2.Effects of Hantaan Virus and IFN-gammaon Induction of Surface ICAM-1 in Primary Cultured Buman Nasal Epithelial Cells and Human Lung Fibroblasts.

The primary culture of human nasal epithelial cells was performed using the inferior nasal turbinate tissues, and infected with Hantaan virus to examine the hypothesis of airborne transmission of Hantaan virus in humans. The primary culture cells were identified as epithelial cells by morphologic and immunologic analyses. The viral antigens were detected in the primary human nasal epithelial cells infected with Hantaan virus by immunofluorescence staining. The ICAM-1 induction by Hantaan virus or IFN-gammawas examined in the primary human nasal epithelial cells and human lung fibroblasts (WI-38). Hantaan virus induced the surface ICAM-1 in Wl-38 cells in a time-dependent manner, and IFN-gammainduced the surface ICAM-1 in a dose-dependent manner in HNEC and WI-38 cells. These results revealed that the human nasal epithelial cells are susceptible to Hantaan viral infection supporting the hypothesis of airborne transmission of Hantaan virus in humans. The human lung fibroblasts also might have an important role in the pathogenesis of Hantaan virus through the induction of ICAM-1.

3.Fertility Outcome after Treatment of Ectopic Pregnancy.

OBJECTIVE: It is generally accepted that ectopic pregnancy (EP) may adversely affected on the female fertility. However, it is not fully understood how it influences on the future fertility after treatment of an EP, so we intended to evaluate its effects, METHODS: This study was undertaken on 473 patients with clinically and pathologically proven diagnosis of ectopic pregnancies at the Department of Obstetrics and Gynecology, Anam Hospital, Kroea University Medical College hom Jan. 1, 1989 to Aug. 31, 1996. RESULTS: The incidence of ectopic pregnancy was 1 in 19.6 deliveries (5.1%). The overall spontaneous conception rate after treatment of ectopic pregnancy was 67.4%, and among them, the rate of intrauterine pregnancy (IUP) and repeat ectopic pregnancy (rEP) was 56.3% and 11.1% respectively. The occurrance rate of infertiity after treatment of EP was 32.6%. The mean time to IUP after treatment of EP was 16.5 months, and 79.0% of all IUP were conceived within 2 years after treatment. The mean time to repeat ectopic pregnancy was 15.9 months. With increasing maternal age, IUP rate was decreased and rEP rate was increased but they were statistically not signiTicant. Repeat EP rate was also increased in multiparous women but it was also statistically not significant. Overall PR (IUP and rEP) was decreased in women who have organic lesions (adhesions, endometriosis, PID etc.) in pelvic cavity(p=0.003). Patients who were treated with conservative surgery achieved a lower conception rate without statistical significance and it may be due to low cases. CONCLUSION: Future fertility rate was not significantly altered by EP itself, but rather affected by patient's age, organic lesions and previous history of pelvic surgery. Recently, the advent of assisted reproductive technology and its associated techniques improved the female fertility in women with such a problem.

4.Transvaginal Selective Fetal Reduction in Multifetal Pregnancy induced by Assisted Reproductive Technology.

OBJECTIVE: The prevalence of multifetal pregnancies has increased up to 30% as a result of the introduction of ovulation inducing agents for assisted reproductive teclmology(ART). An exttemely poor pognosis could be expected for viable pregnancies in multifetal gestation. So, to decrease the consequence of multiple pregnancies and prevent complications, especially premature baby irreversibly damaged, selective fetal reduction to the smaller number of fetuses should be considered in an early gestational period. METHODS: From May 1994 to Apr 1998, transvaginal selective fetal reduction in 13 pati including 9 triplet, 3 quadruplet and 1 quintuplet. Of the 13 patients, 4 were obtained by controlled ovarian hyperstimulation with intrauterine insemination (COH with IUI), 6 were by IVF-ET, 2 wae by controlled ovarian hyperstimulation with natural contact and 1 was by natural conception. Selective fetal reduction using intracardiac KC1 injection and aspiration of amniotic fluid carried out in 8-11 weeks of gestation. RESULTS: After procedures, 8 patients were remained as twin pregnancies, 5 patients as singleton pregnancies and 1 of the remaining twin embryos vanished after procedure. There have been 7 sets of twin delivery including 1 stillbirth and 3 singleton delivery. 1 cases are ongoing state. All of the singleton delivery were completed after 37 weeks of gestation. Of the twin delivery, 2 cases were delivered after 37 weeks of gestation, 2 cases in 35-37 weeks, and 3 cases before 35 weeks of gestation. Unfortunately, 1 stillbirth occurred in 20 weeks of gestation and 2 cases of singleton were aborted. As 3 losses(2 singleton, 1 twin) occurred, the delayed fetal loss rate in this selective fetal reduction was 25.0%(3/12). There was no fetal anomaly related to the procedure. CONCLUSION: Selective fetal reduction in multifetal pregnancies is a rather safe procedure and it may improve the outcome of multiple pregnancies.

