3.Rapid Detection of Rifampin Resistant Mycobacterium tuberculosis Using the Line Probe Assay.

BACKGROUND: Tuberculosis continues to be a major threat to health throughout the world, with an estimated 8 to 10 million new cases and 3 million deaths annually. And control of the disease is further threatened by the emergence of drug resistance. Recent major advances have been made in unravelling the molecular basis of M. tuberculosis resistance to isoniazid, streptomycin, quinolones and rifampin. And rifampin resistance is the useful indicator for the occurance of the multi-drug resistance. Hence, the rapid detection of rifampin resistant strain of M. tuberculosis is the key to have successful anti-tuberculosis therapy. Here we present our experience using PCR and line probe assay (INNO-LiPA) for easy and rapid detection of rifampin resistance of M. tuberculosis. METHODS: Thirty rifampin resistant and twenty susceptible strains of M. tuberculosis were collected from the routine culture and analyzed with INNO-LiPA. And results were compared with conventional antibiotic susceptibility testing. After amplification of the region of the RNA polymerase(rpoB), the amplified product is hybridized with a set of 10 oligonucleotides immobilized onto a membrane strip. From the pattern obtained the presence or not of rifampin resistance M. tuberculosis can be assessed. RESULTS: Ninety three percent of patients who had rifampin resistant strain revealed the multidrug resistance while only two showed resistance to rifampin only. The INNO-LiPA test results were generally agreeable with that of the conventional susceptibility testing(90%). The mutations in codon 531 (absence of 55 probe) were most commonly observed. In 55.2% of the 31 rifampin resistance M. tuberculosis confirmed on mutation by R-probes on the INNO-LiPA strips. CONCLUSIONS: The line probe assay after polymerase chain reaction is a fast and convenient method to detect both presence of M. tuberculosis complex strains and its resistance to rifampin in clinical specimens. We have suggested that detection of rifampin resistance may play a key role in monitoring multi-drug resistance. Consequently, the INNO-LiPA test may constitute an important tool for the control of tuberculosis.

4.A Study of Relation Between Number of Daily Food Intake and Nutritional Balance.

During the period of May 10 to June 6, 1987, we have conducted the survey about the number of the food daily taken and the in take of nutrition of the 83 pregnant women who have registered at Sudong Myun and Mikum-myun, Yangchu-gun which are demonstration are of the public health and medicine of the College of Medicine, Ewha Women's University, and of the 100 pregnant women who have been to the University Hospital attached to the above School. The result of the survey is as follows: 1) Among the above pregnant women, primigravida constituted 91 women and multigravida 92 women. As for the month of pregnancy, those who have been pregnant for ten months numbered 43 women (23.5%), which was the highest number. As for age, those who were 25 to 29 years old constituted 63.9%, which was the highest in percentage. 2) As for the level of education, those who graduated from high school constituted 49.2%, which was the highest in percentage. As for occupation, those who were entirely in charge of household affairs as housewives were 89.6%, which comprised most of the jobs held by the above women. 3) As for the composition of a family in environmental food and nutrition, one generation was 68.3%, and two member family was 42.6% which was the highest. As for those taking care of cooking and serving meals, housewives constituted 95.1%. 4) As for the economic situation of dietary life, those who were in the highest level (A level), were 47.0% in an urban area and 41.0% in rural area, which were the highest in percentage. 5) The food which the above people selected after being pregnant was fruit not only in an urban area, but also in a rural area. And in terms of the percentage of fruit, and urban area and a rural area showed 44.0% and 54.2% respectively. As for milk, and urban area and a rural area showed 28.0% and 8.4% respectively, which disclosed that the rate of a rural area was much lower than that of an urban area. 6) As for the number of food intake, 10-14 was 54.0% in an urban area, which was the highest. And 5-9 was 73.5% in a rural area, which was the highest. 7) As for the composition of food taken according to the number of food taken, meat showed the lowest rate in 5-9 group. (P<0.05) As the number of food taken increased, the rate of food taken(Points) increased (2.4). Milk and milk products showed al low rate in 5-9 group and 15-19 group (P<0.05). Fruit intake showed a similar rate in all groups. As for vegetables, the rate of intake was high, as the number of food taken increased. Cereals showed a similar rate in all troups. And the points of intake was high. As for fat, hte rate of intake was high, as the number of food taken increased. 8) In 5-9 group in the number of food intake, energy was 79.9% of RDA, protein was 64.9%, fat was 90.0%, carbohydrate was 81.6%, iron was 83.0%, calcium was 71.1%, vitamin A was 94.3%, vitamin B1 was 77.8%, vitamin B2 was 79.2%, and vitamin C was 88.2%, so that it showed the lowest sufficiency rate of RDA. 15-19 group maintained the balance in energy and all nutrients nearly. In 20-24 group, fat was 142.4%, vitamin B2 was 105.6% and vitamin C was 102.2%, so that it exceeded RDA.

