1.Characteristic analysis and nursing of vagrant psychiatric patients with different gender

Objective To explore the clinical characteristics of vagrant psychiatric patients with different gender and summarize nursing measures. Methods Toally 1165 vagrant psychiatric patients who all were inpatient in Shenzhen Corning Hospital between July 2014 to July 2015 were analyzed retrospectively. Seperately we analyzed incidence of nosocomial infection, aggressive behaviour, trouble-making level, restraint and protection, and activity daily living. Results There was 735 male (63.1%) and 430 (36.9%) female in 1165 vagrant psychiatric patients. Comparing with female patients, We found male patients had higher rate of nosocomial infection , aggressivity and accident level , who were susceptible to be restrained and protected ( P < 0 . 001 ) and needed more care (P<0.05). Conclusion We should specially take precautionary measures when nursing vagrant psychiatric patients with different gender.
2.The relationship of breast cancer molecular subtype and location and time of recurrence metastasis

Objective To investigate the risk distribution of breast cancer for location and time of recurrence metastasis in molecular subtype.Methods We studied retrospectively the female patients who were diagnosed as invasive ductal breast cancer in our hospital from July 2004 to June 2012,detected ER,PR,and HER2 expressions in the paraffin sections.The patients with recurrence metastasis were divided into local recurrence and distant metastasis with the first transfer site as standard for analyzing the distribution in molecular subtype and the time of the first site of recurrence metastasis.Results Sixty two patients were encountered recurrence metastasis,including 23 patients with local recurrence,and 39 patients with distant metastasis,death 11.The rates of distant metastasis for patients who belonged to HER2 type and basal-like type were higher than that of local recurrence (P =0.01,P =0.001).The risk distribution of recurrence metastasis time in molecular recurrence metastasis showed that 35 percent of recurrence metastasis time of luminal A type was first 3 years,75 percent of molecular subtype of basa1-1ike type recurrence metastasis time in first 3 years and advanced.The peak of luminal B and HER2 type was first 3 years,and very low in 5 years.Conclusions Molecular subtype of breast cancer is an important complement for TNM method in accurately assessing the patients of recurrence metastasis for location and time,and is helpful for the individual screening of patients for recurrence metastasis.
3.Hyperbaric oxygen for children with dyskinetic cerebral palsy

Objective To evaluate the effectiveness and safety of hyperbaric oxygen (HBO) therapy as an adjunctive therapy for children with dyskinetic cerebral palsy. Methods Seventy-one children with dyskinetic cerebral palsy aged 6 mouths to 2 years were randomly assigned to a HBO group ( n = 35 ) or a control group ( n = 36).All children were given conventional rehabilitative treatment, but the children in the HBO group in addition received 40 sessions of HBO therapy. HBO was administered for 1 h with 85% ~ 90% oxygen at 1.4 atmospheres absolute pressure. All the treatments in both groups continued for 8 weeks. Gross motor function was evaluated with a gross motor function measure ( GMFM ), global motor performance was assessed with a psychomotor development index (PDI), and intelligence was assessed with a mental development index (MDI). Clinical assessments were done before and after treatment. At the same time, hearing impairment was measured using brainstem auditory evoked potentials (BAEPs) in the HBO group. Results All outcomes in both groups improved significantly over the course of study. The average improvement in GMFM in the control group was significantly greater than in the HBO group but other differences were not statistically significant. Hearing impairment developed in 8 children treated with HBO.Conclusion There was no evidence that HBO therapy improved the condition of children with dyskinetic cerebral palsy, and there is a risk of side effects with HBO therapy.
4.Effect of melatonin on expression and activity of myosin light chain kinase in the artery wall of atherosclerotic rabbits

Aim To study the effect of melatonin on expression and activity of myosin light chain kinase in the artery wall of atherosclerotic rabbits.Methods The rabbit model of atherosclerosis was induced by a high-cholesterol diet.The blood lipid levels were assayed in the serum of each group.MLCK expression was detected by Western blot and immunohistochemical method.MLCK activity was measured by ?-32P-ATP incorporation into myosin light chain.Results The atherosclerosis model was established successfully.The levels of lipids decreased after MLT treatment.After fed with cholesterol for twelve weeks,the expression and activity of MLCK in the artery of atherosclerotic rabbits increased markedly,whereas there was no obvious difference in expression of MLCK in the artery of atherosclerotic rabbits fed with cholesterol and melatonin for twelve weeks compared with that of control.Conclusions It was suggest that high expression and activity of MLCK in the artery might be closely correlated with the development of atherosclerosis.Melatonin played an important role in inhibiting the development of atherosclerosis by decreasing the expression and activity of MLCK.
5.Effects of FAR on myocardial fibrosis of diabetic rats

