2.Change of Skin Temperature of Workers Using Vibrating Tools in Anthracite Mines.

By implementing epoch-making policies for industrial promotion, the national economy has made a remarkable development. As a result of such economic growth, industrial accidents and occupational diseases have become a serious problem in Korean society. In the presidential order for the execution of the Korean Labor Standard Law, neuritis and other diseases stemming from health impairments due to vibrations in industrial processes are designated to be dealt with as vibration diseases. In the case of vibration disease, industrial accident compensation is not effectively paid. In order to investigate the vibration hazards of rock-drill operation, the authors studied the subjective symptoms and performed physical function tests on a total of 79 persons (vibration exposed group) who used rock-drills, and 39 persons (control group) who did not use rock-drills at anthracite mines. The results of the physical function test were as follows : 1. The right hand was more affected by white finger than the left hand. 2. Independent variables such as duration of rock-drill operation, age, drinking and smoking were identified as statistically significant factors for the occurrence of white finger. 3. In the pain sense threshold, the group with Raynaud's phenomenon showed a statistically higher level than that of the control group. 4. The skin temperature of the group with Raynaud's phenomenon was lower than that of the control group. The recovery time of skin temperature after cooling was delayed compared with the value of the control group.

3.The Change of Anti-HBs Titer after Injection of HBIG and HBV in Newborn Delivered from HBsAg Carrier Mothers.

PURPOSE: We studied prevention effects of vertical transmission of Hepatitis B with follow-up, through the change of Anti-HBs titers during 9 months after injection of Hepatitis B immunoglobulin (HBIG) and Hepatitis B vaccine (HBV) at birth in newborn delivered from Hepatitis antigen carrier mothers. METHODS: This study was performed on newborn delivered from HBsAg carrier mothers at Hae Sung Hospital from Feb. 1995 to May 1996. These newborn were injected intramuscularly with HBV and HBIG after evaluation of HBsAg and Anti-HBs titer directly at birth. Regular Hepatitis vaccination was given at 1 month, 6 months as well as HBsAg and Anti-HBs titer were evaluated at 24 hours, 1 month, 3 months, 6 months, and 9 months. RESULTS: The incidence of Hepatitis B carrier mothers is 5.6%, and newborn babies born to HBsAg carrier mothers showed HBsAg in 10.6%. In most newborn, Anti-HBs titer maintained in 100-1,000mIU/ml after injection of HBIG and HBV at birth, and mean Anti-HBs titer decreased a little at 1 month, but therafter increased gradually by regular Hepatitis vaccination. HBsAg positive newborn maintained mean HBsAg titers below 2 (S/N) until 9 months old after injection of HBIG and HBV. CONCLUSIONS: A screening test for Hepatitis B must be performed on all pregnant women, and that infants of Hepatitis B carrier mothers must be immunized by HBIG and HBV directly at birth.

4.Kinematic Analysis of Lower Extremity Joint Movements during Sidestep at Different Sidestep Speeds.

The purpose of this study were to examine the effects of the speed of sidestep in a specific direction on the kinematic variables of the lower extremities and to provide basic information on sidestep. The subjects of this study were eight students at Hanseo University who did not have a problem in lower extremity alignment. Biomechanical data were collected using a motion analysis system and force plate and Kruskal-Wallis test was performed in order to test differences according to speed. The results of this study were as follow. First, as to hip joint angle according to speed, when the speed increased, in the sagittal view, the maximum flexion angle decreased, the range of motion (ROM) decreased, and the flexion angle at the highest ground reaction force decreased, and in the transverse view, the internal rotation angle at the highest ground reaction force increased. Second, as to knee joint angle according to speed, when the speed increased, in the sagittal view, the maximum flexion angle decreased, the ROM decreased, and the flexion angle at the highest ground reaction force decreased, and in the coronal view, the maximum varus angle decreased, and the ROM decreased. Third, as to ankle joint angle according to speed, when the speed increased, in the sagittal view, the ROM decreased, and the dorsiflexion angle at the highest ground reaction force decreased, and in the coronal view, the minimum inversion angle increased. This suggests that high-speed sidestep during a game increases the risk of injury and degenerative arthritis among athletes.

5.Regenerate Bone Healing According to Osteotomy Methods in Ilizarov lengthening

The Ilizarov technique for gradual distraction osteogenesis was developed in the 1950s. A correctly performed osteotomy is essential to the success of distraction osteogenesis and prepares for limb lengthening. Between Sept. of 1991 and 1994, thirty-four patients were treated by Ilizarov technique at St. Benedict Hosp. and Gang-Dong Hosp.. And then assigned to two separate groups : a corticotomy group (group A) and osteotomy group (group B; osteotomy with Gigli saw or osteotomy with multiple drill holes and osteotome). The regenerate segments were evaluated weekly for the first six weeks after operation. After the initial six-week evaluation period, observations of these segments were continued through a series of monthly radiographs. Distraction began on postoperative day seven in group A and on day eleven in group B; and continued at a rate of 1 mm/day and a frequency of 4 times/day. Group A displayed new bone formation earlier than group B: group A's mean was 16.5 ± 4.9 days, while B's mean was 25.3 ± 4.6 days. The first bridging callus occurred earlier in group A than it did in group B: A's mean was time of 36.7 ± 9.9 days, while B's mean was 44.0 ± 7.9 days. There was no significant difference between groups A & B in terms of first cortical formation : A's mean was 86.9 ± 24.0 days, and B's mean was 100.6 ± 25.2 days. There was no significant difference between groups A & B in terms of the bone healing index : A's mean was 41.6 ± 13.5 days and B's mean was 41.15 ± 8.10 days.

