2.Pedal Indirect Lymphangiography.

PURPOSE: Recently, indirect lymphangiography has been developed as a relatively good and noninvasive imaging modality of the lymphatic system at extremities. But the disadvantage of the indirect lymphangiography is a low contrast ratio between the surrounding tissues and the contrast media in lymphatic vessels, because dimeric nonionic contrast media is water soluble and diluted in the proximal leg lymphatic vessels. We could have relatively better image than previously published images for the leg lymphatic system, when we injected contrast media with adequate high pressure in intradermal space of the interdigital areas at the foot dotsum. So, we would like to report the results. MATERIALS AND METHODS: We could study all 9 lymphedemas(primary :6, secondary: 3) from April 1990 to May 1993 on outpatient base. They were diagnosed as lymphedema clinically and radiologically. Ten ml of dimeric nonionic aget, iotrolan(Isovist 300 ) was injected into intradermal space with five 30-gauge needles The injection speed was more than 0.2mi/min. We have done one side pedal lymphangiogram in 30 minutes. The evaluation of the anterior superficial lymphatics was accordig to the criteria of the Weissleder(2). RESULTS: The results were as follows:1. All lymphatic vessels from foot to inguinal area could be visualized. 2. Two or three inferior inguinal lymph nodes could be visualized about 42%. 3. The most common abnormal finding of the lymphedma was the neovascularization of the lymphatics on indirect pedal lymphangiogram. CONCLUSION: If we use adequate technique relatively high pressure injection, correct intradermal needle insertion, adequate soft tissue exposure technique indirect lymphangiography is considered to be a safe and noninvasive imaging modality for the evaluation of the lymphedema of lower extremity lymphatics including inferior inguinal lymph nodes.

3.Job Stress and Work-Related Musculoskeletal Symptoms of 119 Emergency Medical Technicians.

PURPOSE: This study was done to investigate the prevalence of job stress and musculoskeletal symptoms, and to identify the factors that affect work-related musculoskeletal symptoms of the 119 Emergency medical technicians (EMT). METHOD: From August 26 to September 10, 2010, the data were obtained from 456 EMT working in Daejeon city or Choongnam province. For data analysis, descriptive statistics and multiple logistic regression were performed using SAS version 9.1. RESULTS: Physical environment, job demand, interpersonal conflict, and occupational climate were stressful to 119 EMT. The prevalence of musculoskeletal symptoms of female EMT was higher than those of the male EMT's. After gender and age were adjusted, there ware significant relationships between musculoskeletal symptoms and some risk factors including occupational class, hours of intensive musculoskeletal use, previous injury or work-related injury, physical burden, and job stress. Overall, a higher degree of job stress increased musculoskeletal symptoms. CONCLUSION: Job stress is a major cause of musculoskeletal symptoms. To prevent and manage musculoskeletal disease of 119 EMT, there is a need to develop a management program for musculoskeletal symptoms to reduce occupational stress, considering gender differences.

4.Successful hybrid operation of an acute mobile thrombus in the abdominal aorta induced by chemotherapy.

Acute mobile thrombus of the abdominal aorta after chemotherapy is a very unusual finding, which can be a potential source of arterial embolism. We report here on a case of an acute mobile aortic thrombus with renal infarction. We successfully treated the patient with hybrid operation-open surgical and endovascular approach. Our case shows that hybrid treatment using wire-directed balloon catheter thrombectomy is a feasible, minimally-invasive treatment for a mobile aortic thrombus.

5.Differences in Life Transition Process of Parents Caring for Children with Autism: Based on the Socio-Demographic Characteristics.

PURPOSE: This study was done to identify differences in the life transition process of parents caring for children with autism according to parents' socio-demographic characteristics. METHODS: Participants were 194 parents caring for children with autism. Data were collected from December 2013 to February 2014 through self-report questionnaires, and analyzed using descriptive statistics, t-test, ANOVA with SPSS/WIN 20.0. RESULTS: Mean scores for despairing or accepting phases were higher than those for denying or wandering phases in the life transition process. According to parents' gender, educational level, and job, there were some significant differences in the denying and wandering phases. Differences in denying phase by education (p=.033), job (p=.004) were significant. Respondents with higher educational level, and having a job showed a lower level of denying than other respondents. Wandering phase differed significantly by gender (p=.009) and job (p=.001). Mothers and those who did not have a job showed a higher level of wandering than fathers and those who had a job. However, there was no difference in the despairing or accepting phases. CONCLUSION: The life transition phase of parents with autistic children needs to be assessed to provide differentiated and intensive support program and help them move to the accepting phase.

6.The Effects of PolyMem(R) on the Wound Healing.

