3.Immediate Hypersinsitivity Reaction in Chronic Urticaria.

In a large proportion of patients with chronic urticaria, at present, the definite tiology can't be found. There have been occasional attempts to identify the aIIergic causes uaing allergic skin tests, only ta suggest the Candicla albicans and. some food yeasts ae probabIe causes. Thia atady was designed to investgate how often patients with chronic urticaria from Chonnam Province react positively to skin tests with extracts of 42 common. allergene in the standard group Al-13 produced by Bencard Ltd. in England. The authora alao surveyed the clinical characteristics of 63 patients with chronic urtimria of more than 3 months duration, who bad visited the skin clinic of Chosun Univeresity HcepitaI from May. 1977 throug 3. Apr. 1979. The results were summarized as follows; 1. Among 63 patients, the maIe to female ratio was 1: l.2, and the peak incidence was in the 20s and 30s. 2, In regard to the duration of chroic urticaria, the highst frequency waa below one year duration. 3. Among 60s of the pa,tients who experienced seasonal changes, 32.4% had suffered aggravation during the winter. 4. The most common diseases combined with chronic urticaria were allergic rhinitis and atopic dermatitis, occuring in 9.5% of all subjects. 5. Of thase examined, 69.8% showed positive reactions to the prick test with the peak incidence in the 20s and 30s. 6. Among 42 allergens used far the prick test, 28 allergens turned out to be possibly casal agents, the rnost frequent group of allergens involved were grain du sts (27.1%), animal hairs(21.8%) and moulds(16.5%) in that order of frequency and occupying over 65% of the total positive reactions. And the most frequently involved specific allergens were hay dust, mixed grain dust straw dust and tree in that order of frequency. Moreover, 90 of the involved allergens were inhalants. 7. In teen-agers, the reactions to grain dusts, animal ha.irs, rnoulds and pollens were the rnost frequent while reactions to food allergens were most frequent in the 20s, and cotton and house dust are most frequently involved in tbe 40s. 8. Multiple sensitivity was not:d in a majority of cases and the most frequent combination was 2 to 5 allergens. 9. The rnost common abnormal hematologic finding in chronic urticaria was eosinophilia.

4.Ramsay Hunt Syndrome.

A case of Ramsay Hunt syndrome, though more often seen at the ENT clinic because of the patients' concern with symptoms in the ear, was seen and treated at the Dermatologic Clinic of Chosun University. A 29 year old male patient presented earache, pinhead to rice sized erythematous vesicular eruption on the external auditory canal, auricle and superior auricular area, and facial paralysis, all involving the right side of the face, duration of which were 7 days, 5 days and 1 day respectiveIy. High doses of prednisolone alone made it possible to remit the otalgia, control the vesicular eruption and resolute the facial paralysis almost completely within 18 days in the order of their appearance, indicating that this is one of the more easily treated diseases for dermatologists, The 22 cases of this syndrome reported in Korean literature were reviewed and the conceptual development of its pathogenesis and definition was described.

5.A Case of Generalized Normolipemic Plane Xanthoma.

We report a case of generalized plane xanthoma which showed no-rmolipoproteinemia and was not associated with unerlying disorders or other type of xanthomas. A 48-year-old female has noticed well defined, slightly elevated, yellowish plaques which occurred on the periorbital areas, sides of the neck, scapular areas, anterior chest, buttocks and lower extremities for 3 years. Biopsy specimen taken from the neck showed perivascular accumulations of foam cells in the upper and mid dermis. Paper electrophoresis didn't, show elevation of any lipoprotein.

6.Analysis of factors affecting the unbalanced distribution of physicians’ working regions in South Korea

Background:
Many countries need help with the problem of an unbalanced distribution of physicians and hospitals. Moreover, various policies and strategies have been proposed to solve this problem. This study aimed to examine the determinants of the unbalanced distribution of physicians.
Methods:
From the 2020 Korean Physician Survey data, 4,181 physicians practicing in Korea were limited to the subjects of the study. We analyzed the factors influencing physicians’ choice of practice location and their willingness to change their practice location from an urban to a rural area.
Results:
The region of physicians’ hometown, medical school, and residency training hospitals determined their choice of practice location. The type of affiliated healthcare organization and the location of physicians’ hometown, medical school, and residency training hospitals affected their willingness to change their practice location from an urban to a rural area. Furthermore, the concordance rate of the regions of physicians’ hometown, medical school, and residency training hospitals with the region of their practice location was 24.9%.
Conclusion
In South Korea, policies for doctors have been designed without considering why they are reluctant to work in rural hospitals. To have a balanced distribution of physicians and hospitals, it is necessary to accurately analyze the status of medical resources based on regions and identify the current and future medical demand. The social situation, such as future demographic change and regional extinction, must also be fully considered. Furthermore, policies should be implemented that encourage physicians to work in rural hospitals.
7.Correction of Foot Drop Utilizing the Posterior Tibialis

A clinical review was performed on 64 cases of the foot drop treated by transferof the Posterior Tibialis tendon to the dorsum of the foot at Taegu Presbyterian Hospital during the period from January, 1965 to December, 1971. The follow-up result of 42 cases were obtained from 13 weeks to 4 years and 8 months. The result are summarized as follows. 1. Among 63 patients, there were 46 males and 17 females, ranging 12 to 57 years old of age. 2. The cause of foot drop were peroneal nerve palsy due to leprosy in 61 cases, anterior poliomyelitis in 2 cases, and traumatic peroneal palsy in 1 case. 3. Of 42, cases which follow-up study was done, the degree of active dorsiflexion of the ankle were 90 degree or more in 29 cases (69.0%) and range of active movement was 25 degree or more in 31 cases (73.8%). 22 cases (52.4%) had active dorsiflexion of the ankle which was 90 or more in degree and range of active movement of the ankle which was 25 degree or more. 4. Iterosseous route was used in 26 cases and circumtibial route in 38 cases. Their end results revealed slight better in the latter method. 5. The major causes of complication were detachment at insertion site in 7 cases (53.8% of total failure). weak motor power of the Posterior Tibialis in 4 cases, and adhesions in 2 cases.

