1.A Study on Sickness and the Status of Medical Care in a Rural Area.

This survey was made to determine the overall health situation on (1) the status of sickness; (2) the medical care utilization; (3) the medical cost in Mi-Kum Myun, Nam Yang Ju Gun, Kyung_Gi Do. The survey with questionnaire was carried out with 2,840 peoples in 560 households from August 9th to 16th, 1979. The findings from the survey were as follows; 1. Annul morbidity rate of the prolonged ill cases was 97.2 per 1,000 population (male 94.7, female 99.6). The highest age specific morbidity rate was 274.5 of the 45-to 64-year group and the lowest was 21.9 of the 5-to 14-year group. 2. Annul morbidity rate of the new patients was 777.5 per 1,000 population(male 644.5, female 909.5). 3. The chief complaints distribution of the prolonged ill cases was: local pain 36.6%, indigestion 22.4%, and coughing 7.3%, respectively. In terms of age and sex distribution, a large number of female of the 45-to 64-year group complained of local pain or general pain a large number of both sexes of the 25-to 44-year group complained of indigestion. 4. The major diseases of the new patients which classified with International Classification of Disease (I.C.D) were disease of the respiratory system, disease of the digestive system, and disease of the musculo-skeletal system and connective tissue for male, disease of the respiratory system, disease of the digestive system, and accident, poisoning, violence for female. 5. Total ill days of the 92 new patients were 536 days and average ill days per were 6+/-38.3 days. 6. The rate of receiving treatment in the prolonged ill case was 82.2%(medical facilities 46.4%, drug stores 27.5%, herb medicine 8.3%). 7. The rate of receiving treatment by first choice of the new patients was 88.0% (drug stores 57.%, medicine facilities 28.2%, and herb medicine 2.2%), and the rate of receiving treatment by second choice was 30.9% of first treatment cases (medicine facilities 44.0%, drug store 44.0% and herb medicine 12.0%). 8. Annul hospitalization rate per 1,000 population was (male 12.0, female 11.9). 9. The locations of medical facilities utilized by out-patients were: in the prolonged ill cases Seoul or other places 66.4%, Nam Yang Ju Gun 33.6%, in cases of the new patients Seoul or other places 35.1% and Nam Yang Ju Gun 64.9% respectively. 10. The satisfaction rate of the new patients by mode of receiving treatment was: in cases of primary utilization by first choice herb medicine 100.0%, medical facilities 88.5%, and drug stores 69.8%, in cases of secondary utilization medical facilities 100.0%, herb medicine 100.0%, and drug stores 72.7% respectively. 11. The medical cost per utilized facilities was as follows; in average medical fee per case out-patient 8,947 won, in-patient 266,000 won, drug stores 1,532 won, and herb medicine 15,607 won, in average medical fee per day out-patient 4,829 won, in patient 14,178 won, drug stores 891 won, and herb medicine 4,906 won respectively. 12. The sources of the hospital changes paid out were: there own expense 50.0%, debt 35.3%, and security of medical care 14.7% respectively.

3.Acute myocardial infarction in young patient probably due to Kawasaki disease.

Patient with Kawasaki disease who presented with acute myocardial infarction in young age Although the clinical course of Kawasaki disease is self limiting, coronary aneurysm or myocardial disorders develop in about 20% of the patients. In recent years there have been reports of ischemic heart disease as sequelae of Kawasaki disease in adults, especially young adults. In Korea, there was only one case report in 1995. We report a young woman who presented as acute myocardial infarction with coronary artery aneurysms which highly suggest the underlying Kawasaki disease. The acute myocardial infarction in this patient seems to be the late cardiologic sequelae of Kawasaki disease, which has been silent clinically until the presentation as an acute myocardial infarction. New noninvasive diagnostic tests are being tried to improve the detection of coronary artery complication. However, coronary angiography is still widely used to determine the presence of coronary artery lesions and access the feasibility of coronary intervention. Another advantage of coronary angiography is that, it is possible to analyze the shape and the size of aneurysm and predict the prognosis. In this case we performed the percutaneous transluminal coronary angioplasty(PTCA) instead of surgery. Further study comparing surgery with percutaneous coronary angioplasty is needed.

