2.Application of HHIE-S(Hearing Handicap Inventory for the Elderly-Screening version) to screening test of noise-induced hearing loss.

The study conducted from May to September in 1994 to investigate applicability of the Hearing Handicap Inventory for the Elderly-Screening version(HHIE-S) in parallel with the puretone audiometer to the initial screening test of noise-induced hearing loss(NIHL) in some noise-exposed workers. Subjects were selected by systemic sampling that took every fifth person from 6,700 workers taking the annual occupational health examination by the department of Health Maintenance of Dongsan Hospital Keimyung University in Taegu. The authors administered the pure-tone audiometric test and self-reported questionnaire of HHIE-S including items of sociodemographic and job-related variables concurrently. The final subjects analysed were 1,019(488 males and 531 females) excluding fourteen persons who had many missing values in their questionnaires. The reliability coefficients of HHIE-S scale by Cronbach's alpha were 0.84. In the univariate analysis of hearing handicap measured by the HHIE-S, work duration, military service and the hearing threshold loss at 1kHz and 4kHz was the only selected variable explaining the hearing handicap in males and hearing threshold loss at 1kHz and 4kHz, age, and work duration were selected in females. In ROC curves for HHIE-S scores against NIHL as gold standard which was defined by the follow-up audiogram as more than 30dB of the average of 0.5/1/2kHz and 50dB at 4kHz, the optimal cutoff for the parallel HHIE-S appeared to be 8. The results suggest that HHIE-S appeared to have some reliability and validity in this data and might be used in screening NIHL in parallel with pure-tone audiometer in noise-exposed workers.

3.Air Shadow Containing Mass Lesion in the Chest.

A 70-year-old female Patient was admitted due to the impression of lung abscess via local clinic. Chest P-A showed air shadow containing huge mass lesion in the right lower lung field, pericardio phrenic angle. But the patient complained of only mild upper respiratioy tract infection symptoms and the laboratory tests were within normal limits. Barium enema and chest C-T taken right after barium enema were performad in suggesting of the diaphragmatic hernia. Barium tilled transverse colon and associated omentum were in the right anterior hemithorax surrounded by the hernial sac. The mass lesion shown in the chest P-A was compatible with the diaphragmatic hemia, Morgagni type. Operative reduction of the herniated bowel and simple closure of the diaphragm was performed.

4.A Case of Ocular Ischemic Syndrome with Neovascular Glaucoma.

Ocular ischemic syndrome is a disorder which includes the ocular symptoms and signs secondary to marked carotid artery occlusion, and typically occurs in elderly men. The ocular signs are unilateral injected eye with corneal edema, anterior uveitis, rubeosis iridis, neovascular glaucoma and cataract. On ophthalmologic examinations, unilateral venous dilation, narrowed retinal arteries, equatorial dot-blot hemorrhage and disc or retinal neovasculization are oftenly noted. The characteristic angiographic findings are delayed choroidal and retinal circulations associated with ipsilateral internal carotid artery occlusion. We have experienced a case of ocular ischemic syndrome with neovascular glaucoma, who is 61 year old man with right ocular pain and visual disturbance. Angiographic examinations revealed nearly total occlusion of ipsilateral proximal internal carotid artery.

5.Renal Excretion of Pancreatic Enzyme in Dogs.

