1.Distribution and antimicrobial resistance profile of ESBLs-producing Escherichia coli

Objective? To?investigate?the?distribution?and?antibiotic?resistance?profile?of?the?extended-spectrum?β-lactamase-producing Escherichia coli (ESBL-EC) isolates during the period from 2012 to 2015. Methods ESBL-EC isolates were reviewed retrospectively from 2012 to 2015 and subjected to analysis with WHONET 5.6 software. Results A total of 725, 732, 760, and 770 ESBL-EC strains were isolated each year from 2012 to 2015. The isolates were mainly from urine, sputum, blood, drainage fluid, peritoneal fluid and bile. The ESBL-EC strains were mainly collected from the patients treated in departments of urology, hepatobiliary surgery, nephrology and gynecology. More than 90% of the ESBL-EC strains were resistant to ampicillin, cefazolin, cefuroxime,?and?ceftriaxone,?and?more?than?50?%?of?these?strains?were?resistant?to?trimethoprim-sulfamethoxazole,?ciprofloxacin,?levofloxacin,?and?aztreonam.?However,?less?than?15?%?of?the?ESBL-EC?isolates?were?resistant?to?amikacin,?nitrofurantoin,?cefotetan,?cefoperazone-sulbactam, piperacillin-tazobactam, amoxicinllin-clavulanic acid, ertapenem, imipenem, meropenem, and tigecycline. The resistance rate of ESBL-EC to cefoperazone-sulbactam, piperacillin-tazobactam, ertapenem, imipenem, and meropenem was increasing over time. Conclusions ESBL-EC isolates show extremely high resistance to the commonly used cephalosporins. Cefoperazone-sulbactam, piperacillin-tazobactam, ertapenem, imipenem, and meropenem are still active for these pathogens, even though increasing resistance rate is observed over time.

The application and expanding of nasal endoscopic surgery,an epoch-making transform of rhinology,have a very close associations with medical imaging,which includes anatomy photograph,conceptual diagram,radiological imaging,nasal endoscopic image display system,operation photographic recording and computer assisted instruction courseware,etc.It is important to collect the medical imaging through various means and set up medical imaging resources library for nasal endoscopic surgery teaching.
4.Experiences of Clinical Teaching of Advanced Otolaryngology-head & Neck Surgeons

5.Practice and analysis of bilingual teaching method in Otorhinolaryngology on bilingual education for seven-year-program medical students

Rhinosinusitis is a popular disease in otorhinolaryngology.A guideline document,European position paper on rhinosinusitis and nasal polyps (EPOS) developed on the basis of evidencebased medicine was used in clinical teaching.Theoretical teaching was promoted by reading and understanding EPOS.Meanwhile,EPOS was used in clinical practices of preclinical ward round,case analysis and independent bed charge,which laid a solid foundation to help otorhinolaryngology postgraduates fully grasp the theoretical knowledge of rhinosinusitis and nasal polyps,improve clinical practice abilities and develop standardized philosophy of diagnosis and treatment finally.
7.Association of Epression with the Level of MicroRNA in Plasma and Mononuclear Cells in Patients with Schizophrenia

