1.Grafting of vascularized fibula and ilium to repair bone defects of limbs

Objective To study the clinical results of grafting of vascularized fibular and ilium to repair bone defects of limbs. Methods From January 1992 to January 2004, 49 cases of bone defects of limbs were treated with vascularized fibular grafting (33 cases) and vascularized iliac grafting (16 cases). Their limb functions were graded according to Ennekings system. The changes of the vascularized fibular and iliac grafts were evaluated radiographically according to the International Symposium on Limb Salvage. Results All the patients were followed up for 1 to 12 years. In the vascularized fibular group, the recovery rate for the operated limbs was 82.7%, the average healing time of the bone was 16.8 weeks and the healing rate of the bone was 87.9%. In the other group, the recovery rate of the operated limbs was 88%, the average healing time of the bone was 15.2 weeks and the healing rate of the bone was 93.8%. Conclusion The vascularized fibular graft can be adopted to repair long bone defects of lower limbs and distal radius, while the vascularized iliac graft can be adopted to repair the bone defects of upper limbs.

Objective To study the effect of the PDCA management on safety management of venous transfusion by indwelling needles?.Method The measures included standardizing nurses manipulation,improving the management system related to venous transfusion,and regulating details management process for tube fixing and dressing and tube maintenance.Results The rates of unstandardized maintenance and obstruction of tubes after the enforcement of details management were significantly lower than those before its enforcement(P<0?05).Conclusions PDCA management is effective for standardizing nursing implementation and ensuring the safety of venous transfusion by indwelling needle?
3.Application of STR-PCR in monitoring the effect of allogeneic hematopoietic stem cell transplantation

Objective To establish multiple short tandem repeat (STR) amplification by fluorescence labeling polymerase chain reaction (PCR) for monitoring the effect of allogeneic hematopoietic stem cell transplantation .Methods Sixty‐five patients were analyzed .DNA from peripheral blood of donors and recipients in pre transplantation and post transplantation were extracted , 15 STR loci and sexual loci were amplified by PCR .Results After allo‐HSCT ,54 patients obtained type of donors ,but 3 patients did not ;eight patients showed mixed chimerism .Two cases of type of donor converted into mixed chimerism and two cases of mixed chimerism converted into type of donors after some time .The time of earliest detection ,duration and transformation of each state was different .The earliest detection showed on the 16th day after surgery ,and the last one showed five months later .As to the du‐ration ,the shortest and longest were two months and seven months ,respectively .Conclusion The key factors that significantly in‐fluence the application of STR‐PCR in monitoring the effect of allogeneic hematopoietic stem cell transplantation were samples ,STR loci and proper monitoring time .So to establish the suitable method for this region could truly take maximum advantage of the PCR‐STR method .While a appropriate detecting time and STR loci should be chose .
4.Clinical studies of the low-intensity anticoagulation of Warfarin after heart valve replacement

Objective To investigate the necessity of low-intensity anticoagulation standard in patients after heart valve replacement and the rationality of INR in our hospital.Methods 681 eligible candidates were anticoagulated under the current guidelines for postoperative anticoagulation therapy in our hospital(AVR 1.5-2.0,MVR 2.0-2.5,DVR 2.0-2.5,TVR 2.5-3.0).We monitored the patient 's PT regularly and analyzed the occurrence of anticoagulation-related complications,such as bleeding,thrombosis and embolism.Results 602 cases completed the follow-up.During the period of follow-up,66 patients had bleeding tendencies,the incidence of bleeding complications was 10.96% (66/602).1 1 patients had embolism complications,the incidence of thrombotic complications was 1.83 % (11/602).The average of INR was 2.24± 0.68,the mean oral Warfarin dose was(3.12± 1.14) mg/d.Conclusion Our study suggest that the effect of low-intensity anticoagulation after heart valve replacement is reliable.Further more,the current anticoagulation standards of our hospital meet the requirements of postoperative clinical anticoagulant after heart valve replacement in our region.
5.Changes of Magnetoencephalography during Stretching and Flexing Forefinger under Optical Instructions in One Hemiplegic Patient before and after Rehabilitation Training

Objective To observe the changes of magnetoencephalography (MEG) during stretching and flexing forefinger in one hemiplegic patient before and after rehabilitation training.MethodsThe cerebral electromagnetic wave of one hemiplegic patient during stretching and flexing both forefingers was recorded by MEG and superposed with magnetic resonance imaging (MRI) to form magnetic source imaging (MSI). The changes of MEG before and after rehabilitation training were analyzed.ResultsThere was no movement evoking magnetic fields in right hemisphere motor cortex at two MSI detections, but detected in left hemisphere motor cortex. The latent period of the first and the second detection was -34.2 ms and -61.7 ms respectively. The exiting motor cortex was located in precentral gyrus. The exiting motor cortex at the second detection was located more front medial and low than at the first detection. The volume of the exiting motor cortex (9569.6 m3) at the second detection was more larger than the first detection (2309.7 m3). There was no movement evoking somatosensory magnetic fields in right hemisphere motor cortex at first MSI detection, but found at the second detection, the latent period was 91.1 ms, and the exiting cortex was located in postcentral gyrus.ConclusionThe cortex somatosensory function of patient with stroke recovers early than the motor function and the uninjured hemisphere function can improve obviously after rehabilitation training.
6.Study on HLA-A,B,DRB1 high-resolution alleles polymorphism in Chongqing Han population

