2.Effect of Nicardipine on Pressor Response to Raised Intracranial Pressure and alpha-Adrenoceptor Agonist.

The effect of nicardipine was investigated on hypertension due to raised intracranial pressure, pressor response of alpha-adrenoceptor agonists in the dissected thoracic aorta. Intracerebroventricular(icv) and intravenous(iv) nicardipine produced dose-dependent depressor response and bradycardiac effect, especially marked response was observed following iv injection. The pressor response to raised intracranial pressures was potentiated following iv injection of 50 microgram/kg nicardipine but was markedly inhibited following iv 100 microgram/kg injection, and was not affected following icv 50 microgram/kg administration but was markedly inhibited following icv administration of 100-200 microgram/kg nicardipine. The nicardipine inhibited contractile effect of KCI 35 mM in a dose-dependent fashion but did not affect that of Ne and ME. These data suggest that nicardipine caused hypotensive effect by blocking calcium influx in the peripheral vessels and that direct effect of nicardipine on central nervous system involves the hypotensive action. Conclusively, the inhibitory effect of nicardipine on the pressor response to the intracranial pressure elevation may be induced by these two mechanisms.

6.Supracondylar Fracture of the Femur Treated by Ender nail

Supracondylar fracture of the femur is not as common as the shaft fracture. There is wide range of potential complication in the treatment of these fractures and the method of the treament and the evaluation of it's result is variable and remains controversial. Until 1960, we believed conservative treatment supirior to the operative treatment of the supracondylar fracture of the femur. But recently, anatomical reduction and internal fixation with early joint motion showed satisfactory results and is the trend of treatment of these fracture. The authors of this thesis performed treatment of supracondylar fracture of femur by utilizing Ender nails and screws in 13 cases during the period from October 1987 to November 1988 and it was found that operation was relatively simple, bleeding was little and early exercise of knee was allowed and even for patients of old age internal fixation could be maintained easily.

7.Clear Cell Sarcoma of the Kidney: A case in 39 year old man.

Clear cell sarcoma of kidney(CCSK) is a rare pediatric neoplasm characterized by a predominating component of clear cells, a predilection for metastases to bone, and a poor prognosis. The incidence of CCSK peaks during the 2nd year of life and adult cases are very rare. We report a case of CCSK encountered in the right kidney of a 39-year-old man. Grossly, it was a lobulated mass showing infiltrative margin, measured 7x5.5x5cm and had a homogeneous gray-tan color with a soft, fish-flesh consistency. Microscopically, about half of the tumor revealed the classic pattern of CCSK, having tumor cell cords or nests separated by the characteristic alveolar capillary networks. The tumor cells had clear pale cytoplasm, bland looking round nuclei and inconspicuous nucleoli. The other half showed the epithelioid-trabecular pattern forming pseudorosette or cord-like structures. Immunohistochemically, there was only a focal positive reaction to vimentin. Ultrastructurally, the tumor cells showed the primitive nephrogenic mesenchymal differentiation such as electron lucent cytoplasm, a small amount of organelles, scanty heterochromatin, inconspicuous nucleoli, and a lack of flocculant basal lamina material around the cytoplasmic membrane. We consider that this is a case of CCSK occuring in the oldest patient ever reported, confirmed by both immunohistochemistry and electron microscopy.

9.A Comparison of Serum & Nasal Wash Fluid ECP According to Their Ages in Asthmatic Children.

PURPOSE: Asthma features not only infiltration of various inflammatory cells including eosinophils into bronchus but increased bronchial hyperresponsiveness which comes from bronchial epithelial cells damaged by toxic granular protein like MBP and ECP released from the activated eosinophils. Recently, several authors have reported that nasal mucosa histologic findings are similar to an inflammatory change of lower airway in respiratory allergic diseases. Based on the latter, we performed this study to compare serum ECP with nasal wash fluid ECP by their ages and to find out contribution of eosinophils in infants and children with wheezing. METHODS: The 62 patients in age from 6 months to 14 years who were admitted due to acute asthma from June 1995 to May 1996 were divided into 3 groups by their ages as follows : group A for less than 2 years old, group B for 2-4 years old and group C for 5-14 years old. The 32 children with no previous history of wheezing or no respiratory symptoms on admission were enrolled as control subject. Serum ECP & nasal wash fluid ECP from each age group with asthma were measured and compared with control, and the relationship between serum ECP and atopic findings were studied. RESULTS: 1) The geometric mean level of serum ECP were significantly higher in wheezing children than in control in all three age group as follows : 16.5+/-7.1micorgram/l for group A, 28.9+/-18.7micorgram/l for group B, 45.9+/-34.8micorgram/l for group C. 2) The geometric mean level of nasal wash fluid ECP were significantly higher in wheezing children than in control in all age group as follows : 11.3+/-7.1micorgram/l for group A, 21.3+/-15.2micorgram/l for group B, and 30.2+/-18.2micorgram/l for group C. 3) Serum ECP had correlation with nasal wash fluid ECP & blood eosinophils in group B and C, except group A. CONCLUSION: On acute asthma attack, the mean ECP level of serum & nasal wash fluid from all children including infancy were significantly higher than control and this suggests that the role of eosinophils to airway inflammation in many wheezing infants may be the same as that in older children with asthma.
