PURPOSE: To compare the functional and radiographic results of the treatment in unstable intra-articular fractures of distal radius by closed reduction and external fixation versus open reduction and internal fixation. MATERIALS AND METHODS: Forty cases of unstable intra-articular fractures of distal radius were treated either by application of closed reduction and external fixation (CREF) or by open reduction and internal fixation (ORIF) between March 1989 and June 1997. They were followed up for more than one year. To assess the functional results, we used Green and O' Brien' s score system and for the radiographic results, measured volar tilt, radial inclination and radial length. RESULT: In functional results, excellent to good results were obtained in 19 cases (76%) in the CREF group and 10 cases (80%) in the ORIF group, and the average score was 81.4 and 82.8 by the Green and O' Brien' s score system. In radiographic results, mean loss of volar tilt, radial inclination and radial length were 1.4 degree (13.4%), 2.0 degree (9.0%), 1.3 mm (10.3%) in the CREF group and 1.2 degree (10.8%), 1.6 degree (6.1%) and 1.2 mm (11.5%) in the ORIF group on last follow-up radiographs. There was no evidence of statistical difference between two groups in functional and radiographic results (P>0.05). CONCLUSIONS: If an appropriate operative method is selected according to the presence of several properties, including the pattern of fracture, the general condition and activity of patient and the degree of soft tissue injury, the two operative managements are considered useful to restoring articular congruity and alignment and to allow early postoperative range of motion exercise

Mucocutaneous Lymph Node Syndrome

Nuclear DNA content in 31 cases of thyroid neoplasm was determined by flow cytometry with the use of paraffin-embedded archival tissue. DNA aneuploidy was found in 6 cases (19.4%) of the 31 thyroid neoplasms; such as 2 of 8 (DI=1.16, 1.56) follicular adenomas, 1 of 6 (DI=1.10) follicular carcinomas, 1 of 15 (DI=1.18) papillary carcinomas and 2 of 2 (DI=1.76, 2.07) medullary carcinomas. The remaining tumors were diploid. No significant difference between follicular adenomas and carcinomas was detected with respect to the S phase fraction(SPF). In the papillary carcinoma group the SPF was higher than in the follicular neoplasm group, but it was statistically insignificant. Regional lymph node metastasis was present in 8 of 15( 53.3%) papillary carcinomas but absent in all of the 14 follicular neoplasms. In the medullary carcinoma group one case showed regional node metastasis at the time of resection and the other developed metastasis 11 months after surgical removal of the primary lesion. In this study tumors predominantly composed of Hurthle cells were found to have a significantly higher D.I. than those with few or no Hurthle cells. No significant difference was found between tumors with metastasis and those without metastasis.

A clinical observation was made on 20 patients with testicular torsion admitted at Seoul Red Cross hospital between May l988 and F8bruary 1991 to find out reliable digcriminations on the differential diagnosis and useful informations about the treatment. Clinical features. operative findings and laboratory data were reviewed through the medical records. Of the patients 15(75%) were in age of second decade and 5 (25%) were in age of third decade. Torsion occurred during the cold weather in 14 cases(70% ). The onset of testicular pain was sudden in 14 cases(70%) and previous episodes of testicular pain were present in 10 cases(50%). Torsion occurred on the left side in 17 cases(85%). On physical examination. 13 patients(65%) showed horizontal lie of affected testis. Urinalysis was negative in 18 cases(90%). According to the interval time between the onset of testicular pain and of detorsion, testicular salvage rate was 100% within 6 hours, 89% within 12 hours but 9% in case of over 12 hours. According to the degree of torsion, testicular salvage rate was 50% in case of below 360` and 33% in cases of over 360`. In all 20 cases of testicular torsion, a type of anatomical anomaly was round in torsed testis. that was bell-clapper deformity in 18 cases(90%), long mesorchium in one and ectopic testis in one case. In the contralateral testis, bell-clapper deformity was round in 13 cases(65%). We found the following informations about the testicular torsion. I. It second. there are some reliable features in differential diagnosis, those are age of patients in his second decade. onset in cold weather. more sudden onset of pain, presence of previous episodes, occurrence on the left side, horizontal lie of affected testis and negative urinalysis. 2. Testicular salvage rate after testicular torsion seemed to relate more closely with time between the onset of symptoms and detorsion rather than the degree of torsion. It is advisable to detorse testis at least within 12 hours after the onset of symptoms for testicular salvage. 3. Bell-clapper deformity was found in majority of cases in torsed testis and 65% in the contralateral testis. Therefore. it is reasonable to explore contralateral testis and fix it if needed.

OBJECTIVE: The purpose of this study was to analyze risk factors that are associated with intracranial lesion, and to propose criteria for classification of mild head injury (MHI), and appropriate treatment guidelines. METHODS: The study was based on 898 patients who were admitted to our hospital with Glasgow Coma Scale (GCS) score of 13 to 15 between 2003 and 2007. The patients' initial computerized tomography (CT) findings were reviewed and clinical findings that were associated with intracranial lesions were analyzed. RESULTS: GCS score, loss of consciousness (LOC), age and skull fracture were identified as independent risk factors for intracranial lesions. Based on the data analysed in this study, MHI patients were divided into four subgroups : very low risk MHI patients are those with a GCS score of 15 and without a history of LOC or headache; low risk MHI patients have a GCS score of 15 and with LOC and/or headache; medium risk MHI patients are those with a GCS score of 15 and with a skull fracture, neurological deficits or with one or more of the risk factors; high risk MHI patients are those with a GCS score of 15 with abnormal CT findings and GCS score of 14 and 13. CONCLUSION: A more detailed classification of MHI based on brain CT scan findings and clinical risk factors can potentially improve patient diagnosis. In light of our findings, high risk MHI patients should be admitted and treated in same manner as those with moderate head injury.
