1.Commercial Sex Workers'Condom Use Behavior in Korea.

PURPOSE: With the continuous spread of the AIDS virus and other sexually transmitted diseases (STDs) throughout Asia, it has become imperative for HIV/AIDS prevention outreach programs to stress the need for using prophylactics as one form of protection. This is particularly true for commercial sex workers. This pilot study was conducted to investigate the rate of condom use among the commercial sex workers in Korea and reasons why they frequently do not wear condoms during sexual intercourse. METHODS: Fifty female commercial sex workers who worked in the Yong San area were recruited while outreach was performed and forty eight responses were analyzed due to two incomplete questionnaires. The data were collected on June 28, 2004 as part of an outreach program for condom use promotion. RESULTS: The subjects ranged in age from 21 to 45 years. Nearly half of the subjects (47.9%) did not consistently use condoms. The major reason for not using protection was clients'reluctances for using condoms. When condoms were used, the subjects preferred lubricant-rich and less painful condom during intercourse. CONCLUSION: Due to the inconsistent use of condoms, it is clear that many commercial sex workers and clients are susceptible to sexually transmitted diseases. Therefore, it is important to not only educate commercial sex workers, but also to educate their pimps and clientele about the importance of condom use. In general, it is recommended that practical strategies and a policy for condom use promotion be developed.

2.Influence of Experiences of Witnessing Tobacco Advertising and Preferences of Tobacco Companies' Social Responsibility on Current and Future Smoking Intentions in Adolescents.

PURPOSE: To evaluate the influence of experiences of witnessing tobacco advertising and preferences of tobacco companies' social responsibility (CSR) on current and future smoking intentions in adolescents. METHODS: Data were collected from 700 adolescents living in Korea representing 7 metropolitan cities and 8 provinces in the method of proportional random sampling. The Gallup's data collection system was adopted using 1:1 face to face interviews. The witness of tobacco advertising was determined by 'yes', or 'no' responses to three survey items and to four survey items for preferences of tobacco CSR. For statistical analysis, chi2 test, Fisher's exact test and logistic regression were used. RESULTS: 98.4% of the adolescents had experiences of witnessing tobacco advertising. POP (OR=103.44, 95% CI: 8.22~1301.45) and magazine advertisement (OR=6.07, 95% CI: 1.34~22.58) had a significant effect on their current smoking. Also, POP (beta=.24, p<.001) and movie advertisement (beta=.42, p<.001) had a significant effect on their future smoking intentions. As for the preferences of tobacco CSR, cultural-art support activities (beta=.15, p=.025) and environmental purifying campaigns (beta=-.15, p=.034) had a significant effect on their future smoking intentions. CONCLUSION: Witness of tobacco advertising and positive responses to tobacco CSR are associated with current and future smoking intentions in adolescents.

3.Development and Application of a Community-based Meal Program Model for the Elderly in Rural Area.

Improved nutritional intake contributes to maintaining health and quality of life in elderly population and also reducing individual and social medical costs. Most of nutrition assistance programs for elderly, such as congregate or home-delivered meal programs, are not currently serviced in rural communities mainly due to low cost efficiency of program operation. However, the needs and necessity of such programs are presumed to be higher in rural area where the population density of elderly at nutritional risk is relatively high. Therefore, the purpose of this study was to develop a community-based meal program for the rural elderly. In 2007, four rural communities located in Jeon-Nam province were selected and the pilot meal program was applied for three months. Following are key features of the meal program model developed in this study: 1) meal production and service are operated by elderly participants to overcome the voluntary personnel shortage, 2) utilization of locally-produced foods is maximized to reduce the meal cost, 3) traditional cooking methods are applied to adjust the food preference of elderly, and 4) foods are serviced on site to minimize the food safety problem possibly caused by delivery process. The pilot programs resulted in high satisfaction with the programs of participating elderly. The community-based meal program model developed in this study is expected to be used as an effective nutrition and health intervention model for the rural elderly.

