2.A Case of Advanced Gastric Cancer, Developed from Endoscopic Diagnosis of Early Gastric Cancer.

Early gastric cancer is defined carcinoma that is limited to the gastric mucosa and submucosa and that does not extend into the muscularis propria. The development and widespread use of upper gnstrointestinal endoscopy with biopsy and cytology have enhanced our ability to make early diagnosis of gastrie carcinoma. Gastric cancinoma is being diagnosed at an ealry stage aad the survival rates for these patients are significantly greater than those with advanced gastric carcinoma. These ealry lesion can be cured in over 90% of the cases. In fact, it is almost impossible to follow up cases of early gastric cancer without surgical treatment. We report a case developed advanced gastric carcinoma 34 months after initial endoseopic diagnosis of early gastric cancer. A, 48-year-old woman was diagnosed early gaatrie cancer on June 21, 1985 by endoscopy. Proper surgical intetvention was not carried out due to her liver cirrhosis. The follow-up endoscopic examination was done on March 16, 1988. The gastric cancer looked like an advanced stomach and extended to other site.

3.A Case of Severe Air Leak Syndrome Treated with High - Frequency Oscillatory Ventilation ( HFOV ).

In air leak syndrome, a significant portion of the volume delivered during a positive pressure breath can be lost through the leak. HFOV can achieve adequate ventilation at lower peak and/or mean intrapulmonary pressure than conventional mechanical ventilation (CMV) and has been an effective treatment of already established air leak syndrome. We report a 1-day-old male infant with severe respiratory failure from pneumothorax and pneumomediastinum, who was refractory to CMV with chest tube drainage. HFOV was applied to this patient for 114 hours, and improvement of oxygenation and ventilation as well as significant reduction of pneumothorax followed.

4.Open Reduction and Primary Ligament Suture for Irreducible Knee Dislocation: A Case Report

Irreducible knee dislocation among acute knee dislocations is very rare. If reduction and treatment are not performed early, serious complications such as skin necrosis, vascular and nerve injury, compartment syndrome, and instability can occur. So far, treatment options, such as two-staged operation with early open reduction and delayed ligaments reconstruction or repair, one-staged arthroscopic reduction without ligaments reconstruction or repair, one-staged operation with open reduction and ligaments reconstruction have been reported for the irreducible knee dislocation. In this case, the authors have found satisfactory results following one-staged operation with open reduction and primary ligament suture after diagnosing the exact injury site using magnetic resonance imaging preoperatively.
5.Plastination: An Improved Method for Preservation of Pathology Specimens.

The gross tissue specimens are a valuable aid to the teaching of pathology and anatomy. However, traditional methods for storage and handling of them are discouragingly difficult and, recently, minimal surgical resections as well as preoperative interventions make it more difficult to have instructive gross specimens. Plastination is a process of tissue preservation by impregnation with silicone polymers or epoxy resins. The process in our study involves dehydration by cryosubstitution in aceton, defatting, forced impregnation of silicon polymer in a vacuum, curing and finishing. We submitted 40 surgically resected specimens to plastination. The resulting specimens are odorless, relatively dry, durable, life-like, non-hazardous, maintenance-free, and do not deteriorate with time. Plastinated specimens are a useful adjunct to the teaching of pathology, particularly suited for use in small groups, and appropriate method of tissue preservation. They are much preferred to wet preparation and conventional pots by both students and teachers owing to their accessibility, superior illustrative powers, and comparative ease of interpretation.

6.A Study of Factors Affecting Time of First Stool in Premature Infants.

PURPOSE: To assess the effect of gestational age and illness severity, and the effect of antenatal exposure to magnesium sulfate, glucocorticoids, and antibiotics, on the timing of the first stool in preterm infants. METHODS: Medical records of all preterm infants admitted to the neonatal ward at Kangnam Sacred Heart Hospital between March 1998 and August 1998 were reviewed. We studied the time of the first stool in 55 infants. RESULTS: The median age of the infant at the time of first stool was 18 hours, and 90% of the infants passed stool by 50 hours. Both the gestational age and the illness severity, as measured by the score for neonatal acute physiology(SNAP), correlated significantly with the timing of the first stool(r=0.47 and P<0.001 for SNAP; r=0.29 and P<0.05 for gestational age). An analysis of covariance showed that the relationship between SNAP and the timing of the first stool was significant even after adjustment for gestational age(P<0.01), but the relationship between the gestational age and the timing of the first stool was not significant after adjustment for SNAP (P=0.14). Antenatal exposure to magnesium sulfate for tocolysis, glucocorticoids for enhancing fetal lung maturity, and antibiotics, had no effect on the timing of the first stool. CONCLUSION: Delayed passage of first stool is a function of illness severity, not of gestational immaturity. Antenatal exposure to magnisium sulfate, dexamethasone, and antibiotics, does not affect the timing of first stool in premature infants.

9.Clinical Evaluation of the Glaucoma after Penetrating Keratoplasty.

We retrospectively evaulated the frequency and risk factors of glaucoma after penetrating keratoplasty by reviewing charts of 96 patients(112eyes) who received the penetrating keratoplasty from May 1992 to Novermber 1995 at Chonnam Universty Hospital. Intraocular pressure was increased over 21mmHg in 18 eyes after surgery. Underlying disease of them were graft failure (5 eyes), bullous keratoplasty(5 eyes), corneal ulcer(5 eyes), trauma(3 eyes). Ten eyes(55.5%) were glaucomatous preoperatively. Eight eyes(44.4%) were phakic, 7 eyes(38.9%) aphakic, and 3 eyes(16.7%) pseudophakic. Ten eyes(55.5%) underwent only penetrating keratoplasty, 4 eyes(22.2%) vitrectomy at the time of penetrating keratoplasty, and 4 eyes(22.2%) combined cataract extraction. The eyes with previous history of glaucoma(19 eyes) had higher incidence of increased intraouclar pressure than those 93 eyes with no glaucoma history(P<0.0001). Aphakic eyes(21 eyes) had higher incidence of glaucoma than phakic eyes(80 eyes)(p<0.05).

10.Radiological evaluation of congenital pulmonary vein obstruction.

Congenital obstuction of pulmonary vein without anomalous drainage can cause long-standing pulmonary congestion and pulmonary arterial hypertension, and it may include stenosis of individual pulmonary veinsor total pulmonary vein atresia. We reviewed seven cases of pulmonary vein obstruction, five of which accompanied other cardiac anomalies. Right pulmonary veins were involved in all seven cases including one bilateral case. Pulmonary veins were occluded totally in five and partially in three lungs. Pumonary catheterization and angiography were done for diagnosis. Chest radiographs of total occlusion cases showed decreased lung volume, features of pulmonary edema, interstitial lesions, and pleural thickening, which were quite specific, whereas pulmonry venous dilatation was dominant findings in partial obstruction cases. Pulmonary perfusion scans (n=3) showed total perfusion defects in the cases of total occlusion of veins. MR imaging (n=2) demonstrated total occlusion of pulmonary veins in the venoatrial junction in two, and membranous focal obstruction in one lung. Two patients had pneumonectomy and histological confirmation, Although catheterization and angiography are essential for the diagnosis, MR imaging is thought to be useful for the diagnosis of pulmonary vein obstruction.
