1.A Case of Generalized Lichen Nitidus.

Lichen nitidus(LN) is an unaommon chronic eruption of numerou tiny, discrete, usually flesh colored, shiny papules most cornmonly located on the penis, arms, for arms, and abdomen. It was described by Pinkus, first in 1901. The distribution of LN is most often locaized, but in some cases it may become generalized. A 10-year-old healthy boy was seen by our department because of many flesh colored, discrete, shiny papules on the whole body. The papules are skin colored, pinhcad sized, round, and flat-topped. The lesions appeared first at the age of 9 on the forehead and then neased in the number and extended downward to the neck, trunk, abdornen and both extremities. This case was successfully treated with topical corticosteroid.

2.Focal Dermal Hypoplasia Associated with Ichthyosis.

We report a case of focal dermal hypoplasia associated with ichthyosis in an 18-year-old Korean female. The patient showed a yellowish atrophic patch on the left side of her abdomen, abnormal skin pigmentation, nail dystrophy and ichthyosis since birth.We could also find skeletal and dental deformities. The biopsy specimen obtained from the yellowish atrophic patch showed marked attenuation of collagen fibers and ectopic fatty tissue nearly reaching up to the upper dermis.

6.Effects of Walking Exercise Program for Health Status in Patients on Continuous Ambulatory Peritoneal Dialysis.

BACKGROUND: It is necessary to exercise in Continuous Ambulatory Peritoneal Dialysis (CAPD) patients but it's difficult because of their physical condition and little motivation. The purpose of this study was to evaluate the effects of the walking exercise program on health status in CAPD patients. METHODS: A nonequivalent control group with pre- and post-test was designed to examine the effects of the program. Data collection was done from December, 2002 to June, 2003. The study participants were selected according to the criteria among the patients who were receiving CAPD at the department of Kidney Center, NIHC Ilsan hospital. The walking exercise program was consisted of an exercise education protocol, an exercise prescription, and a counselling protocol based on a framework of self-efficacy promotion. The exerciser group which was composed of 19 participants was educated based on an exercise education protocol and carried out walking exercise for two to four times a week upon taking verbal persuasion biweekly through telephone or face-to-face interview for 12 weeks, while 17 participants in control group were received no intervention. RESULTS: There was significant differences between two groups; the exerciser group showed better physical function (p=0.03), higher VO2 max (p=0.01) and higher serum albumin level (p=0.02) than the results of control group. However, there were no differences in lipid metabolism. CONCLUSION: The walking exercise program was found to have an effect to increase health status of CAPD patients. The results provided evidences for the importance of continuous physical activity in CAPD patients.

7.A Case of Pityriasis Rotunda Aggravated During Pregnancy.

Pityriasis rotunda is an uncommon dermatosis characterized by asymptomatic, multiple, widely distributed, strikingly circular hypopigmented or hyperpigmented, scaly patches on the trunk and extremities. Histopathologic findings are consistent with ichthyosis vulgaris. It has been associated with a variety of underlying systemic diseases. We report a case of pityriasis rotunda aggravated during pregnancy.

8.A Case of Childhood Hobnail Hemangioma.

Hobnail hemangioma is a benign vascular tumor that typically presents as a small, single lesion on the skin of the trunk or limb. It occurs mainly in young or middle-aged adults (mean age: 30 years) but rarely in childhood. Histologically, hobnail hemangioma is characterized by a biphasic growth pattern, with dilated vascular spaces lined by hobnail endothelial cells in the superficial dermis, and collagen dissection narrow vascular channels in the deeper dermal parts, as well as inflammatory aggregates, fibrosis, and hemosiderin deposits. We report a case of hobnail hemangioma in a 10-year-old girl who had a violaceous papule surrounded by an ecchymotic halo on the left thigh. She underwent treatment with surgical excision with primary closure. To the best of our knowledge, our case is the first reported case of childhood hobnail hemangioma in Korean dermatological literatures.

9.Immunohistochemical Labeling of Melanocytic Differentiation Antibodies in Melanocytic Nevi and Malignant Melanomas.

BACKGROUND: Malignant melanoma is the cancer derived from melanocyte and its prevalence and mortality has increased constantly. Recently, newly developed monoclonal antibodies such as antibodies to MART-1, tyrosinase, TRP-1, MITF and NKI-beteb are frequently used in the diagnosis of melanoma. OBJECTIVE: We evaluated these antibodies to see if there are differences of positivity and expression pattern of immunohistochemical labelings between melanocytic nevi and malignant melanomas. METHODS: We have examined 6 antibodies (antibody to S-100, tyrosinase, TRP-1, MART-1, MITF and HMB-45) in their expressions in 8 malignant melanomas and 24 benign melanocytic nevi. RESULTS: No remarkable difference in positivity of staining between benign melanocytic nevi and malignant melanomas was observed. The staining pattern for S-100 protein is homogenous in all melanocytic nevi and malignant melanomas. HMB-45 showed staining confined to the junctional and superficial dermal components in both diseases. Tyrosinase and TRP-1 showed staining confined to the junctional and superficial dermal cells in most benign melanocytic nevi and variable in malignant melanomas. Although more variable patterns were found in the malignant melanomas, MART-1 demonstrated homogenous staining of all cells in the benign lesions. The staining pattern of MITF in melanocytic nevi was principally in junctional and superficial dermal components, whereas it was homogenous in malignant melanomas. CONCLUSIONS: It should be stated that none of the melanocytic differentiation antibodies has any diagnostic value in differentiating between melanoma and nevus cells, but new monoclonal antibodies taken together could contribute to the detection of malignant melanoma due to their high specificity and complementary natures.

10.The Effects of Intravitreal Bevacizumab Injection According to the Type of Diabetic Macular Edema.

PURPOSE: To investigate the effects of intravitreal bevacizumab injection on diabetic macular edema (DME) of different types classified using Optical Coherence Tomography (OCT). METHODS: A total of 82 eyes with refractory DME were enrolled. The DME was classified into diffuse, cystoid, or serous type based on the OCT findings. All cases had received an intravitreal injection of 1.25 mg bevacizumab each month for three months. Foveal thickness, macular volume, and best corrected visual acuity (BCVA) were measured before and one month after the injection, and the interval changes in these parameters were compared. RESULTS: The types of DME were classified as follows: diffuse macular edema 50%, cystoid macular edema 31.7%, and serous macular detachment 18.3%. Foveal thickness and total macular volume after intravitreal bevacizumab injection decreased in all types, and the cystoid and serous types showed better response than did the diffuse type with regard to foveal thickness. However, there were no significant differences in the extent of the change in total macular volume or BCVA among the three types of DME. CONCLUSIONS: There were differences in the therapeutic effects of intravitreal bevacizumab injection among the different types of DME classified using OCT. These differences may be associated with the stabilizing effect of the bevacizumab. This effect was stronger with regard to vascular permeability, the primary factor in the pathogenesis of the cystic and serous types, than with regard to leakage from the microaneurysm, the primary factor in the pathogenesis of the diffuse type. Practical application of bevacizumab to eyes with different DME types will help in further evaluating intravitreal bevacizumab injection as a treatment option for DME.
