrhea and 6.5% with syphilis, in one hand, on the other hand 78.7% with gonorrhea and 16.3% with syphilis at the author's clinic. 2. By both the report of the ministry and author, highest infection rates were found in 196601968, and this fact was considered by the higher infection rate of G.I. in same years. 3. Contrarily, the lowest infection rates were found out in 1970-1971 through both reports.

5.Assessment of Pregnancy Outcome in IUGR Fetuses, Using the Doppler Flow Velocimetry of the Umbilical and Middle Cerebral Artery Authors.

OBJECTIVES: The purpose of this study in intrauterine growth retardation(IUGR) is to assess the relationship between the Doppler flow velocimetry and pregnancy outcome. STUDY DESIGN: 356 normal and 159 growth retarded fetuses between 21 and 42 gestati- onal weeks were investigated retrospectively. All IUGR fetuses were divided into four gro- ups according to the results obtained by Doppler flow velocities as follows. 1) Group 1(n= 89) : patients had normal umbilical artery resistance index ( Rlu ) and normal middle cerebral artery resistance index ( Rlmca ) 2 ) Group 2 ( n= 9 ) : patients had normal Rlu and abnormal Rlmca 3 ) Group 3 ( n=25 ) : patients had abnormal Rlu and normal Rlmca 4) Group 4 ( n=36 ) : patients had abnormal Rlu and abnormal Rlmca. RESULTS: We obtained normal values of RIu and RImca in oriental. Abnormal cardiotoc- ography(CTG) findings and operative delivery for fetal distress were significantly increased in group 2(p < 0.05) and markedly increased in group 3 and 4 ( p < 0.01 ) compared with group 1. Adverse neonatal outcome was significantly increased in group 3 and 4 compared with group 1. There is significant increase of adverse neonatal outcome in group 4 compared with group 3 ( p < 0.05 ). CONCLUSIONS: We obtained the normal values of Rlu, Rlmca and cerebroplacental ratio ( CPR ) in oriental. Adverse neonatal outcome was significantly increased in group 2, 3 and 4 compared with group 1. There is significant increase of adverse neonatal outcome in gr- oup 4 compared with group 3. We also found a strong correlation between the CPR and adverse neonatal outcome in IUGR. In IUGR, centralization of the fetal circulation turned out to exist, resulting in a preferential shunting of blood flow to the fetal brain. Brain-sp- aring effect with Doppler flow velocimetry could be demonstrated by this result.

6.A Mycoparasitic Ascomycete Syspastospora parasitica on the Entomopathogenic Fungus Paecilomyces tenuipes Growing in Bombyx mori.

While studying Bombyx mori "Zipnuae" inoculated with an entomopathogenic fungus, Paecilomyces tenuipes, a mycoparasitic hymenoascomycete with a long neck was isolated from the silkworm rearing room. The fungus was identified as Syspastospora parasitica which has been recognized as a hyperparasite on various moniliaceous hyphomycetes. A descriptive illustration is presented in this study.

9.A Case of Epidermolysis Bullosa Simplex ( Dowling - Meara Type ).

The Dowling-Meara type of epidermolysis bullosa simplex(EBS) is genetic disease that is transmitted as an autosomal dominant trait and is charscterized clinically by pherpetiform clustering of blisters and palmo-plantar keratoderma. The disease usually presents at with or in early infancy. Although serious and hemorrhagic Wers may occur on any part of the body, the lesions heal without scaning in general. The disease shows a tendency to improve by progressian of age and it usually follows a relatively benign course. Microecopically, there are intraepidermal bli.ter s forming as a result of cytolysis of basal cells. In addition, the is a highly characteristic clumping of tonofilaments of keratinocytes in the lower epidermis, which is not seen in any other form of EBS. We report a case of Dowling-Meara type of EBS that is first destribed in Korean medical literatures.

10.The Clinical Significance of High Risk HPV Infection and Positive Margin of LEEP in the Management of CIN III.

OBJECTIVE: To determine the significance of high risk human papilloma virus (HPV) infection and involvement of the resection margins of cervical cone biopsy specimens removed by loop electrosurgical excision procedure (LEEP) in the management of cervical intraepithelial neoplasia (CIN III). METHODS: Records of 506 LEEP procedures performed due to abnormal Pap result between December 1995 and December 1998 were reviewed. Among them, 232 cases with histologically proven as CIN III in LEEP and follow-up Pap smear more than 6 months was available were assessed. All patients were assigned according to HPV infection and resection margin involvement in LEEP, and recurrence rate in the follow-up was compared with reference to post-LEEP treatment regimen, observation or hysterectomy in each group. RESULTS: Of 232 cases reviewed, 10 cases(4.3%) were ASCUS, 19 cases(8.2%) were LSIL, 199 cases(85.8%) were HSIL, and 4 cases(1.7%) were invasive cancer in initial Pap smear. Abnormal Pap result in the follow-up was as follows: 4 cases of ASCUS, no case of LSIL, and 6 cases of HSIL. Excluding ASCUS, the overall recurrence rate in the follow-up was 2.6%(6/232). HPV positive was 164(70.7%) and HPV negative was 68(29.3%) in the LEEP specimen. Of 164 HPV positive, 41 were followed up without further treatment and 123 with hysterectomy. There was no significant difference in the recurrence rate between the two groups(P=0.41). Of 68 HPV negative, 27 were only followed up and in 41, hysterectomy was done. And also, there was no significant difference(P=0.32). Of 232 cases, resection margin was positive in 167(72.0%) and negative in 65(28.0%). Of 65 margin negative, 29 were followed up without further treatment and 36 with hysterectomy. There was no significant difference in the recurrence rate between the two groups(P=0.36). Among 167 margin positive, 39 were only followed up and in 128 hysterectomy was done. Recurrence rate was significantly higher in followed up only group(P=0.04). CONCLUSION: HPV infection in LEEP specimen was not helpful in deciding post LEEP treatment modality in CIN III patient. But, when resection margin was positive, hysterectomy should be performed because by doing so, recurrence rate was significantly lower than when only follow up was done.
