4.A Case of Congenital Arteriovenous Fistula at Left Subclavian Artery Accompanied by Notching on Superior Aspect of Rib.

High output heart failure due to congenital arteriovenous fistula is a rare disease and is difficult to treat. A 27 year old man, suffering from dyspnea, palpitation, lower extremity edema, and abdominal distension admitted May, 1987. At admission there were continuous thrill and bruit at left supraclavicular and infraclavicular area. The heart sound was irregular and rapid, and grade IV/VI pansystolic murmur was audible at the apex. Chest PA showed marked cardiomegaly with pleural effusion and notching at superior aspect of left 2nd rib. Selective left subclavian artery angiogram showed extensive congenital arteriovenous fistula at the area of left subclavian artery and its branches. After setrial feeding artery embolization and palliative operation, the fistula size decreased much with clinical improvement.

6.Work Stress and Mental Health of the Industrial Workers.

Information on work stress and mental health status of industrial workers was required for optimum staffing and health care management. This study dealt with the relationship between mental health status examined by PSI(psychiatric symptom index) and socio-demographic variables. Especially this study attempted to find relevant determinants of the stress and mental health through multiple stepwise regression analysis based on data obtained from 687 occupational workers. The findings of this study were as follows; Mean scores of symptom dimension were higher in youth, female, more educated, unmarried and divorced, and workers in the noisy condition and short duration in job. And it showed high scores in long duration in job for anxiety; college educated for anger; unmarried for cognitive disturbance. The factors affecting the mean scores of psychiatric symptom index were varied according to the types of symptom; age, sex and duration in job for anxiety; age, sex, living with parent, education and marital status for anger; sex and marital status for depression; noisy condition and sex for cognitive disorder.

7.Symptomatic Os Subfibulare: Two Cases Reports.

Os subfibulare is a accessory bone arround the lateral maleolus, but it is different from many other accessory bones in foot and ankle in its development related to trauma and frequent symptoms of ankle instability. Accessory bone is a developmental anomaly which fails to unite to main secondary ossification center and usually asymptomatic. But os subfibulare is developed by recurrent ankle sprains in younger age or non-union of avulsion fracture of anterior talofibular ligament and causes instability of ankle. We experienced two cases of symptomatic os subfibulare in adolecents and report them with review of literatures.

8.Different Patterns of Meniscal Tears in Recent Anterior Cruciate Ligament ( ACL ) Ruptures and Chronic ACL-deficient Knees.

PURPOSE: We tried to outline the incidence and characteristics of meniscal tears in recent lesions and chronic insufficiency of the ACL. MATERIALS AND METHODS: Through this retrospective study of 103 reconstructions of the anterior cruciate ligament (ACL) performed between Jun 1996 to Sep 1998, according to a more accurate evaluation, ACL-deficient knees can be classified into two different stages (recent, chronic injuries). RESULTS: Overall, there was a significantly increasing incidence of meniscal tears in chronic injuries (P<0.05). A predominance of lateral meniscal tears were demonstrated with recent injuries, whereas the incidence of medial meniscal tears increased significantly with chronic injuries (P<0.05). CONCLUSIONS: This study highlights the increase of lateral meniscal lesion in recent ACL rupture and increase of severe meniscal lesion, especially medial meniscus, with progressive worsening of knee instability after an ACL injury. Although not significant, meniscal tears in the recent injuries were more amenable to repair. The results suggest that maximal meniscal preservation is best achieved with ACL reconstruction shortly after injury.

9.A Clinical Study on the Anti-Hypertensive Effect of Cilazapril in Mild to Moderate Hypertensive Patients.

In order to investigate the efficacy and safety of oral cilazapril, a new angiotensin converting enzyme inhibitor, on essential hypertension, a single daily dose of 2.5 to 5.0mg cilazapril was administered in 30 hypertensive patients with diastolic blood pressure in the range of 95??15mmHg while off all other anti-hypertensive agents for 10 weeks. Blood pressure and heart rate were measured every 2 weeks. The complete blood count with platelet count, blood chemistry by SMA-12 including lactic dehydrogenase and serum electrolytes, and urinalysis were performed at 4th and 10th week of therapy. The electrocardiography was performed at the beginning and the end of treatment period. Any kinds of side effects were actively questioned by the examining physicians. The following results were obtained : 1) The mean age was 49.2 years, and the ratio of male-to-female was 1 : 1.3. 2) Blood pressure started to fall significantly within 2 weeks of treatment with cliazpril 2.5mg(M+/-S.E., 15.4+/-17.4mmHg vs 138.5+/-23.3, 100.3+/-6.2 vs 89.4+/-6.6, p<0.05), and after 6 weeks of treatment with a mean dosage of 2.84mg. diastolic blood pressure of all subjects was maintained below 90mmHg throughout the rest of trial. 3) Pulse rate or body weight were not significantly changed during the entire treatment period(69.3+/-6.0/min vs 10th week : 69.0+/-7.7, 64.7+/-7.4kg vs 63.6+/-6.7, p>0.05). 4) There were no significant changes in blood chemistry including blood sugar, cholesterol and electrolytes, except mild changes of serum creativine and alkaline phosphatase values. 5) Hematologic findings, urinalysis and electrocardiographic findings remained unchanged. 6)Side effects were mostly mild in nature without potentially serious episodes(dry cough : 20%, indigestion, headache, dizziness, in order), but there was 1 cases in whom the dosage was redyced due to postural hypotension. From the above results, cilazapril with the dosage of 2.5 to 5.0mg was effectvie and well tolerated in essential hypertensive patients with diastolic blood pressure of 95 to 115mmHg, and cilazapril seems to be appropriate for monotherapy of mild to moderate hypertensive patients.

10.Acute Generalized Exanthematous Pustulosis Probably Induced by Ampicillin.

Generalized pustular skin eruptions as a form of drug eruption is a rare entity. Recently this unique pustular dermatosis has been termed as acute generalized exanthematous pustulosis. We report on a 19-year-old man with acute generalized exanthematous pustulosis probably induced by ampicillin. The patient presented with erythematous and pinhead-sized subcorneal pustules after taking ampicillin for abdomimal pain. The patient complained of high fever and malaise. On further review of his history, generalized skin eruptions had been noted on at least three other occasions. Each episode occured following oral administration of antibiotics. A histological examination of a skin biposy specimen showed a subcorneal pustules with a few neutrophils, dermal edema and necrotic keratinocytes. After discontinuation of ampicillin, the eruption cleared within 4 days.