4.Stem Cell ; New Paradigm in the Era of Genomic Medicine.

Human stem cells have been isolated from embryos as well as diverse somatic tissues. Much efforts have been made to apply those stem cells for therapeutic purpose. Molecular dissection of "stemness" and differentiation process will provide us a clue to further understanding of stem cell biology. The fate of stem cell is determined in part by internal regulation mediated by transcriptional control. A set of genes related to specific functions might be expressed and suppressed upon external or internal signaling. Therefore, it is necessary to identify the complete gene expression profile that defines the stem cell. Moreover, we should verify the potential of therapeutic stem cells regarding differentiation, specific function, tumorigenecity, and toxicity rigorously before the clinical application. At present we have only limited information on "stemness" and differentiation into specific lineages. In the genome era, we can draw a molecular portrait on these functional and morphologic changes along the differentiation, trans-differentiation as well as de-differentiation using the DNA chip technology. We need to accumulate database for gene expression at various steps of stem cell differentiation, which will enhance our knowledge for research on and medical application of stem cell.

5.A Survey of Sexual Knowledge, Attitude of adolescents: Junior High School 1 Grades Students in Chun-Nam Area.

The purpose of the survey was to identify the degree and the relationship of sexual knowledge, sexual attitude. This information will provide useful data for a more systematic, desirable, and practical sex education. The data was collected from 187 first grade schoolchildren(95 boys and 92 girls) of two Junior high school in Chun-nam area. The data was gathered by questionnaire from Oct. 1 to 15, 1996. The questionnaire was modified by authors using the questionnaire developed by You-Jeong, Han(1996). Data was analyzed by using the statistical computer package, SAS to manipulate the data long with percentages, means, standard deviation, t-test, chi2-test, GLM, and Pearson correlation coeffiency. The results in this study were summarized as follows : 1. Sex related knowledge. - The mean score of sex related knowledges showed significant differences between boys and girls(boys ; 22.3, girls ; 21.0, p<0.025), and in the area of pregnancy & delivery(boy ; 9.4, girls ; 7.9, P<0.000) showed significant difference between boys and girls, but in the area of physical differences of the others sex(boys ; 12.9, girls ; 13.1, p<0.551) showed no significant difference between boys and girls. 2. Sex related attitudes. - The mean score of sex related attitudes showed significant difference between boys and girls(boys ; 56.7, girls : 60.5, p<0.002) and in the area of psychological difference the other sex(boys : 28.9, girls : 32.5, p<0.000), but in the areas of form of friendship with the other sex(boys : 11.5, girls : 11.2, p<0.335), sexual delinquency & its prevention(boys : 16.3, girls : 16.9, p<0.171) showed no significant difference between boys and girls. 3. The relationships between sex related knowledges and sex related attitudes. - Those who had higher sex related knowledges showed more positive attitudes towards sex, and showed no statistical significance(r=0.08, p<0.286). 4. Differences of the sex related knowledges according to general characteristics. - The students had disease experience, health interest and more school performance had higher knowledge scores about sex. 5. Differences of the sex related attitudes according to general characteristics. - Those who were in high school performance had higher attitude scores about sex as well.. From the result of the study, it is recommended to develop systematic sex education through formal school education according to age.

6.A Case of Darier's Disease.

Darier's diseaae is relatively rare dyskeratotic and inheritable dermatoses of an. autosomal irregular dominant gene. The authors observed one case of typical Dariers disease with the family hietory of three generatian of autoaomal daminant trait. 22 year-oid patient we observed has had the generalized pruritus and dark dirty and warty patehes distribnted almost on the whole skin surface especially on trunk, back and face for about 10 years. Histopathologically, the lesion showed lacunae, corps ronds and grains which are compatible with typical Dariers disease.

