1.A Case of Arrhythmogenic Right Ventricular Dysplasia.

Arrhythmogenic right ventricular dysplasia is a rare heart muscle disorder of unknown cause that primarily involves the right ventricle. It is characterized pathologically by fibrofatty replacement of the right ventricular myocardium. Clinical manifestations include structural and functional abnormalities of the right ventricle, electrocardiographic depolarization/repolarization changes, and presentation with sudden death or ventricular arrhythmias of right ventricular origin. It is one of the important causes of the ventricular arrhythmia or sudden death among apparently healthy young people. We report a case of arrhythmogenic right ventricular dysplasia with the review of the literature.

2.A Clinical Study of 381 Patients with Childhood Vitiligo.

BACKGROUND: Vitiligo, the pigmentary disorder of the skin and hair, affects patients of various ages from neonates to old adults. According to the onset age, vitiligo is classified as adult or childhood type. Previous clinical studies on childhood vitiligo are rare in Korea. OBJECTIVE: Our purpose was to evaluate the clinical characteristics of childhood vitiligo. METHOD: A clinical investigation was made of 381 outpatients with childhood vitiligo (between 6months and 15years of onset) who had visited the Department of Dermatology, Pusan National University Hospital from January 1988 till December 1997. Results : The results a summarized as follows 1. The subjects consisted of 206(54.1%) females and 175(45.9%) males. 2. The mean age of onset was 7.7 years(males : 8.0, female 7.5). 3. The mean duration of the disease was 2.6 years(male : 2.5, female : 2.7). 4. Familial history was obtained in 30 cases(7.9%) and the most common precipitating factor was trauma(40.0%). 5. Both vulgaris(43.3%) and segmental type(30.4%) were common clinical types in the classification of Ortone et at. Trigeminal distribution(53.4%) was the most common segmental types in Koga's classification. 6. The most common site of initial involvement was the face and neck(52.2%) 7. The most common degree of depigmented lesion was within 5%(84.5%). 8. The progression of vitiligo was present in 68.2% of patients and the incidence of poliosis was 11.5%. 9. Koebner phenomenon was present in 12 cases(3.1%). 10. The most common associated diseases were acne and halo nevus (18.0%). Conclusion : Childhood vitiligo is relatively unique in the clinical findings such as the clinical type(high incidence of segmental type), precipitating factor(trauma is the main factor), depigmented area(small involvement in the extent of the lesion) and the progression of the disease.

3.Development of Health Education Program Components for Early Childhood and the Investigation of Teachers' Awareness and Performance Rate of Health Education.

PURPOSE: This study was conducted to develop health education program components for early childhood and to investigate teachers' awareness of the importance of health education and their performance rate of health education. METHODS: Early childhood health education program components were developed using two rounds of the Delphi method. The Delphi group consisted of 21 experts in childhood education. After health education program components were developed, they were used in surveying child care teachers' awareness of importance and performance rate and 151 teachers sampled from 30 child care centers participated in the survey. RESULTS: The early childhood health education program components consisted of 5 areas, 16 subjects and 33 specific contents. Early childhood teachers' awareness of importance was over 4.5 points in all the areas and in 11 out of the 16 subjects. The most frequently educated subjects were 'the importance of hand washing' and the least frequently educated subject was 'obesity prevention'. The teachers' awareness of importance and their performance rate of specific contents were high in 'maintaining order' and 'using children's rides safely,' and low in 'obesity prevention' and 'infectious disease prevention.' CONCLUSION: The components of this health program were developed in consideration of field feasibility and the relationship of health program education in elementary, middle and high school.

5.The Effectiveness of Catheter-directed Thrombolysis for First Line Deep Vein Thrombosis Therapy.