Little objective information is avilable on the influence of occlusive dressings on the healing of cutaneous partial skin defect wounds. Our purpose was to examine the effects of occlusive dressing by using the synthetic dressing mateial, PolyMem in the management of 2nd degree burn wounds and donor sites of split thicknes skin graft and partial-thickness wounds in rabbits. New Zealand white rabbits, 12 to 14 weeks of age, were divided into 2 groups. Two partial thickness skin wounds measuring approximately 40x30 mm were induced using a scalpel on the back of each anesthetized animal. They were designated as group I (dressing with conventional method, n=15), group II (dressing with PolyMen, n=15). Each treated wound was individually covered with the assigned dressing immediately after wounding. Wound were examined and measured at 10 days to determine the extent of healing. By day 10, the PolyMem dressed wounds were approximately 67% healed, while all vaseline gauze dressed wounds were about 50% healed. Standardized 20 mm full-thickness biopsy wounds were treated for 10 days. Section of PolyMem group at POD 10 days showed complete epidermal regeneration above fibrotic dermis (H&E, x40). Section of conventional group at POD 10 days showed marginal epidermal regeneration (H&E, x40). 72 patients (44 patients with 2nd degree burn and 28 patients with skin graft donor sites) were divided into four groups. They were designated as group I (Burn patients with PolyMem, n=24), group II (Burn patients with conventional methods, n=24), group III (S.T.S.G. patients with PolyMem, n=14), group IV (S.T.S.G. patients with conventional methods, n=14). We investigated wound site pain, healing time, comfort and numbers of dressing change. As compared with the control group, the PolyMem dressed group had less pain, more rapid healing time, more comfort, less frequent dressing changes. From these results, we concluded that the occlusive dressing with PolyMem was an effective alternative to the conventional gauze dressig on the wound healing. Our results suggest tat PolyMem is one of the ideal dressing materials.

7.Health Status for the Elderly Living in a Home for the Aged.

BACKGROUND: The number of the elderly living in a home for the aged is increasing as the socio-culture changes, but health management for the institutionalized elderly has been discontinuos, centered on the single treatment of the disease. The aim of this study is to collect baseline data which is for development of the health maintenance program for the elderly living in a house for the aged. METHODS: We reviewed the journals related to the health maintenance program for the elderly. We selected the contents about health management program for the elderly. 3 domain of the contens were done. The 3 domain were physical health (interview for health habit, physical exam, clinical test), mental health (Korean version of geriatric depression scale, Korean version of mini mental status examination), assessment of functional status (katz activities of daily living, lawtons instrumental activities of living).. 158 elderly people living in a home for the aged in Seoul have been researched since August 1996. RESULT: Total subjects were 158 (men 73, women 85). The average age was 78.1(+/-7.2). The rate of smoking, alcohol drinking, exercise were 32%, 20.4%, 34.4%, each. The rate of assitive device use was 28.3%. The prevalence of sleep distur-bance, urinary incontinence were 69.7%, 17.3%. The prevalence of gait disturbance was 38.4%. The occurrence rate of falling for last 1 year was 29.6%. The prevalence of fear of falling was 57.2%. The positive rate of VDRL was 12.3%, the prevalence of anemia was 48.7%. The prevalence of degenerative arthritis, hypertension were 44.2%, 31.3%, each. The prevalence of depression, dementia were 71.3%, 56.7%, each. The rate of elderly person dependent on one and more ADL was 8.1%. CONCLUSION: We found the health status of the elderly living in a home for the aged. On the basis of the results of the study, it is needed that continuous application of the program for management of the elderly in a home for the aged, keeping searching proper contents of examination and methods of measurement.

8.Factors influencing Health-related Quality of Life in Korean Medicaid Beneficiaries.

PURPOSE: The purpose of this study was to identify the factors which influence health-related quality of life (HRQoL) in Korean Medicaid beneficiaries. The relationships among sociodemographic factors, health status, health behavior, and HRQoL were analyzed METHODS: Data from the 2007 survey on Health Services Use and Health Status of Medicaid Beneficiaries conducted by the Ministry for Health Welfare and Family Affairs were examined. To analyze the sample survey data, descriptive statistics, correlation and hierarchical multiple survey regression analysis with SAS 9.1.3 package were used with SURVEYMEANS and SURVEYREG procedures, which incorporate the sample design into the analyses in order to make statistically valid inference for the whole Medicaid population. RESULTS: The HRQoL correlated with limitations in Activities of Daily Living (ADL) (r=-.509, p<.001), stress (r=-.387, p<.001), depression (r=-.385, p<.001), alcohol consumption (r=.216, p<.001), and exercise (r=.293, p<.001). Significant factors that affect HRQoL of Medicaid beneficiaries were gender, region, limitations in ADL, stress, depression, alcohol consumption, and regular exercise. These variables explained 44.6% of HRQoL (F= 215.00, p<.001). CONCLUSION: The results indicate that to improve the HRQoL of Medicaid beneficiaries it is important to develop nursing intervention programs that focus on psychological health and health behavior and to give consideration to differences in gender and region.

9.Mullerian Adenosarcoma of the Ovary with Sex Cord-Stromal Differentiation: A case report.

Mullerian adenosarcoma is a tumor composed of a mixture of glandular and stromal elements in which the glandular component appear to be neoplastic but, histologically, benign with the stromal component showing varying degrees of malingancy. We report a case of ovarian m llerian adenosarcoma with sex cord stroma differentiation in the stromal components. A 57 year-old female who presented with palpable mass in the right lower abdomen had undergone through salingo-oophorectomy. Grossly, the ovary was multicystic, and partly showed a solid appearance with multiple polypoid projections into the dilated cystic spaces. On microscopic examination, the tumor consisted of benign to borderline epithelial glands that were lined by variety of mullerian epithelia and sarcomatous component with sex cord-stromal elements, which include sertoliform tubules, Leydig cell like clusters, and granulosa cells.
