1.Susceptibility of cockroaches (Blattella germanica Linneaus) to various insecticides.

During the period from May to October 1969, the authors carried out a series of tests with the aim to ascertain the present status of susceptibility of adult cockraoches to various insecticides. The tests on the susceptibility of cockroaches to dieldrin, D.D.V.P., Diazinon; Dibrom, Malathion and lindane were performed with the topical application and the residual film methods under the conditions of 26 degrees C and 80% of relative humidity. The insects tested were collected from Seoul City and were reared at the laboratory. Some of the important results so far obtained can be summarized as follows: The median lethal doses (LD(50)) of the five insecticides against female adult cockroaches were: 0.4 ug/roach or 4.02 ug/body weight (gm) by Dieldrin, 0.29/roach or 2.88 ug/body weight (gm) by D.D.V.P., 0.29 ug/roach or 2.96 ug/body weight (gm) by Diazinon, 0.54 ug/roach or 5.37 ug/body weight (gm) by Dibrom, and 2.96 ug/roach or 29.60 ug/body weight (gm) by Malathion. And those against male adult cockroaches were: 0.18 ug/roach or 3.29 ug/body weight (gm) by Dieldrin, 0.27 ug/roach or 4.89 ug/body weight (gm) by D.D.V.P., 0.16 ug/roach or 2.91 ug/body weight (gm) by Diazinon, 0.49 ug/roach or 8.97 ug/body weight (gm) by Dibrom, and 1.32 ug/roach or 24.19 ug/body weight (gm) by Malathion. The level of susceptibility of adult cockroaches to the insecticides tested was 1.0-1.6 times higher as compared with the results obtained on the Japanese Takashi strain. The median knockdown times (LT(50)) of the female adult cockroaches by insectcides at 1% concentration of 3 mg per square centimetre were 1.82 hours by Malathion, 1.36 hours by Diazinon, 0.56 hours by Lindane, 0.50 hours by Dibrom, and 2.56 hours by D.D.V.P. A marked difference was seen to exist between the males and the females. The female cockroaches were more resistant than the male to Dieldrin, Diazinon and Malathion.

2.Correlation of Virologic Property and Phylogenetic Analysis of Hantaan Viruses Isolated from Patients and Reservoirs in Korea.

Twenty isolates of Hantavirus were isolated from patients and reserovirs from 1988 to 1994 in Korea. Isolation rate was 1.9% (10/538) in patients, 6.2% (5/81) in Apodemus sp., 2.6% (1/38) in Rattus sp. and 0.6% (4/677) in bats. Reciprocal mean IFA titers ranged from 27.5 to 1,024 at the specimen collection. According to the growth rate and reaching peak titer of infectivity, the isolates were grouped as rapid, intermediate, and slow growing groups. All isolates were confirmed as Hantaan type by the nested RT-PCR on the Gl region of the M segment. Comparison of nucleotide sequence (Nt: 2101 - Nt: 2280) of the G2 region revealed that the sequence homology between Hantaan 76/118 virus and the isolates was more than 90%. Several nucleotide positions of the isolates showed high variation. The variation rate of patientisolates was about one-half when compared with that of rodentisolates. On the basis of phylogenetic analysis Hantaan viruses isolated were divided into two genogroups. These results indicate that Hantaan virus is highly dominant serotype in Korea and the virologic property and genogroup are not correlated.

4.A Case of Reconstruction of Orbital Floor Fracture by Grafting Parietal Bone.

An orbital floor fracture is caused by a sudden increase in the intraorbital pressure, resulting from the application of a traumatic force to the soft tissues of the orbit. The fracture is complicated by diplopia or enophthalmos. Orbital floor fractures are caused by the automobile accident, human fist or the ball. Our hospital performed reconstruction of an orbital floor fracture by grafting parietal bone on the fractured orbital floor area of the patient who visited our hospital 4 months after the hit on his left eye by a fist. We now report this case with the review of various aspects of an orbital floor fracture.

5.Normal values of Positive wave in the Multifocal Electroretinography in Korean.

