2.A Clinical Investigation of Patients with Psoriasis.

Psoriasis is one of the relatively common cutaneous disorders, affecting 0.1% to 3.0% of the general population in the world. However, it is recorded that the disease is characterized by its highly variable clinical manifestations and incidence influenced by the racial, geographic and environmental factors. There is a dearth of information for the incidence and clinical manifestations of psoriasis, with the exception of two small-scale and one questionnaire surveys in Korea. This study was aimed to investigate the clinical manifestations and clinic incidence of psoriasis and was based on analysis of 301 psoriatics seen between January, 1978 and August, 1981 (3 years and 8 months) at the Department of Dermatology, Seoul National University Hospital. The results were summarized as follows; 1. In this series of 301 patients, 50.5%(152) were male and 49.5%(149) were female. No significance was attached to the sex preponderance. More than three quarters were aged between 20 and 29 years. 2. The mean age of onset in male was 25.7 years and in female it was 23.0 years, indicating a slightly earlier onset in female. The peak age of incidence in male was between 20 and 29 years and in female, between 10 and 19 years. 3. The mean duration of t.he diseases for the total sample was 7.9 years.

4.A Case of Postcaval Ureter.

Post-caval or retrocaval ureter is a congenital anomaly of the vascular system. This anomaly almost. always involve the rt. ureter and occurs more frequently in male We have reported a 36 years male of rt. postcaval ureter that was resulted marked hydronephrosis due to compression of rt. ureter by vena cava. Three months after the operation, excretory urogram showed previous caliectasis and pyelectasis were moderately reduced and previous dislocated ureteral course was corrected.

5.PSYCHOLOGICAL ASPECT OF PATIENT SATISFACTION AND ACCEPTANCE OF COMPLETE DENTURES.

In spite of the progress in techniques and materials of the prosthodontic rehabilitation of patients with complete edentulous arches, prosthodontists still face patients' complaints about dissatisfaction and discomfort from their dentures. In the past several decades, prosthodontists tried to find the factors that influence the patient's satisfaction with dentures. However the results are contraversial especially with the psychological factor. In this study using patient denture satisfaction questionnaire' and Hopkins Symptom Checklist, we tried to find the correlation between the patient's denture satisfaction and the patient's psychological aspects. 23 complete edentulous patients who have visited the Department of Prosthodontics, Yonsei University Dental Hospital from September 1998 to June 1999 for complete denture treatment were asked to complete the questionnaires 4 to 6 weeks after the upper and lower complete delivery. After the measurement of validity of the questionnaires, correlations between the patient's general satisfaction with their new upper and lower complete dentures and the other satisfaction questions including the satisfactions with esthetics, retention, mastication, speech, comfort and other peoples's opinion and between the patient's general satisfaction with their new upper and lower complete dentures and the 5 symptoms of Hopkins Symptoms Checklist including somatic, obsessivecompulsive, interpersonal sensitivity, depression and anxiety were analyzed. Among the several satisfaction questionnaire items, comfort with the lower denture showed highest relation to the patient's general satisfaction with dentures. However, only the anxiety scale of Hopkins Symptom Checklist among the other symptom scales was related to the patient's general satisfaction with dentures. The two questionaires used in this study turned out to be valid means of analyzing patient's denture satisfaction and psychological status before and after the complete denture treatment.

6.Eosinophilic Granuloma of the Proximal Humeral Epiphysis: A Case Report

Eosinophilic granuloma of bone is the most common variant of histiocytosis X. Most of the lesions occur in the skull, ribs, spine or long bone and may be single or multiple. The occurrence in a growing epiphysis is extremely rare and only seven cases have been reported in the literature. A 13 year old boy with eosinophilic granuloma occurring in proximal humeral epiphysis was seen at Seoul National University Hospital in Nov. 1984 and was treated with curettage and autogenous bone graft followed by radiotherapy. The patient was followed for 13 months and complete healing was obtained.

7.Total Wrist Arthroplasty (Three Cases Report)

We have performed three total wrist arthroplasty of Meuli type on the dominant wrist of three female patients who have been suffering from the classical rheumatoid arthritis for more than two years, and after follow them for average twenty months postoperatively, we have found, even the complications and reoperation rate after total wrist arthroplasty are reported high in literature, three patients have reasonably satisfied with the remarkable relief of pain and the functional abilities of their wrists. It is our belief, even if the total wrist arthroplasty is also still in controversy, that in those patients with bilateral wrist involvement by the rheumatoid arthritis in whom arthrodesis was done on one wrist, then a total wrist arthroplasty on dominant wrist is a most prefer way to reconstruct the disabled wrists.

9.Application of HHIE-S(Hearing Handicap Inventory for the Elderly-Screening version) to screening test of noise-induced hearing loss.

The study conducted from May to September in 1994 to investigate applicability of the Hearing Handicap Inventory for the Elderly-Screening version(HHIE-S) in parallel with the puretone audiometer to the initial screening test of noise-induced hearing loss(NIHL) in some noise-exposed workers. Subjects were selected by systemic sampling that took every fifth person from 6,700 workers taking the annual occupational health examination by the department of Health Maintenance of Dongsan Hospital Keimyung University in Taegu. The authors administered the pure-tone audiometric test and self-reported questionnaire of HHIE-S including items of sociodemographic and job-related variables concurrently. The final subjects analysed were 1,019(488 males and 531 females) excluding fourteen persons who had many missing values in their questionnaires. The reliability coefficients of HHIE-S scale by Cronbach's alpha were 0.84. In the univariate analysis of hearing handicap measured by the HHIE-S, work duration, military service and the hearing threshold loss at 1kHz and 4kHz was the only selected variable explaining the hearing handicap in males and hearing threshold loss at 1kHz and 4kHz, age, and work duration were selected in females. In ROC curves for HHIE-S scores against NIHL as gold standard which was defined by the follow-up audiogram as more than 30dB of the average of 0.5/1/2kHz and 50dB at 4kHz, the optimal cutoff for the parallel HHIE-S appeared to be 8. The results suggest that HHIE-S appeared to have some reliability and validity in this data and might be used in screening NIHL in parallel with pure-tone audiometer in noise-exposed workers.

10.A Case of Generalized Familial Benign Pemphigus.

Familial benign pemphigus(Hailey-Hailey disease) is a rare hereditary dermatosis that begins in the 2nd or 3rd decade of life. The skin lesion is characterized by a localized, recurrent eruption of small vesicles on an erythematous base. During its course there are remissions and exacerbations. It seldom begins in early childhood or after the age of 50. The main treatment modalities are conservative ones. A 59-year-old woman with familial benign chronic pemphigus presented with a 10 year history of generalized pruritic recurrent skin lesions on her neck, axilla, inguinal, antecubital, and trunk area. Physical examination showed moist, macerated, fissured and scaly patches on an erythematous base in the axillae, groins, neck, antecubital, and trunk. A biopsy specimen showed extensive suprabasal separation containing acantholytic cells.
