As increasing numbers of antibiotics, adrenocorticosteroids, and antitumor agents have come into use, so increasing numbars of mycotic diseases have been identified. Qtomycosis, a mycotic infection of the extcrnal ear which is occasionally accompanied by dermatitis of the surrounding skin, is corsidered to be such a disease. While opinion vari.es as to whether otomycosis is caused by a saprophytic or a pathogenic organism, our clinical obs:rvations revealed pathological inflammation of the external auditory canal with profuse fungal elements. During August 1975, l3 specimens of ear wax were taken from patients whv visited the out-patient ENT clinic of this hospital. All patients had had their ears clea,ned at a barber shop prior to visiting the ENT clinic. Because ar cleaning instrumenta (scoops and brushes) are re-used m Korean Sarber shops, 204 of theae in- strum.ent;s were randomly selected from different barber shops and examined. KGH mounts and Lactophenol cot@ten blue stains were used to examine the specimens. Qf the lR ear wax specimens, I2 samples (92. 3%) reveaIed Aspergillus sp. and 1 sample (7. 7%) revealed Penicillium sp. Of the 204 ear cleaning instrumenta tested, l94 colonies grew in culture. Of these cultures, 146 (75. 3 %) were Aspergillus sp., 9 (4. 6%) were Mycelia sterila, 5 (2. 6%) were Fenicillium sp., 4 (2. 1 %) wr Al- terna,ria, 2 (l. 0%) were Stemphylium sp., 1 (0. 5%) was Geotrichum sp., and 27 (13. 9%) were of an unknown species. Of the 146 Aspergillius sp. cultures, 99 (67. 8%) were Asp. terreus, 17 (11.6%) were Asp. glaucus, 5 (3.4%) were Asp. niger, 17 (11. 6%) were Asp. flavus-oryzRC, R (2. 1 %) Were Asp. versicolor, and 5 (3. 4 %) were athers. Beca.use of the close correlation between the fungi cultivated from ear wax apc imens and those cultured from ea,r cleaning instruments, the authors believe that the ear cleaning instruments used in the barber shops could well have been the primary source of infection. In this study the authors also discuss the probable causative factbrs of otomycosis arrd a review of the literature is presented.

Dermatophytosis of the feet, i.e. tinea pedis, is one of the most frequent fungal infections and is caused by a dermatophyte. There are many predisposing factors to the disease such as living or environmental conditions; underlying endocrine or metabolic diseases, complicated individual immune mechanisms, long-term use of adrenocorticosteroids, antibiotics or other drugs and feet, per se, succeptable to infection of the organism by physio-anatomical condition. The authors performed clinical and mycological studies on 225 outpatients visiting the dermatological clinic of Chonnam University Hospital from April to September in 1977, who had clinical evidence of tinea pedis, dermatophytes infection of the feet. The incidence of tinea pedis was 8.9% of the whole 2531 out-patients who visited our clinic during that time. Of the 225 patients, the male patients were53%, the female patients 47%, and patients living in urban areas were affected about 2 times more frequently than those living in rural areas. The age distribution of the patients varied from 5 to 68 years old, but the most of them were in their third decade, at an actively working age. Concurreat infection with tinea pedis and other dermatophytosis was noted in 106 patients (47.1%). The patients with T. pedis and manus were most commonly observed, and the incidence of concurrent infection with T. pedis and T. manus was 31%;, T. cruris 19.8%, onychomycosis 17.0%, and with T. corporis 10.4% respectively. Id reaction or autoeczemation to the disease itself and inadequate or over treatment dermatitis developed in 19 patient*8.4%). It happened more in patients with tinea pedis alone than in thos with concurrent infection, but there was no difference between sexes. The positive rate of 10%. KOH mount was 59% in 173 preparations. The culture rate was about 7 times higher in the KOH positive group than in the negative group. Among the 39 strains of dermatophytes isolated by Sabouraud's dextrose agermedia from 111 patients and identified by 1% dextrose com meal agar media, bloo3 agar media and hair culture method, T. rubrum were 28 strains (71.l%) and T. mentagrophytes were 11 strains (28. 2%) The straivs of all noted hair perforation did not produce the pigmentation on 1% dextrose com meal agar media, and the strains which produced pigmentation on 1% dextrose com meal agar media did not perforate hair in hair culture. We cultured fungi from each lesion of 3 patients with concurrent infection of T.pedis, T.cruris, T.corporis, and dbtained T.rubrum only in all lesions of each patient. Upon making this observation, we presumed concurrent infection generally to be autoinoculation from one to another part.

