2.Comparison of Propofol-N2O and Propofol-fentanyl Anesthesia for Outpatient Surgery.

BACKGROUND: The choice of anesthetic agents and adjuvants during outpatient surgery is of critical importance. Propofol is widely used for the induction and maintenance of outpatient anesthesia. Because propofol lacks analgesic properties, very high concentrations may be required when propofol is used as the sole anesthetic drug. Propofol is used with adjuvants such as nitrous oxide or opioid. This study was designed to evaluate the intraoperative hemodynamic response, recovery characteristics and side effects of propofol-fentanyl anesthesia compared with propofol-N2O anesthesia for outpatient surgery. METHODS: Twenty six healthy and unpremedicated patients scheduled for outpatient surgery were randomly allocated to receive either propofol-N2O anesthesia (N-group) or propofol-fentanyl anesthsia (F-group). The patients in N-group were ventilated with nitrous oxide 60~70% in oxygen and the patients in F-group were ventilated with oxygen 40% in nitrogen via laryngeal mask airway (LMA). RESULTS: There was no significant difference in blood pressure during anesthesia, recovery time and side effects between two group. There was significant decrease of heart rate in F-group. CONCLUSION: We concluded that nitrous oxide and fentanyl are reasonable adjuvants of propofol anesthesia in outpatient anesthesia.

3.The Effects of Combined High-Frequency Jet Ventilation and Conventional Mechanical Ventilation on Cardiovascular and Respiartory System with Pulmonary Edema Induced by Oleic Acid in Dogs.

BACKGROUND: High-frequency jet ventilaion is considered a reliable technique for anesthesia and critical care including respiratory failure but there are adverse reactions such as carbon dioxide retension and dry of respiratory mucosa. The purpose of this study was to confirm the effects of combined high- frequency jet ventilation (HFJV) and converntional mechanical ventilation (CMV) on the cardiovascular system, arterial blood gases tension and mean airway pressure in 9 Korea mongrel dogs with pulmonary edema induced by oleic acid. METHODS: During CMV with 20 breaths/minute, 10 ml/kg of tidal volume and F1O2 1.0, parameers were evaluated (base line value). When pulmonary edema was developed, HFJV was applied initially with 120 breaths/minute, inspiratory time 30% and driving pressure 40 psi F1O2 1.0 for 60 minutes (control value) and thereafter simultaneous use of CMV was applied with the tidal volume of 10 ml/kg and each respiratory rate 8, 4, 2, 1, 0.5 per minute for 30 minutes. RESULTS: Combined application of HFJV and CMV (above repiratory rate 1 per minute) achieved the improvement of oxygenation and carbon dioxide elimination, and Paw was decreased without undesirable effects on cardiovascular system in case of the induced pulmonary edema. CONCUSIONS: From above results we recommanded that HFJV combined with CMV may be a useful method of treatment for respiratory failure.

Trisomy 9p syndrome was first described by Rethore et al in 1970 and about 100 cases have been reported since. The phenotypic spectrum of this syndrome is characterized by craniofacial malformation, facial deformity, skeletal and dermatoglyphic anomalies with variable degrees of mental retardation. We experienced a case of partial trisomy 9 syndrome in a 15-month-old female who had multiple congenital anomalies of frontal bossing, oblique antimongoloid palpebral fissures, enophthalmos, hypertelorism, globular prominent nose, down-turned mouth, prominent low-set ears, simian creases of both hands, clinodactyly and single crease of 5th finger, congenital dislocation of both knees and mental retardation. In cytogenetic studies using G banding technique and fluorescent in situ hybridization(FISH), she presented with an extra derivative chromosome No. 9. The karyotype of the patient was confirmed as 47,XX,+der (9),t (6:9) (q27;q21.2) mat. We report the case with the review of the associated literatures.

