2.A clinical and radiological observation on urolithiasis

A clinical and radiological observation was mde on 167 cases of urolithiasis among the number of 150 patientsduring 1 year and 6 months from June, 1981 to Nov. 1982. The results were summerized as follows. 1. There were 101man and 49 women, a ratio of 2:1. The ages of the patients ranged from 2 to 74 years, showing the highestincidence in 31 to 60 years(68.1%). 2. Locational distributions of urolithiasis were 80 cases (47.9%) in theureter, 66 cases(39.5%) in the kidney, 11 cases (6.5%) in the urethra and 10 cases (5.9%) in the bladder. 3. Amongthe 66 cases of renal stone, pelvis stone was 55 cases (83.3%) with staghorn types in 23 cases(34.8%), andcalyceal stone was 11 cases(16.6%). 4. The location of ureteral stone was 47.5% in lower, 40% in upper ureter and12.5% in mid-ureter. 5. The location of urethra stone was 82% in the anterior urethra and 18% in the posteriorurethra. 6. The chief complain of urolithiasis was flank pain in 59.2%, gross hematuria in 20.3%, renal colic in13.1% ,dysuria in 8.3%, nausea and vomiting in 4.7%, and sudden stoppage of urine stream in 3.5%. 7. Onurinalysis, gross hematuria was found in 54.4%, pyuria in 28.7%, bacteriuria in 23.3%, microscopic hematuria in18.5% and normal in 7.1%. 8. The size of urinary stone was 0.6-2.0cm in length in 105 cases(62.8%). 9. On I.V.P.study of renal stones(66 cases), mild and moderate hydronephrotic changes were detected in38 kidneys(57.5%), andthe relationsip between the urinary stasis and renal stone size was relatively good. 10. On I.V.P. study ofureteral stones(80cases), mild to severe hydronephrotic changes were detected in 64 kidneys(80%). 11. On K.U.B.film, paralytic ileus was found in 25 cases (14.9%). 12. Among the urinary stones, the radiolucent stones weredetected in 8 cases (4.7%). 13. Urinary stones diappeared in 11 cases (6.5%) spontaneously or medical treatment.

3.Artificial prosthesis of the knee joint combining with homogenous bone transplantation

Here, we report a case of artificial prosthesis of the knee joint combining with homogenous bone transplantation for the treatment of giant cell tumor of the right distal femur. At one year follow up, the patient complains almost no pain nor tenderness at operative site and ambulates with partial weight bearing with crutches. The X-ray finding shows satisfactory bony union with progressive bony remodelling.

6.Role of Redistribution and 24 Hour Reinjection Images to assess Myocardial Viability in Patients with Acute Myocardial Infarction.

PURPOSE: We evaluated the importance of redistribution and 24 hour reinjection images in Tl-201 SPECT assessment of myocardial viability after acute myocardial infarction (AMI). MATERIALS AND METHODS: We performed dipyridamole stress-4 hour redistribution-24 hour reinjection Tl-201 SPECT in 43 patients with recent AMI (4-16 days). The myocardium was divided into 16 segments and perfusion grade was measured visually with 4 point score from 0 to 3 (absent uptake to normal uptake). A perfusion defect with stress score 2 was considered moderate. A defect was considered severe if the stress score was 0 or l (absent uptake or severe perfusion decrease). Moderate defect on stress image were considered viable and segments with severe defect were considered viable if they showed improvement of 1 score or more on redistribution or reinjection images. We compared the results of viability assess-ment in stress-redistribution and stress-reinjection images. RESULTS: On visual analysis, 344 of 688 segments (50%) had abnormal perfusion. Fifty two (15%) had moderate perfusion defects and 292 (85%) had severe perfusion defects on stress image. Of 292 severe stress defects, 53 were irreversible on redistribution and reversible on reinjection images, and 15 were reversible on redistribution and irreversible on reinjection images. Two hundred twenty four of 292 segments (76.7%) showed concordant results on stress-redistribution and stress-reinjection images. Therefore 24 hour reinjection image changed viability status from necrotic to viable in 53 segments of 292 severe stress defect (18%). However, myocardial viability was underestimated in only 5% (15/292) of severe defects by 24 hour reinjection. CONCLUSION: The 24 hour reinjection imaging is useful in the assessment of rnyocardial viability. It is more sensitive than 4 hour redistribution imaging. However, both redistribution and reinjection images are needed since they complement each other.

7.Dose effect of LPS on Cytokine mRNA Expressions by Mouse Peritoneal Macrophages.

