Systemic lupus erythematosus is a connective tissue disease which can affect every organ system. Neurologic abnormalities are common, occuring in approximately half of all patients at some time during the course of their illness. But symptoms of nervous system as the sole presenting symptoms occur in less than 1% of lupus patients. In patients initially presenting with neurologic symptoms and signs, differential diagnosis is difficult and sometimes it may be misdiagnosed. Therefore extensive laboratory investigations should be carried out in all patients with unusual neurological symptoms, since early diagnosis of lupus can help in providing effective treatment. We report a patient with systemic lupus erythematosus who presented with dysarthria and dysphagia resembling multiple sclerosis.

A wide variety of cancers metastasize to the ovaries. In a majority of instances the primary site is the gastrointestinal tract, breast, or other gynecologic organs. The best known tumor of this type is signet-ring cell adenocarcinoma. The gallbladder and bile duct are rare sources of these metastases. The authors have had an experience of a case that was presented of Krukenberg tumor metastatic from the gallbladder and report the case with brief review of literature.

5.Treatment of Bone and Tendon-Exposed wounds using-Terudermis.

Deep skin and soft tissue defects with exposed bone and tendon is difficult to treat, because skin graft rarely survives and flap surgery is sacrifice of donor site. Since "Stage I" membrane was developed by Yannas and Bruke in 1980, numerous kinds of artificial skin have been developed. The adaptability of "Terudermis", developed by the Terumo Co., as an artificial skin composed of sponge made of a fibrillar atelocollagen and a heat-denatured atelocollagen, was clinically evaluated on application to 13 cases presenting deep skin and soft tissue defect with exposed bones and tendons from October 1997 to march 1998. Terudermis has the advantage of allowing early incorporation of fibroblasts and capillaries into its collagen sponge due to very weak dehydrothermal cross-linking. Before Terudermis graft, several days of wet dressing and debridement were required to prepare healthy well-vascularized bed because Terudermis was weak on unsanitary wounds. After bed preparation, Terudermis was grafted like usual skin graft. Tie-over bolster dressing or compressive dressing was used case by case. The dressing was opened 2~3 days after Terudermis grafting. Wet dressing was done daily until the skin graft was done. Autologous skin graft was done 2-3 weeks after Terudermis graft. Our clinical results indicated that Terudermis was beneficial in treating 77% of our patients. Through the use of this new method, treatment of severe skin and soft tissue defects that are usually treated by musculocutaneous or other conventional skin flaps can be replaced by Terudermis as an new artificial dermis.

6.Cancer Incidence in Kangwha County(1986 - 1992).

OBJECTIVES: This paper presents the information on the incidence of cancer from the population-based cancer resistry in Kangwha County. Material and METHODS: This investigation is based on Kangwha cancer registry. The data included cases of cancer diagnosed from 1986 through 1992. The diagnosis of cancer was confirmed by a team of physicians and nurses with the medical records kept in the clinics and hospitals based on the diagnostic criteria recommended by WHO. Home visitings were also made to cancer patients confirmed in every 6 months for the follow up and for the collection of relevant information directly from the patients. RESULTS: A total of 992 cancer cases were registered during 1986 - 1992. The age-adjusted cancer incidence rate of all site is 201.7 in men and 110.7 in women. The most common cancer is the stomach cancer in both sexes. The age-adjusted incidence rate of the stomach cancer is 65.9 in men and 25.0 in women per 100,000 population. The lung cancer(33.8) and liver cancer(27.7) are next common cancers in men. The cervical cancer(21.8) and lung cancer(8.4) are next in women. CONCLUSION: The most common cancer is the stomach cancer in both sexes. The annual age-adjusted incidence rate of the stomach cancer is 65.9 in men and 25.0 in women per 100,000 population.

7.Supracondylar Fracture of the Femur Treated by Ender nail

Supracondylar fracture of the femur is not as common as the shaft fracture. There is wide range of potential complication in the treatment of these fractures and the method of the treament and the evaluation of it's result is variable and remains controversial. Until 1960, we believed conservative treatment supirior to the operative treatment of the supracondylar fracture of the femur. But recently, anatomical reduction and internal fixation with early joint motion showed satisfactory results and is the trend of treatment of these fracture. The authors of this thesis performed treatment of supracondylar fracture of femur by utilizing Ender nails and screws in 13 cases during the period from October 1987 to November 1988 and it was found that operation was relatively simple, bleeding was little and early exercise of knee was allowed and even for patients of old age internal fixation could be maintained easily.

8.The Clinical Consideration of Consecutive Extropia.

We evaluated retrospectively the clinical characteristics of 16 consecutive exotropia patients over 10PD following surgery for esotropia, the patients were differentiated into nonoperative and operative group. The nonoperative group comprised of 11 patients with exodeviation 10PD through 20PD(mean 12.9PD) and the orthphoria through 15PD(mean 10.5PD) at final visit. The operative group of 5 patients with exodeviation of 10PD through 30PD(mean 21PD) at postoperatively 2 weeks and 20PD through 35PD(mean 24PD) before operation of consecutive exotropia. In the second operatio, 3 patients with adduction limitation were operated with advancement of medial rectus or recession of lateral rectus of same eye. Four patients showed orthotropia at postoperatively 2 weeks, and 2 patients revealed orthotropia at the final visit. The results suggest that the degree of deviation and adduction limitation will influence the choice of therapy and second operation method.
