1.Treatment of 212 patients with limited-disease small cell lung cancer

Objective To review the treatment effect of limited disease small cell lung cancer (LD-SCLC) and to evaluate the prognosis factors for SCLC. Methods From Aug. 2002 to Feb. 2006, 212 patients of SCLC confirmed with pathology and cytology were. Treated by combined modality. All patients were treated with chemotherapy 2-6 cycles, 59 patients of which were treated with only chemotherapy, 108 patients of which were treated with chemotherapy plus radiotherapy, 45 patients of which were treated with surgery plus chemotherapy with or without radiotherapy. The patients of radiotherapy were exposed to 6 or 10 MV X-ray with a total dose of 40-66 Gy in 4-7 weeks. Results The overall median survival time was 15 months. The 1-,2- and 3-year overall survival rate were 58.0%, 33.2% and 22.1%, respectively. Univariate analysis indicated that weight loss, age, LDH, ECOG performance status, operation, response to radio-chemotherapy, cycles for chemotherapy, radiotherapy irflueoced survival significantly in LD-SCLC. Multivariate analysis suggested that ECOG performance status, response to therapy, cycles for chemotherapy were the independent prognostic factors for LD-SCLC. Conclusions For LD-SCLC patients, ECOG performance ≤ 1, good response to radio-chemotherapy, and ≥ 4 cycles chemotherapy show encouraging survival rate, Distance metastasis is still the mainstay of treatment failure.
2.Clinical application of pedicled flap supplied by radial osteo cutaneous branch of anterior interosseous artery

Objective To expore the method of clinical application and the final result of pedicled flap supplied by radial osteo cutaneous branch of anterior interosseous artery Methods Basing on the anatomic basis that the osteo cutaneous and myocutaneous branches of the anterior interosseous artery have constant collateral anastomoses which can give forth a large size dorso radial flap on dorsum of the forearm Four cases of skin defect of first web had been reconstructed by this flap Results All 4 cases were successful with good functional and cosmetic results Conclusion Reconstruction with dorso radial flap of forearm is simple,reliable procedure which bring minimum trauma to the patient and is an ideal alternative operation
3.Dose calibration of high-energy electron for linear accelerator

Objective To calibrate the dose of 12MeV high-energy electron for Siemens MD7745 medical linear accelerator.Methods The international protocols,contents and technical standards were expatiated.Some existing problems in dose calibration of high-energy electron were analyzed.Results The working precision met the requirement of quality assurance in radiotherapy by WHO.Conclusion In order to improve the therapeutic effect,the dose must be calibrated,and the error in dose calibration of high-energy electron should be reduced to the lowest.

5.Association of R405Q polymorphism of GLI1 gene with tetralogy of fallot

ObjectiveTo investigate the association between R405Q polymorphism of GLI1 gene and tetralogy of fallot(TOF).Methods In the case-control study,the R405Q polymorphism of GLI1 gene in 112 children with TOF and 200 healthy controls were detected with polymerase chain reaction-restriction fragment length polymorphism(PCR-RFLP).The distribution of genotype and allele frequency at R405Q polymorphism site were analyzed and to investigate its relationship with the risk of TOF.ResultsThe distribution of genotype frequency at R405Q polymorphism site was not different between TOF group and the healthy control group(x2 =5.317 ,P = 0.07) .However, the distribution of allele frequency at R405Q polymorphism site was significantly different between TOF group and the healthy control group (x2 = 6.790, P = 0.009) , and the relative risk for TOF in A allele carriers was higher than that in G allele carriers (OR = 1.561,95% CI 1.116 ~ 2.185) Conclusion The R405Q polymorphism of GLI1 gene is associated with TOF and people with A allele have higher risk with TOF.GLI1 gene might be the genetic susceptibility gene of TOF.
6.A study on clinical characteristics for T-cell acute lymphoblastic leukemia with SIL-TAL1 fusion gene