It is generally accepted that in acute pancreatitis, the enzymes normally excreted by the pancreas are released from the disrupted parenchyma into the extraductal space and taken up by way of the lymphatics and capillaries. The enzymes in the blood stream may appear in high concentration in the serum. Therefore, serum amylase and lipase determinations has long been a mainstay in the diagnosis of acute pancreatitis and other pancreatic diseases. However, many investigators have claimed that the urinary output of amylase may be elevated more consistently in acute pancreatitis than in the serum concentration of either amylase or lipase, and urinary amylase measurement is a more sensitive reflection of the presence of pancreatitis and of its clinical course than is the measurement of serum amylase or lipase. Clinically, one of the ominous signs which may develop during the early course of acute pancreatitis is severe hypotension. But, no agreement has been reached among investigators as to the cause of the hypotension, although several investigators have implicated a blood volume deficiency resulting form inflammatory process, and hypercalcemia. Perhaps, the majority have attributed the hypotension to systemic effect of some of the pancreatic enzymes, especially trypsin. Nevertheless, the correction of these factors sometimes fail to restore a normal blood pressure clinically. The purpose of the present investigation was to observe the relationships between serum concentration and urinary output of pancreatic enzymes, and to determine the degree of hypotension resulting from the systemic administration of pancreatic enzymes. These experimental procedures, consisted of heteroinfusion of human pancreatic juice and homoinfusion of canine pancreatic emulsion intravenously, and pancreatic ductal ligation in dogs. Blood and urine samples for the enzyme analysis were collected serially thorough the femoral vein and ureteral catheter before and after the procedure. Blood pressure was measured consistently by the kymograph before and after infusion of pancreatic juice. Activities of amylase and lipase were determined by methods of Nelson and, Cherry and Crandall, respectively. The results obtained are summarized as follows; 1. Following intravenous infusions of pancreatic juice exogenously. serum and urine concentrations of amylase and lipase increased rapidly, but these enzymes decreased rapidly in urinary excretion and gradually in serum concentration. Urinary recovery of amylase was approximately 10% of the total infused amount of pancreatic juice at the end of 4 hours. 2. Following ligation of the pancreatic duct, the amylase and lipase levels of serum rose gradually and reached the maximum at 24-48 hours after ligation and then gradually fell. The output of these enzymes in the urine were relatively constant while serum enzymes were increased. 3. When the human pancreatic juice was infused, hypotension was pronounced, and it was deeper and more prolonged in hypotensive effect with infusion of highly concentrated juice in the enzyme activities. With human pancreatic juice, a more sustained hypotension occurred than was observed after infusion of canine pancreatic emulsion. As a result of this investigation, it is felt that the hypotension in acute pancreatitis is probably the result of pancreatic enzymes itself. 4. In postinfusion period, the urine volume was markedly decreased following hypotension, and the urine volume was increased following blood pressure to normal level. This suggests that urine volume may diminish resulting from transient acute renal failure due to hypotensive effect by pancreatic enzymes.

6.Video-guided Laser Ablation of the Prostate (V-LAP): Evaluation for Effect of Laser Prostatectomy with Transrectal Ultrasonography.

The number of old patients suffering from benign prostatic hyperplasia is growing. TURP is still "gold standard" of relieving infravesical obstruction due to BPH, but the persistently high postoperative morbidity after TURP recently spurred the development of less invasive treatment options. Laser ablation of the prostate is a relatively new surgical treatment for BPH. According to studies published to date, laser prostatectomy is a simple and effective BPH-treatment modality with a low complication rate. We carried out Nd : YAG laser prostatectomy under visual control (VLAP) in 57 patients with BPH, aged 53-80 years (mean: 64.9 years). Preoperatively, the maximal flow rate (Qmax) ranged from 6-14 ml/sec (mean: 9.5 ml/sec), the residual urine volume (RUV) was 20-300 ml (mean: 109 ml), and the I-PSS ranged from 18-28 (mean: 20.4). The prostatic volume, measured with transrectal ultrasonography, was 26-72 gm (mean: 43.3 gm). There was no significant changes of hemoglobin, hematocrit and serum electrolytes, perioperatively. In 6 patients (10.5%), retention was developed after removal of urethral catheter at postoperative fifth day and so urethral catheter was reinserted for 5-7 days. After 6 months, Qmax was 9-22 ml/sec (mean: 15.4 ml/sec), RUV was 0-50 ml (mean: 15 ml), the I-PSS was 4-21 (mean: 8.3), and the prostatic volume was 11.3-51.9 gm (mean: 24.9 gm). In most patients, the prostatic volume was decreased as 42.6%, and Qmax, RUV and I-PSS were improved noticeably. Transrectal ultrasonography showed defect of bladder neck and prostatic urethra during immediate postoperative period, but normal prostatic architecture after 6 months In conclusion, the laser prostatectomy for BPH is considered the most promising alternative to TURP.