PURPOSE: To obtain normal values of positive wave (b-wave) in the multifocal ERG in terms of age and sex of Koreans. METHODS: The multifocal ERGs by using RETIscan visual evoked response imaging system were tested for 80 eyes. 61 retinal locations were stimulated concurrently. We analyzed the average responses of 5 concentric rings. RESULTS: In the amplitudes of b-wave, ring 1, 84.1 5.4 nV/deg, was the largest of all the other rings and ring 5, 23.1+/-1.5 nV/deg, was the smallest of all the other rings (P<0.05). No statistically significant difference was observed in amplitude between sexs. The implicit time of b-wave was the longest in the ring 1, 40.8 1.6 ms, and shortest in the ring 4, 36.7+/-0.9 ms (P<0.05). CONCLUSIONS: The amplitude of b-wave was the largest in the fovea, and became smaller with eccentricity. The largest amplitude of b-wave was observed in the age of twenties. The amplitude of b-wave was decreased with age in the ring 3, 4, and 5. The interindividual variation of the amplitude of b-wave was greatest in the fovea. The implicit time of b-wave in the fovea was the longest, and that in the ring 4 was the shortest. It became longer from the ring 5, again.

6.Use of Polymyxin B Hemoperfusion in a Patient with Septic Shock and Septic Cardiomyopathy Who Was Placed on Extracorporeal Membrane Oxygen Support.

Although shock in sepsis is usually managed successfully by conventional medical treatment, a subset of cases do not respond and may require salvage therapies such as veno-arterial extracorporeal membrane oxygenation (VA ECMO) support as well as an attempt to remove endotoxins. However, there are limited reports of attempts to remove endotoxins in patients with septic shock on VA ECMO support. We recently experienced a case of septic shock with severe myocardial injury whose hemodynamic improvement was unsatisfactory despite extracorporeal membrane oxygenation (ECMO) support. Since the cause of sepsis was acute pyelonephritis and blood cultures grew gram-negative bacilli, we additionally applied polymyxin B direct hemoperfusion (PMX-DHP) to the ECMO circuit and were able to successfully taper off vasopressors and wean off ECMO support. To the best of our knowledge, this is the first adult case in which PMX-DHP in addition to ECMO support was successfully utilized in a patient with septic shock. This case indicates that additional PMX-DHP therapy may be beneficial and technically feasible in patients with septic shock with severe myocardial injury refractory to ECMO support.

7.A Change of Adrenal Androgen and Cortisol in Kawasaki Disease.

PURPOSE: Endocrine and immune systems are connected and interdependent. Adrenal glands play an important role in this network and control the balance between serum levels of dehydroepiandrosterone sulfate(DHEAS) and cortisol. These steroids have an antagonistic effect on the T cell progression into Th1 and Th2 cells and on the induction of correlated interleukins. Therefore we evaluated the role of adrenal androgen and cortisol as immune modulators in Kawasaki disease(KD) with changes of T cell immunity. METHODS: From April to August in 2001, we examined serum DHEAS and 24 hour urine free cortisol(F) before administration of immunoglobulin and steroids by radioimmunoassay in 14 KD patients. It's clinical severity was determined by Harada score and coronary lesion. RESULTS:The age of the patient group ranged from 4 months to 4 years; its average age was 2.3 years. Three patients(21.4%) were below 1 year, 2(14.3%) between 1 and 2 years, 5(35.7%) between 2 and 3 years, 4(28.6%) between 3 and 4 years of age. Male to female ratio was 1:1.3. DHEAS was significantly decreased in patients(11.1+/-6.0 ng/dL) more than controls(81.6+/-13.3 ng/dL)(P<0.05). Twenty-four hour urine free cortisol was significantly increased in patients(36.9+/-21.9 ng/dL) more than controls(13.6+/-5.5 ng/dL)(P<0.05). Ratio of DHEAS/F was decreased remarkably in patients(0.33+/-0.20) more than controls(6.65+/-2.56)(P=0.016). There was no difference between ratio of DHEAS/F and Harada score, but its ratio was very low in patients with coronary aneurysm. CONCLUSION: These data demonstrate that there are changes of DHEAS and cortisol in acute stage of KD and the dis-equilibrium between two steroids may be relevant in the T cell immune response induction of Kawasaki disease. These changes support the use of DHEAS/F ratio as one of the predictive factors of coronary arteries complication.

8.Two rare cases of Diphyllobothrium latum parvum type infection in Korea.