5.Treatment and Clinical Application Of a Triangular Splint Method for Fracture of the Humerus

Fracture of the shaft of the humerus had been treated by many different way, such as hanging cast, shoulder spica cast, abduction splint, and Velpeau bandage, etc, The authors have employed the new trianglar splint designed by ourselves for the 54 cases of fracture of the humeral shaft from Jan., 1965 to Oct., 1976 in the department of Orthopedic surgery of Han-il Hospital. In our experience, the triangular splint method is useful for not only fracture between the head of the humerus and supracondylar region, but also shoulder joint injuries and diseases which need firm immobilization. The triangular splint method has merits, such as spontaneous reduction by the fractured arm weight with gravity, the stability during care, relatively comfort during procedure. Satisfactory results had been obtained by the triangular splint method.

6.Diagnosis and Treatment of the Traumatic Knee Joint Injury

Sixty five cases of the knee joint injuries diagnosed and treated at the orthopedic department of Han-Il hospital during the period from January 1966 to December 1970, were analysed clinically. The results obtained were as follow: (A) Age, sex, and side of injury were reviewed. 1) Age ranged from five to fifty-eight years, with thirty-six of the patients falling in the twenty one to forty years old age group. The greatest incidence (twenty) was in the twenty one and thirty year old. 2) Fifty four patients were male and eleven were female. This reflected the degree of exposure to injury in the sexes. 3) No significance was attached to the fact that thirty patients injured right knees, twenty five injured left knees and ten injured both knees. The Most common site was medial collateral ligament. (40%) (B) In order to observe rentgenographic changes of the traumatic knee joint, artificial stress(varus and valgus) was applied during taking photograph. When the medial site injury exist, medial intercondylar length widen between both condyle of tibia and femur, angle enlarged in degree and the point that made by opposite intercondylar lines of the tibia and femur, deviated to lateral interarticuIar space from spine of tibia. When the lateral site injury exist, above mentioned process is not always apply to application. (C) The results of surgical and conservative treatment have been reviewed; 38 cases conservative and 37 cases operative. The results were successful in both. groups. Surgery is not always the treatment of choice for collateral ligament injuries.

7.Symptomatic Graular Cell Tumor Involving Intra- and Suprasellar Area: A case report.

Symptomatic parasellar granular cell tumor is a very rare tumor. To the best of our knowledge, 43 cases was be found in the English literatures. We recently experienced a case of a parasellar granular cell tumor in a 61-year-old female who had bilateral temporal hemianopsia and severe panhypopituitarism. The tumor was composed of diffuse sheets of polygonal cells with abundant eosinophilic PAS positive granular cytoplasm. In the immunohistochemical and ultrastructural examinations, the tumor failed to show any evidence of Schwann cell or glial differentiation. These findings suggest that granular cell tumor has heterogenous cell types of origin.

9.Clinical Observation of Corneal Graft: An Interim Report of 25 Keratoplasties.

Authors performed 25 corneal grafts in 24 patients since the inauguration of The Central Eye Bank, attached to the Department of Ophthalmology, St. Mary's Hospital, Catholic Medical College, in April 19, 1967. Method and results of these cases are as follows; METHOD: In technique of surgery, the grafts in most of the cases were 7 mm in diameter(Table 3), in two types of penetrating and lamellar corneal grafts(Table 4). The donor material came from patient age group of 51-60 years old in most and was used within 24 hours after death (Table 5). In most cases we placed 12 interrupted sutures except of two cases for continuous suture by 8-0 virgin silks. Preoperative and postoperative cares were routine with systemic dexamethasone, 7.0mg a day was given routinely on the 5 th postoperative day for 30-50 days because of prevention of graft rejection. RESULTS AND SUMMARY: Table 7 summbrizes the results of our observation. Nineteen of the 25 grafts remained clear. And there was improved vision above 20/200 in ten eyes out of nineteen clear grafts. Table 10 lists the reasons why nine clear grafts did not improve vision. Postoperative complications occurred in fourteen grafts (Table 11). Penetrating corneal grafts for adherent leucoma courneae, staphyloma corneae and corneal scars from alkaline burn were failed to maintaining the clear graft. The donor material, the recipient cornea, graft rejection as a complication and its prevantion were discussed.