Objective To investigate the changes and clinical significance ot the expression ot microRNA in plasma and peripheral blood mononuclear cells in patients with schizophrenia.Methods 174 patients with schizophrenia in Danzhou People 's Hospital were selected as the case group and the other 80 healthy persons as control group The relative expression levels of 8 microRNA in two groups of plasma and pcripheral blood mononuclear cells were detected by real-time quantitative fluorescent PCR(MiR-195,MiR-346,MiR-181b,MiR-212,MiR-30e,MiR-432,MiR 7,MiR-34a),and the differences of microRNA in the plasma and peripheral blood mononuclear cells were compared between the two groups.ROC curve was used to analyze the sensitivity and specificity of microRNA as diagnostic criteria for schizophrenia and Logistic regression analysis of the relative risk of microRNA in schizophrenia.Results The expression levels of MiR-195,MiR-181b,MiR-132,MiR-30e,MiR-7 and in the patients of the case group(3.11±1.05,2.18±0.72,1.85±0.74 and 9.61±1.87) were significantly higher than those in the control group(4.48±1.07,2.92±0.86,3.53±1.07 and 11.96±2.73,P<0.05 or P<0.01).The expression levels of MiR-181b,MiR-212,MiR-30e and MiR-34a in peripheral blood mononuclear cells of patients in the case group (-4.20±1.16,0.27 ±0.55,-4.83± 1.05 and 2.64± 1.08) were significantly higher than those in control group (-3.56±0.81,0.91±0.68,-3.49±1.22 and 3.95±1.03,P<0.05 or P<0.01).Logistic regression analysis showed that plasma MiR-181b and MiR-30e were significantly relative risk (OR=2.357,95 % CI:1.361 ~ 4.093;OR=2.064,95 % CI:1.147~3.815),and peripheral blood mononuclear cells MiR-30e also had significant relative risk (OR=1.628,95 %CI:0.914~2.926).ROC curve analysis showed that 95%CI and AUC in plasma and peripheral blood mononuclear cells of MiR-181b were 0.702 (0.784~0.632),0.658 (0.593 to 0.736),and plasma and peripheral blood mononuclear cell of MiR-30e were 0.775 (0.706~0.857),0.758 (0.686~0.839),respectively.Spearman correlation analysis showed that plasma MiR 181b and plasma MiR-30e were significantly correlated (r=0.547,P =0.043).Conclusion Abnormal expression of microRNA in patients with schizophrenia,and plasma and peripheral blood mononuclear cells MiR-181b and MiR-30e had good diagnostic value for schizophrenia patients.
8.Expression Level of CCP/AST in Synovial Fluid and Synovial Tissue of Rheumatoid Arthritis Patients and Its Clinical Significance

Objective To investigate the expression level and clinical significance of cyclic citrullinated peptide antigen-specific T cells(CCP/AST)in synovial fluid and synovial tissue of rheumatoid arthritis(RA)patients.Methods A total of 128 RA patients in Shijiazhuang Hospital of Traditional Chinese Medicine from January to December 2021 were selected as the RA group,and 50 patients who needed arthroscopy for joint pain in the hospital during the same period were selected as the control group.Among the RA group,there were 46 cases in the mild group,52 cases in the moderate group,and 30 cases in the severe group.The protein expression levels of rheumatoid factors(RF)and anticitrullinated protein antibodies(ACPA)in synovial tissues of the subjects in each group were analyzed by Western blot.The frequency of CCP/AST in the synovial fluid of the subjects was analyzed by flow cytometry.The intensity of the staining of CCP/AST in synovial tissues was observed by double immunofluorescence staining/laser confocal scanning.Pearson correlation analysis was used to assess the correlation between the CCP/AST expression of synovial fluid and synovial tissue and RF and ACPA.Logistic regression was used to analyze the risk factors for the development of rheumatoid arthritis.Results In the order of control,mild,moderate and severe groups,RF(1.01±0.01,1.53±0.03,2.01±0.08,2.66±0.12 kDa)and ACPA proteins(1.03±0.01,1.61±0.03,2.04±0.10,2.59±0.13 kDa)in synovial tissues of patients were sequentially elevated,and the differences were all statistically significant(F=14.207,12.446,all P<0.05).The expression of CCP/AST in synovial fluid of patients in the control,mild,moderate and severe groups was increased sequentially(8.26%±1.68%,22.46%±3.28%,33.58%±4.37%,46.15%±5.44%),and the difference was statistically significant(F=25.306,P<0.05).Meanwhile,the intensity of CCP/AST staining in synovial tissues of patients in the control,mild,moderate and severe groups was also increased sequentially(1.05±0.26,1.35±0.89,2.04±0.56,2.78±0.15 score),and the difference was statistically significant(F=70.67,P<0.05).The expression of CCP/AST in the synovial fluid and synovial tissues of patients with RA was positively correlated with RF(r=0.861,0.934,all P<0.05)and ACPA in synovial fluid and synovial tissue(r=0.854,0.913,all P<0.05).Logistic regression analysis showed that hypertension(OR=3.241,95%CI:1.491~6.752),diabetes mellitus(OR=2.565,95%CI:1.126~5.813),synovial fluid(OR=4.450,95%CI:1.652~11.622),and CCP/AST expression in synovial tissues(OR=5.629,95%CI:2.474~12.390)were independent risk factors for the development of RA(P<0.05).Conclusion CCP/AST showed high expression in synovial fluid and synovial tissue of RA patients and related to disease activity and joint destruction,which can be used to assess the clinical joint mobility and bone destruction degree in such patients.
9.Application of event-related brain potential in intelligence impairment assessment after craniocerebral trauma

