1.The Genetic and Clinical Study of Pseudoxanthoma Elasticum.

A genetic and clinical study was done on 20 index cases and their families who had lived in Chonnam province, visited Chonnam University Hospital from 1968 through 1983, and were diagnosed as peudoxanthoma elasticum based on clinical and histopathological findings. In Chonnam province, pseudoxanthoma elsticum occurred in a incidence of more thar, one per 125,000. The male to female ratio was 1: 1.25 (8: 10) with only the cases of familial occurrence although the sex ratio was 1: l. 85 (7: 13) among 20 index cases. Among 8 index cases showing familial occurrence, 6 showed autosomal recessive inheritance with 5 Type 1 and 1 Type 2 whereas 2 showed the autosomal dominant inheritance with both of thern Type 2 (Pope), pointing out the genetic heterogeneity in pseudoxanthorna elsticum. The age of patients ranged from 12 to 42 years old at the time of their first visit to our hospital, but the age that the patients first recognized the disease onset ranged from 9 to 22 years old. The duration of t he disease was also variable ranging from 11/2 to 30 years. The angioid streaks were observed in 38g of the cases whose disease duration ranged from 11/2 to 20 years. The sites of skin lesions were the neck, axillae and groin in the order of frequency.

2.Diagnosis and Treatment of the Traumatic Knee Joint Injury

Sixty five cases of the knee joint injuries diagnosed and treated at the orthopedic department of Han-Il hospital during the period from January 1966 to December 1970, were analysed clinically. The results obtained were as follow: (A) Age, sex, and side of injury were reviewed. 1) Age ranged from five to fifty-eight years, with thirty-six of the patients falling in the twenty one to forty years old age group. The greatest incidence (twenty) was in the twenty one and thirty year old. 2) Fifty four patients were male and eleven were female. This reflected the degree of exposure to injury in the sexes. 3) No significance was attached to the fact that thirty patients injured right knees, twenty five injured left knees and ten injured both knees. The Most common site was medial collateral ligament. (40%) (B) In order to observe rentgenographic changes of the traumatic knee joint, artificial stress(varus and valgus) was applied during taking photograph. When the medial site injury exist, medial intercondylar length widen between both condyle of tibia and femur, angle enlarged in degree and the point that made by opposite intercondylar lines of the tibia and femur, deviated to lateral interarticuIar space from spine of tibia. When the lateral site injury exist, above mentioned process is not always apply to application. (C) The results of surgical and conservative treatment have been reviewed; 38 cases conservative and 37 cases operative. The results were successful in both. groups. Surgery is not always the treatment of choice for collateral ligament injuries.

3.Comparison of Biomechanical Stability of the Ilizarov External Fixation Methods for Treatment of Tibial Pilon Fractures

In order to compare mechanical stability of the Ilizarov external fixation methods for treatment of pilon fracture of the tibia, six human amputed tibiae were utilized to simulate Ruedi type I, II, and III fractures anatomically and non-anatomically reduced. Six fracture models which were either anatomically(3) or non-anatomically(3) reduced were first stailized by the Ilizarov construct composed of two tibial rings and foot assembly with connecting rods and ankle hinges and underwent axial load compression test using Instron. And then same tests were repeated on the models in which ankle hinges between the tibial rings and foot assembly were removed to examine biomechanical effect of the ankle hinges on the fracture stability. When the fracture model was fixed by the construct without ankle hinges, 720 N-780 N were required to disrupt fracture stability. When the fracture was non-anatomically reduced and was fixed by the construct without ankle hinges, 420 N-520 N were required to disrupt the fracture stability. The current study suggests that Ilizarov fixation system should have the construct composed of two tibial rings and foot assembly with connection rods and ankle hinges to prevent fracture destabilization during early ankle motion and weight-bearing in the tibial pilon fracture.

4.Clinical features of bee venom anaphylaxis.

