1.Effects of Wuhaiyingliu Wan plus Jiaheng therapy on the simple goitre

Objective To investigate the clinical curative effects of Wuhaiyingliu Wan(WHW) plus Jiaheng (L-T4) therapy on simple goitre. Methods 120 patients with simple goitre were randomly divided into three groups, 39 cases were given WHW,39 eases were given L-T4 ,and 42 cases were given both WHW and L-T4. All patients' goi-tre volume were tested before initiating medication and 6 ~ 12 months after therapy to compare the medicative effects among the groups. Results There was no significant difference in goitre volume among the three groups before medi-cation. It was found that significant dimunution of gnitre volume in all groups, but similar dimunution in WHW and L-T4 group, significant diminution in combination group after 6 ~ 12 months of initiating medication(P < 0.01). Conclu-sion WHW ean diminish goitre volume in short term,its medicative effect is similar to L-T4 ,conjoint use will boost its effect on gnitre volume,it is a good therapeutic method to be gnneralized.
2.Meta-analysis of Radiomics to predict the efficacy of non-surgical treatment for esophageal cancer

Objective:To evaluate the value of radiomics in predicting the efficacy of non-operative treatment of esophageal cancer by meta-analysis.Methods:Search terms included "Esophageal Neoplasms", "Esophageal Neoplasms", "Neoplasm, Esophageal", "Esophagus Neoplasm", "Esophagus Neoplasm", "Neoplasm, Esophagus", "Neoplasms, Esophagus", "Neoplasms, Esophageal", "Cancer of Esophagus", "Cancer of the Esophagus", "Esophagus Cancer", "Cancer, Esophagus", "Cancers, Esophagus", "Esophagus Cancers", "Esophageal Cancer", "Cancer, Esophageal", "Cancers, Esophageal", "Esophageal Cancers" and "radiomics", "radiomics features", "radiomic", "texture", "texture analysis", "textural analysis", "histogram", "machine learning", "artificial intelligence", both in English and corresponding Chinese. The Chinese and English literatures related to radiomics prediction of the efficacy of non-surgical treatment of esophageal cancer published in PubMed, Web of Science, Embase, China National Knowledge Internet, Wanfang Medical Online and VIP Chinese Journal Service Platform from the establishment of the database to November 2022 were searched, and screening, quality evaluation and data extraction were carried out. Meta analysis was performed by using Stata 15.1, Meta-disc 1.4 and Review Manager 5.3 software.Results:Seventeen literatures of Chinese and English with 1389 patients with esophageal cancer who received non-surgical treatment were included. There was no significant threshold effect in predicting the effect of non-operative treatment of esophageal cancer by radiomics ( r=0.103, P=0.694), and there was high heterogeneity ( I2>50%). The combined sensitivity of all included literatures was 0.86 (95% CI: 0.81-0.89), specificity was 0.81 (95% CI: 0.76-0.85), positive likelihood ratio was 4.4 (95% CI: 3.5-5.6), and negative likelihood ratio was 0.18 (95% CI: 0.13-0.24). The diagnostic odds ratio was 25 (95% CI: 16-39) and the AUC was 0.90 (95% CI: 0.87-0.92). Conclusions:Radiomics can better predict the efficacy of non-surgical treatment of esophageal cancer, MRI and PET/CT radiomics has higher accuracy in predicting the efficacy of esophageal cancer, and machine learning can also improve the accuracy of prediction. It is helpful to make individualized treatment plan and improve the efficiency of treatment by effectively predicting the curative effect of patients with esophageal cancer before treatment.
3.Role of mitochondrial ATP-sensitive potassium channel in sevoflurane preconditioning-induced delayed cardioprotection against ischemia-reperfusion injury in isolated rat hearts

Objective To evaluate the role of the mitochondrial ATP-sensitive potassium (mito-KATP)channel in sevoflurane preconditioning-induced delayed cardioprotection against ischemia-reperfusion (I/R) injury in isolated rat hearts.Methods Seventy-two adult male Sprague-Dawley rats were randomly divided into 6 groups (n =12 each):control group (group CON),I/R group,sevoflurane control group (group SEVO),sevoflurane preconditioning group (group SWO P),5-hydroxydeconoate (5-HD) + sevoflurane preconditioning group (group 5-HD+ SWOP) and 5-HD control group (group 5-HD).The rats were exposed to 33% pure oxygen for 1 h in groups CON and I/R.The rats were exposed to 2.5% sevoflurane for 1 h in groups SEVO and SWOP.5-HD (a mito-KATP channel inhibitor) 10 mg/kg was injected intraperitoneally 30 min before sevoflurane preconditioning in group 5-HD + SWOP.5-HD 10 mg/kg was injected intraperitoneally in group 5-HD.The hearts were immediately removed and perfused in a Langendorff apparatus.The hearts were made globally ischemic for 30 min followed by 120 min reperfusion in groups I/R,SWOP,5-HD + SWOP and 5-HD.The expression of phosphorylated protein kinase C-epsilon (p-PKC-ε) and caspase-8 was measured by Western blot immediately before ischemia (T0) and at 120 min of reperfusion (T1).The myocardial infarct volume was measured by TTC staining.Results Compared with group CON,the myocardial infarct volume was significantly increased at T1 in groups I/R,SWOP,5-HD +SWOP and 5-HD,p-PKC-ε expression was up-regulated at T0 in groups SEVO and SWOP and at T1 in groups I/R,SWOP,5-HD + SWOP and 5-HD,and caspase-8 expression was down-regulated at T1 in group SEVO (P <0.05).Compared with group I/R,the myocardial infarct volume was significantly decreased at T1 in groups SWOP and 5-HD + SWOP,p-PKC-ε expression was up-regulated at T0 in groups SEVO and SWOP,and caspase-8 expression was down-regulated at T1 in group SWOP (P < 0.05).Compared with group SWOP,the myocardial infarct volume was significantly increased,p-PKC-ε expression was down-regulated at T0,and caspase-8 expression was up-regulated at T1 in group 5-HD + SWOP (P < 0.05).Conclusion The mito-KATP channel is involved in sevoflurane preconditioning-induced delayed cardioprotection against I/R injury in isolated rat hearts through upregulation of p-PKC-ε expression before ischemia and inhibition of cell apoptosis during reperfusion.
4.Exposure risk assessment of plasticizer in dietary food in Xiamen.