Objective To analyze the HLA-A ,B and DRB1 alleles high-resolution polymorphism in Chongqing Han population . Methods The PCR-SSOP and PCR-SBT methods were applied for the HLA high-resolution genotyping of 2 067 unrelated healthy donors in the registry of Chongqing branch of Chinese National Marrow Donor Program (CMDP) .The allele frequencies of HLA-A , B and DRB1 were estimated by the direct counting method and the Hardy-Weinberg equilibrium inspection was performed by using the Arlequin software 3 .1 .Results 168 high-resolution alleles were detected out ,in which 42 alleles of A*11 :01 ,A*24:02 ,A*02:07 ,A*02 :01 and A*33:03 at the HLA-A locus were observed with the frequencies greater than 0 .05 ;81 alleles were detected at HLA-B locus ,including B*46 :01 ,B*40:01 ,B*58 :01 ,B*13 :01 and B*15 :02 with the frequencies greater than 0 .05 ;45 al-leles of DRB1*09:01 ,DRB1*15 :01 ,DRB1*12 :02 ,DRB1*08 :03 and DRB1*11 :01 at the HLA-DR locus were observed with the frequencies greater than 0 .05 .Conclusion The data of the HLA-A ,B and DRB1allelic frequencies at high-resolution level in Chongqing Han population are obtained ,which provides the reliable reference data for the studies of anthropology ,forensic medi-cine ,transplantation matching and disease association .

Aim To investigate the effect of Ding''s herb enema prescription on intestinal tissue related target in rat colitis induced by dextran sulfate sodium(DSS), and to elucidate the mechanism of Ding''s herb enema prescription in improving the intestinal inflammation and intestinal fibrosis.Methods Rats were fed with 3.5% DSS.The rats were randomly divided into positive drug group, model group, Control group, and Ding''s herb enema prescription group.The positive drug group was treated with mesalazine enema, and Ding''s herb enema prescription group was treated with Ding''s herb enema prescription.The colon mucosa was taken once a day for 6 weeks.The changes of intestinal inflammatory response and intestinal fibrosis related proteins were detected by GSR-CAA-67 antibody protein array, and the differentially expressed proteins were screened out.Results Eight proteins showed statistical differences, including IFN-γ, erythropoietin(EPO), TIMP-2, TIM-1, IL-6, TIMP-1, TNF-α, IL-22 (P<0.05).On the other hand, Ding''s herb enema prescription and mesalazine significantly antagonized the effect of IL-6 and TIMP-1 (P<0.05).The antagonized effect of Ding''s embolization enema on TNF-α and IL-22 was also significant(P<0.05), but mesalazine had no similar effect (P>0.05).Conclusions Ding''s herb enema prescription has the effect of multiple targets, which may improve the intestinal inflammatory response and intestinal fibrosis to achieve the purpose of treatment of ulcerative colitis(UC).

Objective To observe the clinical effect of auricular bean-burying therapy in acupoints for patients with hepatocellular carcinoma after interventional therapy.Methods A total of 48 patients with hepatocellular carcinoma were divided into control group and observation group.The control group received western conventional nursing.The observation group applied auricular seeds.Pain scores and sleep quality were compared between two groups.Results The intervention group had lower painscores and quality of sleep at 2,3 and 7 d after intervention had lower than the control group (P < 0.05).Conclusion Auricular bean-burying therapy in acupoints can effectively improve postoperative pain,and improve sleep quality for patients with hepatocellular carcinoma after interventional therapy.

10.Effects of Baduanjin exercise in traditional Chinese medicine on fatigue and quality of life in patients with moderate inflammatory bowel disease

Objective To investigate the effects of Baduanjin exercise on the fatigue and quality of life in patients with moderate inflammatory bowel disease (IBD).Methods Totally 63 moderate IBD patients who were treated via the IBD diagnosis and treatment platform Nanjing Hospital of Chinese Medicine between March and July 2017 and met the inclusion criteria were selected and randomly divided into the control group (n=31) and the observation group (n=32) according to the random number table. Patients in the control group received general guidance for anti-fatigue via the WeChat platform on the basis of conventional medication, while patients in the observation group did Baduanjin exercise in addition to general guidance and medication. The fatigue and quality of life scores were compared between the two groups.Results The observation group scored lower on the whole and in the dimensions of general fatigue, physiological fatigue, less physical activities and mental fatigue than the control group after intervention (t=6.403, 6.746, 4.440, 3.409, 3.893;P< 0.05); but there was no significant difference in the dimension of lower enthusiasm between the two groups (t=0.486,P>0.05). In terms of the quality of life, the observation group scored higher on the whole and in the dimensions of bowel symptoms and general symptoms (t=-2.045, -2.254, -6.870;P<0.05); but there was no significance in the dimensions of emotional function and social function between the two groups (t= -0.041, 0.257;P>0.05).Conclusions Baduanjin exercise in traditional Chinese medicine can alleviate the fatigue symptoms in moderate IBD patients and improve their quality of life.
