1.Congenital Fibrous Papule of the Face.

A 2-month-old Korean boy presented with a solitary papule on the cheek which was noted at birth. Histopathologic findings were consistent with angiofibroma of fibrous papule of the face (FPF). FPF is known to affect adults, and congenital occurrence has not been reported to the best of our knowledge. We report a case of congenital FPF which showed a facial papule clinically and an angiofibroma histologically.

2.A Study on Expression of Ki-67 Antigen in Melanocytic Skin Lesions.

BACKGROUND: Evaluation of the cell proliferation activity of neoplasm is useful in making a diagnosis and prognosis. Immunohistochemical staining using MIB1 monoclonal antibodies has recently been demonstrated to assess tumor cell proliferation activity easily, because it can be performed on paraffin-embedded specimens and is available in a retrospective study. OBJECTIVE: The purpose of this study to investigate (a) the use of Ki-67 antibodies in distinguising between benign melanocytic nevus and malignant melanoma, and (b) the correlation between the Ki-67 labelling index(positive number per 100cells) and histological parameters such as the Breslow index, Clarks level, mitotic rate(mitoses per 100cells) in malignant melanoma. METHOD: A total of 37 formalin-fixed, paraffin-embedded melanocytic skin lesions(benign melanocytic nevus 21; malignant melanoma 16) were stained with MIB1 using the one hour method. The values of the Ki-67 labelling index were measured in all the lesions. For each case of malignant melanoma, the Breslow index, Clarks level and mitotic rate were quantitatively evaluated. RESULT: The Ki-67 labelling index averaged 2.6 +1.4 in benign melanocytic nevus and 22.4+ 6.2 in malignant melanoma. In malignant melanoma, the values of the Ki-67 labelling index were significantly correlated with Breslow index(r=0.764), Clark's level(r=0.885) and mitotic rate(r=0.754). CONCLUSION: Our findings suggest that Ki-67 may be a useful tool in distinguising between benign melanocytic nevus and malignant melanoma. According as Ki-67 labelling index correlates with well-established prognostic parameters(Breslow index, Clark's level, mitotic rate), it may therefore carry additional prognostic value for patients with malignant melanoma.

3.Palisaded Encapsulated Neuroma.

Palisaded encapsulated neuroma (PEN) is a distinctive benign cutaneous tumor that was first described by Reed et al. in 1972. We have recently experienced two cases of PEN. Clinically, these lesions were asymptomatic, flesh-colored or reddish brown papules or nodules. Light microscopy revealed several lobulated, partially encapsulated dermal tumors composed of interlacing Schwann cell fascicles. In some areas, palisading of nuclei was observed, but this was not characteristic. On Bodian staining, moderate to large numbers of tiny axons were observed. Electron microscopy demonstrated many myelinated axons incompletely invested by Schwan cell cytoplasm.

4.A Case of Lymphomatoid Papulosis with Atypical Clinical Manifestation.

We report herein a case of lymphomatoid papulosis showing atypical clinical manifestations. A 54- year-old man had had recurrent erythematous necrotic papules and nodues, numbering more than 200 lesions at the most aggravated time, on the trunk, extremities, buttok, and face for the last twenty years. Recently, he complained of tender swelling on the neck, axillae, and inguinal lymph nodes accompanied by mild fever and gerneral weakness. We had performed thorough examinations including biopsy from the skin lesions, lymph node, and bone marrow to detect some evidences of transforming to malignant lymphoma, but found no evidence of malignant lymphoma except aneuploidy on DNA histogram. The patients showed proinpt response to methotrexate 10-30mg par week wit,hout showing distinct side reactions to the accurvulated dose of 780mg for the last 16 months.

5.Efficacy of oral terbinafine the treatment of onychomycosis.

