1.Effects of Steroids and vitamin D Derivatives on Differentiation and Proliferation of Cultrured Osteoblast Cells.

This study was designed to evaluate of the effects ofB-estradiol, progesterone, dexamethasone, hydrocortisone, calcitriol and alfacalcidol on the cell proliferation and differentiation in the rat osteoblast-like osteosarcoma cells ROS 17/2.8. The proliferation of ROS 17/2.8 cell treated with 1 M B-estradiol, 1 M progesterone, 1 M hydrocortisone, 1 M dexamethasone, 10 (-8) M calcitriol and 10 (-8) M alfacalcidol was decreased by 19%, 56%, 60%, 64%, 28% and 38% of control, respetively. Production of TGF-Bl by ROS 17/2.8 cells treated with B-estradiol, progesterone, hydrocortisone, dexamethasone, calcitriol and alfacalcidol was elevated by 30%, 18%, 60%, 48%, 405% and 452% of control, respectively. Alkaline phosphatase activity of ROS 17/2.8 cell treated with hydrocortisone, dexamethasone, calcitriol and alfacalcidol was increased by 3.4 times, 12.9 times, 10.0 times and 5.1 times of control, respectively, but with B-estradiol and progesterone decreased by 39% and 10% of control respectively. Northern blot experiments demonstrated that calcitriol and alfacalcidol increased mRNA levels of both osteocalcin and osteopontin.

3.Clinical Evaluation of Unmarried Maternal Neonates Admitted to NICU.

PURPOSE: This study was done to see whether neonates born to unmarried mothers have higher rates of prematurity, low birth weight, perinatal morbidity and mortality than normal married group. METHODS: 88 neonates of unmarried mothers who were admitted in our NICU during the 5-year period from January 1987 through December 1991 were evaluated, and as the control group served 276 neonates of married mothers in 12 randomly selected weeks extended over the year during the same period. In statistical analyses, all data were analyzed by x2 test, Student t-test, and regression equation. RESULTS: 1) Unmarried mothers were 19.9+/-2.5(SD) year-old, with the majority(64.8%) falling into to age group between 16 and 20 years, whereas the control group was significantly older with 28.8+/-3.9 years. 2) Sex ratio was 1.38 : 1, with male prepondering, mean gestational age 35.5+/-3.7 weeks and mean birth weight 2,240+/-640g, in the unmarried group, differing significantly from the control group with 38.5+/-37 weeks and 2,910+/-750g. 3) Incidences of prematurity and low birth weight (LBW) were 70.5% and 75.0%, and total mortality was 28.4%. These values were significantly higher than in the control group, with 22.5%, 32.2%, 7.2%, respectively, and prematurity and LBW rate were also significantly higher in both groups of the same maternal age. Birth weight of unmarried group was significantly lower than control group in the same gestational age. Gestational age, birth weight, mortality rate were not significantly correlated to maternal age in both unmarried and control groups. 4) For the neonates of unmarried mothers, the majority was delivered either by midwife or in local obstetric clinics, and most of them(57.9%) were delivered by induction. 5) Neonatal diseases were jaundice, respiratory distress syndrome(RDS) and infection in the decreasing order, and incidence of RDS was significantly higher than in control group. But the death rate of RDS only tended to be higher in the unmarried-group. CONCLUSIONS: The neonates born to unmarried mothers have significantly higher rates of prematurity, LBW and mortality, and lower birth weight than the control group in the same gestational age. For neonatal diseases, incidence of RDS was significantly higher, and its mortality was significantly higher in the unmarried-group. Awareness on the seriousness of the problems leading to preventive measures against juvenile and unmarried child births, along with improved managements of babies born to unmarried mothers are urgently advocated.

4.The change of myoepithelial cell after ligation and cut of submandibular gland duct in rabbit.

Obstructive sialadenitis is one of common disease in salivary gland, and most common histologic features are loss of acinar cell and ductal dilatation associated with fibrosis, and infiltration of inflammatory cells. Although many experimental studies has been accomplished for the salivary acinar cell change in obstructive salivary gland disease, studies for myoepithelial cell were deficient. This study is designed for salivary gland tissue change, especially myoepithelial cell when nonspecific chronic sialadenitis or salivary duct injury by duct obstruction or cut can be occurred that is common encounted clinically. After ligation and cutting of submandibular gland of rabbit, groups of aminmal were sacrificed at 1, 2, 4 weeks postoperatively, submandibular gland were removed. The histopathologic evaluation was done with light microscopy. And, with immunohistochemical staining with alpha-smooth muscle actin, characteristics of myoepithelial cell were examined. With transmission electron microscopy, ultrastructure of myoepithelial cell were examined for distribution and ultrastructure of myoepithelial cell. The results were obtained as follows: 1. In the histopathologic evaluation, ligation and cutting group of 1 week, linkage of myoepithelial cell associated with acinar atrophy and degeneration were disappeared in both group. 2. More prominent squamous metaplasia was seen in acinar cells of ligation group of 2 weeks experimental rabbit than cutting group. 3. Acinar cells are nearly disappeared in both ligation and cutting group of 4 weeks, and myoepithelial cell also disappeared associated with acinar cell atrophy, and duct-like structure composed by squamous cells by squamous metaplasia in acinar cells were distributed. 4. In immunohistochemical study, both ligation and cutting group alpha-SMA distribution were diminished at 1 week experimental rabbits, but myoepithelial cell was more diminished in ligation group than cutting group, which were distributed around cells of squamous metaplasia. 5. Nuclear condensation, chromosome margination, and cytoplasmic vaculoation were appeared in myoepithelial cell of both cutting and ligation group after 1 week with transmission electron microscopy. But degenerative substance were seen in cytoplasm of myoepithelial cell of ligation group of 4 weeks. From the results obtained in this study, atrophy and degeneration of myoepithelial cell was more prominent in duct ligation group than duct cutting group, and myoepithelial cells were seen around cells squamous metaplasia of acinar cell.