Two rare cases of human infection with parvum (dwarf) type of Diphyllobothrium latum [syn. D parvum (Stephens,1908)], were discovered in Korea. The first case was a 46-year old houseife, from whom a kind of pseudophyllidean tapeworm eggs was detected in the feces. She was treated with praziquantel and purged, and a complete strobila with scolex, 120 cm in total length, was recovered. She recalled that she had eaten raw trouts at a raw-fish restaurant near the Chungju Lake. Another patient was a 22-year old medical student (male), who used to eat raw sea-foods. He discovered a chain of tapeworm proglottids, 15 cm in length, discharged spontaneously in his stool and brought it for identification. The worms from the two cases were compatible with D. parvum (Stephens, 1908) of which the taxonomic significance has long been questioned. After a detailed morphological study and review of literature, we designated the worms as D. latum parvum type. This is the first report on the occurrence of this rare type of D. latum infection in Korea.

9.Surgically Induced Astigmatic Change after Laser in Situ Keratomileusis using Manual Microkeratome.

We performed spherical LASIK operations using manual microkeratome and assessed the axis and magnitude of surgically induced astigmatism by vector analysis[Jaffe's rectangular coordinates]. Six month follow-up of 124eyes of 98 patients who were followed for 6 months or longer were included. Myopia ranged from -4.00 to -9.75 diopter. We divided eyes depending on the degree of myopia;group 1, 31 eyes between -4.00 and -5.75 diopter, group 2, 46 eyes between -6.00 and -7 .75 diopter, and group 3, 47 eyes between -8.00 and -9.75 diopter. Also, we compared the axis of surgically induced astigmatism between right eyes[73 eyes]and left eyes[48 eyes]. Preoperative astigmatic refraction was -0.63 +/-0.30 diopter in group 1, -0.67 +/-0.35 diopter in group 2, -0.67 +/-0.41 diopter in group 3.After 2 months, magnitude of surgically induced astigmatism was 0.84 +/-0.57 diopter in group 1, 1.08 +/-0.72 diopter in group 2, 1.19 +/-0.84 diopter in group 3[P=0.15], after 6 months, 1.01 +/-0.59 diopter in group 1, 1.16 +/-0.98 diopter in group 2, and 0.99 +/-0.59 diopter in group 3[P=0.52]. Change in the axis of vectorial-induced astigmatism was not statistically significant between right eyes and left eyes after 2 months[P=0.16], but significant after 6 months[p<0.05 ]. In conclusion, spherical LASIK using manual microkeratome may induce a significant change in magnitude of the astigmatism regardless the degree of myopia. The difference of astigmatic axis was statistically significant between right eyes and left eyes after 6 months. These changes and differences may require long-term follow-up and further investigation.

10.Preferential Suppression of the On Pathway by r-Aminobutyric Acid in the Catfish Retina.

The effects of r-aminobutyric acid(GABA) agonsits and antagonists were explored by the intracellular recording method to discern the preferential suppression of the ON component by GABA on the ON-OFF transient cell in the catfish (Ictalurus punctatus) retina. Experiments were performed in the superfused eyecup preparation. The animals were decapitated and pited before the eye, and the surrounding tissue was removed from the skull. The retina was exposed by excising the cornea, iris, and vitreous. This preparation rested on a wad of Ringer`s soaked cotton in contact with an Ag/Agcl reference electrode. Solutions were delivered through a manifold system that was connected to a pipette located near the absorbent wick. Electro-physiological recordings were made using standard intracellular electrodes filled with 2 M potassium acetate. The electrical signal was recorded with an amplifierand a penwriter, viewed on an oscilloscope, and stored on a data recorder. The light sources were red light-emitting-diode (LED) and the stimuli were full field illumination covering the cntire retina. GABA preferentially reduced ON light responses in ON-OFF transient cell. and GABA hyperpolarized bipolar cells, but the effects on ON bipolar cells were more sensitive than OFF bipolar cells. CACA and TACA, GABAc receptor agonist, did not act on bipolar cells. CACA and TACA, GABAc receptor agonists, hyperpolarized bipolar cells but the sensitivity deferences between ON and OFF bipolar cell were not observed. These results suggest that the preferential suppression of the ON component of the ON-OFF transient cell by GABA was resulted from the presynaptic mechanism that reduced bipolar cell input.