An open multicenter trial was conducted to assess the efficacy and tolerability of oral terbinafine in the treatment of onychomycosis during 10 months from April, 1992 to February, 1993. Of patients with clinically and mycologically proved onychomycosis recruited from the Department of Dermatology of 29 university and training hospitals in the Korea, 143 patients who could be followed up at the end of the study were evaluated. The group of fingernail infection received orally terbinafine 250mg once daily for 8 weeks and the group of toenail infection for 16 weeks. After stopping treatment, all patients were followed up for 12 weeks. The results were as follows: 1. On the mycological study at the initial visit, 101 causative organisms were isolated from 99 pateints and Trichophyton rubrum was common organism (40 strains). 2. The length of healthy, unaffected nail were increased significantly more than two-fold after the end of treatment and at the follow-up investigation, those were 11.8mm in fingernail group and 9.8mm in toenail group. 3. The mean of all clinical symptom scores on entry diminished significantly by the time of the end of treatment and cleared mostly at the follow-up. 4. The mycological cure rate(negative microscopy and culture) was 56.5% for fingernails and 69.3% for toenails at the end of treatment and 100% for fingernails and 89% for toenails at the follow-up investigation. 5. overall efficacy assessed by the patient and by the investigator were evaluated as more than moderate in 93.1% and 94.5% of patients and overall tolerability were evaluated as more than moderate in 100% of patients by both of them. 6. The adverse effects of terbinafine were noted in 10 patients(7%) out of the 143. The most frequent drug-related adverse events were mild-to-moderate gastrointestinal disturbances. Any changes in liver biochemical tests were not considered clinically relevant. In conclusion, oral terbinafine is effective and safe in the treatment of onychomycosis. Our results demonstrate that a treatment period of 8 weeks for fingernail onychomycosis and 16 weeks for toenail onychomycosis is sufficient.

6.A Clinical Study of Recombinant Interferon - alpha Ointment in the Treatment of Several Viral Skin Diseases: Multicenter Trials.

A study evaluating the therapeutic effect of recombinant interferon-a ointment on several viral skin diseases was done. A total of 204 patients, 92 of herpes simplex, 90 of herpes zoster and 22 of varicella, were included in this study. Two different concentration of recombinant interferon-a ointment(l*10IU/g; group A, 1*10IU/g; group B) and placebo vehicle(control group) were applied topically on the early lesion of the disease. The evaluation of therapeutic effect was done by the measurement of time interval from the day of vesicle formation to the day of crust formation, The results were as follows; 1. Herpes sirnplex: Crust formation observed within 6 days in 70. 1% of group A. 63.9% of group B and 26.7%, of control group. The time of crust formation of group A and B was shorter than that of control group (p<0.01, P<0.05). 2. Herpes zoster: Crust formation observed within 10 days in 64. 3% of group A. 46.7% of group B and 33. 3% of control group. The time of crust formation of group A was shorter than that of control group (p<0.05). 3. Varicella: Crust formation observed within 6 days in 87. 5% of group A, 72.7% of group B and 66.7% of control group. Therefore we have been impressed that recombinant interferon-a ointment is an effective topical therapeutic agent and 1*10IU/g recombinant interferon-a clintment is more effective than 1*10IU/g recombioant interferon-a ointment for above viral skin diseases.

8.A Case of Giant Premature Sebaceous Gland Hyperplasia Clinically Simulating Nevus Sebaceus.

The occurrence of sebaceous gland hyperplasia commonly develops in elderly individuals. Clinical appearance is a papule with a central umbilication corresponding to the histological opening of sebaceous duct in the center. The usual size is 2 to 3mm in diameter. The histological features include hyperplastic mature sebaceous lobules grouped around a widely dilated sebaceous duct. We report a case of a 7-year old child with mental retardation and giant size of sebaceous gland hyperplasia which lead us to such clinical impressions as a nevus sebaceus

9.Serratia marcescens Skin Abscess.

Serratia marcescens is the most important member of the genus Serratia and causes opportunistic infections, particularly pneumonia and septicemia in patients with malignancy, renal failure (acute and chronic), and diabetes mellitus. The most common portals of entry are known to be, in descending order, lung, genitourinary tract, intravenous line, gastrointestinal tract, and skin. S. marcescens rarely causes skin infection because it does not normally colonize human skin. Only six cases of S. marcescens cellulitis were reported. Five of them were immunocompromised patients. We have experienced a case of skin abscess caused by S. marcescens, which was found in a 59-year-old woman. She was undergoing prior antibiotic treatment after insulinoma surgery. S. marcescens was isolated from the skin abscess as a sole organism. She was treated with appropriate antibiotics that exhibited sensitivities for the organism and cured without any complication. The authors report a case of S. marcescens infection on the skin of a 59-year-old woman and review the literature concerning this organism as a causative agent.

10.A Case of Acute Generalized Exanthematous Pustulosis(AGEP), An uncommon Condition in Children.

Acute generalized exanthematous pustulosis(AGEP) is characterized by acute onset of widespread pustular eruption in association of fever and leukocytosis. Most cases appear to be related to drugs, mainly antibiotics, but viral infections and hypersensitivity to mercury may cause AGEP. Histologically, AGEP is composed of neutrophilic subcorneal and spongiform pustules. A 4-year-old female, with no history of psoriasis, was given ampicillin for scarlet fever. One day after taking the medication, pustular eruptions appeared on the whole body. We describe a 4 year old girl with AGEP secondary to ampicillin. This is an uncommon condition in children.
