Splenic artery pseudoaneurysm is a relatively rare and potentially life-threatening complication of chronic pancreatitis. The authors present a case of splenic artery pseudoaneurysm complicating ,chronic pancreatitis. It was converting into a pseudoaneurysm by vessel rupturs. In this case report, color doppler US, CT, and MRI made the definite diagnosis.

PURPOSE: To evaluate the effectiveness and the most accurate element of the diagnostic criteria of the Japan Society of Uitrasonics in medicine(JSUM, Nov. 1989) for distinguishing between benign and malignant solid breast masses on the US. METHODS AND MATERIALS: We analyzed the ultrasonic findings of histopathologically proved 51 fibroadenomas, 12 fibrocystic disease, and 39 breast cancers in relation to the diagnostic criteria of the JSUM (shape, border, boundary echo, internal echo, posterior echo, lateral echo, and depth/width ratio). RESULTS: The number of cases of fibroadenoma, fibrocystic disease, and breast cancer corresponding to the diagnostic criteria was in the shape(26/51, 5/12, 33/39), border(41/51, 9/12, 29/39), boundary echo(48/51, 12/12, 27/39), internal echo(43/51, 9/12, 24/39), posterior echo(32/51, 3/12, 21/39), and lateral shadowing(15/51, 1/12, 35/39). All diagnostic criterias showed statistical significance for differentiation of benign/malignant breast mass on the US(Chi-square test: p<0.05). The order of accuracy was boundary echo, internal echo, and border. The mean of depth/width ratio was 0.54 +/- 0.15, 0.52 +/- 0.12, and 0.69 +/- 0.21 in fibroadenoma, fibrocystic disease, and breast cancer respectively and it had statistical significance for differentiation of benign/malignant breast mass on the US(ANOVA test: p=0.0002). CONCLUSION: The diagnostic criteria of JSUM is effective for differentiation of benign/malignant breast solid masses on the US and has accuracy in the order of boundary echo, internal echo, and border. Depth/width ratio also has statistical significance.

Fifty-one consecutive 186 patients with aneurysmal subarachnoid hemorrhage were treated from the day of admission with nimodipine which was given first as an IV infusion at 30ug/kg/hr for 1 week and then orally in a dose of 360mg/day for 2 weeks and compared with 135 patients which were treated without nimodipine for the past 2 years. A comparision based on clinical and radiological variables influencing both the coruse and the outcome of the disease showed no significant difference between the nimodipine treated group and the control group except the delayed timing of surgery in the control group. There was no significant difference in the outcome between the nimodipine treated patients and the patients treated without nimodipine, however in Hung & Hess grade IV patients nimodipine treatment was associated with a significantly better outcome. Nimodipine treatment reduced the occurrence of delayed ischemic deficts(DID) in grade III, IV patients. Significant improvement in the outcome occurred in the nimodipine treated patients with subarachnoid hemorrhage of large amount(Fisher classification III).

The cyclophilins (Cyps) are family members of proteins that exhibit peptidylprolyl cis-trans isomerase (PPIase, EC 5.2.1.8) activity and bind the immunosuppressive agent cyclosprin A (CsA) in varying degrees. During the process of random sequencing of a cDNA library made from Giardia intestinalis WB strain, the cyclophilin gene (gicyp 1) was isolated. An open reading frame of gicyp 1 gene was 576 nucleotides, which corresponded to a translation product of 176 amino acids (Gicyp 1). The identity with other Cyps was about 58-71%. The 13 residues that constituted the CsA binding site of human cyclophilin were also detected in the amino acid sequence of Gicyp 1, including tryptophan residue essential for the drug binding. The single copy of the gicyp 1 gene was detected in the G. intestinalis chromosome by southern hybridization analysis. These radiologic studies were carried out on 265 cases of silicosis which were diagnosed clinically atindustrial Accident Hospital affiliated with Catholic Medical Collage, suring the period of 3 years from 1974 to1976. 265 cases of silicosis consisted of 96 cases (35%) of coal miner, 91 cases (34%) of coal choicer and 82cases (31%) of others. The average age was 42.2 years and average working period was 9.2years. Qualitative andquantitative features in the analysis of roentgen findings were based on UICC/Cincinnati and KLO classification.The qualitative features showed 26% of "p" opacity, 46% of "q" opacity 4% of "r" opacity as rounded profusion and5% of "s" opacity, 11% of "t" opacity, 2% of "u" opacity as irregular profusion of small opacities.Large opacitiesshowed 3% of group A, 2% of group B and 1% of group C. Quantitative features revealed 25% of Type 1, 54% of Type2, 16% of Type 3 and 5% of Type 4. The qualitative and quantitative features showed significant differences as ageand working period increased with age and working period. Major complications were pulmonary emphysema,tuberculosis and pleural abnormalities.

