1.Noduloulcerative syphilis.

A 26-year-old female presented herself to our clinic due to noduloulcerative lesions of one month duration on her face and trunk with accompaning headache and mild fever. In family history, her husband was reported to have a history of chaner. On physical examinat.ion, rice sized eythematous papules and bean sized nodules with central ulceration were seen on the face and trunk. VDRL was 1:31 positive. A skin biopsy iif a noduloulcerative lesion showed ulceration, vascular dilatation with fibrinoid changes and perivascular inflarnmatory cellu infilt.rations, especially plasma cells. Immunoperoxidase stain showed T. pallidum at the epidermis and papillary dermis. The diagnsis was made asecondary syphilis with unusual clinical presentation as noduloucerative skin lesions. The skin lesions completely disappeared one month after treatment with injection of benzathine pelnicillin G 2.4 million unit intramuscularly weekly for three times.

2.Early experience of hyperbaric oxygen therapy in radiation-induced cystitis.

The effects of hyperbaric oxygen therapy have been documented in 10 patients with radiation-induced cystitis refractory to conventional management. Seven patients were treated for 40-70 consecutive days, 90 minutes each day with 100 per cent oxygen at 2.4 atmospheres absolute pressure in a multiplace unit. Seven patients achieved the relief of gross hematuria and improvement of bladder irritative symptoms without side effect for an average 16.7 months (range 6 to 27 months). Hyperbaric oxygen therapy was stopped in three patients because of febrile sense in trunk, pneumonia and cancer recurrence. This experience suggests that hyperbaric oxygen therapy might be considered the primary treatment of patients with symptomatic radiation-induced cystitis.

3.Prostate specific antigen as a tumor marker for adenocarcinoma of the prostate.

Serum prostate specific antigen (PSA) and prostate acid phosphatase (PAP) were measured in 106 patients including 9 patients with prostate cancer, 63 patients with BPH. and 34 normal control group free of prostate disorders. The mean PSA value was 61.14+/-43.87 ng/ml for prostate cancer, 6.07+/-5.57 ng/ml for BPH and 1.42+/-0.98 ng/ml for normal control group, and the mean PAP value was 9.81+/-10.16 ng/ml for prostate cancer, 1.58+/-2.36ng/ml for BPH and 1.00+/-0.17 ng/ml for normal control group. The positive rates of PSA were 78% for prostate cancer, 22% for BPH and 0% for normal control group, and those of PAP were 67% for prostate cancer, 6% for BPH and 0% for normal control group. The sensitivity and specificity for PSA were 78%, respectively. We conclude that PSA may be a useful tool for detection and therapeutic monitoring of the prostatic cancer.

4.A case of true hermaphroditism with 46, XX/47, XXX karyotype.

Authors experienced a case of true hermaphroditism which was confirmed an ovary in the left abdomen and testis in the right scrotum with a karyotype of 46, XX,47, XXY and report this case with the brief review of literatures.

5.Two Cases of Malignant Melanoma in the Bulbar Conjunctiva.

Two cases of malignant melanoma which are located in the bulbar conjunctiva are presented. A case of 56 years old male revealed melanosis in his medial palpebral conjunctiva and wedge shaped resection was performed. The other case of 76 years old male had dark brownish strawberry like pedunclated mass in his lateral bulbar conjunctiva and limbal area with history of progressive enlargement. This tumor mass was excised radically. Above two cases were confirmed as malignant melanoma histopathologically.

6.A Clinical Study: Change of the Eye Position under General Anesthesia.

This study was undertaken to evaluate the relationship between eye positions in the awake state and in the surgical plane of anesthesia induced with anesthetics and muscle relaxants in 167 non-paralytic horizontal strabismus patients. All 27 patients with esotropia demonstrated divergence under general anesthesia when compared with their pre-anesthetized state. In 140 patients with exotropia, 107 patients(76.4%) demonstrated convergence under general anesthesia, 6 patients(4.3%) demonstrated divergence, 27 patients demonstrated no change of the eye position. A statistically significant relationship was found between the preanesthetized position of the eyes(P) and the eye position under general anesthesia(A). The relationship is linear and can be written as the approximate regression fomula: A=0.44P+6.34, R=0.73(p<0.01). When surgical success was defined as an eso- or exodeviation within 10 delta of orthotropia at postoperative 12 weeks, final success rate revealed statistically significant difference between the following two groups of exotropic patients(p<0.05). The patients whose amount of vergence were under 15 delta had a success rate of 96.6%, while those over 16 delta had a success rate of 86.3%. In esotropia, surgical success was not satisfactory in patients whose amount of deviation were over 41 delta. These results suggest the significant relationship between eye positions in the awake state and those under general anesthesia, which could be clinically applied in deciding the surgical amount and predkting the success of strabismus surgery.

