2.Clinical Analysis of the Pattern of Anterior-Posterior Circulation in Patients with Posterior Communicating Artery Aneurysm.

OBJECTIVE: The purpose of study was to assess the status of posterior communicating artery in patient with aneurysms arising from the internal carotid-posterior communicating artery and the angiographic blood flow pattern of anterior-posterior circulation through the posterior communicating artery especially to the posterior cerebral artery. The clinical outcomes according to the pattern of posterior communicating artery and the blood flow to the posterior cerebral artery were analysed. PATIENTS AND METHODS: The data includes 41 cases of internal carotid-posterior communicating artery aneurysm among 248 cases of aneurysms which were operated from September 1993 to August 1998. The clinical informations were obtained by review of the records and radiologic films retrospectively. The statistical analysis were performed using Chi-square tests. RESULTS: The female was predominant and peak age was fifties. The operative result was in close relation with initial Hunt-Hess grades(p=0.039), Fisher grade(p=0.001) but showed no statistical differences with age(p=0.106). The flow pattern of posterior cerebral artery was mainly from both anterior and posterior circulation(28cases, 68%), and the size of posterior communicating artery was smaller than P1 segment of posterior cerebral artery in 28cases(68%) but each of them showed no statistical differences with outcome. CONCLUSIONS: Even though there were no statistical differences of outcome according to the size of posterior communicating artery and the pattern of blood flow to the posterior cerebral artery, it is mandatory to save the posterior communicating artery for the preservation of otherwise normal blood flow to the perforators and main branches of posterior circulation.

4.Crescentic Glomerulonephritis in a Patient with Rheumatoid Arthritis: A case report.

Crescentic glomerulonephritis in rheumatoid arthritis is described recently with increasing frequency. It can occur directly as a manifestation of rheumatoid arthritis or may be a reaction to drugs such as D-penicillamine and bucillamine. We report a case of crescentic glomerulonephritis in a 46-year-old woman with rheumatoid arthritis for 20 years who had been treated intermittently with herb medicine or nonsteroidal anti-inflammatory drugs (NSAIDS). Light microscopic examination showed severe focal segmental and global necrotizing glomerulonephritis with crescent formation in 50% of the glomeruli. Immunofluorescent study revealed scanty amount of mesangial granular deposits of IgA, IgM, C3 and fibrinogen in a diffuse pattern.

5.Effect of a Follow-up Program Involving an Early Intervention and Self-help Group on Parenting Stress, Depression, and Parenting Efficacy for Mothers of Premature Infants

Purpose:
This study is intended to investigate the impact of an early intervention and follow-up program involving self-help groups on maternal parenting stress, depression, and parenting efficacy in families with premature infants.
Methods:
The study included 30 mothers in the experimental group and 29 in the control group, all of whom had premature infants in the neonatal intensive care unit. Changes in dependent variables before and after the followup program were analyzed using paired t-tests with mean and standard deviation, and variables that differed in presurvey scores were analyzed using analysis of covariance with covariates.
Results:
Parenting stress decreased in the experimental group that participated in the follow-up program, while it increased in the control group. Depression decreased by 3.44 points in the intervention group and 1.76 points in the control group. Parenting efficacy increased by 3.03 points in the experimental group and decreased by 1.03 points in the control group after the program.
Conclusion
This study highlights the significance of offering family-centered early interventions within existing hospitals or institutions, rather than relying solely on home visits, to promptly address the early developmental issues of premature infants, share information, and provide emotional support through regular self-help meetings.
6.Pigmented Squamous Cell Carcinoma Arising from Pigmented Actinic Keratosis.

Pigmented squamous cell carcinoma is a very rare malignant, pigmented, epidermal tumor. The rarity of pigmented squamous cell carcinomas may reflect in part their misdiagnosis as other pigmented neoplasms, particularly malignant melanoma. To our knowledge, only five cases have been reported in literature. We recently experienced a case of pigmented squamous cell carcinoma arising from pigmented actinic keratosis in a 77 years old female. Physical examination showed a 0.8 0.6 cm, smooth, dark brown pigmented patch with irregular but sharply defined borders located on the upper left chest. The biopsy specimen showed histologic findings of pigmented actinic keratosis with abundant melanin pigments, which became pigmented squamous cell carcinoma. Most of pigments in the squamous cell carcinoma were contained within the melanocytes along with the neoplastic squamous cells.

7.Sarcomatoid Carcinoma with Heterologous Osteosarcomatous Component of the Prostate: A case report.

Sarcomatoid carcinoma of prostate has been rarely reported and occasionally difficult to distinguish from a true sarcoma or carcinosarcoma. A case of sarcomatoid carcinoma of the prostate, which has been occured in 61-year-old male patient is presented. The tumor consists of carcinomatous areas with epithelioid cells, sarcomatoid areas with spindle cells and foci of heterologous osteosarcoma component. The phenotypic nature of the tumor was confirmed immunohistochemically by positive reaction for cytokeratin, epithelial membrane antigen, vimentin and prostate specific acid phosphatase in both sarcomatous and carcinomatous components.

8.Development of Hospital Nurses' Job Description based on DACUM Method: Focusing on General Ward and Intensive Care Unit.

PURPOSE: This study was done to clarify nurses' duties, tasks and task elements and to develop a job description for clinical nurses on general and intensive care units in tertiary-level hospitals, and dramatically reflect changing medical trends in Korea. METHODS: The job description was developed based on the Developing a Curriculum Method (DACUM). The questionnaire included frequency, importance, and difficulty of duties, tasks and task elements, measured on a 4-point scale. Results were analyzed using SPSS version 21.0. Data were collected from September 4 to 7, 2017, and analyzed using descriptive statistics and a scattered graph. RESULTS: The job description consisted of 10 duties, 38 tasks, and 51 elements. Of the 10 duties, the highest duty in order of importance was ‘Direct nursing care’ followed by ‘Infection control’. The highest duties according to frequency and difficulty were ‘Document and notify’ and ‘Research and quality improvement’. ‘Safety management’ and ‘Infection control’ were considered as relatively simple duties, however, these two duties were still included as important jobs for clinical nurses. CONCLUSION: The job descriptions for clinical nurses developed from this study contain nursing ethics and safety as well as infection control, to faithfully reflect clinical nurses' jobs.

10.Well-Differentiated Thymic Carcinoma, Spindle Cell Type, Arising from Anterior Mediastinum: A case report.

Well differentiated thymic carcinoma(WDTC) was recently separated from cortical thymoma. It is characterized by a predominance of epithelial cells with usually low mitotic rate, an epidermoid differentiation with slight to moderate cytologic atypia and lobular growth pattern. In recent reports, an uncommon spindle cell variant of WDTC, which is composed of spindle shaped epithelial cells, has been described. We investigated an unusual case of WDTC consisted of purely spindle shaped epithelial cells in a 66-year-old female. Radiologically, the well demarcated mass was located in the anterior mediastinum with focal invasion into the surrounding left upper lung. The tumor, 10 x 8 x 5 cm, was encapsulated with thin fibrous tissue and showed a pale yellow solid and lobulated cut surface. Microscopically, it consisted of solid sheets of purely spindle shaped epithelial cells with mild atypism, a low mitotic rate and focal epidennoid differentiation.