6.Analysis of Published Papers and their Keywords in the Korean Journal of Women Health Nursing (2003-2006).

PURPOSE: This research was aimed to explore the major subjects and research theme and to classify characteristics of the key words in recent Korean Journal of Women Health Nursing. METHOD: With survey design, whole 103 published papers during 2003-2006 were analyzed with structured analysis form; research subjects, research methodology, data analysis, and key words of the published papers were classified and extracted. Various theme was founded and classified to 9 domains. RESULT: Most of the research subjects were women. Eighty-eight percent of papers conducted quantitative research; 83% chose convenience sampling and 69% used survey design, while experimental design was 29%. Key words were categorized 9 domains: demographic, health related concept, health behavior(intervention), sex-related, life event, disease, et al. Among 9 domains, health related concept, especially psycho-social topics such as depression, anxiety, stress were mostly studied and then physiologic topics such as labor pain, fatigue, and menstrual symptoms. CONCLUSION: Most of the subjects were women and the most common domain studied was health related concepts in Korean Journal of Women Health Nursing. And key words and topics were on women's health issues. We can conclude that Korean Journal of Women Health Nursing has published by her own philosophy.

7.A Case of Fibroma in the Female Urethra.

Fibromas of the urethra are extremely rare. We experienced a case of fibroma in the female urethra recently. 48-year-old female patient was admitted to the Department of Urology of Seoul National University Hospital due to a protruded mass on external urethral meatus. The mass was removed surgically and confirmed as a fibroma pathologically. It was a huge mass, 17 X 9 X 7 cm in size, irregularly lobulated, firm and well encapsulated. Histologically it was composed of loosely textiled spindle cells of fibroblastic origin with myxoid changes.

8.Two Cases of Foreign Bodies in the Bladder.

We discovered two plastic tubes which were used as masturbating tools in the bladder. The following is a report of two young boys who slipped plastic tubes into the bladder through the urethra for the purpose of masturbation and lost them in the bladder while attempting to remove them. Case 1. : Y. D. S., 18 years-old, Admitted because of hematuria for 5 days. Cystoscopically, the bladder showed diffuse hyperemia and a coiled calcified plastic tube. It was removed by operation. The length of the tube was 0.58m and the diameter was O.4 cm. Case 2. : K. S. K., 18 years-old. Admitted because of urinary incontinence for 4 months. Cystoscopically. the bladder showed diffuse hyperemia and a coiled calcified plastic tube. It was removed by surgical operation. The length of the tube was 1.2m and the diameter was 0.5cm.

9.A Study on the Proteins that Interact with Human Nebulin SH3 Domain.

OBJECTIVE: bjective: By identifying the unknown substance responsible for binding with nebulin SH3 domain within the sarcomeric Z-line, we tried to find out Z-line structure which plays an important role on muscle contraction and maintenance of muscle funtion. METHOD: First, the bait plasmid was made by binding the DNA binding domain of Gal4 protein of yeast and the SH3 domain. Second, library plasmid was made by binding activation domain and human skeletal cDNA library. Then, the base sequence of the clone, produced by combining the two proteins expressed by transgenically converted plasmid in yeast, was analyzed. RESULT: We screened out six true positive clones and analyzed the base sequence of the two of six clones. We identified them to be alpha-actinin2. CONCLUSION: We can theorize that Neublin SH3 domain and alpha-actinin2 plays a vital role for the integration of Z-line. Thus, this is an important data in further studying muscle functions, mechanisms, and muscular disease as well.

10.Usefulness of Chromoscopy Using Lugol Solution for Detection of Superficial Esophageal Cancer.

Background/Aims: Esophageal cancer is not an uncommon cancer in Korea, however, the prognosis still remains very poor with a 5 year survival rate bemg less than l0% mainly becauae of the delayed diagnosis. Although chromoscopy with lugol solution has been received to diagnose the esophageal cancer in an early stage without difficulty, its clinical use has not been popular yet in Korea. This study was performed prospectively to evaluate the usefulness of the chromoscopy for the detection of superficial esophageal cancer in risk patients for esophageal cancer. Methods: Ninety-five patients were selected among persons who received gasiroscopy at Asan Medical Center between Jan. 1996 and May 1996 and were prospectively included for chromoscopy. Inclusion criteria for the chromoscopy were patients older than 60 years of age with smoking history of more than 30 packyears, and/or past or family history of cancers. After conventional endoscopic examination, lugol solution was sprayed to stain the glycogen granules in the epithelial cells. The size of unstained lesion was measured and stainability was classified into 5 grades. All lesions unstained were biopsied for histological diagnosis.(continue...)
