1.Comparison of polymer-based temporary crown and fixed partial denture materials by diametral tensile strength.

PURPOSE: The purpose of this study was to investigate the diametral tensile strength of polymer-based temporary crown and fixed partial denture (FPD) materials, and the change of the diametral tensile strength with time. MATERIAL AND METHODS: One monomethacrylate-based temporary crown and FPD material (Trim) and three dimethacrylate-based ones (Protemp 3 Garant, Temphase, Luxtemp) were investigated. 20 specimens (the empty set 4 mm x 6 mm) were fabricated and randomly divided into two groups (Group I: Immediately, Group II: 1 hour) according to the measurement time after completion of mixing. Universal Testing Machine was used to load the specimens at a cross-head speed of 0.5 mm/min. The data were analyzed using one-way ANOVA, the multiple comparison Scheffe test and independent sample t test (alpha = 0.05). RESULTS: Trim showed severe permanent deformation without an obvious fracture during loading at both times. There were statistically significant differences among the dimethacrylate-based materials. The dimethacrylate-based materials presented an increase in strength from 5 minutes to 1 hour and were as follows: Protemp 3 Garant (23.16 - 37.6 MPa), Temphase (22.27 - 28.08 MPa), Luxatemp (14.46 - 20.59 MPa). Protemp 3 Garant showed the highest value. CONCLUSION: The dimethacrylate-based temporary materials tested were stronger in diametral tensile strength than the monomethacrylate-based one. The diametral tensile strength of the materials investigated increased with time.

2.Metabolism of C(14)-glucose by plerocercoid of Diphyllobothrium sp..

The glucose uptake rate by plerocercoid of Diphyllobothrium sp. was a mean value of 5.35+/-0.80 micro-mole/hr/g of wet wt, and total CO(2) production rates by the plerocercoid larva averaged 7.54+/-0.73 micro-mole/hr/g of wet wt. The relative specific activity into respiratory CO(2) showed a mean value of 7.30 +/-0.90 per cent. The rate of CO(2) production derived from medium C(14)-glucose was a mean of 0.58+/-0.13 micro-mole/hr/g of wet wt. Therefore, the average value of 1.92+/-0.38 per cent of glucose utilized by the larvae from the medium C(14)-glucose was oxidized to respiratory CO(2). The tissue concentration of glycogen in plerocercoid larva was a mean of 46.28 +/-2.23 mg/g or 4.63+/-0.22 per cent/g of wet wt., and the turnover rate of glycogen pool was a mean of 0.049 +/- 0.012 %/hr or 0.010 +/- 0.003 mg/hr/g of wet wt. The average value of 2.76+/-1.00 per cent of glucose utilized by the larvae from the medium C(14)-glucose was incorporated to the glycogen. These data accounts for that only 5 per cent of the utilized glucose by the plerocercoid larvae participated in furnishing the oxidation into respiratory CO(2) and the synthetic process into glycogen.

3.Treatment of Fixed Lumbosacral Kyphosis by Posterior Vertebral Column Resection: A preliminary report.

STUDY DESIGN: This is a retrospective study evaluating the efficacy of new surgical technique for treatment of fixed lumbosacral kyphosis. OBJECTIVES: To report a new method of vertebral column resection and to determine its efficacy in the treatment of fixed lumbosacral kyphosis. SUMMARY OF BACKGROUND DATA: The treatment of fixed severe spinal deformity is very difficult and only a few surgical methods are reported. The vertebral column resection from anterior and posterior is a radical method to treat the severe deformity but it has many problems with anterior and posterior approachs. MATERIALS AND METHODS: A new surgical method of vertebral column resection from posterior approach only(PVCR) was carried out in 5 patients of fixed lumbosacral Tb kyphosis from October 1997 to March 1998. The mean age was 43.4 years(range 35-61 years), and four were female and one male. The average postoperative follow-up period was 5.5 months(ranged from 2 months to 12 months). An average of 2.6 vertebrae was resected. The degree of sagittal curves was measured using the Cobb technique preoperatively and postoperatively. the rejional lumbosacral kyphotic angle and compensatory thoracic curvature from 74 to 712. The sagittal imbalance was measured by distance from C7 plumb line to postero-superior corner of 51 using 14 x 36 inch standing lateral radiograph. RESULTS: The sagittal deformities were corrected from an average of kyphosis 37.4 to lodosis 8.4 at lumbosacral level and from an average of lodosis 31.6 to lodosis 8.2 at thoracic level. The sagittal imbalance was improved from an average of +11.7cm to +2.2cm, for a posterior immigration of 9.5cm. The mean operating time was 334 minutes and with an average blood loss of 4338m1. Complication comprised of motor weakness with deep wound infection in one case and superficial wound infection in another case. The transient hip flexor weakness was seen in all the patients. The hip flexor power recovered in 3-4 weeks. The patient of motor weakness and deep infection showed partial motor improvement and complete wound healing at last 2 month follow-up. CONCLUSION: one-stage posterior vertebral column resection is a promising technique for treatment of the severe fixed lumbosacral kyphosis.