PURPOSE: Untreated deep vein thrombosis (DVT) is associated with morbidity and mortality, such as pulmonary embolism and post-thrombotic syndrome. Anticoagulation therapy is efficient for reducing thrombus propagation but is insufficient for clot lysis or preventing post thrombotic syndrome. Current catheter-directed thrombolysis is an important DVT treatment. We compared the outcomes between anticoagulation therapy and DVT catheter-directed thrombolysis. The purpose of this study was to evaluate treatment outcomes in patients with symptomatic DVT who had undergone catheter-directed thrombolysis. METHODS: From January 2003 to January 2009, we retrospectively reviewed 91 patients who had been admitted and treated for DVT in our hospital. We divided 91 patients into two groups according to treatment method; 42 patients (46.2%) were treated with only anticoagulation, and 49 patients (53.8%) were treated with catheter-directed thrombolysis. We compared the results of the two treatments groups. The results included gender, symptom onset, age, location, complications, risk factors, days to improved patient symptoms and patency. All patients underwent Doppler sonography or CT-venography at 3 and 6 months to evaluate venous patency after treatment. RESULTS: 49 patients who underwent catheter-directed thrombolysis for DVT had more symptom-relief days (25.00 days, P<0.001). The 43 patients (88%) of the catheter-directed thrombolysis group showed more complete resolution on Doppler sonography or CT-venography after 6 months (P=0.001). We obtained better clinical outcomes in the catheter-directed thrombolysis group than in the anticoagulation only group. CONCLUSION: Catheter-directed thrombolysis is an effective treatment for acute DVT.

6.Comparison of Emotional Labor and Job Stress of Hospital Nursing Staff.

PURPOSE: This study was to investigate the relationship between the level of emotional labor and occupational job stress of hospital nursing staff. METHODS: The survey was conducted from Mar. to Oct. 2009 to collect data from clinical nurses (N=496). RESULTS: The study showed that nurses without religion and those working in big-sized hospitals had higher scores in emotional labor, and those of 26~30 years old felt the highest stress in the occupational role. The same applied to the married, more work experienced, atheist and those working in big-sized hospitals. Unmarried, under educated nurses with less work experience working in the big-sized hospitals showed higher scores in personal strain. The scores of the personal resources depended on religion, work experience, marital status and position of their jobs. Emotional labor was positively correlated with an occupational role and personal strain, respectively, but the emotional labor, occupational role and personal strain were negatively correlated with personal resources respectively. CONCLUSION: Intervention program is required to reduce overload of nurse's role and to relieve physical and psychological strains of the under-30-age-group. Also, social support and rational/cognitive coping must be reinforced.

7.Clinical Electrophysiological Study on Sick Sinus Syndrome.

Nine patients of mean age 47.8 years, with suspected sinus node dysfunction, underwent extensive electrophysiological studies. Sinus bradycardia(6the electrophysiological study, AH and HV intervals were prolonged in 2/9 and 1/9 patients, respectively. Maximal sinus node recovery times were prolonged in 7/9 patients, ranging from 1,330 msec to 12,330msec. Sinoatrial conduction times measured by atrial premature stimulation technique were prolonged in 5/7 patients, ranging from 137msec to 310 msec. And sinoatrial conduction time measured by continuous pacing technique also revealed prolonged value in 4/6 patients ranging from 140 msec to 195 msec. The effective refractory periods, of atrium were prolonged in 6/8 patients (320 msec to 470 msec). The effective and functional refractory periods of AV node were prolonged in 3/8 patients (440 csec to 490 msec) and 4/8 patients (530 msec to 560 msec), respectively. Retrograde VA conduction could be observed in 3/7 patients and ventricular effective refractory periods were normal in 7/7 patients. Atrial flutter was induced in 1/9 patients by electric stimulation during electrophysiological study. Above data suggest that the electrophysiological study is very useful in assessing the sinus node function and other electrophysiological properties in sick sinus syndrome patients and also suggest that the data could be utilized in choosing the proper mode of artificial pacemaker for each patient.

8.Radiologic Evaluation of Intestinal Obstruction in the Neonates.

