1.A case of traumatic pseudoaneurysm of the dorsalis pedis artery.

An aneurysm occurs in association with trauma, atherosclerosis, infection and congenital diseases. Among these, the traumatic arterial aneurysms developed from blunt trauma, crushing injury, perforation, and lacerations of vessels. Generally, most traumatic arterial aneurysms are false type, so called pseudoaneurysm. A pseudoaneurysm occurs by incomplete laceration of the vessel wall or partial division of the vessel. The vascular injury will make hematoma and fibrosis of neighboring tissues. After two to eight weeks post-trauma, the organized hematoma is liquefied and the fibrous tissue acts as an aneurysmal sac. We have experienced a case of dorsalis pedis artery pseudoaneurysm associated with arterial cannulation. So, we report a case of traumatic pseudoaneurysm with a review of literature.

3.Genetic Polymorphism of PAI-1 Gene and Cardiovascular Disease: eta-analysis of Case-Control Studies.

BACKGROUND: Previous reports have suggested that alleles at the plasminogen activator inhibitor-1 (PAI-1) gene are associated with increased risk of developing coronary artery disease, including myocardial infarction and stroke through their effect on PAI-1 levels. Method: We attempted to search English literatures for all reports of possible effects of PAI-1 gene on cardiovascular disease in human published prior to November 1998. We used a Mantel-Haenszel method (fixed effect model) and random effect model, respectively, to perform a meta-analysis of 7 case-control studies that provided information related to the effects of PAI-1 gene on risk of cardiovascular disease. RESULTS: From 7 studies for diagnosed cardiovascular disease, the relative frequencies of the three genotypes among controls was (5G/5G) (homozygous normal), 24.5%; (4G/5G) (heterozygous), 48.2%, and (4G/4G) (homozygous for the mutant, 675 GGGG), 27.3%. These relative frequencies in cases were 21.7% for 5G/5G, 48.0% for 4G/5G, and 30.3% for 4G/4G. In fixed effect model, compared with those with genotype (5G/5G), the overall odds ratio (OR) for cardiovascular disease among those with (4G/5G) was 1.12 (95% CI, 0.93 to 1.34), and it was 1.20 (1.01 to 1.44) for the (4G/4G) genotype. For five studies with myocardial infarction as the outcome, the overall OR of myocardial infarction was 1.20 (0.99 to 1.47) for those with (4G/5G) and 1.24 (1.00, 1.54) for those with (4G/4G) genotypes, respectively. CONCLUSION: Our findings provide support for the weak association between PAI-1 gene and cardiovascular disease, in particular, myocardial infarction.

4.Clincal Effect of Ketotifen in Allergic Rhinitis.

Ketotifen, a benzocycloheptathiophene, has an orally effective antiallergic as well as antihistaminic properties. In pervious studies, Ketotifen has shown encouraging results on patient with allergic rhinitis, either perennial or seasonal. 39 patients with allergic rhinitis had been treated with Ketotifen 1 mg twice daily for 8 weeks. And we obtained following results. 1) The efficacy rate in sneezing attack was 73.5%, in nasal discharge 71%, in nasal obstruction 58%. 2) Some improvements in at least one of three-major symptoms were noted within 1 week in 30.7%, within 2 weeks in 55.8%, within 3 weeks in 66.7%, within 8 weeks in 87.2%. 3) Physical findings such as colour, swelling of turbinate, character of rhinorrhea were not improved significantly. 4) Side effect was observed only in one patient with abdominal pain and diarrhea, which was subsided after interruption of administration. These results suggested that Ketotifen was effective in treatment of allergic rhinitis.

7.The Observation of Arbuscular Mycorrhizal Roots in Horticultural Plants.

To determine the degree of variability among the host plant species in their abilities to become colonized by arbuscular mycorrhizal fungi (AMF), the inoculum for AMF was collected from the various sites in Korea and was inoculated to the three horticultural plants; Tagetes patula, Torenia fournieri, and Salvia splendens. After 4-month growth under greenhouse, mycorrhizal root colonization rates and spore density were measured. The roots of T. patula showed higher colonization rate than both plants of T. fournieri and Salvia splendens. The mycorrhizal root colonization was influenced by both of the AM fungal inoculum and the host species or their interactions. The combination of the host and fungal species was suggested to be important for the application of AMF to horticultural crops.

10.Residency training guideline for esophagogastroduodenoscopy skills in family practice.

BACKGROUND: Esophagogastroduodenoscopy(EGD) is a useful diagnostic procedure and an important basic skill in family practice residency program. But currently we have no guidelines for EGD training in family practice residency program. METHODS: In March 1999, we mailed a self-administered questionnaire to the residency directors of all KAFM-accredited family practice residency programs. 63 out of 109 programs responded to the questionnaire(57.8%). RESULTS: Among the total of 63 hospitals that responded to the questionnaire, secondary hospitals were 55.6% and tertiary or university hospitals were 44.4%. 98.4% reported that their residents receive training to perform EGD. 57.1% of the total received training to perform EGD during internal medicine training. EGD was performed by family physicians in 38.1% of total family practice residency programs. In the majority of these programs(34.9%, cumulative percent 74.6%), the minimal requirements for technical skills in EGD training were 50 cases and for both technical skills and cognitive skills at least 100 cases of EGD(41.3%, cumulative percent 79.3% was necessary). The most difficult component during EGD procedure seemed to be esophageal intubation followed by retroflexed maneuver and entering the pylorus in series. CONCLUSION: To perform EGD in primary care, it is required for trainee to perform at least 50 cases of EGD under supervision in a family practice residency program.