8.MR Patterns of Bone Marrow of Calvarium and Vertebral Body in Normal Subjects; Pattern Analysis According to Age Distribution.

PURPOSE: The purpose of this study is to illustrate MR patterns of bone marrow of calvarium and vertebral body in normal subjects according the age distribution and to understand the course of the fatty replacement from red marrow. METHODS AND MATERIAL: We retrospectively evaluated MR examinations of the calvaria(n=71), cervical spine(n=71), thoracic spine(n=65), Imbar spine(n =68) in subjects without bone marrow abnormality whose age ranged 3 weeks to 74 years. Three distinctive patterns were categorized on Tl-weighted images of the skull. In pattern 1, uniformly low signal intensity with or without very small areas of high intensity in frontal and occipital bones is noted. In pattern 2, frontal and occipital bones have uniformly high signal intensity, and patchy area of high intensity appears in parietal bone. In pattern 3, the entire skull has uniformly high signal intensity. In the spine, four patterns were categorized on Tl-weighted MR images. In pattern 1, the vertebral body has uniformly low signal intensity except for linear areas of high intensity superior and inferior to basivertebral vein. In pattern 2, bandlike and triangular areas of high signal intensity are found in the periphery. Pattern 3 and 4 have diffusely distributed areas of high signal intensity; pattern 3 consist of numerous indistinct dots measuring a few millimeter or less, and pattern 4 consist of fairly well marginated areas ranging in size from 5 to 1.5cm. RESULT:In the calvaria, 73% of pattern 1 were younger than 20 years, pattern 2 were evenly distributed, and 86% of pattern 3 were older than 40 years. In the spine, 87% of pattern 1 were younger than 40 years, 72% of pattern 3 were in 40 to 50 years, and 87% of pattern 4 were older than 50 years. Pattern 2 were evenly distributed in the cervical and thoracic spine, but in the thoracic spine 62% were younger than 30 years. CONCLUSION:It is concluded that younger age group shows mainly pattern 1, whereas elderly group has pattern 3 or 4 in the calvarial and vertebral body marrow. This suggests that conversion to fatty marrow begin locally and progress diffusely with age.

9.Doctors’ consciousness on CCTV installation in operating rooms: A survey

Background:
A bill for the mandatory installation of closed-circuit television (CCTV) in operating rooms, still likely in breach of the constitution, was approved. When a bill infringing on individuals’ fundamental rights is drafted, alternative means of minimizing the infringement of the offender’s rights should be considered ahead of the draft. To this end, alternatives on the bill identified through the consciousness of the offender would be most realistic and much more effective. Thus, this study examined doctors’ consciousness on the mandatory installation of CCTV in operating rooms, the appropriateness of punishment for members who commit immoral and unethical behaviors, and doctors’ alternative ideas to CCTV installation in operating rooms.
Methods:
The online survey was conducted for a week from July 9, 2021, to July 16, 2021, by the Korean Medical Association Doctors News, and 2,345 doctor members responded to the investigation.
Results:
According to the survey, the following alternatives to CCTV installation in operating rooms were proposed: strengthening punishment for performing ghost surgery (38.3%), placing cameras at the entrance of the operating room (21.8%), mandatory written consent (pledge) (13.7%) to prevent ghost surgery for medical staff participating in the surgery, promoting self-purification (whistle-blowing) (11.5%), and a biometrics function for entering operating rooms (8.8%).
Conclusion
The revised medical law delegated legislative devices to subordinate statutes for minimizing infringement. Thus, new regulations should be set to reduce infringement of fundamental rights. It is hoped that doctors’ consciousness on new law could be preliminary data to regulate new rules in discussing lower statutes.
10.Doctors’ perception on the infringement of basic rights and the collapse of essential medical services following CCTV installation in the operating rooms

Background:
On September 25, 2023, the law requiring the mandatory installation and operation of closed circuit television (CCTV) in the operating room went into effect. In this study, doctors’ perceptions of the law were re-examined on September 23, 2023 (just prior to the law’s enforcement), following a survey conducted on July 21, 2021 (before the re-examination of the bill). This study aimed to confirm doctors’ perceptions of the infringement of fundamental rights of this law, the collapse of essential medical services due to the avoidance of surgery, and other concerns and priority solutions ahead of the law’s enforcement.
Methods:
The survey was conducted from 8 to 18 September, 2023, by the Korean Medical Association Doctor Survey; a total of 1,267 doctors responded to the survey.
Results:
Out of 1,267 respondents, 1,156 (91.2%) said “yes,” and 111 (8.8%) said “no” to issue concerning constitutional violations of fundamental rights–such as the freedom of medical personnel to practice their profession–and moral rights. A total of 1,149 (90.7%) respondents agreed with the concern regarding the collapse of essential medical care due to the avoidance of surgeons.
Conclusion
Sufficient guidance should be provided, along with guidelines that provide explicit standards for the installation and operation of the CCTV systems, along with safety management measures. Medical disputes and the heavy legal responsibility of medical personnel are the main reasons as to why essential medical care collapses, given the avoidance of surgery. Medical disputes should be avoided in order to prevent essential medical care collapse, as the video is used as evidence for criminal sanctions.