PURPOSE: The purpose of this study is to evaluate the radiologic findings of the intestinal obstruction in the neonate according to the causes and to determine the findings useful for the differential diagnosis. MATERIALS AND METHODS: The materials consisted of 29 neonates with surgically proven gastrointestinal tract obstruction. We analized simple abdominal radiography and barium study comparing with the operative findings. RESULTS: The causes of intestinal obstruction were gastric atresia in 1 case, duodenal atresia in 3 cases, small bowel atresia in 11 cases(jejunal ;3 cases, ileal ;8 cases), colonic atresia in 2 cases, Hirschsprung's disease in 9 cases, ano-rectal anomaly in 6 cases, midgut volvulus in 2 cases, and Meckel's diverticulum in 1 case. Vomiting was noted in the all cases. The abdominal distension was not noted in the cases of gastric atresia, duodenal atresia, and proximal jejunal atresia. The meconium passage was noted in 2 cases of ileal atresia and 3 cases of Hirschsprung's disease. On barium study, site of obstruction was predicted accurately in gastric atresia, duodenal atresia, proximal jejunal atresia, and colonic atresia but it was not possible in distal jejunal atresia and ileal atresia. The microcolon was noted in 2 cases of jejunal atresia, 4 cases of ileal atresia, and 2 cases of colonic atresia. Out of 9 Hirschsprung's disease transition zones were seen on rectum or rectosigmoid junction in 7 cases and barium study was normal in 2 cases. CONCLUSION: In the diagnosis of neonatal intestinal obstruction, the basic radiologic studies such as simple abdominal radiography and gastrointestinal contrast study was useful for the differential diagnosis of the proxireal bowel loop atresia colonic ateria, and midgut volvulus.

9.New Viewpoint of Surface Anatomy Using the Curved Sectional Planes of a Male Cadaver.

BACKGROUND: The curved sectional planes of the human body can provide a new approach of surface anatomy that the classical horizontal, coronal, and sagittal planes cannot do. The purpose of this study was to verify whether the curved sectional planes contribute to the morphological comprehension of anatomical structures. METHODS: By stacking the sectioned images of a male cadaver, a volume model of the right half body was produced (voxel size 1 mm). The sectioned images with the segmentation data were also used to build another volume model. The volume models were peeled and rotated to be screen captured. The captured images were loaded on user-friendly browsing software that had been made in the laboratory. RESULTS: The browsing software was downloadable from the authors' homepage (anatomy.co.kr). On the software, the volume model was peeled at 1 mm thicknesses and rotated at 30 degrees. Since the volume models were made from the cadaveric images, actual colors of the structures were displayed in high resolution. Thanks to the segmentation data, the structures on the volume model could be automatically annotated. Using the software, the sternocleidomastoid muscle and the internal jugular vein in the neck region, the cubital fossa in the upper limb region, and the femoral triangle in the lower limb region were observed to be described. CONCLUSION: For the students learning various medical procedures, the software presents the needed graphic information of the human body. The curved sectional planes are expected to be a tool for disciplinary convergence of the sectional anatomy and surface anatomy.

10.Prevalence of Urinary Incontinence and Its Related Factors among the Rural Residing Elderlies.

OBJECTIVES: This study was conducted to find out the frequency of urinary incontinence among the rural elderly people and its related factors. METHODS: Informations have been obtained through interviews from the 464 rural residents of advanced age over 65 years on September 1st through November 30th, 2007, in Chungnam Province. RESULTS: As for the rate of experiencing urinary incontinence, the group with the experience rate of 'every day' were 9.5% and 'occasionally' 35.5%, with the total of 45.0%. The higher rates of urinary incontinence were in the elderly women(58.5%) than in the elderly men (29.8%), in the more advanced in age, in the higher educated, and in the groups with higher monthly income. Based on life styles, the rate of experiencing urinary incontinence was significantly higher in smoking groups and non-drinking groups. Based on subjective senses of health, it was more highly associated with the groups who reported that they were not healthy, that they concerned themselves about health, that they had physical disability, that they had forgetfulness, and they needed assistance in terms of activity of daily living(ADL) and instrumental activity of daily living(IADL) than their respective counterparts. By the result of multiple logistic regression, sex, age, smoking status, anxiety, physical disability, amnesia, and IADL was indicated the affecting factors to the prevalence of urinary incontinence. CONCLUSIONS: The above results reveal that the rate of urinary incontinence was higher in the elderly women than the elderly men, and in more advanced age. Moreover, its rate increases in the groups with undesirable life styles or lower senses of subjective and physical health conditions. It is highly suggested that efforts to manage urinary incontinence of the elderly need to be narrowed to the more advanced, especially those with lower standards of health conditions.