Objective To investigate the clinical characteristics and outcome of T-cell acute lymphoblastic leukemia (T-ALL) with SIL-TAL1 rearrangement.Methods 62 newly diagnosed T-ALL patients including 15 patients with SIL-TAL1 rearrangement were systemically reviewed.Results Compared with SIL-TAL1-T-ALL patients,SIL-TAL1 + T-ALL patients was characterized by higher white blood cell count (P =0.029) at diagnosis,predominant cortical T-ALL immunophenotype (P =0.028) of the leukemic blasts,a higher prevalence of acute tumor lysis syndrome (P < 0.001) and disseminated intravascular coagulation (P < 0.001),which led to a higher early mortality (26.7 % (4/15) vs 4.3 % (2/47),P =0.011).Compared with SIL-TAL1-patients,SIL-TAL1+ patients had shorter relapse free survival (2 months vs 12 months,P =0.007) and overall survival (4 months vs 25 months,P =0.002).Conclusion SIL-TAL1 rearrangement identifies a distinct subtype with inferior outcome which could allow for individual therapeutic stratification for T-ALL patients.
7.Reconstruction of contracture of the first web space with snuff-box flap

Objective To summarize and investigate the therapeutic effects of reconstruction of contracture of the first web space with snuff-box flap. Methods Eighteen patients with contracture of the first web space were treated by snuff-box flap. The width and the angle of the first web space was 19 mm and 20°on average. According to the first web space skin defects, the flap designed to nasopharyngeal fossa centers as rotation point, the radial line when the forearm in the neutral position as the axis. Results The followedup for 5-26 months revealed that 17 flaps had a success, but 1 case was partly necrosis. The width of the first web space was augmented by an aveage of 45 mm, the angle of the first web space was augmented by an average of 50°. Conclusion It is ralatively simple and reliable to repair the contracture of the first web space using the snuff-box flap.
8.Aesthetic reconstruction of distal finger or middle-distal finger degment with transplantation of 2nd, 3rd or 4th toe

Objective To report the results of aesthetic reconstruction of distal finger or middle-distal finger segment with transplant harvested from 2nd, 3rd or 4th toe. Methods In a series of 108 cases of distal finger or middle-distal finger segment defect, 141 fingers were reconstructed with transplants harvested from 2nd, 3rd or 4th toe according to the outward appearance of donor toe. The blood-supply was restored by anastomosis of digital artery and vein. In first-stage reconstruction, refinement of outward appearance was applied in doner toe if the toe pulp is bulky or middle part of toe was relatively thinner. Results One hundred and forty fingers survived. The successful rate was 99.3%. Among them, 81 patients had been followed up from 1 to 10 years (mean 3.5years) postoperatively. 61 cases were graded to be excellent and 16 eases good as assessed with Criterion of Functional Evaluation on Finger Reconstruction issued by Chinese Society of Hand Surgery. Conclusion For aesthetic reconstruction of distal finger or middle-distal finger segment, the 2nd toe is usually considered to be donor of first choice. In case the outward appearance of 2nd toe was inferior to that of 3rd or 4th toe, the 3rd or 4th toe should be chosen for better result. In some cases, revisional aesthetic surgery were necessary to refine the outward appearance in first-stage reconstruction.
9.Apoptosis of K-562 cells induced by oleum curcumae wenchowensis and its mechanisms

Purpose To investigate the effects and the mechanism of oleum curcumae wenchowensis (OCW) with different concentrations (0, 2.5, 5, 10 and 20 mg/mL) on chronic myeloid leukemia cell line K-562 cells in vitro.Methods The apoptosis of K-562 cells was dyed by Hoechest 33258 and detected by flow cytometry marked with Annexin V/PI. The Expression of Fas/FasL, bcr/abl, bcl-2 and p53 was detected by semi-quantity reverse transcript-polymerase chain reaction (RT-PCR) and Western blot.Results The results showed that the apoptosis rates were gradually elevated. The expression of Fas and FasL protein was increased in a concentration dependent manner, while bcr/abl, bcl-2 and p53 had no significant changes.Conclusion OCW could induce the apoptosis of K-562 cells by up-regulating the expression of Fas/FasL protein.
10.Small size toe flap repair tissue defect of thumb and ringer

Objective To explore small size toe tissue flap for aesthetic reconstruction of the thumb and / or finger. Methods Six kinds of small size toe tissue transplants had been applied in repairing skin-bone-joint composite tissue defects of the thumb or finger in 74 cases. Results Among 83reconstructed flaps of the 74 patients, 81 flaps survived completely. Follow-up examination made three to forty-eight months postoperatively showed that the outward appearance were excellent in most cases. The function of the thumbs or fingers were good. The donor feet can walk normally with no pain. Conclusion A variable combinations of toe tissues including skin, soft tissue, bone and joint can be harvested to form a lot of small size transplants for refined aesthetic reconstruction of thumb and finger. The functional and aesthetic results are good and the treatment course is shortened.
