With polymerase chain reaction and dot-blot hybridization with allele-specific oligonucleoade probes,we analyse DQA1 gene of 42 insulin-dependent diabetes mellitus (IDDM) and 40 normal controls (the Han nationality in Shanghai). We found that the frequency of DQA1 * 0301 and DQA1 * 0501 allele in IDDM patients was greatly increased, while the frequency of DQA1 * 0103 and DQA1 * 0201 allele was significantly decreased as compared with the controls. The result indicates the contribution of the DQA1 * 0301 and DQA1 * 0501 alleles to IDDM susceptibility in Han population in Shanghai. Our results also provide confirmation of the role of DQA1 * 0103 and DQA1 * 0201 to IDDM resistance in the same population.

Objective To explore the test-retest reliability of elevated plus maze (EPM) which is taken as an animal model of state anxiety in Kunming mice.Methods The behavior of adult,male and female,Kunming mice in EPM was recorded twice with one-week inter-trial interval for lasting five minutes.The following parameters were evaluated:percentage of time on EPM open arms (Otime％),percentage of time on EPM closed arms (Ctime％),time on EPM open arms (Otime),time on EPM closed arms (Ctime),percentage of entries into EPM open arms (Oentries％),entries into EPM open arms (Oentries),entries into EPM closed arms (Centries),total entries into EPM open and closed arms (Entries).Subsequently,the intraclass correlation coefficient (ICC) and the kappa agreement index (κ) were calculated for these parameters.Results ICC for Otime％ (♂ ICC=0.506,P＜0.05;♀ ICC=0.456,P＜0.05;♂ + ♀ ICC=0.492,P ＜0.01),Oentries (♂ ICC=0.673,P＜0.01;♀ ICC=0.648,P＜0.01;♂ + ♀ ICC=0.665,P＜0.01),Centries (♂ ICC=0.704,P＜0.01;♀ ICC=0.661,P＜0.01;♂ + ♀ ICC=0.686,P＜0.01),Entries (5 ICC=0.701,P＜0.01;♀ ICC=0.661,P＜0.01;5 + ♀ ICC=0.685,P＜0.01) showed a fair test-retest reliability,respectively.Only κ for Entries (♂ κ=0.253,P＜0.05;♀ κ=0.339,P＜0.05;♂ + ♀ κ=0.298,P＜0.01)displayed a significant but poor reliability between the test-retest trials.Conclusion The Otime％ Entries parameters as an acceptable index for Kunming mice in EPM can be used for exploratory and locomotion activity.Sex and repeated measurement have different effects on the Otime％ and Entries parameters,especially the more on the former.Therefore,Otime％ and Entries can be considered when EPM as an animal model of state anxiety in Kunming mice;meanwhile,more attention should be paid to both the sex (male animals recommended to EPM,especially) and repeated measurement effects.

Objective To explore the value of CT diagnosis in broadening of right upper mediastinum.Methods X-ray plain film and CT manifestations of broadening of right upper mediastinum in 56 cases proved by surgery and pathology or CT enhancement or heart ultrasound were analyzed. All cases were undergone radiography and CT scan. The contrast-enhanced CT scans were carried out in 51 cases.Results The study showed:un-invasive thymoma in 6 cases,invasive thymoma resulted in superior vena cava syndrome in 2 cases,intrathoracic thyroid tumors in 2 cases,ascending aorta or innominate artery dilatation in 14 cases,thoracic aortic aneurysm in 3 cases,pulmonary artery stenosis resulted in aortopulmonary dilatation in 1 cases,right sided aorta in 2 cases,lymphoma in 4 cases,the right mediastinal lung carcinoma in 16 cases,larvate focus of lung carcinoma in 2 cases,low esophageal carcinoma resulted in upper esophagectasia in 2 cases,foreign body of esophageal secondary upper mediastinal abscess in 2 cases. The cases proved by plain and contrast-enhanced CT scan,operation,pathology and heart ultrasound were 33,10,12 and 1 cases respectively. Conclusion CT scan is a effective method in the diagnosis of broadening of right upper mediastinum.

Objective To investigate the leukoaraiosis(LA)influencing factors of sleep disorder patients. Meth-ods 90 cases of LA patients which were conformed MRI diagnostic criteria were chosen as LA group,90 cases of normal old people which the gender and age were matched with LA group were chosen as NC group;by asking the medical history to inquirying blood vessel hazards;pittsburgh sleep quality index(PSQI)was adopted to evaluate the condition of sleep disorders,according to the performance MRI of skull to part LA group as four grades. Results In LA group,hypertension and diabetes incidence were significantly higher than that in group NC(P < 0. 05);LA group,PSQI score was significantly higher than that of NC group(P < 0. 01). LA group with the LA increasing grade,PSQI score increased(P < 0. 05). Regression analysis showed that age,hypertension,diabetes and MoCA were associated with LA in patients with sleep disorders. Conclusion Patients with LA have a sleep disorder, which is closely related with the degree of cognitive impairment,LA lesions,age,hypertension,diabetes etc.