6.A Case of Coats' Disease Accompanying A Retinal Macrocyst.

PURPOSE: To report a case of laser photocoagulation treatment for the patient of Coats' disease accompanying a retinal macrocyst. CASE SUMMARY: A three-year-old boy visited the hospital whose chief complaint was visual acuity decrease of his left eye. Fundus examination showed macular scar, foveal hard exudates and inferior retinal cystic lesion in his left eye. Two months later, examination under anesthesia (EUA) and fluorescein angiography (FAG) was performed. The results revealed inferior retinal macrocyst, nasal avascular retina and telangiectasia around the retinal macrocyst. Laser photocoagulation was performed around the retinal macrocyst and at the nasal avascular retina. One year after the laser photocoagulation, retinal macrocyst did not further progress and the retina was stabilized. CONCLUSIONS: Laser photocoagulation was done around the retinal macrocyst and at the nasal avascular retina of the Coats' disease accompanying a retinal macrocyst and the lesions did not further progress and the retina was stabilized.

7.Change in the Sensation of the Arms, the Range of Motion in the Shoulders and Depression in Breast Cancer Patients.

PURPOSE: The purpose of this study was to examine the association among change in the sensation of the arms, the range of motion in the shoulders and depression in breast cancer patients. METHOD: This is a descriptive study on correlation. The participants were 132 patients who had been diagnosed with breast cancer and had mastectomy at a university hospital, and had participated in a breast cancer self-help group. The degree of depression was assessed by SCL-90-R scores. Collected data were analyzed with SPSS 12.0 for Windows. RESULTS: The participants complained of 'heaviness' as the most frequent and serious symptom of change in the sensation of the arms. 'Completely zip up the dress with a back-fastening zipper' was the most difficult motion of the shoulders. Participants who were in a worse stage of disease were in a worse condition in shoulder functions, and on a higher degree of depression. The degree of change in sensation and shoulder functions showed a positive correlation with each other, and both of them showed a negative correlation with the degree of depression. CONCLUSION: Sensation and motion change in the arms and the shoulders were common phenomena that affected depression in patients who had mastectomy. Nurses should consider not only depression but also discomfort of the arms and the shoulders for patients with mastectomy.

8.Linear Atrophoderma of Moulin with Bilateral Involvement.

Linear atrophoderma of Moulin is a rare acquired atrophic skin condition following the lines of Blaschko. It is characterized by asymptomatic, band-like, hypopigmented atrophoderma, without obvious induration or sclerosis. Usually, the lesions begin in childhood or adolescence and are localized on the trunk or extremities with a unilateral distribution. Extensive lesions with bilateral involvement are extremely rare, and only three cases affecting patients bilaterally have been previously described. We herein present the case of a 17-year-old boy with multiple hyperpigmented atrophic patches, arranged in a linear pattern on both sides of the trunk, right buttock and leg, and left arm.

9.The influence of pH and lactic acid concentration on the formation of artificial root caries in acid buffer solution.

The purpose of this study is to compare and to evaluate the effect of pH and lactic acid concentration on the progression of artificial root caries lesion using polarizing microscope, and to evaluate the morphological changes of hydroxyapatite crystals of the demineralized area and to investigate the process of demineralization using scanning electron microscope. Artificial root caries lesion was created by dividing specimens into 3 pH groups (pH 4.3, 5.0, 5.5), and each pH group was divided into 3 lactic acid concentration groups (25 mM, 50 mM, 100 mM). Each group was immersed in acid buffer solution for 5 days and examined. The results were as follows: 1. Under polarized microscope, the depth of lesion was more effected by the lactic acid concentration rather than the pH. 2. Under scanning electron microscope, dissolution of hydroxyapatite crystals were increased as the lactic acid concentration increased and the pH decreased. 3. Demineralized hydroxyapatite crystals showed peripheral dissolution and decreased size and number within cluster of hydroxyapatite crystals and widening of intercluster and intercrystal spaces as the pH decreased and the lactic acid concentration increased. 4. Under scanning electron microscope evaluation of the surface zone, clusters of hydroxyapatite crystals were dissolved, and dissolution and reattachment of crystals on the surface of collagen fibrils were observed as the lactic acid concentration increased. 5. Under scanning electron microscope, demineralization of dentin occurred not only independently but also with remineralization simultaneously. In conclusion, the study showed that pH and lactic acid concentration influenced the rate of progression of the lesion in artificial root caries. Demineralization process was progressed from the surface of the cluster of hydroxyapatite crystals and the morphology of hydroxyapatite crystals changed from round or elliptical shape into irregular shape as time elapsed.

10.The Effect of Anti-TNF-alpha on Refractory Uveitis.

PURPOSE: To investigate the effects of Anti-TNF-alpha (infliximab, Remicade(R), Shering-Plough) on refractory uveitis. METHODS: A retrospective observational survey was conducted on 12 eyes of 10 patients with refractory uveitis. These patients had no response to immunosuppresants or systemic steroid therapy and were intolerable due to drug toxicity. After an intravenous injection of Anti-TNF-alpha, best corrected visulal acuity was measured. Improvement in inflammation was then observed at the first, third and last month of follow-up. RESULTS: Patients were observed for a mean period of 15 months. They received anti-TNF-alpha intravenously, ranging from one to eleven infusions. After infusion with Anti-TNF-alpha, remission of intraocular inflammation was evident in all patients within 9.7 days. Best corrected visual acuity also improved at the first, third, and last month of follow-up. Two patients were infected by tuberculosis and mumps respectively. CONCLUSIONS: There was an effect on refractory uveitis with anti-TNF-alpha therapy. Many participants may also require further evaluation after treatment.