OBJECTIVETo understand the dietary consumption of residents in Xiamen and the content of phthalic acid esters (PAEs) in food, and to assess the plasticizer exposure risk of diet in Xiamen.
METHODSThe survey was conducted by stratified cluster random sampling method in Xiamen from September to October in 2010. According to the Xiamen administrative division, six neighborhood communities were selected as sampling units, then 25 families were randomly chosen from each sampling units.From the above 150 families, the permanent residents over the age of six were permitted to our study. The survey included 495 residents totally. These participants' information, such as basic personal information, physical activity levels, meal frequency and the average consumption of 33 kinds of food in 13 categories were collected using questionnaires. Thirteen categories included cereal and tubers, beans, vegetables, fungi and algae, fruits, dairy products, meat, seafood, eggs, snacks, beverages, cooking oil and spices. The height and weight of residents were measured and the average daily dietary intake was calculated. Thirty-three kinds of food in 13 categories were collected in supermarkets in Xiamen. According to the annual sales ranking, the top three-five brands of each kinds of food were selected and numbered, then two or three brands were chosen by random number table method from them; three completely individual packed samples in the same batch of each brand were detected; 243 samples were included in our study.100-500 g solid samples or 100-500 ml liquid samples were collected. The content of diethyl phthalate (DEP), dibutyl phthalate (DBP), di (2-ethylhexyl) phthalate (DEHP) in food were detected by liquid chromatography mass spectrometry, which expressed by median (minimum-maximum). The exposure dose, contribution rate and risk index of PAEs were calculated by point estimation method.
RESULTSAccording to the average daily dietary intake of residents in Xiamen, the top three ones in 13 categories of food were cereal and tubers (337.16 g/d, 18.21%), vegetables (309.12 g/d, 16.69%) and fruits (213.20 g/d, 11.51%). The content of DEP, DBP or DEHP among different categories of food was significantly different (χ² values were 58.05, 50.19 and 102.10, P < 0.01). Among 13 categories of food, seafood contained the most DEP (0.090 (0.000-0.324)mg/kg); cooking oil had the most DBP (0.700(0.000-2.980) mg/kg) and DEHP (5.115(0.000-24.160) mg/kg). DEP, DBP and DEHP exposure(0.19, 4.20, 18.10 µg × kg⁻¹ ×d ⁻¹)in dietary food in Xiamen were less than the reference dose(RfD) (800, 100, 20 µg × kg⁻¹ × d⁻¹) proposed by the United States Environmental Protection Agency (EPA), and the risk indexes were 0.02%, 4.20% and 90.50%, respectively. Among 13 categories of foods, seafood was the main source of DEP dietary exposure. The exposure dose and contribution rate of DEP in seafood were 0.18 µg × kg⁻¹ × d⁻¹ and 94.74%, respectively.Vegetables were the main source of DBP and DEHP dietary exposure. The exposure dose and contribution rate of DBP and DEHP were 1.48 µg × kg⁻¹ × d⁻¹, 35.24% and 6.07 µg × kg⁻¹ × d⁻¹, 33.54%, respectively.
CONCLUSIONThe food consumed by residents in Xiamen was overall in a safe state, but to some extent, there still exists DEHP exposure risk in foods.

5.Diagnosis and treatment of acute pancreatitis complicated with splenic infarction and literature review

Objective:To investigate the etiology, pathogenesis, clinical manifestations, and treatment of acute pancreatitis (AP) complicated with spleen infarction.Methods:A case of AP combined with splenic infarction was retrospectively analyzed and the literature was reviewed.Results:AP complicated with splenic infarction suggested that the patient′s condition was serious. The impairment of splenic immune function caused by splenic infarction may be an important cause of severe acute pancreatitis (SAP). Early detection and active treatment can obtain a good prognosis, and anticoagulant or antiplatelet aggregation treatment was not the key treatment.Conclusions:AP combined with splenic infarction may be an risk factor for SAP.