Many studies for LPS-induced cytokines, chemokines gene expressions have been reported, but results are highly diverse. The dose of LPS used, cell type studied or other technical factors may contribute to the differences. We investigated the cytokine mRNA expressions, interleukin-2 (IL-2), IL-6, IL-10 and IFNr, on thioglycollate (TG) elicited-mouse peritoneal macrophages stimulated with various concentrations, 1 ng/ml, 10 ng/ml, 100 ng/ml, 1 ug/ml and 10 ug/ml, of LPS used by reverse transcription polymerase chain reaction (RT-PCR) and northern blot analysis. TG- elicited peritoneal macrophages expressed IL-6 and IFN-r mRNA in a dose-dependent manner. The expressions of IL-2 and IL-10 mRNA were dose-independent manner. In northern blot analysis, IL-6 mRNA expression was detected only in 10 ug/ml LPS concentration for short stimulation time (0.5 h), while IL-10 mRNAs were expressed evenly in all LPS concentrations. The expression of IL-6 mRNA was sustained until 72 h at 10 ug/ml concentration only, but IL- 10 mRNAs of all cases of LPS concentrations were sustained evenly until then. These results suggest that the concentration of LPS required for cytokine induction must be determined differently from gene to gene types, and although LPS concentration 10 ug/ml has not been used as an ordinary concentration in vitro cytokine study, 10 ug/ml of LPS might be more appropriate concentration in the study of IL-6 expression on mouse peritoneal macrophages.

8.Erythromycin Resistance Phenotype of Streptococcus pyogenes.

BACKGROUND: The erythromycin-resistance rate and phenotype distribution of Streptococcus propenes are quite different by geographical variation and study period. The aim of the present study was to determine the evolution of resistance to erythromycin and the frequency of erythromycin resistance phenotype of S. pyogenes isolated from Wonju Christian Hospital. METHODS: The minimal inhibitory concentrations (MICs) of erythromycin and clindamycin for 94 S. pyogenes isolated from clinical specimens between 1990 to 1998 were investigated. Double disk test of erythromycin (78microgram) and clindamycin (25microgram) were performed for 15 isolates of erythromycin resistant S. pyogenes to evaluate the erythromycin resistance phenotype. RESULTS: The resistance rates of 94 isolates of S. pyogenes were 16%(15/94) to erythromycin and 4%(4/94) to clindamycin. The frequency of erythromycin resistance phenotype in decreasing order were M phenotype (47%), inducible resistance phenotype (40%), and constitutive resistance phenotype (13%). Erythromycin-resistant S. pyogenes did not exist until 1993, but was isolated since 1994, and ranged from 14.0% to 24.0% during the period of 1994-1998. CONCLUSIONS: Our finding documents the emergence of high resistance rates to erythromycin in S. pyogenes at Wonju area since 1994. The M phenotype (47%) and inducible resistance phenotype (40%) account for the majority of erythromycin-resistant S. pyogenes.

9.A Study on the Production of Blood Group Antibodies in Korean Children.

BACKGROUND: It has been known that blood group antibodies are not produced in the neonatal period and that if the antibodies exist, they are probably maternal in origin which had crossed the placenta. There have been several studies conducted abroad on when these antibodies are formed but none has been done in Korea. This study was carried out to determine the ABO blood type and blood group antibodies in children from neonates up to 5 year old. We hoped to determine when and in what pattern blood group antibodies were produced. METHODS: We selected 337 children from neonates up to 5 year old who were admitted to Hanyang university Hospital in Seoul or Kuri from 1994 to 1996. Cell typing was done immediately by the slide method. The anti-A and anti-B used for cell typing were supplied by Immucor (Norcrosis, Ga) . Sera were stored at -70 degrees C until they were tested for ABO blood group antibodies by the standard saline test tube method. When uncertain results were obtained, a drop of the mixture was placed on a slide and observed under a microscope. RESULTS: ABO blood group antibodies were detected in 9 of 50 (18%) infants less than 1 week old and in 10 of 51 (20%) infants between 1 week and 3 months of age. The pattern of ABO blood group antibody production was similar to that of the fetal period up to 3 months after birth, after which antibody production increased rapidly to reach approximately 80% at 6 months of age, There was no difference in ABO antibody production between boys and girls. The antibody formation pattern of group A and group B infants less than 6 months of age showed anti-A to be 35% and anti-B to be 20%. In group O infants of the same age, anti-A was positive In 42% and antral-B In 33%. However, after 6 months of age, there was no difference in antibody production among groups A, B, or O. CONCLUSIONS: Antibodies directed toward ABO antigens were detected in 19 out of 101 (19%) infants less than 3 months old. We therefore believe it is necessary to Perform serologic typing as well as cell typing in these Infants. Furthermore, the emergency transfusion of type A or B blood to a type O infant under the impression that anti-A and anti-B do not exist should be forbidden.

10.Treatment Strategy of Growth Hormone Secreting Pituitary Adenoma.

The goals of the pituitary tumor surgery are restoration of the normal pituitary function and relieving the mass effect on the adjacent normal brain structures. The main purpose of this study is to analyze our surgical results of growth hormone(GH) secreting pituitary adenomas which were treated extensive and aggressive surgical removal. The surgical results were reviewed in 36 patients treated between 1993 and 1997. The criteria of clinical remission are postoperative basal GH less than 5ng/ml and suppression of GH less than 2ng/ml in oral glucose tolerance test. As the results, 25 of 36 patients(69.4%) achieved surgical remission. Eleven patients who could not achieve remission by surgery were treated with medical treatment(octreotide, bromocriptine) and Gamma-knife surgery. In conclusion, the tumor morphology and extensiveness of tumor removal were well correlated with surgical outcome, and the aggressive resection of tumor seems to be the most important factor achieving remission in GH secreting pituitary adenomas.