PURPOSE: To evaluate the effectiveness and the most accurate element of the diagnostic criteria of the Japan Society of Uitrasonics in medicine(JSUM, Nov. 1989) for distinguishing between benign and malignant solid breast masses on the US. METHODS AND MATERIALS: We analyzed the ultrasonic findings of histopathologically proved 51 fibroadenomas, 12 fibrocystic disease, and 39 breast cancers in relation to the diagnostic criteria of the JSUM (shape, border, boundary echo, internal echo, posterior echo, lateral echo, and depth/width ratio). RESULTS: The number of cases of fibroadenoma, fibrocystic disease, and breast cancer corresponding to the diagnostic criteria was in the shape(26/51, 5/12, 33/39), border(41/51, 9/12, 29/39), boundary echo(48/51, 12/12, 27/39), internal echo(43/51, 9/12, 24/39), posterior echo(32/51, 3/12, 21/39), and lateral shadowing(15/51, 1/12, 35/39). All diagnostic criterias showed statistical significance for differentiation of benign/malignant breast mass on the US(Chi-square test: p<0.05). The order of accuracy was boundary echo, internal echo, and border. The mean of depth/width ratio was 0.54 +/- 0.15, 0.52 +/- 0.12, and 0.69 +/- 0.21 in fibroadenoma, fibrocystic disease, and breast cancer respectively and it had statistical significance for differentiation of benign/malignant breast mass on the US(ANOVA test: p=0.0002). CONCLUSION: The diagnostic criteria of JSUM is effective for differentiation of benign/malignant breast solid masses on the US and has accuracy in the order of boundary echo, internal echo, and border. Depth/width ratio also has statistical significance.

Splenic artery pseudoaneurysm is a relatively rare and potentially life-threatening complication of chronic pancreatitis. The authors present a case of splenic artery pseudoaneurysm complicating ,chronic pancreatitis. It was converting into a pseudoaneurysm by vessel rupturs. In this case report, color doppler US, CT, and MRI made the definite diagnosis.

In a large proportion of patients with chronic urticaria, at present, the definite tiology can't be found. There have been occasional attempts to identify the aIIergic causes uaing allergic skin tests, only ta suggest the Candicla albicans and. some food yeasts ae probabIe causes. Thia atady was designed to investgate how often patients with chronic urticaria from Chonnam Province react positively to skin tests with extracts of 42 common. allergene in the standard group Al-13 produced by Bencard Ltd. in England. The authora alao surveyed the clinical characteristics of 63 patients with chronic urtimria of more than 3 months duration, who bad visited the skin clinic of Chosun Univeresity HcepitaI from May. 1977 throug 3. Apr. 1979. The results were summarized as follows; 1. Among 63 patients, the maIe to female ratio was 1: l.2, and the peak incidence was in the 20s and 30s. 2, In regard to the duration of chroic urticaria, the highst frequency waa below one year duration. 3. Among 60s of the pa,tients who experienced seasonal changes, 32.4% had suffered aggravation during the winter. 4. The most common diseases combined with chronic urticaria were allergic rhinitis and atopic dermatitis, occuring in 9.5% of all subjects. 5. Of thase examined, 69.8% showed positive reactions to the prick test with the peak incidence in the 20s and 30s. 6. Among 42 allergens used far the prick test, 28 allergens turned out to be possibly casal agents, the rnost frequent group of allergens involved were grain du sts (27.1%), animal hairs(21.8%) and moulds(16.5%) in that order of frequency and occupying over 65% of the total positive reactions. And the most frequently involved specific allergens were hay dust, mixed grain dust straw dust and tree in that order of frequency. Moreover, 90 of the involved allergens were inhalants. 7. In teen-agers, the reactions to grain dusts, animal ha.irs, rnoulds and pollens were the rnost frequent while reactions to food allergens were most frequent in the 20s, and cotton and house dust are most frequently involved in tbe 40s. 8. Multiple sensitivity was not:d in a majority of cases and the most frequent combination was 2 to 5 allergens. 9. The rnost common abnormal hematologic finding in chronic urticaria was eosinophilia.