5.Immunochemical Study on the Changes of Carbonic anhydrase-II and Iron-binding Proteins in the Demyelinationand and Remyelination model Mouse induced with Cuprizone.

O1igodendrocytes are known to be responsible for the synthesis and maintenance of myelin sheath in the central nervous system, and their functional disturbance leads to defect in myelination. But, the fine mechanism of myelination by oligodendrocytes is not yet known, and iron metabolism in central nervous system is suspected to be related with myelination process by oligodendrocytes. Carbonic anhydrase-II[CA-II], transfe-rrin, and ferritin are known to be present at oligodendrocytes and suspected to play a role in iron metabolism of central nervous system. In this study, demyelination and remyelination of ICR mouse brains were induced using cuprizone, the copper-chelating agent, and immunohistochemical changes of CA-II-, transferrin-, and ferritin-immunoreactive oligodendrocytes at corpus callosum were observed. During demyelination by cuprizone feeding, the numbers of CA-II- and transferrin-immunoreactive oligodendrocytes were decreased. Especially, the decrease ratio of CA-II-positive cells was great. In contrast, the number of ferritin-positive oligodendrocytes was increased during demyelination by cuprizone feeding. Cessation of cuprizone feeding leaded remyelination and the numbers of CA-II-, transferrin-, and ferritin-immunoreactive oligodendrocytes were returned to normal level. In conclusion, the derangement of iron metabolism in oligodendrocytes may be related to demyelination mechanism of central nervous system, and the CA-II is suspected to have an important role in iron metabolism of oligodenrocytes in relation to demyelination and remyelination induced with cuprizone.

According to the donor shortage, the marginal donor should be concerned as one of the potential kidney sources. The pediatric kidney could be transplanted to adult recipient, but it is well known that small functioning nephron mass could cause the late graft dysfunction according to the hyperfiltration syndrome. Here, we report a case of successful kidney transplantation in adult using en bloc kidneys from a child with good long term graft function.

9.Pulsed-Field Gel Electrophoresis Pattern, Phage Type and Drug Susceptibility of Salmonella typhi Isolates from an Outbreak in Pusan City in 1996.

BACKGROUND: During the period from April to June 1996, an outbreak of typhoid fever occurred in SuYoung-Ku, Pusan city. We performed the molecular epidemiological analysis and drug susceptibility test for 48 isolates of Salmonella typhi from the outbreak. METHODS: Chromosomal DNA of S. typhi was digested with the restriction endonuclease Spe I and the resulting restriction polymorphism was observed on pulsed field gel electrophoresis (PFGE). Phage typing was performed using Vi-phages, and antimicrobial susceptibility test was done by Disc diffusion method. RESULTS: On phage typing, 2 isolates were M1 phage type, 2 E1 phage type and 44 untypable. The PFGE analysis on 14 to 16 chromosomal DNA restriction fragments ranged from 50 Kb to 1000kb showed 2 different patterns of restriction fragments, divided into group A and B. Forty-seven isolates belonged to group A that were allocated into A1 (42 strains), A2 (1 strain), A3 (3 strains), A4 (1 strain), and one strain was grouped into group B. In the F-value of genetic similarity of the epidemic S. typhi strains, there was low similarity (F value:0.2-0.258) between group A strains and group B strains. However, the subgroup strains (A1-A4) showed high similarity (F value:0.8-0.897) each other. The drug susceptibility test showed susceptibility to ampicillin, carbenicillin, cephalothin, chloramphenicol, tobramycin, gentamicin, kanamycin, nalidixic acid, neomycin, polymyxin B, streptomycin, tetracycline, trimethoprim-sulfamethoxazole, ciprofloxacin and norfloxacin. CONCLUSION: This study showed that phage typing and PFGE were very useful as a tool to investigate molecular analyses of epidemic S. typhi strains this study.
