1.New Fixation Method Using Two Crossing Screws and Locking Plate for Cubitus Varus Deformity in Young Adult Elbow: Case Report.

Many types of osteotomy have been proposed for the treatment of cubitus varus deformity of the elbow, and various methods for fixation of the osteotomy site have also been described. However, no method has been perfect. We treated two cases of cubitus varus elbow deformity with step-cut osteotomy using a new fixation method with two crossing screws and an anatomically designed locking plate. Active assisted elbow range of motion (ROM) exercise was permitted at postoperative 3 days, after removal of the drainage. Preoperative and postoperative humerus-elbow-wrist angles and ranges of motion of the two patients were compared. At 3 months follow-up, each patient had recovered the preoperative elbow ROM, and achieved the complete bony union of the osteotomy site and proper correction of the cubitus varus deformity. In addition, the appropriate remodeling of the lateral bony protrusion was observed. Therefore, we introduce a new fixation method for achievement of stable fixation allowing immediate postoperative elbow motion after corrective osteotomy for cubitus varus deformity in young adults.

2.Amytal test in embolization of brain arteriovenous malformation.

Is superselective embolization of the brain AVMs, the possibilities of adjacent normal brain tissue damage necessitates preembolic evaluation. The authors performed the Sodium Amytal test on 15 patients with brain AVMs. In the AVM patients with negative Amytal test(30 cases), all patients showed normal pattern on EEG and neurologic examinations after embolization Among the five patients with positive Amytal test, three cases showed neurologic deficit and the others two didn't. In eleven patients. Repeated embolization without the Amytal test were done. Among them, 5 cases showed neurlogic deficit and the others were quite normal. In conclusion, the Amytal test is a useful method of evaluation of the risk to damage normal brain tissue which are supplied by superselected feeding vessels. Combining the EEG with this test is a sensitive and objective method in evaluating patients following the Amytal test. And it is essential to performed the Amytal test for repeat embolization procedures.

3.Diagnostic Value of Computed Tomography in Acetabular Fracture.

We retrospectively analyzed 22 patients pelvic CT, in whom the acetabular fracture were suspected in plain film. And compared and analyzed the computed tomogram findings and plain radiographic findings. The results were as follows. CT enables better evaluation of shape, extent, and degree of separation of fragment. CT was helpful in detecting the combined fracture and soft tissue injuries. CT showed intraarticular loose bodies, which were invisible on plain film. In patients with pelvic trauma, no necessary changing position during CT examination. CT was useful demonstrates the remnant of intraarticular osseous fragment and adequacy of reduction after surgery.

5.Embolization of carotid-cavernous fistula using a silicone balloon and a tracker catheter system.

With the recent introduction and development of the detachable balloon system, it has become the treatment of choice in the management of carotid cavernous fistulas(CCFs). But, since most delivery systems for embolization of CCF mainly depend on flow guidance for balloon delivery, in cases of small fistula, pseudoaneurysm and arterialized venous collaterals, failure of balloon embolization can occur. To overcome these limitations, the authors designed and used a new versatile, steerable, and flow-guided detachable balloon system by using a Tracker catheter system with silicone or latex balloons. Using this maneuver, we could get successful fistula occlusion in 7 out of 8 patients(silicone balloon). But in one case, we had to occlude the internal carotid artery at the fistula site, proximal and distal cervical portions of the internal carotid artery. This balloon delivery system proved to provide high selectivity for fistula and relatively ease of handing.