7.Comparison of specimen adequacy in two cervical sampling METHODS: Cytobrush and Cervex-Brush.

BACKGROUND: The quality of specimen for Papanicolaou smear depends on the sampling methods of uterine cervix. This study was designed to test specimen adequacy and the detection of disease using t,wo cervical cytological sampling methods. pastic spatula plus Cytobrish and Cervex-Brush. METHODS: Research subjects were t.he exarninee for screening for cevical cancer in Farnily Practice Center and Health Promotion Center of University Hospital. Research sub jects were classified to two groups randomly. 5 physicians used each device at random. A group 80 test smples(plast,ic spatula plus Cytobrush) were compared with B group test samples(Cervex Brush) for the presence rate of endocervial cells and specimen adequacy. The laborat.ory was blind to the sampling met,hods. RESULTS: The plastic spatula plus Cytobrush and Cervex-Brush were comparable in cap- turing in endocervical cells. But the capturing rates of both methods were high(over 70%). Both the pastic spatula plus Cytobrush and Cervex-Brush produced adequate samples, and the adequacy scores between two methods were not different. significantly. CONCLUSIONS: Use of the plastic spatula puls Cytobrush and Cervex-Brush produced adequate smear for interpretation.

8.Characterization of Principal Component Cell of DMBA induced Rat Malignant Fibrous Histiocytoma With Cell Culture and Cloning.

This experiment was performed to elucidate the cytologic origin of chemically induced MFH in Wistar rats. The tumor was produced by injections of DMBA(9,10-dimethyl-1,2-benzanthracene). With the produced MFH, cell culture and cloning were performed, followed by establishment of a cell strain, which was investigated by immunohistochemical and electron microscopic studies. The results were as follows. A) By immunohistochemistry of the tumor tissue, fibroblastic cells were positive for MEP-1(specific antibody for fibroblastlike cell of MFH, Takeya, 1993) and Anti-hPH(beta)(Anti-prolyl 4-hydroxylase beta), but negative for TRPM-3 and F4/80. Histiocytelike cells were positive for TRPM-3 and F4/80, but negative for MEP-1 and Anti-hPH(beta). In immunoelectron microscopy, normal spleen macrophage showed linear reactivity in cell membrane for TRPM-3, whereas histiocytelike cells of the tumor disclosed negative reaction. B) At 5 weeks of the primary tumor cell culture, the cells exhibited typical storiform pattern of MFH. C) The established cell strain revealed immunoreactivity for MEP-1 and Anti-hPH(beta), but negative for TRPM-3. The cloned tumor cells showed morphologic characteristics of undifferentiated fibroblastic cell. Latex particle (0.80 micrometer size) phagocytosis was negative in the cloned cell strain. The results of the current study support the concept that principal component cells of MFH is of fibroblastic cell origin.

9.A Case of Verrucous Hemangioma.

Verrucous hemangioma is congenital hemangioma, which is structural varients of capillary or cavernous hemangioma. And it is vascular malformation, in which reactive epidermal acanthosis, papillomatosis, hyperkeratosis develope secondarily. Though foreign anthors have reported some cases of this disease, it is rare hemangioma and it never has been reported in Korea. This 13 year-old, healthy female patient has erythematous patchs on her left knee at birth. It grew become coin to egg sized. Because of verrcous iuflammatory reaction of irregular verrucous surfaced plague, the lesion become necrotic thick crusty plague with severe offensive odor and patient complained of severe pain & motion limitation on her left knee. Urinalysis, C B C, chest PA were within normal limit. PPD test was negative. Lt. knee AP & lat. revealed. Marked destroyed soft tissue on anterior portion of Lt. knee and no bony pathological changes were demonstrable. Histopathologica.11y, hyperkeratosis, parakeratosis, acanthosis, papillomatosis in the epidermis and numerous capillary lumina, capillary dilatation, proliferation of endothelial cells and mild infiltration of inflammatory cells in the Dermis. And fibrosis in the Dermis & Subcutaneous tissue are seen.