A case of Ramsay Hunt syndrome, though more often seen at the ENT clinic because of the patients' concern with symptoms in the ear, was seen and treated at the Dermatologic Clinic of Chosun University. A 29 year old male patient presented earache, pinhead to rice sized erythematous vesicular eruption on the external auditory canal, auricle and superior auricular area, and facial paralysis, all involving the right side of the face, duration of which were 7 days, 5 days and 1 day respectiveIy. High doses of prednisolone alone made it possible to remit the otalgia, control the vesicular eruption and resolute the facial paralysis almost completely within 18 days in the order of their appearance, indicating that this is one of the more easily treated diseases for dermatologists, The 22 cases of this syndrome reported in Korean literature were reviewed and the conceptual development of its pathogenesis and definition was described.

To evaluate of the relationship between basal ganglia lesion and portal-systemic encephalopathy, eleven patients who had clinically proved liver cirrhosis with superior mesenteric vein larger than 10mm in diameter on ultrasonogram underwent brain MR imaging. No evidence of clinical or neuropsychiatric disturbance was observed in any patient at the time of the MR examination. Brain MR imaging revealed basal ganglia lesion characterized by bilateral, symmetric, high signal intensity without edema or mass effect on spin echo T1-weighted images in nine patients which included three patients with the past history of portal-systemic encephalopathy. It was concluded that excepted in the circumstances of other causes of the high signal intensity in basal ganglia on T1-weighted images such as fat, methemoglobin, melanin, neurofibromatosis, dense calcification, and parenteral nutrition, bilateral and symmetric high signal intensity lesion, in basal ganglia would be a useful MR finding of subclinical portal-systemic encephalopathy in liver cirrhosis patients with no clinical or neuropsychiatric symptoms and larger than 10mm diameter of superior mesenteric vein in ultrasonography.

Recently developed new classifications (Holland, Van Nuys, modified Lagios) of ductal carcinoma in situ (DCIS) linked to outcome have emphasized the importance of nuclear morphology rather than architecture. We have evaluated these three classifications in ductal carcinomas composed of in situ and invasive carcinomas. The reproducibility of three classifications was assessed (n=49), and the histological grade of the DCIS was compared with the histologic differentiation (modified Bloom & Richardson method) and nuclear grade (modified Black method) of the coexisting invasive ductal carcinoma (n=45). According to Holland classification, the DCIS component was poorly differentiated in 51.0%, intermediately differentiated in 40.8%, and well differentiated in 8.2%. Using the Van Nuys classification, the DCIS component was group 3 (high grade with or without necrosis) in 44.9%, group 2 (non-high grade with necrosis) in 28.6%, and group 1 (non-high grade without necrosis) in 26.5%. According to the modified Lagios classification, the DCIS component was high-grade in 42.8%, intermediate-grade in 32.7%, and low-grade in 24.5%. The histologic grades of the three classifications revealed significant correlations between Holland and Van Nuys classification (p<0.0001) and between Holland and modified Lagios classification (p<0.0001), especially in poorly differentiated/group 3/high-grade DCIS. The reproducibility of classification of the DCIS was 71.4% in the Holland, 61.2% in the Van Nuys, and 55.1% in the modified Lagios classifications. The grade of the DCIS showed significant correlation with the grade of coexisting invasive ductal carcinoma (p<0.0001), especially in poorly differentiated/group 3/high-grade DCIS. In conclusion, DCIS grade, determined by the Holland, Van Nuys or modified Lagios classifications, is closely correlated with the histologic grade of the invasive ductal component in tumors composed of in situ and invasive ductal carcinoma, and may be a useful factor to estimate clinical behavior of DCIS. In our experience the Holland classification is recommended for DCIS classification due to its high reproducibility.