7.Regulation of V-cadherin Expression on Human Dermal Microvascular Endothelial Cells.

BACKGROUND: Cadherins are a supergene family of structurally related transmembrane glycoprotein. V-cadherin was a subclass of cadherin which expressed on endothelial cell as an endothelial cell cell adhesion molecule. The role of V-cadherin is still unclear in inflarnmation and immunologic responses. OBJECTIVE: Our purpose was to demonstrate the expression and the regulation of V-cadherin expression on human dermal microvascular endothelial cells(HDMEC) by biological response modifier(BRM). METHODS: The cell surface expression of V-cadherin on HDMEC that was unstimulated or pretreated with IL-1, TNF, IFN-, and PMA was evaluated by immunofluorescence flow cytometry and the enzyme-linked immunosorbent assay. RESULTS: 1. V-cadherin was constitutively expressed on unstimulated HDMEC. 2. Maximal expression of V-cadherin was observed at 16 hrs after stimulation with IL-1, at 6 hrs after stimulation with TNF and at 24 hrs after stimulation with IFN- on HDMEC. 3. V-cadherin expression on HDMEC was increased after stimulation with IL-1 or PMA. CONCLUSION: V-cadherin was constitutively expressed on HDMEC, and V-cadherin expression can be regulated by BRM. These results suggest V-cadherin may contribute to inflammation.

8.Effects of Reactive Oxygen Species on Sperm Function, Lipid Peroxidation and DNA Fragmentation in Bovine Spermatozoa.

OBJECTIVE: To evaluate the effects of the reactive oxygen species (ROS) generated with a xanthine(X) and xanthine oxidase (XO) system on sperm function, the change of sperm characteristics, lipid peroxidation, and DNA fragmentation in bovine spermatozoa. MATERIALS AND METHODS: ROS were produced using a combination of 100 micrometer X and 50 mU/ml XO. The ROS scavengers: superoxide dismutase (SOD)(200mu/ml) and catalase (500mu/ml) were also tested. Spermatozoa were incubated for 2 hours in BWW medium with a combination of X-XO supplemented with or without ROS scavengers at 37degrees C under 5% CO2 incubator. Sperm movement characteristics by CASA (computer-aided sperm analysis), HOST (hypoosmotic swelling test), Ca-ionophore induced acrosome reaction, malondialdehyde formation for the analysis of lipid peroxidation, the percentage of DNA fragmentation using the method of TdT-mediated nick end labelling (TUNEL) by flow cytometry were determined after 2 hours incubation. RESULTS: The action of ROS on bovine spermatozoa resulted in a decreased in capacity for sperm motility, Ca-ionophore induced acrosome reaction and membrane integrity, an increased in malondialdehyde formation and the percentage of sperm with DNA fragmentation. In the effects of antioxidant, catalase completely alleviated the toxic effects induced by the ROS in terms of sperm function and characteristics, however SOD exhibited no capacity to reduce the toxic effects. CONCLUSION: The ROS can induce significant damages to sperm functions and characteristics. The useful ROS scavengers can minimized the defects of sperm function and various damages of spermatozoa.

10.Cyclic Esotropia Following Ocular Surgery.

The cause of cyclic strabismus is not clear but is assumed to be an abnormality of the biological clock mechanism. Secondary factors, combined with surgery or trauma as well as unknown causes stimulate its occurrence and may be followed by the periodicity of strabismus. We report postoperative follow-up results of 4 cases of cyclic esotropia following surgery. Two cases (case 1 and 2), who developed cyclic esotropia after surgery for intermittent exotropia, show good postoperative alignment without periodicity. But in case 3, originally treated for intermittent exotropia after receiving treatment for consecutive esotropia, the patient reverted to original condition. In case 4, originally treated for congenital glaucoma and receiving secondary treatment for cyclic esotropia, the cyclic esotropia was recurred. After surgery for consecutive cyclic strabismus, it is necessary to follow up for a long time.
