3.The side effects of antiepileptic drugs in epileptic children treated with monotherapy.

The goal of therapy of epileptic children is to control of seizures with minimal side effects of antiepileptic drug (AED). The relationship between side effects, serum levels and doses of AED were examined prospectively in 72 epileptic children who received monotherapy with phenobarbital, phenytoin, valproate, or carbamazepine for 6 to 26 months. Our results were as follows: 1) Side effects were observed in 59.7% of all patients; in decreasing order, behavioral (41.6%)>digestive (25.0%)>others (22.2%)>neurologic (11.1%). 2) The most common side effects in children treated with phenobarbital (PB) was behavioral (54.2%); these included hyperactivity, drowsiness, sleeping in decreasing order and there was significant relationship between drowsiness and serum level of PB(P<0.05). 3) Common side effects in patients receing carbamazepine (CBZ) were behavioral (52.0%) and digestive (52.2%); behavioral disorders included drowsiness and sleeping which showed significance with dosage of CBZ (P<0.01), digestive disorders were abdominal pain which showed significance, with dosage of CBZ (P<0.05) and weight gain. Other side effects were dizziness, headache, diplopia, tremor pallor, durg eruption, alopecia, aggravated acne etc. 4) The most common side effect in children treated with valproate (VPA) was pallor which showed significance with dosage of VPA (P<0.05). Abdominal pain had<0.05 significance with serum level of VPA, and Other side effects were anemia and weight gain. 5) In 83.3% of all patients, they had no side effects or could tolerate mild side effects. But in 12.5% of patients, the side effects disappeared when dosages were reduced, and AED had to be withdrawn in only 4.2% of patients.

4.Intraosseous Ganglion: Report of a case.

Ganglia in soft tissue are common and usually occur in close relations with joints, tendon sheaths, or tendons. However, intra-osseous ganglia are very rare. We report a case of intra-osseous ganglion arising from the right sided proximal humerus in a 33 year-old woman. It was multiple cysts surrounded by thin rims of sclerotic bone in the subchondral epiphysis without other features of degenerative joint disease or destruction of adjacent soft tissue.

5.A Study of beta-glucuronidase and Lactic Dehydrogenace Activities in the Neoplastic Tissue of Stomach Carcinoma.

To observe whether the measurement of enzyme activity has any critical role in assesment of diagnostic value, beta-glucuronidase and lactic dehydrogenase activities in neoplastic tissue of stomach were compared with those in the stomach at the stomach mucosa at the antrum and body which was uninvolved by tumor. And then activities of those enzymes were also compared with differentiated and undifferentiated carcinomas. The stomach tested were obtained by gastrectomy in Pusan National University Hospital during the months from March to June in 1983. The results were summarized as follows: 1) beta-glucuronidase activity in the neoplastic tissue of stomach was higher than that in the antral and body mucosa, and this enzyme activity in the antral mucosa with intestinal metaplasia was higher than that in the body mucosa of stomach. 2) Lactic dehydrogenase activity in the neoplastic tissue of stomach was similar to that in the antral mucosa with intestinal metaplasia, and was higher than that in the body mucosa of stomach. 3) Acitivities of both enzymes in the neoplastic tissue of differentiated carcinoma were slightly higher than that of undifferentiated one, but did not found statistically significant difference. 4) In the neoplastic tissue of the stomach there was no significant correlation between beta-glucuronidase and latic dehydrogenase activities. Above results support that the measurement of gastric juice enzymes is useful in the diagnosis of stomach carcinoma and may be of value in the identification of high-risk groups.

7.Erythropoietin Bioassay in 7 Cases of Hydronephrosis and Renal Cell Carcinoma.

It has been well established that the kidney is the main organ producing erythropoietin (E.S.F.). Erythropoietin activity was reported to be increased in cases of renal tumor. cyst, polycystic disease and hydronephrosis. To study erythropoietic activity in hydronephrosis and renal cell carcinoma, erythropoietin bioassay by DeGowin method were performed in 7cases of hydronephrosis and renal cell carcinoma. Erythropoietin activity is significantly increased in renal cell carcinoma but not significantly increasedin hydronephrosis.

