2.Artificial prosthesis of the knee joint combining with homogenous bone transplantation

Here, we report a case of artificial prosthesis of the knee joint combining with homogenous bone transplantation for the treatment of giant cell tumor of the right distal femur. At one year follow up, the patient complains almost no pain nor tenderness at operative site and ambulates with partial weight bearing with crutches. The X-ray finding shows satisfactory bony union with progressive bony remodelling.

3.Primary Malignant Lymphoma of the Gallbladder: A case of report.

Lymphomatous involvement of the gallbladder is rarely identified and manifests mostly as a local involvement of disseminated disease. There have been reported only about 20 primary malignant lymphomas of the gallbladder without involvement of other sites. A 63-year-old woman presented with jaundice, epigastric pain, and weight olss of two months duration. There was no systemic lymphadenopathy elsewhere. Abdominal computed tomograph revealed a focal thickening of the gallbladder wall. The removed gallbladder contained a well circumscribed, 5x3 cm-sized, slightly elevated firm area with accentuated mucosal papillary excrescences near the neck protion. The lesion involved whole thickness of the wall, and was made up of diffuse atypical lymphoid cell infiltrates of variable size accompanying plasmacytoid differentiation in areas. Some large tumor cells were pleomorphic and contained prominent nucleoli. To our knowledge, this is the first case of primary malignant lymphoma of the gallbladder in Korean literature.

6.Induction of IL-12 Experession in Bone Marrow-derived Mouse Dendritic Cells.

Dendritic cells (DCs) are the most potent antigen presenting cells that can activate naive T cells. Mature DCs exress high levels of MHC and costimulatory molecules on their surface and have capacity to produce IL-12, a 75 kDa heterodimeric cytokine composed of p35 and p40 subunit. IL-12 is currently thought to be one of most critical determinants for skewing the immune response towards Th1. Expression of IL-12 in dendritic cells seems to be regulated by various stimuli including CD40L. In the present study we investigated expression of IL-12 in mature DCs, which were cultured from bone marrow cells in the presence of GM-CSF. Maturity of the DCs was confirmed by morphologic characteristics, immunophenotypes, and allostimulatory activities. Exprssion levels of IL-12 p40 in the DCs were measured by semi-quantitative RT-PCR. Increases in IL-12 p40 expression were observed after treatment with lipopolysaccharide (LPS), an anti-MHC class II monoclonal antibody, or an anti-CD40 monoclonal antibody. The most remarkable increases, however, were observed in the DCs treated with an anti-CD40 monoclonal antibody. These results support a previous notion that signals through CD40/CD40L interaction may be important for the production of IL-12 by DCs. Moreover, results of this study show a possibility of using monoclonal antibodies against CD40 molecules for preparing DCs producing high amount of IL-12, which can be used for anti-tumor or anti-viral immunotherapy.

10.Metastatic Adenocarcinoma of Colon in Meningioma: A case report.

Cases of metastases from extracranial tumor to intracranial tumor are very rare. The world wide review of the literatures until 1992 revealed 44 cases of primary intracranial tumors containing metastatic tumors which are unrelated extracranial primary malignant tumors; the intracranial recipient tumor is a meningioma in 35 cases among them. Carcinomas of the lung and the breast are the most common extracranial donor tumors. Metastases from colon cancer to meningioma are extremely rare. A 74 year-old-female presented with headache for 2 weeks. CT revealed a round mass with high signal intensity, measuring 4 cm in diameter, which is located in the left parietal lobe. The patient had colon cancer 2 years ago and lymphoma I year ago. On operation, the tumor is relatively well delineated and attached to the meninx. Microscopically, the tumor is composed of fascicles of long slender, fibroblast-like spindle cells with indistinct cytoplasmic border, variable amount of collagen deposit and many psammoma bodies. A few scattered glands are present in periphery of the meningioma. The tumor glands are composed of columnar cells with basally located hyperchromatic nuclei and similiar to the glands of the adenocarcinoma of the colon.
