2.A Study on the Role of Bacteria in the Pathogenesis of Confluent and Reticulated Papillomatosis.

BACKGROUND: The pathogenesis of confluent and reticulated papillomatosis(CRP) is still unknown, although many theories have been suggested. It is stressed that abnormal host response to colonization of pityrosporum orbiculare might play a role in the pathogenesis of CRP, but is not completely understood. Frequently, we have observed bacterial colonies in the stratum corneum and, especially, within the hair follicles from biopsy specimens of patients with CRP. In addition, successful treatment for CRP with minocycline, a derivative of tetracyclines, has been reported continuously. OBJECTIVE: The purpose of this study was to document the possible association of bacterial factors in the pathogenesis of CRP and to elucidate the effectiveness of minocycline. METHODS: We performed Brown and Brenn gram staining in 12 biopsy specimens, bacterial cultures, identification of microorganisms and antibiotics sensitivity testing including minocycline. We administered oral minocycline with an initial dose of 100mg per day for 1 to 3 months and a maintenance dose 50mg per day for I to 2 months, then evaluated the response of treatment. RESULTS: 1. On Brown and Brenn staining, the gram positive bacterial colonies that stained dark bluish or nearly black were observed within the infundibulum of hair follicles in 10 out of 12 biopsy specimens(83.3%) and on the keratotic invagination of stratum corneum in 11 out of 12 biopsy specimens(91.7%). 2. Histological features of regions where bacterial colonies were observed showed, hyperkeratosis and keratotic invagination on the stratum corneum in all cases. Hyperkeratosis(66.7%), parakeratosis(16.7%), inflammatory cell infiltration(25.0%), perifollicular fibrosis(33.3%), and abnormal keratin in sebaceous ducts(50.0%) were shown in hair follicles. 3. In anaerobic cultures, no bacteria was grown. In aerobic cultures, staphylococcus species were identified in 6 cases, streptococcus viridans in one case, sternotrophomonas maltophilia in one case. In the susceptibiliity test, minocycline was sensitive in 7 from 8 cases. 4. The therapeutic response to minocycline was observed within one month in all cases, and the time to clear the lesions was 1/2 month in 2 cases, 1 month in 4 cases, 2 months in 5 cases, 3 months in one case, respectively. CONCLUSION: Form these results, we suggest that bacterial factors may play an important role in the pathogenesis of CRP and that the action mechanism of minocycline in the treatment of CRP may be due to antibacterial effects.

3.A Case of Idiopathic CD4 + T Lymphocytopenia Associated with Kaposi's Sarcoma.

Idiopathic CD4+ T lymphocytopenia (ICL) is a new disease entity characterized by depletion of helper T cells apparently without any evidence of HIV infection. We report a case of ICL associated with Kaposis sarcoma (KS) and pneumocystis carinii pneumonia (PCP) in a 34-year old woman. She developed violaceous, protruding masses on scalp, back, both extremities, palms, soles, left. first toe and peritonsillar region for 2 months. These lesions were confirmed as KS by histopathologic findings. Chest X-ray and HRCT findings represented PCP and KS. Absolute deficiency of CD4+ T cell was detected in the count of T cell subsets. Serologic tests for HIV-1, 2 and HTLV I, -II were negative. And she was absent any defined immunodeficiency or therapy associated with decreased levels of CD4+ T cells. By CDC criteria, a diagnosis of ICL was made. Because of aggravation of PCP and lung involuement of KS, she died at 22nd day after admission.

5.Epidemiological Studies on Geriatric Hypertension in Korea.

A total of 4757 apparently healthy aged persons over 65 years of age was surveyed for geriatric hypertension. A single blood pressure measurement was taken in the sitting position. Mean systolic blood pressure rose significantly with age in female but didn't in male and mean diastolic blood pressure didn't change with age in both sex. The prevalence of pure systolic hypertension, i.e., a systolic blood pressure greater than or equal to 160 mmhg and a diastolic blood pressure less than 95mmhg, increased with age. The prevalence was slighty lower for female than it was for male, being 7.0% for female and 8.0% for male. The prevalence of classical hypertension-defined as diastolic blood pressure greater than or equal to 95mmhg - was 42.7% for female and 48.2% for male and it rose with age in female. The prevalence of both pure systolic and classical hypertension was 49.7% for female and 56.1% for male.

7.OGILVIE`S SYNDROME AS A COMPLICATION AFTER THE BREAST RECONSTRUCTION WITH A TRAM FLAP.

Acute colonic pseudoobstruction was first reported in 1948 by Sir H. Ogilvie. It is characterized by an acute, abrupt right-sided colonic distention in the absence of physical reasons for obstruction. If untreated, it may progress to cecal perforation, peritonitis, and death. Although primary cases have been reported, 87 percent of cases were secondary results of medical or surgical conditions. The most common medical conditions were infections, cardiac disease, and neurologic problems. In surgical conditions, cesarean section has been reported as the most common cause of Ogilvie's syndrome. In 1995, the first case of Ogilvie's syndrome complicated after a cosmetic surgical procedure(abdominoplasty) was reported by Bradley et al. We report a case of Ogilvie's syndrome complicated after breast reconstruction with TRAM flap that developed in the postoperative third day in 35-year old woman. The complications are primarily related to decreased flap perfusion. No reference of intestinal pseudoobstruction was found in the literature conducted in regard to complications of TRAM flap breast reconstruction.

8.A Case of Captopril-induced Pemphigus Foliaceus.

A 65-year-old male received captoPnl therapy (25mg three times a day, for the treatment of congestive heart failure. Seven months after starting the captopril therapy he developed flaccid vesiculobullae and erythematous patches with oozing, crusted surfaces on his trunk and the proximal portion of his extremities. The mucous membranes were not involved. The mucous membranes were not involved.) The histopathologic findings showed subcorneal bulla containing a few acantholytic cells and direct immunofluorescent studies revealed intercellular deposition of IgG and C3 throughout entire epidermis, consistent with pemphigus foliaceus. After captopril therapy was discontinued, the skin lesions gradually improved within 1 month. Pemphigus foliaceus; is a rare complication of captopril therapy and this is, to our knowledge, the first case report in Korea.

9.A Case of Granuloma Pyogenicum-like Cutaneous Metastasis to the Fingertip in Papillary Thyroid Carcinoma.

Cutaneous metastasis of the papillary thyroid carcinoma(PTC) is very rare. We report a case of PTC that metastasized to the left fourth fingertip. Its cutaneous appearance was similar to granuloma pyogenicum. A 65-year-old woman who has PTC with regional lymph nodes involvement and pulmonary metastasis complained of a protruding hemorrhagic subungual mass on the left fourth finger for two months. The histopathologic findings showed papillary projections of the tumor cells surrounding the axial fibrovascular stroma and mitotic cells. The tumor cells showed a positive reaction with antithyroglobulin antibody in immunoperoxidase stain. To the best of our knowledge, this is the first case report of PTC that metastasized to the fingertip, and that showed granuloma pyogenicum-like cutaneous lesion at the metastatic site.

10.Clinical Experience of Lazy-T Operation.

Five Lazy-T procedures for medial ectropion of the lower lid were performed in 4 patients(3 males, 1 female). Of these 5 procedures, one was for involutional ectropion and 4 were for cicatrical ectropion which occurred after lid surgery or were due to facial xerosis. Ages ranged from 55 to 68 years. After the follow-up period of 8 to 29 months, all cases showed marked improvement of tearing, ocular injection, and discharge without any recurrence or overcorrection.