8.A Study on Total Lactic Dehydrogenase (LD) Activity and Its Isoenzymes in Neoplastic Tissue of Stomach Carcinoma.

This sutdy was performed to find out a LD isoenzyme pattern in neoplastic tissue of gastric carcinoma and in gastric mucosa with intestinal metaplasia, which has been known as premalignant lesion. The specimens used in this study were 18 stomachs with caarcinoma and 8 stomachs with peptic ulcer as control, and wer obtained by subtotal or total gastrectomy at Pusan National University Hsopital from Oct. 1987 to Sept. 1988. Total LD activity and its isoenzymes in body and antral mucosae and neoplastic tissue of stomach were assayed, and analysed statistically from several points of view. The results obtained were summarized as follows: 1) There was no significant difference in total LD activity between univolved body and antral mucosae of stomach with carcinoma, and between mucosa of stomach with peptic ulcer and univolved mucosa of stomach with carcinoma. 2) Total LD activity in the neoplastic tissue of gastric carcinoma was significantly higher than those of univolved body and antral mucosae of stomach with carcinoam or that of stomach with peptic ulcer, but no significant difference was observed among histological types of gastric carcinoma. 3) The uninvolved body mucosa of stomach with carcinoma or peptic ulcer showed a pattern of LD1 predominance and increas of H to M ratio, while the antral mucosa of stomach with carcinoma or peptic ulcer showed a pattern of LD3 predominance and decrease of H to M ratio. 4) The LD4 and LD5 fractions in antral mucosa with moderate to severe intestinal metaplasia were slightly increased than those of antral mucosa with nil to mild intestinal metaplasia, but did not show statistically significant difference. 5) Marked increase of LD5 fraction was found in neoplastic tissue of gastric carcinoma, and thus ratio of H to M subunit in neoplastic tissue was significantly lower, as compared with body or antral univolved mucosa of stomach. In conclusion, the neoplastic tissue of gastric carcinoma and gastric mucosa with intestinal metaplasia show substantially predominant "M" subunit as well as increased total LD activity.
9.Microsurgical Treatment of Distal Anterior Cerebral Artery Aneurysm.

Between Jan. 1983 and Dec. 1990, 17 Patients with distal anterior cerebral artery(DACA) aneurysms were admitted to our institute and underwent microsurgical neck clipping of their aneurysms. This group comprised 3.7% of the 437 aneurysms managed surgically during this period. There were 11 females and 6 males. The mean age was 48.5 years. Most of DACA aneurysms were located at the genu portion of the anterior cerebral artery. Three cases were at the proximal protion of A2 close to the anterior communicating artery. Two cases were at the dista pericallosal artery and distal callosomarginal artery. Eight patients had additional vascular anomalies documented by angiography such as multiple aneurysms, azygos DACA and duplication of DACA. Direct neck clipping was possible in all cases through the interhemispheric approach or the frontotemporal approach according to the location. For the interhemispheric apporach preoperative evaluation of the exact location and direction of the aneurysms on the angiogram was important for operative planning. The surgical outcome was good or excellent without any neurological deficits in 17 of the cases. Operative management, clinical features and incidence of vascular anomalies associated with DACA aneurysms are discussed.

10.Fine needle aspiration cytology of malignant epithelial mesothelioma of the peritoneum.

A case of malignant epithelial mesothelioma of the peritoneum diagnosed by fine needle aspiration cytology is described. The smear showed many individually scattered or clustered large round malignant epithelial cells intermingled with relatively small nonneoplastic mesothelial and mesenchymal cells. Papillary configurations with thick fibrous core were also seen. The malignant cells were virtually reminiscent of reactive mesothelial cells but they were larger in size and had more prominent nucleoli and more frequent binucleated or multinucleated cell formations than reactive mesothelial cells. The characteristic features of malignant cell of mesothelioma compared with the metastatic adenocarcinoma were relatively uniform cellular size, prominent round nucleoli, large round vesicular nuclei with finely granular chromatin pattern, smooth nuclear membrane, abundant glassy cytoplasm rather than bubbly mucin-containing cytoplasm and fuzzy cell border.