Ecologic studies are widely used in all fields of public health on account of accesibility of data. However, two problems related to these studies have been brought up. The first is ecological fallacy occurred in the course of interpreting the ecologic level of exposure-disease associations into individual level. The second is exposure isclassification which leads to serious bias. Nevertheless there is few methodologic study dealing joint effects of the two problems in ecologic study. This study was conducted to suggest an ecologic model not having an ecologic fallacy due to model linkage failure and a methodology for correcting the misclassification bias due to exposure-confounder misclassification. Finally, we suggest a criteria for the ecologic model selection. Main results are as follows: 1. A linear ecologic regression model has a serious ecological fallacy due to model linkage failure and the misclassification bias due to the exposure-confounder misclassification. 2. An interaction ecologic regression model has no ecological fallacy due to model linkage failure, but it is affected seriously by the exposure misclassification. However misclassification bias could be removed mathematically if the information related to the misclassification was known. 3. A log-linear ecologic regression model has an ecological fallacy due to model linkage failure. It is seriously biased as the individual risk ratio are increased, but relatively less affected by the exposure misclassification than interaction ecologic regression model. 4. One of the two ecologic regression model-interaction ecologic regression model and log-linear ecologic regression model- would be selected according to the information of individual risk ratio and exposure misclassification. But using a linear ecologic regression model should be avoided in any circumstance. The above results are only valid in case that there is no other source of ecological fallacy except model linkage failure. Also exposure and confounder are independent each other, measured binary, and having nondifferential misclassification. Since the above assumptions are somewhat strong in considering the real situations of ecologic studies, it is necessary to extend the scope of this study.

Since the placenta is an organ composed of blood vessels, it is not surprising that its primary neoplasm would be a vascular tumor. Placental tumors, primary or secondary, have been known to interfere with placental function. Chorioangioma(primary tumor of the placenta), which is the most common of them, occurs with an incidence for clinically significant ranges from 1~2.8:10000 births. These tumors are benign and are not usually associated with clinical sequelae unless they are larger than 5cm in long diameter. About one third of the large chorioangiomas may be associated with the maternal and fetal complications. For diagnosis of these lesions, the ultrasonography was used. If the chorioangioma is suspected, color doppler study is informative to confirm the presence of the vascular channels. We reviewed ultrasonograms and clinical records of seven patients who had been diagnosed as placental chrioangioma. The appropriate diagnostic tests and treatment can then be initiated in order to prolong gestation and decrease fetal mortality and morbidity.

In an attempt to evaluate the efficacy ef cimetidine in treatment of erosive gastritis, a praspective study wras done in 9 cases of heworrhagie gastritia, 4 cases yunctiform gastritis, 9 caees verrueous gastritia, and 9 caseis mixed type, Four hundred mg of cimetidine was given in the morning and at bedtime for 3 weeks. Evaluation ef the efficacy was assessed by clinical syinptoms acoring and by endoscopic findings of erosion before treatment, on the 8th day, on the 15th day, and on the 22th day of treatment in all patients. The results were as follows; 1) Clinical sytnptoms in patients with erosive gastritis were epigastric discamfortness(83. 9%), heart burn(80. 0%), indigestion(77, 6%), hunger pain(74.2%), belching(71.0%), anotexla (48.4%) and nausea(45.5%). There was no differenees of symptoms among the type of erosion. 2) Percent decrease in clinical eyxeptoms were; 45% on the 8th day, 68% on the 15th day, 81% on the 22th day of treatment. Ciinical symptoms in patients with punetiform gastritis lasted longer than other typea. 3) Disappearance of the gastric mucossl erosion was seen in 8 cases of hemorrhagic gastritis, 6 case of mixed type, 5 cases of verruceous gastritis, 2 cases of punctiform gastritis, and overall 21 Cases(67.7%) on the 22th day of treatment, endoscopieally. Faur eases af punctiform gastritis were transformed into verrueous gastritis during the treatment. 4) Disappearance of the erosion was observed more frequently in the mueosa of oxyntic area than that of antrum. This study suggests that cimetidine appears to be effective for relief of elinical symptoms in patients with erosive gastritis, and for treatment of hemorrhagic gastritis and erosions in the mucosa of oxyntic area,

Channels formed by the transient receptor potential (TRP) family of proteins have a variety of physiological functions. In the present study, we examined the localization of canonical transient receptor potential channels (TRPCs) in rat cerebellum. Twelve adult (4~6 month old) Sprague-Dawley rats were examined in this study. We performed immunohistochemistry using specific antibodies against TRPCs to investigate the detailed and comparative distribution of six TRPCs in rat cerebellum. There was a high density of TRPC1, TRPC3, TRPC4, TRPC5 and TRPC7, with a much lower density of TRPC6 in the rat cerebellar cortex. The somatodendritic Purkinje cell areas were intensely stained with antiTRPC3, TRPC4, TRPC5 or TRPC7 antibodies, whereas the staining intensity of TRPC6 was relatively low in the Purkinje cell bodies. In the cerebellar nuclei, the cell bodies of cerebellar output neurons showed moderate TRPC1, TRPC3, TRPC5 and TRPC7 immunoreactivities, while TRPC6 immunoreactivity was observed in the surrounding neuropil. This study showing the differential localizations of TRPC channels in the cerebellum may provide useful data for the future investigations on the structural and functional properties of TRPCs.