Pulmonary edema is a recognized comphcation of acute airway obstruction. When pulmonary edema occurs, it usually follows relief of obstruction and is likely to be of noncardiogenic origin. We present a case of noncanhogenic pulmonary edema that occured in a ncy woman who was transfered to our hospital, because of endotracheal intubation failure and unrelieved bronchospasm, during general anesthesia for Cesarian section.

6.Evaluation of Puretone Threshold Using Periodic Health Examination Data on Noise-exposed Workers in Korea.

OBJECTIVES: This study was carried out to evaluate hearing impairment judgement and to investigate the differences in various diagnostic criteria for noise-induced hearing loss (NIHL) among workers who required for close observation (C). METHODS: Out of 731,029 workers who had taken the specific periodic health examination in 1994, we used the audiometric data on 37,999 workers (C) eliminating the employees who had previous otologic problems. Many investigators have being using different criteria for the evaluation of hearing impairment. In this study, we used the criteria of early (1989-1994), current, compensation for NIHL in Korea, 2-, 3-, 4-divided classification and hearing loss at 4,000 Hz and compared the evaluation results. RESULTS: The prevalences of C and workers who had occupational disease (D1) diagnosed for NIHL were 11.1 % and 0.44 %. There were significant difference in the prevalences of C and D1 depending on different province of Korea. Pure tone averages (PTAs) were not appropriately applied in their evaluation. 97% of workers whom we studied on were below the level of mild hearing loss judged by ISO standard. However, there were wide variations in the prevalence rate of mild hearing loss by diagnostic criteria. Thus, there were different judgements in determining the degree of NIHL depending on which diagnostic criteria were utilized. PTAs were found 20.54 (Rt) and 20.74 (Lt) when the method of 3-divided classification was applied for audiometric data. The degree of hearing impairment of the left ear was more severe than that of right ear. The prevalence of normal hearing threshold below 20 dB was 75.4% and the range of difference in both ear was below 10 dB. Right sided hearing threshold levels were 21.08 dB (500 Hz), 18.44 dB (1,000 Hz), 22.09 (2,000 Hz) and 52.36 dB (4,000 Hz). There was typical high frequency loss (C5-dip at 4,000 Hz) above 30 - 40 dB in normal hearing level. The increasing trend in hearing threshold level was gradually decreased by the increase of PTAs. The difference between PTAs and threshold at 4,000 Hz was about 10 dB. CONCLUSIONS: We could found that PTAs in the previous examination were not appropriately evaluated. This study revealed that they did not use unique criteria for managing the workers of NIHL. For the prevention of NIHL, it was found that the quality control on diagnosis and comprehensive management program were required, especially for those of hearing loss (C).

Contact dermatitis (CD) is a common skin disease caused by various allergens that can be identified by patch test (PT). Despite the analysis of PT outcomes in various regions of Korea, no specific investigation has targeted the Busan area. We aimed to analyze PT results using Chemotechnique in patients suspected of CD in Busan.
Methods:
We reviewed medical records of 273 patients who underwent PT between 2019 and 2020 at two university hospitals in Busan. PT results were interpreted according to the International Contact Dermatitis Research Group criteria.
Results:
Among the patients, 86.4% exhibited positive reactions to at least one allergen. Those with positive outcomes displayed a higher comorbidity rate of chronic urticaria but a lower rate of atopic dermatitis. Moreover, they reported higher positivity rates to questions about sexual life and treatment on the Dermatology Life Quality Index questionnaire. Major allergens in Busan were cobalt chloride (60.1%), nickel sulfate (52.0%), and potassium dichromate (50.9%), followed by thiomersal, balsam of Peru, and fragrance mix (17.2%, 15.4%, and 13.9%, respectively). Captan, budesonide, and colophony were more prevalent allergens in males. While the positive rate for thiomersal were higher in the younger age group, balsam of Peru allergen was higher in the older age group.
Conclusion
This study represents the first PT analysis conducted in Busan, revealing metal-related allergen as the most common cause of CD, with variations observed across sex and age. Further research is needed to validate these findings and elucidate the sources and pathways of allergen exposure in Busan.