4.The Change of Ocular Aberration after LASIK Surgery.

PURPOSE: To evaluate the change of ocular aberration after LASIK using Zernike's polynomial expansions. METHODS: Sixty eyes of thirty consecutive patients that underwent LASIK were examined prospectively. The ocular aberration was measured with Hartman-Shack aberrometer (WASCA Wavefront Analyzer, USA) at postoperative 30, 90, and 180 days. The postoperative values were compared with that of preoperation. RESULTS: The average age is 27.6 years old. The range of preoperative refractory error were from -3.00 Diopter (D) to -11.00 D. The increase of spherical and triangular coma aberrations (4.7- fold and 3.5- fold, respectively), with respect to preoperative values. There were statistically significant differences between preoperative and postoperative high order RMS at the postoperative 180 days (p<0.05). CONCLUSIONS: The postoperative increases of RMS in higher order aberration, coma aberration, triangular aberration was observed.

5.Direct Vertebral Rotation (DVR): A New Technique of 3-D Deformity Correction with Segmental Pedicle Screw Fixation in Adolescent Idiopathic Scoliosis (AIS).

STUDY DESIGN: A prospective study. OBJECTIVES: To introduce a new technique, direct vertebral rotation (DVR), and to compare the surgical results with those of a simple rod derotation (SRD). SUMMARY OF BACKGROUND DATA: Pedicle screw fixation, with a simple rod derotation maneuver, enables powerful coronal and sagittal plane corrections in scoliosis surgery. However, the ability for rotational correction is still unclear. METHODS: Thirty-eight AIS patients, treated with segmental pedicle screw fixation, were analyzed. The first group (n=17) was treated by DVR, and the second (n=21) by SRD. Having similar preoperative curve patterns, both groups were evaluated for the deformity correction and spinal balance. RESULTS: In the DVR group, the average preoperative AVR of 16.7 degrees was corrected to 9.6 degrees, showing a 42.5% correction, while in the SRD group, the correction was negligible, from 16.1 degrees to 15.7 degrees(2.4%). In the DVR group, the preoperative thoracic curve of 55 degreeswas corrected to 12 degrees(79.6%), and the lumbar curve from 39 degreesto 7 degrees(80.5%). In the SRD group, the preoperative thoracic curve of 53 degreeswas corrected to 17 degrees(68.9%), and the lumbar curve from 39 degreesto 16 degrees(62.2%). The average LIVT correction was 80.6 and 66.3% in the DVR and SRD group, respectively. There were statistically significant differences in the coronal curve, LIVT and rotational correction (p<0.05, Mann-Whitney u test). CONCLUSIONS: The segmental pedicle screw fixation with 'direct vertebral rotation'showed better rotational and coronal corrections than the 'simple rod derotation'.

7.A Case of Insulinoma Diagnosed as Epilepsy for 7 Years

Insulinoma is rare functioning islet cell tumor of pancreas and its main feature is frequent attacks of hypoglycemia. Because of frequent seizure, the disease is occasionally mistook as epilepy. We experienced a case of insulinoma with dilantin toxicity in a 45 year old patient who had been treated with dilantin for 7 years. Selective splenic arteriogram showed 0.8cm mass in the tail of pancreas which was proven insulin secreting tumor on histiopathological examination. After operation no more seizure attack was detected and her blood sugar level was well maintained within normal range. This case might provide an insight that the possibility of insulinoma should be considered in patient with episodic neurobehavioral dysfunction such as epilepsy.

Extraadrenal pheochromocytomas developed from the paraganglia other than adrenal medulla are uncommon tumors. Such lesions have been reported that as much as 10% of retroperitoneal paragliomas arise outside the adrenal gland. But their incidence and prognosis are changing partly as a result of progression in developmental mechanism, diagnostic tools and concepts including ultrastructure, immunohistochemistry. The ultrastructure of chromaffine neoplasm from adrenal and extraadrenal tissues has been studied with chrarnaffine granule specificity and type of catecholamine content. With rare exceptions, extraadrenal chrornaffine tumor mainly has high concentration of norepinephrine which is suggested as dict relationship to numoruos electrone dense granular bodies in the cell cytoplasrn. High or intermediate level of epinephrine in tissue is associated with cytoplasmic granular bodies of much less density, Recently we experienced the exception, a case of norepinephrine secreting pheochrom-ocytoma in the organ of Zuckerkandl with epinephrine granules in cytoplasm and this discrepancy probably due to the plasticity