5.The Effects of Cancer Prevention and Early Detection Education on Cancer-related Knowledge, Attitudes, and Preventive Health Behavior of Middle-aged Women in Korea.

The purpose of this study was to identify the effects of cancer prevention and early detection education on cancer-related knowledge, attitudes, and preventive health behavior of middle-aged women in Korea. The research design was a nonequivalent control group pretest-posttest design. The subjects of this study were 38 middle-aged women from a church in Taegu. An Experimental group of 19 and a control group of 19 women were studied. The study was conducted from September 21, 2000 to October 27, 2000. The cancer prevention and early detection education had been provided to the experimental group for 2 weeks. The contents of the education program for the third most prevalent cancer of Korean women were: 'the risk factors of cancer', 'the early symptoms of cancer', 'the diagnostic test for cancer detection', and 'the cancer prevention methods'. The instruments used for this study were modified, cancer-related knowledge, and attitude, preventive health behavior tools of Suh et al.(1998). Data were analyzed using descriptive statistics, X2-test, t-test, ANCOVA with SPSS WIN 9.0/PC. The results were as follows: 1) Hypothesis 1 that the women who get cancer prevention and early detection education will have higher scores of the cancer-related knowledge than the women do not get cancer prevention and early detection education was accepted(F=4.732, p=.037). 2) Hypothesis 2 that the women who get cancer prevention and early detection education will have higher scores of cancer-related attitudes than the women do not get cancer prevention and early detection education was rejected(F=.118, p=.733). 3) Hypothesis 3 that the women who get cancer prevention and early detection education will have higher scores of cancer-related preventive health behavior than the women who do not get cancer prevention and early detection education was rejected(F=2.250, p=.143). On the basis of the above findings, the following recommendations are suggested: 1)It is necessary to identify the variables affected on cancer-related knowledge, attitudes and preventive health behavior. 2) It is necessary to develop a well organized cancer prevention and early detection education program to change cancer-related attitude and preventive health behavior.

6.A Case of Dermatomyositis Manifestated by the Paroxysmal Supraventricular Tachycardia.

Dermatomyositis is a disease of unknown etiology characterized by inflammation and degeneration of skeletal muscles and cutaneous abnormalities. Cardiac involvement in dermatomyositis-polymyositis is thought to be rare. In recent year, however, there has been an increasing number of reports on cardiac abnormalities in adult dermatomyositis and polymyositis due to development of noninvasive diagnostic techniques. Categorically, these abnormalities have included electrocardiographic changes, cardiac arrhythmias, congestive heart failure, coronary artery disease, and pericarditis. A 56-year-old woman was admittied to the Ewha womans University Hospital with dyspnea and palpitation. She was diagnosed as having dermatomyositis and followed up our department of Dermatology. Electrocardiogram showed a paroxysmal supraventricular tachycardia at as rate of 195/min. The patient was treated with 240mg/day verapamil p.o, 60mg/day prednisone p.o, there was marked improvement of symptoms. Systematic study of cardiac function utilizing echocardiography, Holter monitoring, thallium-201-scan, and gated blood pool studies were conducted in five newly diagnosed patients with dematomyositis-polymyositis. A significant elevation of serum CPK-MB is indicative of cardiac involvement. Cardiac involvement is a serious prognostic sign. We report a case with the review of the literature.