8.An Effect of benzene on Chromosomes in Bone Marrow Cells of Rats

The toxic action of benzene on erythropoiesis and myelopciesis, has been recognized since the early years of the present century. With the advance in high civilization and modern covenience, benzene as a kind of aromatic compound has been used for industrial solvent and its longstanding use has committed a public nuisance to be overcome by medical approach. Chromosomal breakage and rearrangement may be produced by radiation, radiomimetics, virus infection and various chemicals, especially, antibiotics and antitumor agent, causing chroimosomal rearrangement in vitro, whose teratogenic action in rats was previously demonstrated. Several works hsve been published on the chromosome damage as a consequence of benzene intoxication. Recently, it was shown by certain workers that individuals who had been exposed to atmospheric benzene, even without haematological disorders, might have an elevated percentage of structural chromosome aberrations in the lymphocytes cultured from their peripheral blood. Moreover, structural and numerical chromosome aberrations were demons trated in patients with blood disorders which were believed to be due to exposure to beuzene vapors. Accordingly, much interest has been paid to its cytologic effect on the hematopoietic tissues in man and experimental animals. A high incidence of chromosomal aberrations has also been found in rabbits exposed to benzene during a period of peripheral pancytopenia and after hematologic recovery. The significance of these findings was discussed in relation to leukemic transition and to their diagnostic value in human benzene intoxication. Chromosomal anomalies can also be induced by benzene given subcutaneously to rata. A pronounced individual variation of the degree of chromosome damage was shown. The purpose of this investigation was to determine whether benzene could a direct effect on the chromosome complement of mammalian bone marrow cells in vivo and whether characteristic banding patterns might be demonstrated in rat chomosomes by a modified trypsin-Giemsa method. Four-week old Sprague-Dawley strain rats of both sexes(each weighing about 50gm) were used for this experimental study. Three groups of animals were treated-with subcutaneous infections of pure benzene. Group I received benzene, 2.0ml per kg body weight, 24 hours before sacrifice; Group II, 48 and 24 hours and Group III, 72, 48 and 24 hours. A control group was given no treatment. The animais were sacrificed in ether anesthesia. Femur and iliac bone marrow cells were suspended in medium 199 within 30 minutes and transferred to warm Hanks-distilled water(1:3) for hypotonic treatment(10 minutes). A freshly prepared solution of methanol glacial acetic acid (3:1) was used as fixative. Finally, a few drops of the cell suspension were placed on moistened, pre-cleaned slides being dried by rapid-drying technique. The slides were stained with either simple Giemsa or trypsin Giemsa banding technique. From the data obtained, this report was summarized as follows: 1. For the benzene-treated groups, chromosomal aberration rate was 13.4% in group II and 38.6% in group III, while in the controls the rate was 6.4 percent. 2. Numerical aberrations included aneuploidy, polyploidy and monoploidy. The most frequent type was hypodiploidy (5.8–9.4%) in all the treated groups. 3. Structural aberrations could be divided in gaps, ring chromosomes, breaks, deletions, exchanges and dicentrics. Among those, the majority of abnormal metaphases was gaps; 2.4%, 2.2% and 10.8% in group I, II and III respectively, and 1. 6% in control group. 4. The translocations and dicentrics were not demonstated in group I and II. 5. The normal chromosome set of the Sprague-Dawley rat was comprised of 42 chromosomes: 20 pairs of autosomes, and one pair of sex chromosomes, xx or XY chromosomes. The total number of major bands in s chromosome complement was about 40 and minor bands, 13, 6. Sucessful demonstration of banding patterns was available by proper adjustment of the concentration, temperature and duration of trypsin solution.

9.The relationship of common reasons for encounter and major diagnoses of patients admitted to a hospital.

BACKGROUNDS: It is important to know the contents of health problems in patients in a primary care setting. The aim of this study was to explore the main chief complaints and major diagnoses of patients, who were admitted to a hospital using the ICD-10 and to observe difference according to each department and admission route and how diagnoses were made. METHODS: A total of 18,560 patients who were admitted to a hospital located in Chungnam Province from 1 Jan 1998 to 31 Dec 1998. Main chief complaints and major diagnoses made through the admissions departments were chosen as subjects (medical vs. surgical) and by admission route (emergency vs. OPD)were analyzed. How the diagnoses were derived from the most common chief complaints among medical and surgical departments were analyzed by admissions departments and by admission route. RESULTS: The most common 10 chief complaints revealed no significant difference by admission route in medical departments, but there was some difference in surgical departments. There was some difference in the most common 10 major diagnoses by admission routes in both medical and surgical departments. Abdominal and pelvic pain, which was the most common chief complaints, became a significantly different diagnosis by admission route in both departments. CONCLUSION: Main chief complaints were similar regardless of admission routes, but the diagnoses were different. ICD-10 classification may be useful to classify the diagnoses, but have limitations to classify chief complaints or reasons for encounter. It is necessary to introduce a new classification such as ICPC-2 for reasons for encounter in order to explore the dimension of health problems.