Aim To study the effects of Free Anthra-quinone from Rhubarb (FAR)on myocardial CTGF and collagen expression and interstitial fibrosis in dia-betic rats.Methods The male SD rats were randomly divided into normal group (CON),diabetic cardiomy-opathy group (DCM) and FAR treatment group (FAR).Streptozocin was intraperitoneally injected in-to the animals in the latter 2 groups to induce diabetic rat model.The model was expected to be stable for 2 weeks before the treatment.At the end of the 8th week in treatment,fasting plasma glucose and heart mass in-dex were measured.Masson staining was used to ob-serve the myocardial fibrosis.RT-PCR was used to de-tect the mRNA levels of CTGF,procollagen type Ⅰand collagen type Ⅲ.Immunohistochemical method was used to detect the content of CTGF.ELISA was used to detect the depositions of collagen type I and collagen type Ⅲ. Results Compared with CON group,fasting plasma glucose,heart mass index,the degree of myocardial fibrosis,and the expressions of CTGF,collagen type I and collagen type Ⅲ in left ven-tricular myocardial tissue of DCM group were signifi-cantly increased. However, compared with DCM group,fasting plasma glucose,heart mass index,the degree of myocardial fibrosis,and the expressions of CTGF,collagen type I and collagen type Ⅲ in left ven-tricular myocardial tissue of FAR-treated rats were sig-nificantly decreased.Conclusion FAR retards the process of myocardial fibrosis in diabetic rats by down-regulating the expression of CTGF,reducing the syn-thesis and depositions of collagen type I and collagen type Ⅲ.
6.Directive function of intracavitary electrocardiogram bimodal P-wave in the position of PICC tip

Objective To determine the accuracy of intracavitary electrocardiogram bimodal P-wave and characteristic P-wave in the position of PICC tip. Methods In the process of ECG guided catheterization, the morphological changes of P-wave in the intracavitary electrocardiogram were observed, and the bimodal P-wave and characteristic P-wave were recorded. The corresponding catheter length of the bimodal P-wave and characteristic P-wave were recorded. CT three-dimensional reconstruction was performed on 416 patients who underwent CT examination before and after catheterization. The distance between carina of trachea to superior vena cava and the right atrium junction was measured as best position of PICC and the standard distance. After catheterization, the distance from the tip of catheter to the corresponding catheter length of the bimodal P-wave and characteristic P-wave was measured as actual distance, and compared with the standard distance. Results In 416 cases, 168 patients (40.38%) were bimodal P-wave, and the accurate rate of bimodal P-wave was 86.31%(145/416), and the accurate rate of the characteristic P-wave was 13.69%(23/416). The standard distance was (41.96 ± 9.16) mm, the actual distance of corresponding characteristic P-wave was (48.00±15.18) mm and bimodal P-wave was (45.10± 11.16) mm. The distance of bimodal P-wave to control was (1.31±0.33) mm and characteristic P-wave was (6.00 ± 2.88) mm, which was significant statistical difference (t =5.197, P < 0.05), and the standard deviation of the distance of bimodal P-wave to control was smaller (0.33 < 2.88). Conclusions The intracavitary electrocardiogram bimodal P-wave is more accurate than characteristic P-wave in the position of PICC tip and it supplements and improves thePICC's tip position.
7.Influencing factors of suicide-committing risks among hospitalized depressive patients

Objective To study the, influencing factors of suicide-committing risks among hospitalized depressive patients. Methods Based on qualitative diagnoses, 102 patients were divided into non-suicide and suicide group.The two groups were compared in terms of general information, score by the Beck hopelessness scale (BHS) and the suicide subscale of the Chinese version of mini-international neuropsychiatric interview (MINI). Logistic regression was applied to analyze the related factors of suicide-committing risk in depressive patients. Results Among the 102 patients, 70 cases had tendency to commit suicide, accounting 68.6% , and 32 did not, accounting for 31.4%. Multivariate logistic regression suggested that the score of the suicide subscale of MINI could effectively predict the suicide tendency (P<0.001). Conclusions The suicide-committing risk is high in the depressive patients. The MINI suicide risk is the influencing factor of suicide-committing risk and can be used to predict suicide tendency in hospitalized depression patients. More attention should be paid to patients at high risk and relevant nursing countermeasures should be taken for effective intervention.