Abstact: Biqiu is equivalent to allergic rhinitis in modern medicine. Ancient and modern literature has shown that acupuncture has a good clinical efficacy on this disease. However, there is no uniform specification in the choice of acupuncture methods (such as acupuncture, moxibustion, warm acupuncture and so on) and the selection of acupoints. According to the etiology and pathogenesis of allergic rhinitis and its clinical manifestations, Biqiu can be classified as“fluid retention disease”in TCM. This article tried to provide a new idea for acupuncture treatment of Biqiu from the perspective of the treatment of“fluid retention disease”in TCM.

Objective To investigate the correlation between Presenilin 1 gene methylation (PSEN1) and sporadic Alzheimer's disease (SAD).Methods Massarray method was used to perform an analysis of DNA methylation in the promoters of PSEN1 gene in lymphocytes from 35 SAD patients,33 mild cognitive impaired (MCI)patients and 22 age-and gender-matched controls.Reporter gene plasmid of PSEN1 was built by genetic recombination methods,and then transiently transfected into the SH-SY5Y and HeLa cells.After that,cells were treated with several agents including serum deprivation,Aβ25-35,5-aza-2'-deoxycytidine (5-aza Cdr) and S-adenosyl-Lmethionine (SAM).A dual-luciferase reporter assay system was used to calculate the relative luciferase activity (RLA).Results Some CpG sites of PSEN1 (Cytosines at-173,95 and +76) showed hypomethylation patterns in SAD cases as compared with MCI and control group (P＜0.01).A dual-luciferase reporter assay showing significant differences in PSEN1 transcription activities were found in both cell lines under SAM treatment [SY5Y,SAM:(6.40±0.81),control:(9.13±1.67),P=0.000; Hela,SAM:(2.06±0.34),control:(2.99±0.59),P=0.000].Conclusions DNA hypomethylation of PSEN1 gene promoter is significantly associated with SAD susceptibility.PSEN1 may change the gene transcription level by methylation,further affecting the activity of amyloid protein-like γ-secretase,leading to the onset of AD.

The diarrheal rat model was established by orally administering senna. The preventive experiment was concurrent for 6 days. The treatment experiment modeling had lasted for 12 days. The administration started at the 7th day, and lasted for 6 days. During the experiment, efforts were made in symptom score and weighing. After the experiment, hearts, livers, spleens, kidneys, brains, adrenals and thymuses were collected and weighed, lactic dehydrogenase (LDH) and cardiac troponin-I (cTn-I) in serum were detected. The efficacy of aqueousextracts from Aconiti Lateralis Radix Praeparata in preventing and treating rats with diarrheal and its accompanying toxicity were respectively studied. The result shows that aqueous extracts from Aconiti Lateralis Radix Praeparata could improve syndromes of rats with diarrheal. The 50% effective doses (ED50) of preventive and treatment administrations were 1.420 4 g · kg(-1) and 1.048 9 g · kg(-1), respectively. Aqueous extracts from Aconiti Lateralis Radix Praeparata could decrease the ratio of heart to body weight, and increase serum LDH and cTn-I. It was concluded that aqueous extracts from Aconiti Lateralis Radix Praeparata had a specific preventive and treatment effect on rats with diarrheal caused by senna, but with specific toxicity on heart.

OBJECTIVETo evaluate the clinical effects of short-segment transpedicular fixation and vertebroplasty via paraspinal intermuscular approach in treating thoracolumbar fractures.

METHODSFrom January 2009 to January 2012,18 patients with thoracolumbar fractures without neurological symptoms were treated with short-segment transpedicular fixation and vertebroplasty via paraspinal intermuscular approach. There were 11 males and 7 females, aged from 52 to 76 years old with an average of 62.2 years. The duration from injuries to surgery ranged from 8 h to 7 d with an average of 4.2 d. According to the Denis fracture classification, 12 cases got compression fractures and 6 cases got burst fractures.

LOCATION6 vertebra with T12, 9 with L1, 6 with L2, and 3 with L3. Anterior vertebral body height, the sagittal Cobb angle, the sagittal index (SI), condition of internal fixation failure and recurrent kyphosis were observed.

RESULTSAll patients were followed up for 12-28 months with an average of 16.5 months. Operation time was from 80 to 130 min with a mean of 95 min and bleeding volume during operation ranged from 100 to 180 ml with a mean of 145 ml. Anterior vertebral body height ratios preoperation, 3 days after operation and final follow-up was 54.3 +/- 2.8, 90.9 +/- 1.5, 88.6 +/- 1.7, respectively; sagittal Cobb angle was (27.8 +/- 2.5) degrees, (5.3 +/- 0.8) degrees, (6.3 +/- 1.4) degrees, respectively; sagittal index was 52.3 +/- 3.8, 89.2 +/- 5.2, 86.4 +/- 4.5, respectively. Data obtained 3 days after operation obviously improved than preoperation, and there was no statistically significant difference between 3 days after operation and last follow-up. No internal fixation failure, neurological complications and recurrent kyphosis were found.

CONCLUSIONTreatment of thoracolumbar fractures with short-segment transpediclar screw fixation and vertebroplasty via paraspinal intermuscular approach can retain the posterior ligament complex and restore the mechanical strength of the anterocentral column,which proved an ideal method for preventing the failure of internal fixation and reduction of post-traumatic segmental kyphosis.