Objective To investigate the changes of event-related potentials (ERPs) in patients with intelligence impairments after craniocerebral trauma.Methods 60 patients with intelligence impairments after craniocerebral trauma were enrolled as case group,and 60 healthy subjects were enrolled as control group.EEG instrument was used to record P300 and P50 of the two groups and the differences in P300 and P50 components were compared.Results There are significant differences between case group and control group in latency of P300 ((440.430 ± 77.367) ms vs (342.928 ± 36.175) ms,P< 0.01),and case group showed decreased amplitude ((12.692±8.152) μV vs (18.138±6.590) μV,P<0.01).The S2-P50 amplitude of case group was significantly higher than that of control group ((3.75± 1.59) μV vs (2.42±1.43) μV,P<0.01).In addition,the S2-P50 amplitude/S1-P50 amplitude ratio of case group was higher than that of the control group,and the difference was statistically significant (0.78±0.54 vs 0.46±0.18,P<0.01).The latency and amplitude of P300 were significantly correlated with the total score of WAIS-RC (r=-0.31,P<0.01;r=0.17,P<0.01);The amplitude of S2-P50 and the ratio of S2-P50 amplitude to S1-P50 amplitude were significantly negatively correlated with the total score of WAIS-RC (r=-0.33,P<0.01;r=-0.45,P<0.01).Conclusion P300 and P50 components of ERP can provide references for judicial expertise to evaluate intelligence impairments after craniocerebral trauma.
10.Clinical analysis of 258 cases of nasal-sinus benign and malignant tumors

Objective To analyze the clinical features of 258 cases of nasal-sinus benign and malignant tumors to improve the accuracy of clinical diagnosis.Methods The clinical data of 258 patients with nasal-sinus tumor confirmed by pathology were retrospectively analyzed and the clinic differences between benign and malignant tumors were compared.Results The clinic manifestations of sinonasal neoplasm were varied.The incidences of epistaxis or blood in the tears,headache,facial numbness or pain,eye pain or epiphora were significantly higher in the benign tumors group than in the malignant tumors group (P<0.05).Among 196 patients undergoing surgery,the coincidence rates of preoperative and postoperative paraffin sections were similar in the two groups (P>0.05).Among 63 surgical patients with malignant tumors,the coincidence rate of 232 incised-margin frozen sections and postoperative paraffin sections was 98.70% (229/232).Among 34 cases,the coincidence rate of frozen sections and postoperative paraffin sections was 91.18%0 (31/34),in which 6 cases were immediately intraoperative frozen,and 66.67% (4/6) of the cases that changed into malignancy from benignity were not consistent with the preoperative diagnosis of paraffin sections.Conclusion The sinonasal tumors are lacking specificity in clinical manifestations,but rapid intraoperative frozen section examination is helpful to improve the diagnosis rate;continuously frozen pathological examination should be performed in malignant tumor margins.
