1.Photochemotherapy for Psoriasis.

The beneficial effects of sunlight and ultraviolet light for the treatment, of psoriasis vulgaris has long been recognized. Parrish et al were the first to introduce a controlled light source, which allowed the delivery of a known quantity of light energy to the patients skin after the patient had ingested a photosensitizer, 8-methoxypsoralen (methoxsalen) two hours previous to UVA exposure. This successful treatment has become known as PUVA (psoralen and UVA). PUVA photochernotherapy denotes a systemic approach that is based on the interaction of light and a photoactive drug. Parrish et al have described successful treatment of psoriasis with PUVA and other investigators alI over the world have confirmed their beneficial results. The rationale of photochemotherapy in psoriasis is the inhibition of the increased DNA synthesis within the psoriatic lesions by the interaction of psoralen molecule and light energy in the UVA range(320-400nm). The present study was undertaken to evaluate the therapeutic effectiveness of PUVA photochernotherapy combining orally administered methoxsalen and UAA exposure using Dermatron UVA phototherapy system in patients with wi5espread psoriasis. A total of 15 patients with psoriasis over more than 30% of their bodies were selected at the department of dermatology, National Medical Center through March 1978 to September 1978. Oral administration of methoxsalen, 20 to 50 mg doses according to body weight, was followed by exposure to a high-intensity longwave ultraviolet light emitting a continuous spectrum between 320 and 400nm (peak emission, 350 - 365nm). Initial UVA exposure times were based on each patients minimal phototoxicity dose (MPD) and exposure times were increased at each treatment by 1 - 2 minutes to maintain minimal erythema. Frequency of treatment was two to three times weekly. Once the psoriatic lesions had, cleared completely the patient was placed on a maintenance schedule. If psoriasis recurred the frequency of treatment was increased. The results were as follows: l. 5 patients were dropped out due to unknown reasons. 2. 7 cases showed clearing of over 95, of the lesions (Response 5'). Mean number of PU VA therapy was 16. 7. During 1 4 months follow-up period, 4 cases remained free of psoriasis but 3 cases couldn't be followed. 3. Immediate side effects of PUVA were temporary and generally mild. No significant changes in laboratory screenings were noted. Side effects of therapy included transient nausea(33.3%), dizziness(26.7%), erythema.(66.7%), pruritus (60%), increased pigrnentation(100 %). 4. Comparison of pre-and post-PUVA therapy in biopsy specimens by light microscopy revealed histologic improvement evidenced by diminished acanthosis and parakeratosis, and regeneration of granular layers in all of 8 cases. Glycogen accumulation in the affected epiderrnis was significantly decreased or absent after PUVA therapy in all cases.

2.Study on the Musculoskeletal Subjective Symptoms Disorders and Health Promotion Behavior of Daily Plant Construction Workers in One Area.

PURPOSE: This research is a correlative study to assess the relationships of musculoskeletal subjective symptoms disorders and health promotion behavior of the daily plant construction workers. METHODS: The subjects were comprised of 306 daily plant construction workers by self-reported questionnaires on October 19, 2011. The instruments used for this study were musculoskeletal subjective symptoms disorders (NIOSH) and health promotion lifestyle profile II (HPLP-II). The data were analyzed using SPSS/WIN 14.0. RESULTS: The scores of health promotion behavior of daily plant construction workers were 2.13, and the scores of health promotion behavior were significantly different depending on gender (t=-2.51, p=.013), religion (t=3.10, p=.002), owning house (t=2.59, p= .010), exercise (t=6.01, p<.001), time to rest during work (t=2.21, p=.027), and work satisfaction (F=8.97, p<.001). The musculoskeletal subjective symptoms disorders attack rate was 90.5%. The results of the correlative study to assess relationships showed significant negative correlation between musculoskeletal subjective symptoms disorders and health promotion behavior (r=-1.45, p=.011). CONCLUSION: The results of the study showed that it was lower level of health promotion behavior level. Therefore, to increase the health promotion behavior of the daily plant construction workers, a great deal of efforts are required at the level of the government, employers and workers.

4.A Comparison between Home Care Nursing Interventions for Hospice and General Patients.

PURPOSE: The purpose of the study was to compare home care nursing intervention activities analyzed by the Nursing Intervention Classification (NIC) system for hospice and general patients. METHOD: For the descriptive survey study, data was collected by reviewing charts of 151 hospice patients and 421 general patients who registered in the department of home health care nursing at K Hospital. RESULTS: According to the NIC system application, there were 2380 total nursing interventions used for the hospice patients and 8725 for the general home care patients. For both sets of patients (hospice vs. general), the most frequently used nursing intervention in level 1 was the Physiological: Complex domain (40.13 vs. 31.06 percent), followed by the Safety domain; in level 2, the Risk Management class (28.4 vs. 27.70 percent), followed by Tissue Perfusion Management; and in level 3, Vital Sign Monitoring (6.18 vs. 4.84 percent), followed by Health Screening. CONCLUSION: The study showed that there was a lack of specialized hospice nursing interventions such as emotional, family and spiritual support, and care for dying hospice patients.

5.Burkitt's Lymphoma Developed Acute Leukemia: A report of two cases.

Burkitt's lymphoma with a frankly leukemic picture is an extremely rare condition, and usually has a very acute course with a poor prognosis. The authors experienced two cases of Burkitt's lymphoma developed acute leukemia. The first case was 8 years-old boy who had rapidly growing large abdominal mass, which revealed typical Burkitt's lymphoma with characteristic "starrysky" pattern. The second case was 20 year old male who had not definite tumor mass. Their peripheral blood and bone marrow aspiration revealed acute leukemia with multiple vacuolated lymphoblast of Burkitt's type. We report these cases with brief review of literatures.