Background and OBJECTIVE: There has been a few case reports of anaphylaxis due to honeybee in Korea. In order to observe the clinical feature of bee sting anaphylaxis. Moderials and methods: Six patients living in Kyunggi province area were referred under history of anaphylaxis after the bee sting. Atopy was defined by skin prick test result to common inhalant allergen. Serum specific IgE antibody to each bee antigen was detected by radioimmunoassay to identify the causative bee. RESULTS: All six cases were female. Three had atopy and four had combined allergic diseases such as allergic rhinitis, asthma, and urticaria. The etiologic bees consisted of yellow jacket (6 cases), paper wasp (4 cases), yellow hornet (3 cases), white faced hornet (1 case) and honey bee (1 case). Four cases had experienced anaphylaxis after ant bite and they showed positive result on specific IgE to imported fire ant. Specific immunotherapy against causative bee venom was begun using bee venom extracts from Bayer (USA) based upon results of specific IgE anti-body to bee venom. CONCLUSION: The yellow jacket is the most common cause of bee venom anaphylaxis in this area. Further studies will be needed to evaluate possible cross-reactivity between bee and ant venom.

Ipsilateral fractures of the femur neck and shaft show diagnostic difficulties and complexity of the treatment. This femoral double fracture, when not initially diagnosed, carries a much high risk of complications, such as aseptic necrosis of the femoral head. Early recognition is of importance. The definitive treatment of a fractured femur should be begun after the ipsilateral hip and the knee joint have been examined clinically and radiologically. At the Department of Orthopedic Surgery, Dae Han General Hospital, from July 1985 to March 1990, four cases of the concomitant ipsilateral femur neck and shaft fracture had been treated and followed-up for 2.4 years in average (ranging from 1.1 to 4.7 years).

9.Spinal Myoclonus on Lower Extremities Following Spinal Anesthesia or Subdural Administration of Intended Epidural Steroid Injection: Three cases reports.

Clonic movement is an extremely rare complication that occurs after neuroaxial blockade. We have experienced three patients showing intermittent clonic movement on both lower extremities following spinal anesthesia or subdural administration of intended epidural steroid injection.The first and the second patients, 13-year-old boy and 51-year-old man, experienced clonic movement following standard spinal anesthesia with hyperbaric tetracaine with epinephrine for operation on lower extremities. The third one, 24-year-old man, was an outpatient for the treatment of intractable radiculopathy, previously had an L4 laminectomy. His clonic movement may have resulted from the accidental subdural administration of an intended epidural injection of 0.5% lidocaine 6 ml containing triamcinolone 40 mg. Clonic movement has appeared as the occurrence of intermittent, short-timed (about 2~5 seconds), severe annoying, bilateral involuntary muscle contraction on lower extremities at the stage of recovery. It sustained for about 4~19 hours. The treatment was mainly sedation with sedatives and inhalation anesthetics, and partially muscle relaxants or anticholinesterase. They recovered without any sequelae.

10.New Fiduciary Plate and Orientation Marker for High Energy Radiation Therapy.

PURPOSE: A new fiduciary plate and orientation marker have been devised to assist the quality assurance (QA) procedures for port films in radiation therapy department. The plate is used in conjunction with the film/cassette combination during weekly QA procedures, at Seoul National University Hospital (SNUH), in order to verify treatment fields in high energy radiation therapy. MATERIALS AND METHODS: A new fiduciary plate was fabricated using an acrylic plate, cerrobend, standard blocking tray and mercury. The acrylic plate had the dimension of 1x25x25 cm, with two fiduciary markers. The plate was rigidly attached onto the standard blocking tray, thus making it easier to set the fiduciary plate to the center of the radiation field of the linear accelerator. The plate had two 2-mm vertical and horizontal lines, with the minor scales in 2-cm steps. The orientation marker was a small mercury filled disk, which was inserted into the plate. RESULTS: The geometrical structure of the lines in the plate makes it easier to correlate two different images between the simulation and port films. The marker clearly indicated the orientation of the film, for example, the anterior, posterior, left, right and various oblique orientations, without the placement of a conventional orientation marker. Also, the new orientation marker could easily be applied to the simulator by placing the small orientation marker onto the image intensifier or in front of the film/cassette holder. CONCLUSIONSs: The new fiduciary plate appears to be useful in verifying the treatment fields, and the new orientation marker makes the film orientation simple, which is expected to lower the block fabrication errors.
