The patient complained of finding on the right side of the neck fistula with discharge nine years. In recently, fistula spills significantly increased compared with the previous. Special physical examination: Right sternocleidomastoid middle 1/3 front border is seen here in a small fistula, translucence jelly secretion were spilling out when squeezeing the fistula, no smell. Ultrasound: On the right side of the upper cervical skin to submandibular gland rear could see tubular low echo area. MRI: Visible on the right side of the neck tube signal, after the submandibular gland rear. Three-dimensional CT: Visible on the right side of the neck by skin sinus crossings as deep as the tonsillar fossa lumen containing contrast agent. Clinical diagnosis: The second branchial cleft cyst and fistula.

2.Research advances in the role of frailty in elderly patients with chronic kidney disease

The management of frailty in elderly patients with chronic kidney disease (CKD) has been receiving increasing attention.Frailty in elderly patients with CKD has a high incidence and is used for risk prediction of adverse outcomes,including death,hospitalization,falls,dialysis and so on.A range of pathophysiological changes caused by CKD,aging and multimorbidity work together to promote the development of frailty.Treatment for CKD patients with frailty includes mainly nutritional support,exercise,drug therapy and comprehensive intervention.This article reviews recent research progress concerning frailty in elderly CKD patients.
3.Isolation and purification of pancreatic islets in adult donors

Objective To investigate the methods and feasibility of islets isolation and purification,and to get more islets with high purity and good function for clinical transplantation.Methods After being weighted,human pancreatic islets were isolated with type Ⅴ collagenase,and purified by Ficoll's discontinuous density gradient centrifugation.The yield and purity of islets were evaluated by dithizone(DTZ) staining under microscope,and the viability was assessed by insulin release assay in vitro.Results The average number of islets was about 3 600 ± 447 per gram pancreas after isolation and it was about 2140 ±207 after purification with more than 70% purity.2,4,6 days after islet cell culture,the basal insulin concentration of the culture medium was measured,and it was 3.302 ± 1.63,3.504 ±1.10,and 2.921 ±1.13 (mIU/L/100 islets) respectively.Conclusion Collagenase digest and Ficoll's discontinuous density gradient centrifugation are effective methods for isolation and purification of human pancreatic islets.

6.AN EXPERIMENTAL STUDY USING INDOCIN TO INHIBIT THE OTOTOXICITY INDUCED BY FUROSEMIDE

We have observed the protective effects of indocin (IND), a potent inhibitor of pro-sglandins (PGs), on ototoxicity induced by furosemide (FU) using the synchronous recording technique of endocochlear potential (EP) and auditory action potential(AP) in guinea pigs. The FU group were found to have a mean decrease in EP of 65.1?5.6mV and AP amplitude 91.5?10.2%. In contrast, the animals in the FU+IND group had a much smaller reduction of EP(17.5 ?3.8mV) and AP amplitude(31.1?4.7%). The rate of return in EP and AP was much faster in the FU+IND group as compared with the other group. Our findings suggest that the ototoxicity of FU may be related to the mediation of PGs in the inner ear target tissues. IND can interrupt an increase in endogenous PGs in cochlear tissues triggered by FU injection by inhibiting the biological synthesis of PGs, thus effectively diminishing the ototoxicity induced by FU.
7.Comparative Study of Cardiovascular Digital Imaging and Echocardiography inDiagnosis of Transposition of the Great Arteries

Objective To evaluate the value of cardiovascular digital imaging and echocardiography in diagnosis of transposition of the great arteries.Methods Thirty cases of transposition of the great arteries were examined with cardiovascular digital imaging technique,two-dimensional echocardiography and color Doppler.11 cases of them underwent surgery.A comparison was made between the data obtained and surgical findings.Results The rate of coincidence of digital imaging and echocardiography with operation in diagnosing transposition of the great arteries was 92.3%(36/39)and 84.6%(33/39) respectively.Both echocardiography and cardiovascular digital imaging techique could clearly show the abnormalities of the heart and great vessels.Echocardiography was sensitive to valvular pathological changes,though cardiovascular digital imaging was superior to echocardiography in demonstrating abnormalities of great arteries and systemic-pulmonary collateral circulations.Conclusion Combination of echocardiography with cardiovascular digital imaging technique can greatly improve the rate of diagnostic accuracy for transposition of the great arteries.
8.The effects of hyperbaric oxygen therapy on promotion of intellectual rehabilitation in infants with severe hypoxic-ischemic encephalopathy

Objective To explore the long-time effects of hyperbaric oxygen(HBO) therapy on the promotion of intellectual rehabilitation in infants with severe hypoxic-ischemic encephalopathy(HIE). Methods Forty-seven infants with severe HIE were randomly divided into a HBO group(n=24) and a control group(n=23). All the infants were treated with routine therapy for three months,in addition to HBO treatment in the HBO group once a day for four courses of 10 d with the interval of 10~15 d. Neonatal Behavioral Neurological Assessment (NBNA)assay was employed at 7 and 28 d after born,and Bayley Scale of Infant Development (BSID)assay was got at two years in two groups, as well as Wechsler Preschool and Primary Scale of Intelligence(WPPSI)assay at five years, in order to evaluated the short-time and long-time effects of HBO on intellectual rehabilitation in infants with HIE. 9.Exploration of Plasma Levels of Catestatin in Patients With Different Blood Pressure and Metabolic Status

Objective: To analyze plasma levels of catestatin (CST) in patients with different blood pressure (BP) and metabolic status.
Methods: Our research included in 3 groups: Control group,n=60 subjects with normal BP from physical examination in our hospital; 131 patients with essential hypertension (EH) were divided into 2 groups: Isolated EH group,n=90 and MS (metabolic syndrome) group,n=41. Plasma levels of CST, norepinephrine (NE) were examined, the ratio of CST/NE was calculated, and those indexes were compared among different groups.
Results: Compared with Control group, plasma levels of CST and the ratios of CST/NE were decreased in Isolated EH and MS groups,P<0.05-0.01. As BP increasing, plasma levels of CST were gradually decreased; compared with Isolated EH group, plasma level of CST and CST/NE were reduced in MS group,P<0.05.
Conclusion: Plasma levels of CST and the ratio of CST/NE were obviously decreased in EH patients, CST was even lower in MS patients which implied that CST might be involved in the development of EH and MS.