6.A Case of Dermatomyositis Manifestated by the Paroxysmal Supraventricular Tachycardia.

Dermatomyositis is a disease of unknown etiology characterized by inflammation and degeneration of skeletal muscles and cutaneous abnormalities. Cardiac involvement in dermatomyositis-polymyositis is thought to be rare. In recent year, however, there has been an increasing number of reports on cardiac abnormalities in adult dermatomyositis and polymyositis due to development of noninvasive diagnostic techniques. Categorically, these abnormalities have included electrocardiographic changes, cardiac arrhythmias, congestive heart failure, coronary artery disease, and pericarditis. A 56-year-old woman was admittied to the Ewha womans University Hospital with dyspnea and palpitation. She was diagnosed as having dermatomyositis and followed up our department of Dermatology. Electrocardiogram showed a paroxysmal supraventricular tachycardia at as rate of 195/min. The patient was treated with 240mg/day verapamil p.o, 60mg/day prednisone p.o, there was marked improvement of symptoms. Systematic study of cardiac function utilizing echocardiography, Holter monitoring, thallium-201-scan, and gated blood pool studies were conducted in five newly diagnosed patients with dematomyositis-polymyositis. A significant elevation of serum CPK-MB is indicative of cardiac involvement. Cardiac involvement is a serious prognostic sign. We report a case with the review of the literature.

8.The Effect of Sensory Stimulation on Mother-Infant Interaction in Premature Infants.

PURPOSE: This study was designed to investigate effect of sensory stimulation on the mother-infant interaction in premature infants. METHOD: The subjects of this study consisted of 36 pairs of premature infants and their mothers from NICU of one university hospital located in Taegu, 18 pairs for intervention group and 18 pairs for control group. The data were collected from May, 1999 to October, 2000. For the intervention group Field's sensory stimulation(tactile and kinesthetic stimulation) was applied 2 times a day for 10 days(10:00-11:00 in the morning and 7:00-8:00 in the afternoon by researcher and mother). To determine mother and infant interaction during feeding, tool developed by Kim Mi-Ye(1999) was used. Collected data were analyzed with SAS program using chi-square test and t-test. RESULT: Significant differences were found in mother-infant interaction between two groups(t=-5.38, p=.00). It indicates that sensory stimulation was effective in improving mother-infant interaction. In the aspects of the quality of mother-infant interaction, sensory stimulation was most effective in improving sensitivity to mother and infant's synchronic behaviors(t=-5.43. p=.00) and followed by growth fostering(t=-5.07, p=.00), sensitivity to infant's cues(t=-4.53, p=.00), clarity of infant's cues(t=-3.03, p=.00) and responsiveness to the mother's behaviors(t=-2.14, p= 0.04). CONCLUSION: Based on the result of this study, it is suggested that maternally administered sensory stimulation should be applied clinical practice to improve interaction of premature infants and their mothers.

9.Perceived Cognitive Function and Related Factors in Korean Women With Breast Cancer.

PURPOSE: The aim of this study was to explore perceived cognitive function and its related factors in breast cancer patients that had received or were receiving adjuvant chemotherapy after surgery. METHODS: A cross-sectional and correlational design was used in this study. A total of 118 women who had undergone or were undergoing adjuvant chemotherapy after breast cancer surgery recruited from a convenience sample from a university hospital in Korea were included in the study. The attentional function index was used to measure perceived cognitive function while the linear analogue self-assessment scale was used to measure mood disturbance. Hierarchical multiple regression analyses were done to determine possible predictors of perceived cognitive function in patients with breast cancer. RESULTS: Perceived cognitive function and mood disturbance had a mean score of 66.22 (SD = 13.43) and 159.78 (SD = 81.40), respectively. Mood disturbance was a statistically significant predictor of cognitive function in patients with breast cancer. CONCLUSIONS: Breast cancer patients with mood disturbance may experience decrements in perceived cognitive function. Nurses should be aware of mood changes and its influence on perceived cognitive function in breast cancer patients.

10.Gastrointestinal Tumors Associated with von Recklinghausen's Neurofibromatosis: A report of two cases.

Neurofibromatosis, first clearly described by von Recklinghausen in 1882, is a dominantly inherited mesodermal and ectodermal dysplasia with a broad spectrum of clinical findings. Most common is the classical neurofibromatosis, which has three major features: (1) multiple neural tumors dispersed anywhere on or in the body; (2) numerous pigmented skin lesions, some of which are "cafe au lait" spots; and (3) pigmented iris hamartomas also called Lisch nodules. Other lesions sometimes seen in patients with von Recklinghausen's disease include congenital malformations of various types, vascular lesions, neurilemoma meningioma and other intracranial neoplasms, pheochromocytoma, medullary carcinoma of thyroid gland, neuroblastoma, ganglioneuroma and Wilms' tumor. Approximately 10% of the patients with neurofibromatosis have the gastrointestinal tract tumors as neurofibroma, ganglioneuroma and leiomyoma. Some of them show evidence of malignancy. Increased activity of the protein "nerve growth factor" in the sera of the patients with disseminated neurofibromatosis has been reported. We would like to report here two cases of von Recklinghausen's neurofibromatosis with involvement of the gastrointestinal tract.
